1. Pharmacologic restraint use for children experiencing mental health crises in pediatric hospitals

Abstract

Children in mental health crises are increasingly admitted to children's hospitals awaiting inpatient psychiatric placement. During hospitalization, patients may exhibit acute agitation prompting pharmacologic restraint use.To determine hospital-level incidence and variation of pharmacologic restraint use among children admitted for mental health conditions in children's hospitals.We examined data for children (5 to ≤18 years) admitted to children's hospitals with a primary mental health condition from 2018 to 2020 using the Pediatric Health Information System database. Hospital rates of parenteral pharmacologic restraint use per 1000 mental health bed days were determined and compared after adjusting for patient-level and demographic factors. Cluster analysis (k-means) was used to group hospitals based on overall restraint use (rate quartiles) and drug class. Hospital-level factors for pharmacologic restraint use were compared.Of 29,834 included encounters, 3747 (12.6%) had pharmacologic restraint use. Adjusted hospital rates ranged from 35 to 389 pharmacologic restraint use days per 1000 mental health bed days with a mean of 175 (standard deviation: 72). Cluster analysis revealed three hospitals were high utilizers of all drug classes. No significant differences in pharmacologic restraint use were found in the hospital-level analysis.Children's hospitals demonstrate wide variation in pharmacologic restraint rates for mental health hospitalizations, with a 10-fold difference in adjusted rates between highest and lowest utilizers, and high overall utilizers order medications across all drug classes.

Abstract

The Soreq Applied Research Accelerator Facility (SARAF) in Israel will provide an intense source of fast neutrons and radioactive nuclei for neutron applications, to explore rare nuclear reactions and produce new types of radiopharmaceuticals for nuclear medicine. CEA Paris-Saclay is in charge of the design, construction and commissioning of this superconducting linear particle accelerator (linac) to produce the first particle beams in 2022. The superconducting linac is part of the SARAF accelerator and it will be equipped with twenty identical solenoid packages arranged in four 5-metre long cryomodules; they are located between the cavities and they ensure the beam control. The solenoid package is a complex mechanical system and it is designed to operate at cryogenic temperatures. It comprises the main solenoid coil with an integrated square flux density of 3.5 T2 m , two lateral shielding coils to reduce the fringe fields on the cavities as well as two pairs of steering coils. This ensemble is enclosed in a stainless steel helium vessel. In this study, we present the mechanical design of the first prototype of the solenoid package with a particular focus on the main coil, whereby the superconducting wire (Nb–Ti/Cu) is wound and cured on two extremity flanges made of epoxy reinforced with glass cloth (G-10). The modelling of the solenoid components is carried out using Finite Element Analysis (FEA) and the results of simulations are used to assess the structural integrity of the components. Moreover, an extensive literature review is conducted to collect the mechanical properties at cryogenic temperature of the composite materials used for the construction. Finally, the results of the experimental validation of the first solenoid prototype are presented. The experimental campaign is carried out using a cryostat designed on purpose to reproduce both the final operating temperature and magnetic field.

Abstract

International audience; The SEPAGE diagnostic will detect charged particles (electrons, protons, and ions) accelerated in the interaction of the PETAL (PETawatt Aquitaine Laser) laser with its targets on the LMJ (Laser MegaJoule)-PETAL laser facility. SEPAGE will be equipped with a proton-radiography front detector and two Thomson parabolas (TP), corresponding to different ranges of the particle energy spectra: Above 0.1 MeV for electrons and protons in the low-energy channel, with a separation capability between protons and 12C6+ up to 20 MeV proton energy and above 8 MeV for the high-energy channel, with a separation capability between protons and 12C6+ up to 200 MeV proton kinetic energy. This paper presents the calibration of the SEPAGE's low-energy channel TP at the Tandem facility of Orsay (France) with proton beams between 3 and 22 MeV and carbon-ion beams from 5.8 to 84 MeV. The magnetic and electric fields' integrals were determined with an accuracy of 10-3 by combining the deflections measured at different energies with different target thicknesses and materials, providing different in-target energy losses of the beam particles and hence different detected energies for given beam energies.

