Abstract

Objective: To describe the favorable clinical outcome in a case with bilateral choroidal neovascular membrane and quiescent Vogt-Koyanagi-Harada (VKH) syndrome by administering bilateral intravitreal aflibercept injections.Case report: A 30-year-old woman was diagnosed with VKH syndrome at another institution and had been in remission with oral mycophenolate mofetil for two years. However, nearly simultaneous right juxtafoveal and left subfoveal type 2 choroidal neovascular membrane was detected two years after the initial diagnosis. The right eye (OD) received three and the left eye (OS) received four aflibercept injections within a time span of eight months. Visual acuity was 20/30 in OD and 20/25 in OS at the last follow-up visit.Conclusion: Although suppression of inflammation is a must in eyes with inflammatory type choroidal neovascular membranes, anti-VEGF (vascular endothelial growth factor) therapy with agents, such as aflibercept in the present case, is a key therapeutic adjunct and may possibly help improve the visual prognosis.

Abstract

We describe a 36-year-old woman with a relapsing Hodgkin’s lymphoma who developed a severe bilateral sudden visual loss almost three weeks after the initiation of brentuximab therapy. Ancillary fundus tests yielded bilateral severe retinal arteriolar occlusion 360° and serous macular retinal detachment. No visual improvement could be achieved despite the pulse corticosteroid therapy and a single bilateral intravitreal aflibercept administration cessation of the brentuximab therapy. Unfortunately, she succumbed to respiratory failure almost six weeks after the diagnosis of Purtscher-like retinopathy.

Abstract

To elucidate whether wide-field fluorescein angiography (WFFA) can yield additional information in patients with newly diagnosed idiopathic acute anterior uveitis (AAU). The WFFA was performed in patients with idiopathic AAU, and the findings were analyzed according to the scoring system by Angiography Scoring for Uveitis Nomenclature. Forty-four eyes of 30 patients (22 eyes of 13 patients in the pediatric group and 22 eyes of 17 patients in the adult group) were studied. The mean age was 12.41 ± 3.92 (range, 5–18) years in the pediatric group and 42.36 ± 32.07 (range, 24–68) years in the adult group. Thirteen eyes (59%) of pediatric patients and 12 eyes (54%) of adult patients showed some evidence of posterior segment activity on the WFFA (p =.764). Systemic treatment was administered in 53.8% of the pediatric and 5.9% of adult patients, depending on the disease severity. Pediatric patients with idiopathic AAU may have subtle posterior segment manifestations more than adult patients, and posterior segment findings may affect the treatment preferences of physicians. Mean age of the cohort was 65.44 ± 9.14 years (Range; 49– 82 years). Mean diameters of intraretinal macroaneurysms were 238.20 ± 61.12 μm (Range; 163.00– 292.50 μm) and 242.72 ± 49.58 μm (Range; 168.00– 328.00 μm) on fluorescein angiography and optical coherence tomography, respectively. Primary group had 10 eyes with 11 intraretinal macroaneurysms, whereas eight eyes had nine intraretinal macroaneurysms in the secondary group. Three of the eight eyes (37.5%) had diabetic retinopathy, four (50%), retinal vein occlusion, and one (12.5%), posterior uveitis in the secondary group. 19. Evaluation of Macular Pigment Optical Density in Eyes with Hyperopic Anisometropic Amblyopia Using Fundus Reflectometry.

