Purpose: The amount of coronary artery calcification (CAC) is a strong and independent predictor of cardiovascular disease (CVD) events. In clinical practice, CAC is manually identified and automatically quantified in cardiac CT using commercially available software. This is a tedious and time-consuming process in large-scale studies. Therefore, a number of automatic methods that require no interaction and semiautomatic methods that require very limited interaction for the identification of CAC in cardiac CT have been proposed. Thus far, a comparison of their performance has been lacking. The objective of this study was to perform an independent evaluation of (semi)automatic methods for CAC scoring in cardiac CT using a publicly available standardized framework. Methods: Cardiac CT exams of 72 patients distributed over four CVD risk categories were provided for (semi)automatic CAC scoring. Each exam consisted of a noncontrast-enhanced calcium scoring CT (CSCT) and a corresponding coronary CT angiography (CCTA) scan. The exams were acquired in four different hospitals using state-of-the-art equipment from four major CT scanner vendors. The data were divided into 32 training exams and 40 test exams. A reference standard for CAC in CSCT was defined by consensus of two experts following a clinical protocol. The framework organizers evaluated the performance of (semi)automatic methods on test CSCT scans, per lesion, artery, and patient. Results: Five (semi)automatic methods were evaluated. Four methods used both CSCT and CCTA to identify CAC, and one method used only CSCT. The evaluated methods correctly detected between 52% and 94% of CAC lesions with positive predictive values between 65% and 96%. Lesions in distal coronary arteries were most commonly missed and aortic calcifications close to the coronary ostia were the most common false positive errors. The majority (between 88% and 98%) of correctly identified CAC lesions were assigned to the correct artery. Linearly weighted Cohens kappa for patient CVD risk categorization by the evaluated methods ranged from 0.80 to 1.00. Conclusions: A publicly available standardized framework for the evaluation of (semi)automatic methods for CAC identification in cardiac CT is described. An evaluation of five (semi)automatic methods within this framework shows that automatic per patient CVD risk categorization is feasible. CAC lesions at ambiguous locations such as the coronary ostia remain challenging, but their detection had limited impact on CVD risk determination.

Differences in scanning parameters or modalities can complicate image analysis based on supervised classification. This paper presents two representation learning approaches, based on autoencoders, that address this problem by learning representations that are similar across domains. Both approaches use, next to the data representation objective, a similarity objective to minimise the difference between representations of corresponding patches from each domain. We evaluated the methods in transfer learning experiments on multi-modal brain MRI data and on synthetic data. After transforming training and test data from different modalities to the common representations learned by our methods, we trained classifiers for each of pair of modalities. We found that adding the similarity term to the standard objective can produce representations that are more similar and can give a higher accuracy in these cross-modality classification experiments.

Rationale and Objectives The aim of this study was to assess (1) automated analysis methods versus manual evaluation by human experts of three-dimensional proton magnetic resonance spectroscopic imaging (MRSI) data from patients with prostate cancer and (2) the contribution of spatial information to decision making. Materials and Methods Three-dimensional proton MRSI was applied at 1.5 T. MRSI data from 10 patients with histologically proven prostate adenocarcinoma, scheduled either for prostatectomy or intensity-modulated radiation therapy, were evaluated. First, two readers manually labeled spectra using spatial information to identify the localization of spectra and neighborhood information, establishing the reference set of this study. Then, spectra were labeled again manually in a blinded and randomized manner and evaluated automatically using software that applied spectral line fitting as well as pattern recognition routines. Statistical analysis of the results of the different approaches was performed. Results Altogether, 1018 spectra were evaluable by all methods. Numbers of evaluable spectra differed significantly depending on patient and evaluation method. Compared to automated analysis, the readers made rather binary decisions, using information from neighboring spectra in ambiguous cases, when evaluating MRSI data as a whole. Differences between anatomically blinded and unblinded evaluation were larger than differences between evaluations using blinded data and automated techniques. Conclusions An automated approach, which evaluates each spectrum individually, can be as good as an anatomy-blinded human reader. Spatial information is routinely used by human experts to support their final decisions. Automated procedures that consider anatomic information for spectral evaluation will enhance the diagnostic impact of MRSI of the human prostate.