Reifarth et al: ; 12 págs.; 9 figs.; Open Access funded by Creative Commons Atribution Licence 3.0 ; Nuclear Physics in Astrophysics VI (NPA6), The nucleosynthesis of elements beyond iron is dominated by neutron captures in the s and r processes. However, 32 stable, proton-rich isotopes cannot be formed during those processes, because they are shielded from the s-process ow and r-process -decay chains. These nuclei are attributed to the p and rp process. For all those processes, current research in nuclear astrophysics addresses the need for more precise reaction data involving radioactive isotopes. Depending on the particular reaction, direct or inverse kinematics, forward or time-reversed direction are investigated to determine or at least to constrain the desired reaction cross sections. The Facility for Antiproton and Ion Research (FAIR) will oer unique, unprecedented opportunities to investigate many of the important reactions. 6. Temperature Distribution During the Cooling Down of the R3B Magnet Cold Mass

Abstract

The cold mass of the R3B magnet consists of a set of six trapezoidal racetrack coils. There are two main coils and two pairs of lateral coils which are connected in series in a butterfly-like shape. The coils are imbedded in the coil-casings and covers which are made of aluminum alloy 5083. There are four coil-casings (two main coil-casings and two lateral coil-casings) and linking components between the coil casings. The cold mass will be placed in a large cryostat. The purpose of the cooling system is to get the R3B-GLAD coils at the proper temperature for running operations under magnetic field. The coils are indirectly cooled. The cool down is ensured by gas helium forced flow. As the coils are imbedded in 5083 (casings + covers) it has been decided to cut off heat loads, like thermal radiation and conduction from room temperature to cold mass, before they can reach the coils. Heat exchangers are thus glued on the casings and covers. There are 20 heat exchanger tubes glued in square grooves in covers and casings over a length of 2 m. In this paper the temperature distribution over all the coils and their support structure during the steady state and transient cooling down process are presented. The orthotropic properties of the thermal conductivity of the winding are taken into account. The non linearity of the thermal properties of the coils and materials constituting the support structure is also considered.

Abstract

The Cu-NbTi conductor for the R3B-GLAD spectrometer magnet was manufactured. In order to correctly know the thermo-mechanical behavior of this Rutherford cable after insulation and then impregnation, some impregnated cable stacks have been submitted to mechanical tests at room and cryogenic temperatures. The analysis of these measurements will be presented and compared to a theoretical approach. A half-scale mock-up of a real coil was built with the final conductor. After its cooling down to 80 K, the measurements have given the thermal shrinkage coefficients of this coil in the three directions. They confirmed the really orthotropic behavior of these composite coils. The method and the analysis of these measurements are presented.

Abstract

The R3B-GLAD (GSI Large Acceptance superconducting Dipole) magnet provides the magnetic field needed for the R3B (Reaction studies with Radioactive Relativistic Beams) experiment which will be implemented on the future FAIR Facility (Facility for Antiproton and Ion Research). There are six trapezoidal racetrack coils for the R3B-GLAD magnet. Two main coils and four lateral coils are connected in series in a butterfly-like shape. The total weight of the six coils is 5.2 t. The cold mass assembly consists of coils, coil-casings, structural linking components and other non-structural components. The cold mass assembly weighs 22 t with envelope dimensions of 3.1 m (L) × 4.6 m (W) × 2.8 m (H). It is supported by the three cold to warm cryogenic supports. The magnet configuration was finalized in June 2008. The final cold mass design was accomplished end of July 2008. The manufacturing of the cold mass assembly has been launched since January 2009. The cold mass will be ready for test in the test station at CEA Saclay by the end of 2010. This paper gives an overview on the final design of the R3BGLAD cold mass assembly and manufacturing status.