Abstract

Objectives: This study aimed to evaluate the macular pigment optical density (MPOD) scores in eyes with hyperopic anisometropic amblyopia and compare those with their fellow and control eyes using one-wavelength fundus reflectometry. Methods: This cross-sectional study enrolled 33 patients diagnosed with hyperopic anisometropic amblyopia aged 12-40 years. The control group consisted of 36 hyperopic and 42 emmetropic children, age-matched to the patients. Central macular thickness (CMT), MPOD, axial length (AL), best-corrected visual acuity, and refraction errors were measured between the study group and the control group. Results: Eyes with the diagnosis of hyperopic anisometropic amblyopia had significantly higher mean and maximum (max) MPOD scores compared with their fellow eyes as well as hyperopic and emmetropic eyes (p<0.001 for all). The mean AL in eyes with hyperopic anisometropic amblyopia was statistically shorter than that in hyperopic and emmetropic controls (p=0.027, p<0.001, respectively). The mean CMT was found to be thicker in eyes with hyperopic anisometropic amblyopia when compared to their fellow eyes, as well as hyperopic and emmetropic controls, eventhough there was no significant difference was found among the four groups (p=0.052). The mean MPOD levels were significantly correlated with the difference in CMT (r=-0.21, p=0.032), and logMAR visual acuity scores (r=-0.44, p<0.001) in the hyperopic anisometropic amblyopia group. Conclusion: The present study indicates that the MPOD is reduced in eyes with hyperopic anisometropic amblyopia. This reduction may be due to less visual stimulus-induced deterioration of foveal development and microarchitecture in anisometropic amblyopic eyes. [ABSTRACT FROM AUTHOR]

