1. Association between severity of ischemia evaluated by stress CMR and Syntax score in patients with stable coronary artery disease

Abstract

Dipiridamol stress cardiac magnetic resonance (CMR) had proved to be useful to evaluate the benefit of revascularization in stable coronary artery disease [1]. Nevertheless the association between stress CMR imaging parameters and coronary Syntax score has not yet be fully evaluated. Objective To analyze the association of the extent and transmurality of ischemia and necrosis analyzed by vasodilators stress CMR with the severity and prognosis of coronary artery disease assessed by the Syntax score. Methods A retrospective cohort study including consecutive patients with stable coronary artery disease with inducible ischaemia in vasodilators stress CMR who underwent post-study catheterization was performed. The CMR protocol includes a functional study with b-SSFP sequences, first-pass perfusion with b-SSFP sequences obtained after administration of 0.84mg/kg of dipyridamole and during administration of gadolinium chelates and also IR-FGE viability sequences. A post-stress perfusion (PSP) and viability (V) score was developed scoring each segment from 0 to 2 according to transmurality. The number of segments with induced wall motion abnormalities (NSIWA) were also analyzed. The Syntax I and II scores were calculated using data from coronary angiography and computerized medical records. Results 208 patients were included, mean age 65±13 years, 44% women, 109 of them revascularized after stress CMR, 13 of them by surgery. Univariate analysis showed a significant correlation of the PSP, NSIWA and V scores with the Syntax I and Syntax 2. Using a multivariate regression models, NSIWA and V scores were independent predictors for Syntax I while NSIWA and PSP were independent predictors for Syntax 2. Conclusions – CMR stress test in patients with stable coronary artery disease allows an integrative assessment of V, PSP and PSWA and all of this parameters were associated with the severity of coronary artery disease and the prognosis after revascularization estimated by the Syntax I and 2. – NSIWA and V independently predict the severity of coronary artery disease estimated using Syntax I. – NSIWA and PSP independently predict prognosis after revascularization estimated for surgery using Syntax 2. Abstract

Hypertension is a poor prognostic factor following STEMI, however the impact of blood pressure (BP) on the extent of microvascular damage in the setting of acute ischemia -reperfusion injury has not yet been fully evaluated and this evaluation is the purpose of the present study. Methods A cohort of patients with acute STEMI referred to primary percutaneus intervention were prospectively included. Angiographic analysis were performed according to standard clinical practice and decision regarding type of stent and antiplatelet drugs was left to the discretion of cardiologist. Information about high blood pressure needing drug therapy before acute event and also BP levels at catheterization laboratory during reperfusion procedures were assessed. All patients underwent cardiac MRI during the first week post reperfusion with a standardized protocol including 8–10 short axis slices in order to asses: area of myocardium at risk (AR) as an area of signal hyperintensity >2sd with respect to the remote one in TSE-T2 sequences, necrosis size (NS) as signal hyperintensity >5 SDs relative to the remote in IR-FGE sequences, microvascular obstruction (MVO) as signal hypointensity in the infarct core in IR-FGE sequences and intra-infarct haemorrhage (IIH) as an area with T2* Results 94 patients,mean age 62,SD 13, 67% males were included. Hypertensive patients were more likely to have diabetes mellitus 43 vs 28% (p0.05). Conclusions 1. Hypertension is associated with lower extent of necrosis and microvascular damage without differences in myocardial salvage and area of myocardium at risk. 2. Low BP levels at catheterization laboratory were associated with a significantly higher extent of microvascular damage, so maintaining systolic blood pressure over 120mmhg during reperfusion procedures should be advised. Abstract

Classically clinical evaluation of patients with aortic stenosis (AS) is made using clinical and echocardiographic parameters but recently new imaging techniques as cardiovascular magnetic resonance (CMR ) are being used increasingly in this clinical setting. We aim to assess utility and prognostic value of aortic valve area (AVA) by CMR in patients with AS regarding echocardiographic data. METHODS a retrospective cohort of patients with AS referred to CMR and cardiac echocardiography study for evaluation were included. Patients with known coronary artery disease or another significant valve disease were excluded. Clinical Follow up was performed to asses cardiovascular death, hospital admission or aortic valve replacement at the first year after imaging evaluation. Furthermore symptomatic status was assessed. Echocardiographic severity was estimated used peak and mean gradients . Additionally a CMR study including b-SSFPS for left ventricle ejection fraction (LVEF) quantification and phase-contrast sequences acquired at aortic valve plane to analyze aortic valve area (AVA) by planimetry was performed. RESULTS ninety eight patients, mean age 67+ 15 years, 52(58%) males, 23 (25%) with decreased left ventricular ejection fraction. A significant negative correlation was observed between AVA by CMR and echocardiographic gradients ( -0.49 for mean gradient and -0.58 for peak gradient). AVA was strongly associated to symptomatic status (AUROC curve 0.66, p ( OR:1.02 p = 0.01) and AVA (OR: 0.002 p = 0.001) were shown to be independent predictors of events. Conclusion 1.AVA by planimetry in phase-contrast CMR sequences is a valid tool to asses severity of aortic valve stenosis . 2 AVA was strongly associated to symptomatic status.3. Abstract