Abstract

The R3B-Glad superconducting Magnet is a large acceptance dipole, dedicated to the analysis of Reactions with Relativistic Radioactive ions Beams. It takes part in the FAIR Project at GSI. As the superconducting NbTi Rutherford cable was under production, detailed studies of the mechanical structure (with both simulation and experiment on a half-scale mock-up) led to revise the magnet design and to abandon the grading of the coils in three stages. Due to the large magnetic forces (up to 400 tons/m), the maximum shear stress level of 20 MPa was impossible to meet in the coils. The main reasons consist in the orthotropic thermo-mechanical behavior of the coils together with the large differential thermal shrinkage between the Cu stabilized coils and their Al alloy casings. Indeed after several studies of different mechanical designs, we decided to simplify the magnet in order to cope with these difficulties. One innovative point is that the coils are not blocked at room temperature, but only at 4.5 K. This paper presents the magnetic calculations of this active shielded magnet, and shows how the new design features meet the specifications. Currently, the 22 tons magnet cold mass, i.e. the 6 coils and their integration in the casings, is ordered and under construction. Meanwhile, the design of the magnet cryostat has evolved into a shape of elliptical cylinder with a lateral satellite. The total weight is expected to be around 50 tons.

Abstract

The R3B-Glad superconducting Magnet provides the field required for a large acceptance spectrometer, dedicated to the analysis of Reactions with Relativistic Radioactive ions Beams. In the framework of the FAIR Project to GSI and within NUSTAR physics program, the technical study started in 2006, and the engineering design is undertaken. One main feature of this butterfly-like magnet with graded, tilted and trapezoidal racetrack coils is the active shielding. It makes it possible to decreasing the field by two orders of magnitude within a 1.2 m length, despite the large opening on the outlet side of the magnet (around 0.8 square meters). The fringe field is lower than 20 mT in the target area beside the entry, while the main field is larger than 2 teslas, out of 2 m length. The other principal characteristics are as follows: first, a high level of magnetic forces (300 to 400 tons per meter), with little place to block the coils, requiring a very specific mechanical structure; then, the magnet protection system that is based on an external dump resistor, coupled to a strong quenchback effect, to prevent any damage of the coils which could be caused by the 24 MJ of stored energy; lastly, the indirect cooling of the cold mass with a two-phase helium thermosiphon. The overall size of the conical cryostat will be around 3.5 m long, 3.8 m high and 7 m broad.

Abstract

The R3B-GLAD magnet is a large acceptance superconducting dipole magnet. It provides the magnetic field needed for the R3B (Reaction studies with Radioactive Relativistic Beams) experiment which will be implemented on the future FAIR Facility (Facility for Antiproton and Ion Research). The cold mass structure of the magnet is designed to hold the six superconducting coils in position and to resist high level magnetic forces. The cold mass assembly consists of coils, coil casings and linking components. It is supported by the three cold to warm supports. It weighs about 20 tons with envelope dimensions of 3 rm(rL) X 5 rm(rW) X 3 rm(rH). Engineering design of the cold mass structure has been carried out through extensive finite element analyses. This paper gives an overview on the mechanical behavior of the cold mass assembly during energizing phase and cool-down phase.

Abstract

Comparison between active and passive shielding magnets for a large acceptance superconducting dipole magnet has been carried out. The two magnet designs have been studied to answer to the new requirements of Nuclear Physics experiments in order to get a momentum resolution of up to 10-3 with 1 GeV/nucleon heavy-ion beams: A field integral of about 5Tmiddotm]], a very low fringe field, especially in the target region about 1 meter upstream from the magnet entrance, and a large acceptance defined by a maximum bending angle of 40deg together with an opening horizontal and vertical angle of +/-80 mrad for the neutrons and the charged fragments. To fulfil these requirements, we find that the iron shielding magnet needs a heavier mass, about 300 tons, compared to the active one, even though the total device volume is not so different (4 meter diameter). With iron the coil volume is divided by two (compared with the same current density value) but the stored energy decreases only of less than 20%. The trajectory reconstruction algorithms give results within the specifications for the kinematical reconstruction at the target in both cases, either with iron pole and yoke or with innovative shielding coils, as shown by the given figures of merit. The conclusion of this study is that we find here the limits of a traditional iron design and that the physics requirements open the way to the choice of an active shielding design

