Background: The extraction of the mandibular third molar is one of the most frequent intervention in oral surgery. A common indication for wisdom tooth extraction is represented by pericoronitis, which can determine discomfort and pain in patients. The present study aimed to evaluate the impact of patients’ quality of life by comparing a surgical approach with a periodontal approach. Methods: We evaluated 82 patients diagnosed with pericoronitis that occurred at the third molar site. In total, 41 of them received a periodontal treatment and 41 were treated by extraction. The quality of life (QoL) of the patients was assessed by using the Oral Health Impact Profile-14 (OHIP-14) index. Results: A total of 82 patients were included in the study and were followed up for 6 months. Of the patients, 41 received a periodontal treatment and 41 underwent surgical extraction. At the baseline, the OHIP-14 scores of the surgical group were higher (19.71, SD 9.90) than the periodontal group (14.41, SD 8.71). At 1 week, there was a reduction in terms of OHIP-14 in both groups, but the periodontal group showed lower values (12.3, SD 8.11). Long-term follow-up showed a reduction of the OHIP-14 values, with a difference in favor of the surgical group (0.10, SD 0.45). However, there was a reduction in OHIP-14 scores in both groups. Conclusion: Although the periodontal treatment offered a rapid improvement in terms of quality of life during the first week after the treatment, after 1 month and 6 months, the extraction of the mandibular third molar extraction remained the best treatment, removing the occurrence of re-inflammation of the site.

Multiple-beam holography has been widely used for the realization of photonic quasicrystals with high rotational symmetries not achievable by conventional periodic crystals. Accurate control of the properties of the interfering beams is necessary to provide photonic band-gap structures. Here we show, by finite difference time domain (FDTD) simulations of the transmission spectra of 8-fold quasiperiodic structures, how the geometric tiling of the structure affects the presence and properties of the photonic band-gap for low refractive index contrasts. Hence, we show an interesting approach to the fabrication of photonic quasicrystals based on the use of a programmable spatial light modulator encoding computer-generated holograms, that permits an accurate control of the writing pattern with almost no limitations in the pattern design. Using this single-beam technique we fabricated quasiperiodic structures with high rotational symmetries and different geometries of the tiling, demonstrating the great versatility of our technique. Background: Previous studies have documented a negative impact of the COVID-19 pandemic on emergent percutaneous treatment of patients with ST-segment elevation myocardial infarction (STEMI), but few have examined recovery of healthcare systems in restoring prepandemic STEMI care., Methods: Retrospective analysis was performed of data from 789 patients with STEMI from a large tertiary medical center treated with percutaneous coronary intervention between January 1, 2019, and December 31, 2021., Results: For patients with STEMI presenting to the emergency department, median time from door to balloon was 37 minutes in 2019, 53 minutes in 2020, and 48 minutes in 2021 (P < .001), whereas median time from first medical contact to device changed from 70 to 82 to 75 minutes, respectively (P = .002). Treatment time changes in 2020 and 2021 correlated with median emergency department evaluation time (30 to 41 to 22 minutes, respectively; P = .001) but not median catheterization laboratory revascularization time. For transfer patients, median time from first medical contact to device changed from 110 to 133 to 118 minutes, respectively (P = .005). In 2020 and 2021, patients with STEMI had greater late presentation (P = .028) and late mechanical complications (P = .021), with nonsignificant increases in yearly in-hospital mortality (3.6% to 5.2% to 6.4%; P = .352)., Conclusion: COVID-19 was associated with worsening STEMI treatment times and outcomes in 2020. Objectives: The purpose of this study is to provide expert consensus recommendations to establish a global ultrasound curriculum for undergraduate medical students., Methods: 64 multi-disciplinary ultrasound experts from 16 countries, 50 multi-disciplinary ultrasound consultants, and 21 medical students and residents contributed to these recommendations. A modified Delphi consensus method was used that included a systematic literature search, evaluation of the quality of literature by the GRADE system, and the RAND appropriateness method for panel judgment and consensus decisions. The process included four in-person international discussion sessions and two rounds of online voting., Results: A total of 332 consensus conference statements in four curricular domains were considered: (1) curricular scope (4 statements), (2) curricular rationale (10 statements), (3) curricular characteristics (14 statements), and (4) curricular content (304 statements). Of these 332 statements, 145 were recommended, 126 were strongly recommended, and 61 were not recommended. Important aspects of an undergraduate ultrasound curriculum identified include curricular integration across the basic and clinical sciences and a competency and entrustable professional activity-based model. The curriculum should form the foundation of a life-long continuum of ultrasound education that prepares students for advanced training and patient care. In addition, the curriculum should complement and support the medical school curriculum as a whole with enhanced understanding of anatomy, physiology, pathophysiological processes and clinical practice without displacing other important undergraduate learning. The content of the curriculum should be appropriate for the medical student level of training, evidence and expert opinion based, and include ongoing collaborative research and development to ensure optimum educational value and patient care., Conclusions: The international consensus conference has provided the first comprehensive document of recommendations for a basic ultrasound curriculum. The document reflects the opinion of a diverse and representative group of international expert ultrasound practitioners, educators, and learners. These recommendations can standardize undergraduate medical student ultrasound education while serving as a basis for additional research in medical education and the application of ultrasound in clinical practice., (© 2022. Background: Chronic steroid therapy is associated with higher vascular complication rates in patients undergoing transcatheter aortic valve replacement (TAVR). The effect of corticosteroids on aortic annular complications has not been directly assessed in this population., Methods: A retrospective analysis of 1095 patients undergoing transfemoral TAVR was performed. Patients treated with chronic steroids at the time of the procedure (n = 99) were compared with those who received no steroids (n = 992). The primary outcome included a composite of aortic annular complications, defined as a combination of aortic annular rupture, aortic dissection/perforation, and left ventricular perforation., Results: The primary outcome was significantly higher in the steroid group (4.0% vs 0.5%; P<.01). This finding was primarily driven by higher rates of acute annular rupture in the steroid group (2.0% vs 0.2%; P=.04). Steroid use was associated with higher rates of intraoperative cardiac arrest (5.1% vs 1.5%; P=.03), device capture/retrieval (4.0% vs 0.8%; P=.01), and emergent conversion to open heart surgery (4.0% vs 0.6%; P<.01). There were no differences with respect to in-hospital mortality, stroke, myocardial infarction, need for permanent pacemaker, bleeding complications, minor vascular complications, hospital length of stay, hospital 30-day readmission, or 30-day echocardiographic findings. Additionally, within the steroid group, there were no significant differences between balloon-expandable vs self-expanding TAVR prostheses with respect to composite aortic annular complications., Conclusion: Chronic steroid therapy increases the risk of aortic annular complications in patients undergoing TAVR, with detrimental consequences including intraoperative cardiac arrest and conversion to open heart surgery. Steroid use should be considered in patient selection and determination of procedural technique for TAVR.