20. Akut Retinal Nekrozlu Olgularımızın Fonksiyonel ve Anatomik Sonuçları

Abstract

AMAÇ:Akut retinal nekroz (ARN) gelişen olgularda tedavi sonrası fonksiyonel ve anatomik sonuçların değerlendirilmesi.GEREÇ VE YÖNTEM:Kliniğimizde Aralık 2011 – Nisan 2021 yılları arasında ARN tanısıyla en az 6 ay takip edilen, 9 hastanın 11 gözü retrospektif olarak değerlendirildi. Tüm hastalara intravitreal ve sistemik antiviral tedavi uygulandı. Hastaların final fonksiyonel ve anatomik sonuçları incelendi.BULGULAR:Hastaların ortalama yaşı 44.5±18.1 yıl ve takip süresi 24.05±29.67 aydı. Olgularımızın 5’i (%55.6) erkek idi. Vitreus örneğinden çalışılan polimeraz zincir reaksiyonu sonucunda 7 gözde (%63.6) varisella zoster virüsü tespit edildi. Başvuruda tüm olgulara parenteral asiklovir başlandı. 10-14. günden sonra 1 olguda oral asiklovire; 8 olguda oral valasiklovire geçildi. 1 göze sadece foskarnet sodyum, 2 göze hem gansiklovir hem de foskarnet sodyum olmak üzere; 10 göze ortalama 6.18±7.12 kez intravitreal gansiklovir ve 3 göze ortalama 0.63±1.50 kez intravitreal foskarnet sodyum uygulandı. En iyi düzeltilmiş görme keskinliği tanı sırasında persepsiyon projeksiyon (+) ile 0.3 arasında değişmekte iken takip süresinin sonunda el hareketi ile 0.7 arasında değişmekteydi. 7 gözde (%63.6) takip sırasında yırtıklı retina dekolmanı gelişti. 2 gözde (%18.1) 20 G, 2 gözde (%18.1) 23 G ve 2 gözde (%18.1) 25 G pars plana vitrektomi, çevreleyeci skleral çökertme ve silikon yağ endotamponadı ile cerrahi tamir gerçekleştirildi. Retina dekolmanı gelişen 1 olgu (%11.1), önerilen cerrahi tedaviyi kabul etmedi. 1 olgunun (%11.1) takibinde katarakt cerrahisi sonrası dirençli psödofakik kistoid makula ödemi gelişmesi nedeniyle; intravitreal gansiklovir şemsiyesi altında deksametazon implant uygulandı. 1 olguda (%11.1) başvuru sırasında bilateral ARN mevcuttu. Takip süresince, unilateral başlangıçlı olgulardan 1’inde (%11.1) diğer gözde ARN gelişti.TARTIŞMA VE SONUÇ:Akut retinal nekroz, günümüzde halen ciddi görme kaybına yol açabilen bir hastalıktır. Erken tanı ve etkili bir tedavi ile görme kaybı önlenebilir. Sistemik ve intravitreal antiviral ilaçlar tedavinin temelini oluşturmaktadır. Gerekli durumlarda vitreoretinal cerrahi fonksiyonel ve anatomik başarıyı artırabilmektedir. OBJECTIVE: To evaluate the functional and anatomical results after treatment in patients with acute retinal necrosis (ARN).MATERIALS AND METHODS: Eleven eyes of 9 patients who were followed up in our clinic for at least 6 months with the diagnosis of ARN between December 2011 and April 2021 were evaluated retrospectively. All patients received intravitreal and systemic antiviral therapy. Final functional and anatomical results of the patients were reviewed.RESULTS: The mean age of the patients was 44.5±18.1 years and the follow-up period was 24.05±29.67 months. Five of our cases (55.6%) were male. As a result of the polymerase chain reaction studied from the vitreous sample, varicella zoster virus was detected in 7 eyes (63.6%). Parenteral acyclovir was started in all cases at admission. 10-14. oral acyclovir in 1 case after the day; Oral valacyclovir was used in 8 cases. Foscarnet sodium only in 1 eye, and both ganciclovir and foscarnet sodium in 2 eyes; An average of 6.18±7.12 times intravitreal ganciclovir was administered to 10 eyes and an average of 0.63±1.50 times intravitreal foscarnet sodium was administered to 3 eyes. The best corrected visual acuity ranged from perception projection (+) to 0.3 at diagnosis, and ranged from hand movement to 0.7 at the end of the follow-up period. Ruptured retinal detachment developed in 7 eyes (63.6%) during follow-up. Surgical repair was performed with pars plana vitrectomy, circumferential scleral buckling, and silicone oil endotamponade in 2 eyes (18.1%) 20 G, 2 eyes (18.1%) 23 G, and 2 eyes (18.1%) 25 G. One patient (11.1%) who developed retinal detachment did not accept the recommended surgical treatment. In the follow-up of 1 case (11.1%), due to the development of resistant pseudophakic cystoid macular edema after cataract surgery; Dexamethasone implant was applied under the umbrella of intravitreal ganciclovir. Bilateral ARN was present in 1 case (11.1%) at presentation. During the follow-up, 1 (11.1%) of the unilateral onset cases developed ARN in the other eye.DISCUSSION AND CONCLUSION: Acute retinal necrosis is a disease that can still cause severe vision loss today. Vision loss can be prevented with early diagnosis and effective treatment. Systemic and intravitreal antiviral drugs form the basis of treatment. When necessary, vitreoretinal surgery can increase functional and anatomical success.

27. Rheumatologic manifestations in a cohort of patients with Vogt-Koyanagi-Harada disease.

Abstract

Objectives: Vogt-Koyanagi-Harada Disease (VKHD) is a systemic autoimmune disorder characterized by granulomatous panuveitis. Inflammatory rheumatic diseases (IRDs) are among the differential diagnosis of VKHD. However, current knowledge on the rheumatological aspects of VKHD is still limited. We aimed to investigate the prevalence of rheumatic conditions in VKHD patients. Methods: VKHD patients were included in the study and they were reviewed in terms of the presence of any rheumatological manifestations. Results: There were 18 patients with a female preponderance (83.3%, female). Inflammatory type of peripheral joint pain (11%) and sicca symptoms (33%) were the most common rheumatological findings. The frequency of spondyloarthritis-related features such as inflammatory back pain and HLA-B27 rate was not increased. None of the patients had radiographic sacroiliitis. Anti-nuclear antibody was positive in high titres nearly in 30% of the patients and three patients had antibodies against extractable nuclear antigens. Nailfold capillaroscopy was abnormal in about one-third of the patients. Pathergy test was negative in all cohorts. While angiotensin-converting enzyme was elevated in nearly 20% of the patients, there were no abnormalities on chest X-rays. Conclusion: VKHD shares some features with IRDs. The common features were mostly suggestive of connective tissue disease rather than SpA or rheumatoid arthritis. [ABSTRACT FROM AUTHOR]