Abstract

The GSI projectile fragment separator FRS has demonstrated with many pioneering experiments the research potential of in-flight separators at relativistic energies. Although the present facility has contributed much to the progress in the field of nuclear structure physics, major improvements are desirable in the future. The characteristics of the proposed next-generation facility at GSI, the Super-FRS, will be presented and compared to other projects. The Super-FRS is a large-acceptance superconducting fragment separator followed by different experimental branches including a combination with a new storage-cooler ring system. This system consists of a collector ring (CR) and a new experimental storage ring (NESR) which allow precision mass and lifetime measurements as well as in-ring reaction studies. The NESR can be operated in combination with an electron ring to measure electron scattering with exotic nuclei. This electron heavy-ion collider will open up new fields for nuclear structure research.

Abstract

The paper describes the preparation of the short-pulse high-energy laser PETawatt Aquitaine Laser (PETAL), which will be coupled to the Laser Megajoule (LMJ) laser of CEA. The LMJ/PETAL facility will be opened for the academic access of European researchers. In parallel, diagnostics are being developed within the PETAL + project and many physical problems are being addressed ranging from the study of the problems of radiation generation and activation issues to the problem of generation of large electromagnetic pulses.

Abstract

The R3B-Glad magnet is part of a large acceptance spectrometer devoted to the physics of exotic nuclei, under construction at CEA Saclay (France) for the future FAIR facility at GSI (Darmstadt, Germany). The R3B-Glad project is in its final phase where the magnet cold mass is achieved and will be tested at 4.6 K and nominal current in the fall of 2011. We present the results of the R&D tests programs that were implemented to answer some issues raised after the technical design review. First, this superconducting dipole will be cooled down by indirect cooling, and its 22 tons cold mass maintained at 4.6 K by means of a two-phase helium thermosiphon loop. An experimental model set-up has been specifically built to demonstrate the cooling efficiency despite the presence of large quasi-horizontal sections in the circuit. Then the superconducting Rutherford cable joints, required between the 26 coil double pancakes, were designed and tested to minimize their electrical resistance. Finally a reduced scale mock-up of a coil in its casing, with a general design similar to the final magnet, was tested at low temperature to validate the indirect cooling and the mechanical blocking system of the coil in its casing by differential thermal shrinkage. This test was performed at a higher current (8800 A) than the nominal one (3600 A) to reach the same value of temperature margin before quenching, together with similar high level of magnetic forces on the coil (100 to 200 t/m in final magnet).

Abstract

The R3B-Glad magnet is part of a large acceptance spectrometer devoted to the physics of exotic nuclei, under construction at CEA Saclay (France) for the present GSI and the future Fair facilities (Darmstadt, Germany). In this paper is presented the fabrication of the cold mass (22 t), divided into three batches (coil manufacture, machining of coil casings, and integration of the coils inside their casings). The work began in January 2009 and ended in December 2010. A real size prototype coil, was developed to validate the winding process, with double pancakes assembled in a stacking tool, and the procedure for impregnating coils, inserted into a mold. After impregnation of the prototype and the first coil, a failing (short circuit between turns) was detected in both coils by electrical testing. The repair of various defects has been a success. Insulation reinforcements were performed on the following coils to avoid any further electrical default. The whole set of coils was wound and impregnated in 2010 (6 coils, about 3 meter long each). The casings are made of aluminum alloy 5083 (with the exception of a few pins, screws and bolts made of stainless steel) and machined from single blocks without welding. All parts were machined in two stages. The first step was a premachining to perform a blank assembly of the whole cold mass (performed in February 2010). The final machining was performed according to the actual dimensions of the manufactured coils, in order to get a perfect fit of the coils within their casings. The integration of the coils inside their casings (including shims setting, adjusting of pushing systems, bonding of drains and thermal sensors) occurred in autumn 2010. The set of coils in their casings was delivered to Saclay in December 2010.