Space-variant polarization patterns present in the transverse mode of optical beams highlight disclination patterns of polarization about a singularity, often a C-point. These patterns are important for understanding rotational dislocations and for characterizing complex polarization patterns. Liquid-crystal devices known as q-plates have been used to produce two of the three types of disclination patterns in optical beams: lemons and stars. Here we report the production of the third type of disclination, which is asymmetric, known as the monstar. We do so with elliptically-symmetric q-plates. We present theory and measurements, and find excellent agreement between the two.

Topological insulators are fascinating states of matter exhibiting protected edge states and robust quantized features in their bulk. Here we propose and validate experimentally a method to detect topological properties in the bulk of one-dimensional chiral systems. We first introduce the mean chiral displacement, an observable that rapidly approaches a value proportional to the Zak phase during the free evolution of the system. Then we measure the Zak phase in a photonic quantum walk of twisted photons, by observing the mean chiral displacement in its bulk. Next, we measure the Zak phase in an alternative, inequivalent timeframe and combine the two windings to characterize the full phase diagram of this Floquet system. Finally, we prove the robustness of the measure by introducing dynamical disorder in the system. This detection method is extremely general and readily applicable to all present one-dimensional platforms simulating static or Floquet chiral systems.

We here demonstrate on both theoretical and experimental bases a method to recover the topological structure of a monochromatic optical field that has suffered diffuse transmission. This method consists of two steps: first, a linearly polarized sample beam is mixed with a coaxial Gaussian beam in orthogonal polarization states resulting in a Poincaré beam; second, a polarization-related spatial correlation function is considered and measured for the overall speckle field arising by optical diffusion. The singularities of the sample beam turn out to be imaged into the correlation function of the vector speckle field.