Abstract

PurposeTo evaluate the real-life safety profile of intravitreal dexamethasone implant injection for various retinal conditions.MethodsRetrospective multicenter analysis of intravitreal dexamethasone implant injections (700 µg) due to various retinal conditions including central retinal venous occlusion (1861 injections), diabetic macular oedema (3104 injections), post-surgical cystoid macular oedema (305 injections) and uveitis (381 injections). The eyes were evaluated mainly for the occurrence of adverse events such as glaucoma, cataract, retinal detachment and endophthalmitis along during the follow-up period.ResultsA total of 6015 injections in 2736 eyes of 1441 patients (mean age of 65.7±12.9 years) were in total analysed over an average period of 18 months (range 6 months to 102 months). A total of 576 eyes (32.5% of the phakic eyes) developed cataract requiring surgical intervention. However, visually insignificant cataract progression was observed in another 259 phakic eyes (14.6%) which did not require surgical removal. A total of 727 eyes (26.5%) experienced an intraocular pressure (IOP) rise of >25 mm Hg, with 155 eyes (5.67%) having a prior history of glaucoma and 572 eyes (20.9%) having new onset IOP rise. Overall, more than 90% of eyes with IOP rise were managed medically, and 0.5% eyes required filtering surgery. Endophthalmitis (0.07%), retinal detachment (0.03%) and vitreous haemorrhage (0.03%) were rare. There was no significant change in visual acuity (p=0.87) and central macular thickness (p=0.12) at the last follow-up.ConclusionThis is the largest real-life study assessing the safety of intravitreal dexamethasone implant injections in various retinal conditions. Cataract progression and intraocular pressure rise are the most common side effects, but are often rather easily manageable.