Abstract

The R3B-Glad magnet cold mass has been tested in the test facility at CEA Saclay previously used for testing W7X coils. This superconducting dipole has been cooled down by indirect cooling, and its 21 tons were maintained at around 4.8 K by means of helium thermosiphon. For this, the facility was adapted and connected to the cold mass to approximate actual conditions in its final configuration at GSI (Darmstadt, Germany). The magnet cooling down as well as the two-slope current ramping up to the nominal current of 3584 A was successfully achieved in December 2013. This paper presents the cryogenic distribution process, and shows the relevant results.

The first accelerated beam was served to physicists in 1987 fall.Since that time, the source has been running 1500 hours as a preinjector and nuclear physics experiments, distributed in four runs of two weeks, have been performed according to the SATURNE schedule.

Abstract

The SEPAGE diagnostic will detect charged particles (electrons, protons, and ions) accelerated in the interaction of the PETAL (PETawatt Aquitaine Laser) laser with its targets on the LMJ (Laser MegaJoule)-PETAL laser facility. SEPAGE will be equipped with a proton-radiography front detector and two Thomson parabolas (TP), corresponding to different ranges of the particle energy spectra: Above 0.1 MeV for electrons and protons in the low-energy channel, with a separation capability between protons and 12 C 6+ up to 20 MeV proton energy and above 8 MeV for the high-energy channel, with a separation capability between protons and 12 C 6+ up to 200 MeV proton kinetic energy. This paper presents the calibration of the SEPAGE's low-energy channel TP at the Tandem facility of Orsay (France) with proton beams between 3 and 22 MeV and carbon-ion beams from 5.8 to 84 MeV. The magnetic and electric fields' integrals were determined with an accuracy of 10 -3 by combining the deflections measured at different energies with different target thicknesses and materials, providing different in-target energy losses of the beam particles and hence different detected energies for given beam energies.

Abstract

This paper aims to analyze the representations of premarital sexuality among young people in a context where they are strongly encouraged not to engage in premarital sexual relations. This link between prevention messages, representations and behaviours has been studied among a student population in Madagascar. The analysis is based on a socio-demographic survey conducted in 2006. Results show that young men and women share the ideal of abstinence before marriage. The promotion of premarital sexual abstinence within AIDS programs reinforces gender differences. It is mainly young women who are urged to be abstinent, whereas young men have the means--condom use--to transgress the taboo of premarital sex and protect themselves against health risks. Thus, young men have a dual strategy. On the one hand, they attach great importance to premarital abstinence with their future wife; on the other hand, they can have sex with occasional partners and are then protected by condoms.

Abstract

In Madagascar, voluntary termination of pregnancy is totally forbidden. However, many women resort to it, most especially in the capital, Antananarivo. These secret abortions are carried out not only by traditional birth attendants but also by private medical practitioners. This article presents the results of a research conducted on 193 women interviewed in 2005 when they came to consult a doctor and seek abortion. This first survey was complemented by qualitative data collected from students who had already had an abortion, but most especially the young ones, spinsters and those still in school. The latter try to delay the time of first childbirth in order to complete their studies, to find a first job and to get married. The young girls have a very limited access to contraception and their contraceptive choice yet depends on the willingness of their sexual partners

Abstract

In committing itself to achieve the Millennium Development Goals, Madagascar committed itself to reducing maternal and infant mortality. Teenagers must be a target for activities to improve maternal and child health, for analyses of survey data show clearly that teenager pregnancies present a risk to the health of both young mothers and their children; in particular, both have a higher risk of death than older mothers and their children. These differences can be explained both by biological factors and socioeconomic variables (place of residence, educational level, etc). The issue of abortion is also important, for it is an important cause of maternal mortality, especially among young girls. Beyond the actions of prevention, post-abortion care could help to reduce the complications of abortions and thus mortality among women.