We propose a Bayesian smoothness prior in the spectral fitting of MRS images which can be used in addition to commonly employed prior knowledge. By combining a frequency-domain model for the free induction decay with a Gaussian Markov random field prior, a new optimization objective is derived that encourages smooth parameter maps. Using a particular parameterization of the prior, smooth damping, frequency and phase maps can be obtained whilst preserving sharp spatial features in the amplitude map. A Monte Carlo study based on two sets of simulated data demonstrates that the variance of the estimated parameter maps can be reduced considerably, even below the Cramer-Rao lower bound, when using spatial prior knowledge. Long-TE 1H MRSI at 1.5 T of a patient with a brain tumor shows that the use of the spatial prior resolves the overlapping peaks of choline and creatine when a single voxel method fails to do so. Improved and detailed metabolic maps can be derived from high-spatial-resolution, short-TE 1H MRSI at 3 T. Finally, the evaluation of four series of long-TE brain MRSI data with various signal-to-noise ratios shows the general benefit of the proposed approach.

Besides the diagnostic evaluation of a spectrum, the assessment of its quality and a check for plausibility of its information remains a highly interactive and thus time-consuming process in MR spectroscopic imaging (MRSI) data analysis. In the automation of this quality control, a score is proposed that is obtained by training a machine learning classifier on a representative set of spectra that have previously been classified by experts into evaluable data and nonevaluable data. In the first quantitative evaluation of different quality measures on a test set of 45,312 long echo time spectra in the diagnosis of brain tumor, the proposed pattern recognition (using the random forest classifier) separated high- and low-quality spectra comparable to the human operator (area-under-the-curve of the receiver-operator-characteristic, AUC >0.993), and performed better than decision rules based on the signal-to-noise-ratio (AUC

International audience; Knowledge of left atrial (LA) anatomy is important for atrial fibrillation ablation guidance, fibrosis quantification and biophysical modelling. Segmentation of the LA from Magnetic Resonance Imaging (MRI) and Computed Tomography (CT) images is a complex problem. This manuscript presents a benchmark to evaluate algorithms that address LA segmentation. The datasets, ground truth and evaluation code have been made publicly available through the http://www.cardiacatlas.org website. This manuscript also reports the results of the Left Atrial Segmentation Challenge (LASC) carried out at the STACOM'13 workshop, in conjunction with MICCAI'13. Thirty CT and 30 MRI datasets were provided to participants for segmentation. Each participant segmented the LA including a short part of the LA appendage trunk and proximal sections of the pulmonary veins (PVs). We present results for nine algorithms for CT and eight algorithms for MRI. Results showed that methodologies combining statistical models with region growing approaches were the most appropriate to handle the proposed task. The ground truth and automatic segmentations were standardised to reduce the influence of inconsistently defined regions (e.g., mitral plane, PVs end points, LA appendage). This standardisation framework, which is a contribution of this work, can be used to label and further analyse anatomical regions of the LA. By performing the standardisation directly on the left atrial surface, we can process multiple input data, including meshes exported from different electroanatomical mapping systems

Despite its diagnostic value and technological availability, (1)H NMR spectroscopic imaging (MRSI) has not found its way into clinical routine yet. Prerequisite for the clinical application is an automated and reliable method for the diagnostic evaluation of MRS images. In the present paper, different approaches to the estimation of tumor probability from MRSI in the prostate are assessed. Two approaches to feature extraction are compared: quantification (VARPRO, AMARES, QUEST) and subspace methods on spectral patterns (principal components, independent components, nonnegative matrix factorization, partial least squares). Linear as well as nonlinear classifiers (support vector machines, Gaussian processes, random forests) are applied and discussed. Quantification-based approaches are much more sensitive to the choice and parameterization of the quantification algorithm than to the choice of the classifier. Furthermore, linear methods based on magnitude spectra easily achieve equal performance and also allow for biochemical interpretation in combination with subspace methods. Nonlinear methods operating directly on magnitude spectra achieve the best results but are less transparent than the linear methods.

