Abstract

Low-molecular-weight heparin appears to be as effective as regular heparin in treating people with pulmonary embolism. This condition occurs when a blood clot forms in another part of the body and travels to the lungs. Researchers randomly assigned 612 patients with pulmonary embolism to receive daily infusions of regular heparin or injections of low-molecular-weight heparin. At eight days and three months, the incidence of recurrent embolism, bleeding or death was similar in both groups.

26. Minocycline pneumonitis and eosinophilia: a report on eight patients

Abstract

We identified eight patients (six women and two men) who had pulmonary infiltrates during treatment with minocycline hydrochloride between 1989 and 1992 in French referral centers for drug-induced pulmonary diseases. Clinical files, chest roentgenograms, computed tomographic scans, pulmonary function, and bronchoalveolar lavage data were reviewed. Minocycline treatment was given for acne (n=4), genital infection (n=3), and Lyme disease (n=1). The duration of treatment averaged 13 [+ or -] 5 days (mean [+ or -] SE); the total dose, 2060 [+ or-] 540 mg. Patients presented with dyspnea (n=8), fever (n=7), dry cough (n=5), hemoptysis (n=1), chest pain (n=2), fatigue (n=3), and rash (n=3). Chest roentgenograms showed bilateral infiltrates in all cases. Pulmonary function was measured in five patients; four had airflow obstruction and two had mild restriction. Blood gas tests demonstrated hypoxemia in seven patients (58 [+ or -] 3 mm Hg). Seven patients had blood eosinophilia 1.76 [+ or -] 0.2x[10.sup.9]/L). Bronchoalveolar lavage (performed in seven patients) showed an increased proportion of eosinophils (0.30 [+ or -] 0.07). The [CD4.sup.+]/[CD8.sup.+] ratio was determined in four cases and was low in three. Transbronchial lung biopsy, performed in two patients, showed interstitial pneumonitis in both patients, with marked infiltration by eosinophils in one patient. The outcome was favorable in all patients. Because of severe symptoms, steroid therapy was required in three patients. Rechallenge was not attempted. We conclude that minocycline can induce the syndrome of pulmonary infiltrates and eosinophilia, that presenting symptoms may be severe and may culminate in transient respiratory failure, and that the disease has a favorable prognosis.

Researchers in heat transfer field always attempt to find new solutions to optimize the performance of energy devices through heat transfer enhancement. Among various methods which are implemented to reinforce the thermal performance of energy systems, one is the dispersion of solid nanoparticles in common working fluids such as water. The suspension is called nanofluid. On the other hand, utilizing porous media in heat exchangers is another technique to augment of thermal efficiency. Porous media by providing high surface area contact will ameliorate heat transfer rate in ducts. In the present work, a comprehensive review is conducted on the simultaneous application of nanofluids and porous media for heat transfer enhancement purposes in thermal systems with different structures, flow regimes, and boundary conditions. Abstract

In this thesis the main features of Harrison C. White’s general sociology are studied. Since the 1960s White has played a crucial role in the development of the social network approach. He is well known for both the fecundity of the analytical tools he has developed over the years and for the original contributions he has made to several sub­fields of the discipline. White has also developed an unconventional and highly individual approach to social reality that, as the end-result of a sustained synthesizing effort, has grown out of a long and persistent endeavor. Yet, more than a decade after its publication, this general theoretical approach still remains largely unexplored. The main argument of this study is that White’s approach represents one of the mast persistent, elaborated and systematic efforts to enrich the analytical rigorous of the social network approach by adding the substantive theoretical insights that have been elaborated mainly within the symbolic interactionist perspective and the tradition of phenomenological sociology. In this study, first the premises of White’s approach are examined. It is demonstrated how White uses social networks as an analytical tool in order to obtain causal explanations of social phenomena. It is also shown how White re- conceptualizes the notions of social relationship and embeddedness. Furthermore, it is also discussed how White, on the basis of these conceptual innovations, develops a novel image of modern social contexts. This study proceeds by presenting the set of new basic concepts that are derived from this image, seeking to locate these concepts within the larger and more familiar context of theoretical sociology. It is also demonstrated in this study that White’s particular image of modern social contexts leads him to pose new questions and to develop new modes of analysis to answer them. White’s view of modem societies radically alters the very nature and state of the question of social order as well as the premises of its answer. As White dismisses the conventional formulations of the problem of social order, he considers the issue to be a question of identifying the small enclaves of regularity within the social landscape that is dynamic, indeterminate and shifting. In more concrete terms, it becomes a question of identifying the limited, local and stable patterns or configurations of relationships that prove sustainable and thus observable, despite all the dynamics of embeddedness and connectivity. Finally, the basic theoretical features of White’s model of production markets are presented and discussed. Production markets is a topic to which White has devoted a great deal of interest. Ever since the mid-1970s he has produced a long series of work with the ambition of developing a sociological account of these markets. This account represents the most extensive application of White’s general sociology, where he fleshes out his abstract ideas and arguments and where one finds a concrete case of his account of the emergence of social structures and local orders out of network ties and flows. The main conclusion of this study is that, despite all its shortcomings, the general sociological perspective that White has developed is an important contribution. Abstract

This article investigates how a plastic manufacturing firm handles the challenging task of comparing and choosing among the feasible alternative modes of structuring the flow of repeated transaction with one of its key suppliers. Qualitative interviews are used to explore the perceptions of the management of the firm concerning the advantages and disadvantages associated with each alternative, and the main findings suggest that the selection of the transaction structure intended to reduce boundary uncertainty is itself an uncertain choice. Since many of the gains and drawbacks of the alternatives are unknown and even unknowable in advance, the firm is unable to set up a conclusive comparative assessment to underpin its choice. Abstract

Using Swedish entrepreneurs' own explanations, this article searches for the possible regularities in the accounts offered by white collar offenders and for the underlying factors that produce these regularities, tracing in particular the impact of these offenders' attitudes toward regulation and taxation as well as their perceptions of the functioning of the market in which they operate. The results show that justifications are more common among offenders who have a clearly negative view on both factors. Excuses, on the other hand, are more frequently used by those who hold a positive attitude toward taxation and regulation. The results also show that offenders who offer excuses tend to share the same negative perception of the market as the “justifiers.” This unanticipated finding suggests that the “excusers” commit their offenses because they believe that the unlawful behaviors of the relevant others disturb the functioning of the market mechanism and thereby push them toward criminality.