Intramyocardial hemorrhage (IMH) is considered a marker of tissue damage severity in patients with reperfused ST-segment elevation myocardial infarction (STEMI) and has been associated with a poor prognosis despite successful revascularization of the culprit artery . We aim to study the impact of cardiovascular risk factors and treatment strategies on the presence of IMH studied with T2* -w cardiovascular magnetic resonance (CMR) in this clinical setting METHODS A prospective observational study including patients with repefused STEMI who underwent an MRI during the first week post-revascularization were conducted . The presence of IMH was analyzed in ECG triggered T2 * w sequences as presence of hipointensity area . Clinical data including cardiovascular risk factors and treatment strategies at cath lab were studied. RESULTS 94 patients with reperfused STEMI were included. Demographic data are shown at the the table. No significant association was observed between the presence of IMH and the different treatment strategies used. All data were introduced in a multivariate model including presence of thrombus, total ischemia time and culprit coronary artery. The analysis showed previous infarction as an independent risk factor (OR: 6 p = 0.03, CI: 1.1-29) while history of hypertension (OR: 0.9, p = 0.04, CI: 0.1- 0.9) and systolic blood pressure showed independent protective effect (OR: 0.3 p = 0.02 IC: 0.9-0.99.) CONCLUSIONS. 1. Previous infarction was shown to be an independent risk factor for IMH . 2. Arterial hypertension and systolic blood pressure showed a protective effect. Abstract

The prognostic impact of coronary revascularization in patients with stable angina is limited being reserved nowadays for patients with symptoms or severe ischemia in imaging tests. We propose to study the usefulness of stress CMR to assess the utility of coronary revascularization in patients with stable angina. Methods A retrospective cohort study was conducted including consecutive patients with inducible ischemia in dypiridamol stress CMR. Patients with acute coronary syndrome or severe valvular disease were excluded. The decision to revascularize after MRI was made by the responsible medical team following the guidelines of clinical practice. The presence of cardiovascular events (death, admission or need for new revascularization) was analyzed during follow-up. The stress cardiac MR imaging protocol included a baseline functional study (b-SSFP sequences) and after administration of 0.84 mg / kg IV dipyridamole, perfusion sequences (b-SSFP) obtained during the administration of IV gadolinium contrast and F-GRE viability sequences. Clinical data such as the severity of coronary involvement, the type of coronary revascularization and if it was complete as well as data derived from the baseline functional study of ventricular volumes, segmental contractility score, transmurality and extension of necrosis, extension of hypoperfusion and also number of segments with severe ischemia considering as such those segments with transmural hypoperfusion or stress-induced systolic dysfunction. Results We included 424 patients of mean age of 65+ 10 years, 182 (43%) of them had known coronary artery disease and 93 (20%) of them had pevious percutaneous revascularization. Thirty-five (8%) had left main disease and 176 (41%) multivessel disease. Twenty five patients had normal stroke volume (SV) and depressed left ventricle ejection fraction (LVEF) and 60p had normal LVEF and low SV . Abstract

Background Measurement of left ventricular outflow tract (LVOT) diameter and area for estimation of aortic valve area (AVA) using transthoracic echocardiography (TTE) and the continuity equation assumes circular LVOT. The use of direct planimetric measurement of LVOT area by gated-CT can theoretically improve accuracy of AVA calculation. Purpose We aim to assess reproducibility of LVOT echo measurement and its correlation and agreement with Gated CT measurements. In the subgroup with aortic stenosis (AS) we secondarily assessed the potential change in AS severity using LVOT area by CT instead of TTE in the continuity equation. Methods We retrospectively studied 93 patients, 43 of whom with severe AS. LVOT Area was measured with 2D TTE by 2 expert echocardiographers and gated-CT by 2 expert radiologists; inter-reader agreement and inter-method (Echo vs gated CT) agreement and correlation were measured. Finally we used the measurement of CT scan in the continuity equation instead of TTE measurement to assess potential reclassification of AS severity. Results Mean age was 78±11. Table shows inter-reader and inter-method agreement and correlation. The correlation between 2 echocardiophers for LVOT measurements was good (rho = 0,77) although not perfect.
