1. Psychiatric care after Kahramanmaraş earthquake: Evaluation of children being trapped under rubble and in need of intensive care

Abstract

Kahramanmaraş earthquakes occurred on 6 February 2023, and many people died and were trapped under the rubble. The objective of this research was to investigate child victims of the quake, who were most dramatically affected, required intensive care and were trapped under debris/rubble/wreckage in terms of psychiatric status and the disaster-related or personal determinants of psychopathologies. Pediatric patients (n=77) who were hospitalized in intensive care units and inpatient wards within the first month after the earthquake and were consulted to the Child Psychiatry, were examined in terms of psychiatric symptoms, use of psychotropics, duration under debris, loss of relatives, loss of limbs, and intensive care requirement. Anhedonia, loss of appetite, psychosomatic and dissociative symptoms, as well as psychotropic need/use were significantly higher in patients who required intensive care. Dissociative and psychosomatic symptoms were linked with longer durations under debris. The current findings highlight prominent and predominated psychiatric symptoms in child victims of quakes, hospitalized in intensive care units and being prolonged trapped under debris. Prolonged entrapment correlated with increased dissociative and psychosomatic symptoms, emphasizing the need for targeted psychiatric support for vulnerable earthquake survivors. Abstract

OBJECTIVES: The determinants of subjective well-being have been largely investigated in recent years. According to Pavot and Diener who have developed TSWL (Temporal Satisfaction with Life Scale), to assess the life satisfaction level of an individual according to temporal factors, subjective well-being is composed of two constructs: an emotional or affective component (i.e. positive and negative affects) and a conceptual or cognitive component (i.e. satisfaction with life, marriage, work, and leisure). The aim of this study was to adapt the Temporal Satisfaction with Life Scale into Turkish (TSWLS-T) and examine the reliability and validity of this new Turkish version. METHODS: The study was conducted in four phases with randomly chosen 236 participants (113 females, 123 males) and the mean age of sample was 38.6 ± 12.5 years. Cultural adaptation was implemented according to the internationally suggested method and a pre-test was administered to examine the language equivalence. Since the scale had high levels of language equivalence, validity and reliability studies were conducted. For construct validity, confirmatory factor analysis was employed. The Satisfaction with Life Scale (SWLS) was used in order to evaluate the validity of TSWLS-T, since internal consistency (Cronbach’s alpha and intra-class coefficients) and test–retest analysis were employed to estimate instrument reliability. RESULTS: The TSWLS-T demonstrated acceptable internal consistency, with a Cronbach’s alpha of 0.87 for the total scale and ranging between 0.76 and 0.88 for the subscales. Aims:This study aimed to evaluate the mental health of mothers of children with attention deficit hyperactivity disorder (ADHD) after distance learning measures during the Coronavirus disease-2019 (COVID-19) pandemic, considering factors that may influence their well-being.Methods:In a cross-sectional design, the levels of burnout, depression, anxiety, stress, coping strategies, social support, and perceived changes in mental health among the mothers were assessed using the Maslach Burnout Inventory (MBI), Depression Anxiety Stress Scale-21 (DASS-21), Brief Coping Orientation to Problems Experienced Inventory, Multidimensional Scale of Perceived Social Support Scale, and Global Rating of Change (GRC) scale. Furthermore, the mothers completed the Atilla Turgay Scale (T-DSM-IV-S) to evaluate their perceptions of their children’s ADHD and behavioral problems.Results:Of the 72 mothers (mean age: 42.46±6.73 years, mean children age: 11.39±3.12 years), 50%, 44.4%, and 43.1% had depression symptoms, anxiety, and stress, respectively. According to the GRC, most mothers (67%) reported deterioration in mental well-being compared with the pre-pandemic period. Abstract

EgeUn###, Aim: The aim of this study was to determine the prevalence of childhood psychopathologies in Turkey. Method: A nation-wide, randomly selected, representative population of 5830 children (6-13 years-old) enrolled as a 2nd,3rd or 4th grade student in 30 cities were evaluated for presence of a psychiatric or mental disorder by a Sociodemographic Form, Kiddie Schedule for Affective Disorders and Schizophrenia for School Age Children-Present and Lifetime Version (K-SADS-PL), and DSM-IV-Based Screening Scale for Disruptive Behavior Disorders in Children and Adolescents scales. Impairment criterion was assessed via a 3 point-Likert scale by the parent and the teacher independently. Results: Overall prevalence of any psychopathology was 37.6% without impairment criterion, and 17.1% with impairment criterion. Attention-deficit hyperactivity disorder was the most frequent diagnosis, followed by anxiety (19.5% and 16.7% without impairment, 12.4% and 5.3% with impairment, respectively). Lower education level and presence of a physical or psychiatric problem of the parents were independent predictors of any psychopathology of the offspring. Conclusion: This is the largest and most comprehensive epidemiological study to determine the prevalence of psychopathologies in children and adolescents in Turkey. Our results partly higher than, and partly comparable to previous national and international studies. It also contributes to the literature by determining the independent predictors of psychopathologies in this age group.