The 2013 workshop in medical computer vision (MCV) took place in Nagoya, Japan in connection with MICCAI (Medical Image Computing for Computer Assisted Intervention). It is the third MICCAI MCV workshop after 2011 and 2012 also related to a similar workshop held at CVPR (Computer Vision and Pattern Recognition) in 2012. The workshop aims at exploring the use of modern computer vision technology in tasks such as automatic segmentation and registration, localisation of anatomical features and detection of anomalies. It emphasises questions of harvesting, organising and learning from large–scale medical imaging data sets and general–purpose automatic understanding of medical images. We are especially interested in modern, scalable and efficient algorithms which generalise well to previously unseen images. The strong participation of over 70 persons in the workshop shows the importance and timeliness of medical computer vision as a research field. This overview article describes the papers presented in the workshop as either oral presentations or short presentations and posters. It also describes the invited talks and the results of the VISCERAL session in the workshop on the use of big data in medical imaging.

Recently conducted clinical studies prove the utility of Coronary Computed Tomography Angiography (CCTA) as a viable alternative to invasive angiography for the detection of Coronary Artery Disease (CAD). This has lead to the development of several algorithms for automatic detection and grading of coronary stenoses. However, most of these methods focus on detecting calcified plaques only. A few methods that can also detect and grade non-calcified plaques require substantial user involvement. In this paper, we propose a fast and fully automatic system that is capable of detecting, grading and classifying coronary stenoses in CCTA caused by all types of plaques. We propose a four-step approach including a learning-based centerline verification step and a lumen cross-section estimation step using random regression forests. We show state-of-the-art performance of our method in experiments conducted on a set of 229 CCTA volumes. With an average processing time of 1.8 seconds per case after centerline extraction, our method is significantly faster than competing approaches.

Examinations of the spinal column with both, Magnetic Resonance (MR) imaging and Computed Tomography (CT), often require a precise three-dimensional positioning, angulation and labeling of the spinal disks and the vertebrae. A fully automatic and robust approach is a prerequisite for an automated scan alignment as well as for the segmentation and analysis of spinal disks and vertebral bodies in Computer Aided Diagnosis (CAD) applications. In this article, we present a novel method that combines Marginal Space Learning (MSL), a recently introduced concept for efficient discriminative object detection, with a generative anatomical network that incorporates relative pose information for the detection of multiple objects. It is used to simultaneously detect and label the spinal disks. While a novel iterative version of MSL is used to quickly generate candidate detections comprising position, orientation, and scale of the disks with high sensitivity, the anatomical network selects the most likely candidates using a learned prior on the individual nine dimensional transformation spaces. Finally, we propose an optional case-adaptive segmentation approach that allows to segment the spinal disks and vertebrae in MR and CT respectively. Since the proposed approaches are learning-based, they can be trained for MR or CT alike. Experimental results based on 42 MR and 30 CT volumes show that our system not only achieves superior accuracy but also is among the fastest systems of its kind in the literature. On the MR data set the spinal disks of a whole spine are detected in 11.5s on average with 98.6% sensitivity and 0.073 false positive detections per volume. On the CT data a comparable sensitivity of 98.0% with 0.267 false positives is achieved. Detected disks are localized with an average position error of 2.4 mm/3.2 mm and angular error of 3.9°/4.5° in MR/CT, which is close to the employed hypothesis resolution of 2.1 mm and 3.3°.

Background Regularized regression methods such as principal component or partial least squares regression perform well in learning tasks on high dimensional spectral data, but cannot explicitly eliminate irrelevant features. The random forest classifier with its associated Gini feature importance, on the other hand, allows for an explicit feature elimination, but may not be optimally adapted to spectral data due to the topology of its constituent classification trees which are based on orthogonal splits in feature space. Results We propose to combine the best of both approaches, and evaluated the joint use of a feature selection based on a recursive feature elimination using the Gini importance of random forests' together with regularized classification methods on spectral data sets from medical diagnostics, chemotaxonomy, biomedical analytics, food science, and synthetically modified spectral data. Here, a feature selection using the Gini feature importance with a regularized classification by discriminant partial least squares regression performed as well as or better than a filtering according to different univariate statistical tests, or using regression coefficients in a backward feature elimination. It outperformed the direct application of the random forest classifier, or the direct application of the regularized classifiers on the full set of features. Conclusion The Gini importance of the random forest provided superior means for measuring feature relevance on spectral data, but – on an optimal subset of features – the regularized classifiers might be preferable over the random forest classifier, in spite of their limitation to model linear dependencies only. A feature selection based on Gini importance, however, may precede a regularized linear classification to identify this optimal subset of features, and to earn a double benefit of both dimensionality reduction and the elimination of noise from the classification task.