Abstract

Amaç: Glokom, optik sinirin progresif dejenerasyonu ve retinal ganglion hücrelerinin kaybı ile karakterize bir grup hastalıktır. Glokomda humör aköz ve vitreus içerisinde protein özelliğinde birçok biyobelirtecin artışı tanımlanmaktadır. Ancak “proteomik” olarak adlandırılan bu proteinlerin biyokimyasal analizi oldukça güçtür. Diğer yandan manyetik rezonans görüntüleme (MRG) tetkikinde, protein içerik, sinyal değişikliğine neden olmaktadır. Çalışmamızın amacı; vitreus içerisinde bulunan protein özelliğindeki moleküllerin MR sinyal değişikliğine neden olabileceği hipotezinden yola çıkarak glokom tanısının orbita MR ile konulmasının etkinliğini araştırmaktır. Gereç ve Yöntem: Hastanemizin Göz Hastalıkları AD. Glokom birimine Nisan - Kasım 2018 ve Ağustos - Ekim 2019 tarihleri arasında başvuran hastalardan glokom tanısı olan ve orbita MR incelemesi bulunanlar retrospektif olarak saptanarak çalışmamıza dahil edilmiştir. Çocuk hastalar, göz travması ya da hasarı bulunan hastalar dışlanmıştır. Kontrol grubu olarak glokom hastalığı bulunmayan, diğer sebeplerle orbita MR tetkiki elde olunan ve orbita MR incelemesi normal olan hastalar seçilmiştir. Hasta ve kontrol grubunun orbita MR tetkiklerinde T1 ve T2 ağırlıklı aksiyel kesitlerde, bilateral bulbus okuli vitreus düzeyine “region of interest” (ROI) yerleştirilerek intensite değerleri ölçülmüştür. Aynı ROI, incelemeye dahil olan beyin omurilik sıvısı (BOS) alanına da yerleştirilerek ölçüm yapılmıştır. Vitreus ve BOS ölçümleri birbirine oranlanarak normalizasyonu sağlanmıştır. İstatistiksel farklılık Mann Whitney U testi uygulanarak araştırılmıştır. Bulgular: Hasta grubunda 16 hastanın 31 gözü, kontrol grubunda ise 16 olgunun 32 gözü değerlendirilmiştir. İki grubun, T1 ağırlıklı görüntülerden gerçekleştirilen ölçümleri arasında istatistiksel olarak anlamlı fark saptanmıştır (p=0,045). Bununla birlikte T2 ağırlıklı görüntülerde ölçümler arasında anlamlı fark saptanmamıştır (p=0,729). Sonuç: Bu çalışma sonucunda glokom hastalarında izlenen vitreal proteinoz değişikliklerin radyolojik olarak da saptanabileceği gösterilmiştir., Objective: Glaucoma is a disease which causes progressive degeneration of optic nerve and cellular damage in retinal ganglion. in glocoma, increase of some protein nature biomarkers (proteomics) in vitreous were defined. Chemical analysis of proteomics is very difficult. on the other hand, it is known that protein content causes intensity changes in magnetic resonance imaging (MRI). Aim of this study, is to evaluate the role of orbital MRI measurements on detecting the presence of proteomics in vitreous. Materials and Methods: Glaucoma patients with orbital MRI investigations in our ophthalmology clinic from April to November 2018 and from August to October 2019 were collected retrospectively. Patients under 18 years of age or with eye trauma clinic or history were excluded. Patients with normal orbital MRI which had been performed due to reasons other than glaucoma were selected for the control group. Vitreous measurements were performed by placing “region of interest” (ROI) on T1 and T2 weighted axial images. Cerebrospinal fluid (CSF) measurements were also performed by using same ROI on the same sequence. Vitreous measurements were divided by CSF measurements for normalization. Mann Whitney U test were performed to evaluate the statistically significant difference. Results: in glaucoma group 31 eyes of 16 patients and in control group 32 eyes of 16 patients were evaluated. Measurements performed on T1 weighted images showed statistically significant difference between two groups (p=0.045). on the other hand, no statistically significant difference was found between the groups with T2 weighted image measurements (p=0.729). Conclusion: Our study showed that it is possible to detect vitreous changes with orbital MRI measurements.

43. Optic nerve sheath decompression saves sight in severe papilloedema: results from 81 eyes in 56 patients with pseudotumor cerebri.

Abstract

Purpose: To report the outcome of optic nerve sheath decompression (ONSD) for papilloedema in a teaching hospital in western Turkey. Methods: The charts of 56 patients who had ONSD surgery between April 2007 and September 2019 were collated; and a total of 81 operated and 31 fellow eyes were included. Pre‐ and postoperative ophthalmologic examination including best‐corrected visual acuity (BCVA), colour vision (CV), visual field (VF) analysis, fundoscopic examination and demographic and medical characteristics of the patients were noted and outcomes after surgery were investigated. Results: Of all study eyes, 49 (43.7%) eyes had BCVA 0.2 or less and 62 (55.3%) eyes had mean deviation (MD) below – 20.0 dB. 62 (55.3%) eyes had Frisen grade 4 or 5 papilloedema. Almost half of the eyes had severe vision loss. After ONSD, BCVA, CV and MD in both operated and fellow non‐operated eyes improved significantly (p < 0.001, p = 0.009 and p < 0.001 for operated, p < 0.001, p = 0.007 and p < 0.001 for fellow eyes, respectively). Earlier surgery and higher cerebrospinal fluid opening pressure were related to better outcomes. None of the patients had major operative complications. Conclusion: Optic nerve sheath decompression can safely improve vision not only of the operated but also of the non‐operated eye, even in cases with severe vision loss from severe bilateral papilloedema. Regardless of initial VA and VF, patients may benefit from ONSD; the earlier it is done the more likely the better outcome. [ABSTRACT FROM AUTHOR]