Abstract

The stigmatization towards mental illness has significant effects on the quality of life both for the people with a psychiatric disorder and their families. The aim of this study was to translate the Attribution Questionnaire-27(AQ-27) to the Turkish language, and to evaluate the reliability and validity of new Turkish version on a multi-centered selected adult sample. Cultural adaptation was implemented according to the internationally suggested method. The Turkish version of AQ-27 (AQ-27-T) was applied to a total of 424 participants (221 females) included to study. Methods: This study was conducted as a part of the "The Epidemiology of Childhood Psychopathology in Turkey" (EPICPAT-T) Study, which was designed by the Turkish Association of Child and Adolescent Mental Health. The inclusion criterion was being a student between the second and fourth grades in the schools assigned as study centers. The assessment tools used were the K-SADS-PL, and a sociodemographic form that was designed by the authors. Impairment was assessed via a 3 point-Likert type scale independently rated by a parent and a teacher. Results: A total of 5842 participants were included in the analyses. The prevalence of affective disorders was 2.5 % without considering impairment and 1.6 % when impairment was taken into account. In our sample, the diagnosis of bipolar disorder was lacking, thus depressive disorders constituted all the cases. Among depressive disorders with impairment, major depressive disorder (MDD) (prevalence of 1.06%) was the most common, followed by dysthymia (prevalence of 0.2%), adjustment disorder with depressive features (prevalence of 0.17%), and depressive disorder-NOS (prevalence of 0.14%). There were no statistically significant gender differences for depression. Maternal psychopathology and paternal physical illness were predictors of affective disorders with pervasive impairment. Conclusion: MDD was the most common depressive disorder among Turkish children in this nationwide epidemiological study. This highlights the severe nature of depression and the importance of early interventions. Populations with maternal psychopathology and paternal physical illness may be the most appropriate targets for interventions to prevent and treat depression in children and adolescents.

Abstract

Cognitive, emotional and social problems are being experienced to long-term follow up of children with cancer. Children diagnosed with cancer are frequently suffered from major mental illnesses such as depression, post-traumatic stress disorder and somatization disorder. This paper reports, in substantial details, a 16-year-old female adolescent case who had exposed to a long time period of medical treatment after the diagnosis of Ewing's Sarcoma and was admitted to our clinic for psychiatric symptoms related to sequelae of cancer. In this manuscript we are discussing supportive psychotherapeutic approaches implemented to this adolescent and its follow-up therapeutic results., Kanser hastalığı olan çocukların uzun süreli izleminde bilişsel, duygusal ve sosyal alanda sorunlar yaşanmaktadır. Kanser tanısı konulan çocuklarda depresyon, posttravmatik stres bozukluğu ve somatizasyon bozukluğu gibi önemli ruhsal hastalıklara sık rastlanmaktadır. Bu olgu sunumunda Ewing Sarkomu tanısından sonra uzun bir tedavi süreci geçiren ve kanser sekeli sonucu yaşadığı psikiyatrik sorunlar için kliniğimize başvuran 16 yaşında bir ergenin izleminde uygulanan destekleyici psikoterapötik yaklaşımlar ve sonuçları tartışılmıştır.

Abstract

Objective. To assess the children referred to our clinic with a health measure ruling which was given in accordance with Turkey’s no. 5395 Child Protection Law, in terms of socio-demographic characteristics, psychiatric diagnosis, and follow-up. Methods. Thirty children referred to our clinic with a health measure ruling given according to Turkey’s no. 5395 Child Protection Law were assessed. Results. The mean age was 13.20±3.86 (min-max: 2-17). Referral reasons of the children with a health measure ruling included driven to crime in 7 (23.3%), family therapy in 8 (26.7%), sexual abuse in 8 (26.7%), substance abuse in 3 (10%), physical abuse in 1 (3.3%), and medical care and rehabilitation in 3 (10%) children. Of the children referred for a health measure, 19 (63.3%) did not continue following the initial assessment. Conclusions. To enhance the effectiveness of the protective and preventive mental health services, children and adolescents who have a health measure ruling should be treated, followed-up, and rehabilitated at every stage of the process. It is necessary to increase in-house and inter-agency communication in order to implement the system effectively for the benefit of these children.