Optical singularities manifesting at the center of vector vortex beams are unstable, since their topological charge is higher than the lowest value permitted by Maxwell's equations. Inspired by conceptually similar phenomena occurring in the polarization pattern characterizing the skylight, we show how perturbations that break the symmetry of radially symmetric vector beams lead to the formation of a pair of fundamental and stable singularities, i.e. points of circular polarization. We prepare a superposition of a radial (or azimuthal) vector beam and a uniformly linearly polarized Gaussian beam; by varying the amplitudes of the two fields, we control the formation of pairs of these singular points and their spatial separation. We complete this study by applying the same analysis to vector vortex beams with higher topological charges, and by investigating the features that arise when increasing the intensity of the Gaussian term. Our results can find application in the context of singularimetry, where weak fields are measured by considering them as perturbations of unstable optical beams.

Many phenomena in solid-state physics can be understood in terms of their topological properties. Recently, controlled protocols of quantum walk (QW) are proving to be effective simulators of such phenomena. Here we report the realization of a photonic QW showing both the trivial and the non-trivial topologies associated with chiral symmetry in one-dimensional (1D) periodic systems. We find that the probability distribution moments of the walker position after many steps can be used as direct indicators of the topological quantum transition: while varying a control parameter that defines the system phase, these moments exhibit a slope discontinuity at the transition point. Numerical simulations strongly support the conjecture that these features are general of 1D topological systems. Extending this approach to higher dimensions, different topological classes, and other typologies of quantum phases may offer general instruments for investigating and experimentally detecting quantum transitions in such complex systems.

Vortices are archetypal objects that recur in the universe across the scale of complexity, from subatomic particles to galaxies and black holes. Their appearance is connected with spontaneous symmetry breaking and phase transitions. In Bose-Einstein condensates and superfluids, vortices are both point-like and quantized quasiparticles. We use a two-dimensional (2D) fluid of polaritons, bosonic particles constituted by hybrid photonic and electronic oscillations, to study quantum vortex dynamics. Polaritons benefit from easiness of wave function phase detection, a spinor nature sustaining half-integer vorticity, strong nonlinearity, and tuning of the background disorder. We can directly generate by resonant pulsed excitations a polariton condensate carrying either a full or half-integer vortex as initial condition and follow their coherent evolution using ultrafast imaging on the picosecond scale. The observations highlight a rich phenomenology, such as the spiraling of the half-vortex and the joint path of the twin charges of a full vortex, until the moment of their splitting. Furthermore, we observe the ordered branching into newly generated secondary couples, associated with the breaking of radial and azimuthal symmetries. This allows us to devise the interplay of nonlinearity and sample disorder in shaping the fluid and driving the vortex dynamics. In addition, our observations suggest that phase singularities may be seen as fundamental particles whose quantized events span from pair creation and recombination to 2D+t topological vortex strings.

The standard method for experimentally determining the probability distribution of an observable in quantum mechanics is the measurement of the observable spectrum. However, for infinite-dimensional degrees of freedom, this approach would require ideally infinite or, more realistically, a very large number of measurements. Here we consider an alternative method which can yield the mean and variance of an observable of an infinite-dimensional system by measuring only a two-dimensional pointer weakly coupled with the system. In our demonstrative implementation, we determine both the mean and the variance of the orbital angular momentum of a light beam without acquiring the entire spectrum, but measuring the Stokes parameters of the optical polarization (acting as pointer), after the beam has suffered a suitable spin-orbit weak interaction. This example can provide a paradigm for a new class of useful weak quantum measurements.

Recent years have seen vast progress in the generation and detection of structured light, with potential applications in high capacity optical data storage and continuous variable quantum technologies. Here we measure the transmission of structured light through cold rubidium atoms and observe regions of electromagnetically induced transparency (EIT), using the phase profile as control parameter for the atomic opacity. With q plates we generate a probe beam with azimuthally varying phase and polarization structure, and its right and left circular polarization components provide the probe and control of an EIT transition. We observe an azimuthal modulation of the absorption profile that is dictated by the phase and polarization structure of the probe laser. Conventional EIT systems do not exhibit phase sensitivity. We show, however, that a weak transverse magnetic field closes the EIT transitions, thereby generating phase-dependent dark states which in turn lead to phase-dependent transparency, in agreement with our measurements.

Purpose: Arterial stiffness increases with age and is related to an increased risk of coronary artery disease. Poor trunk flexibility has been shown to be associated with arterial stiffness in middle-aged subjects. The purpose of our research study was to measure arterial stiffness and flexibility in healthy middle-aged martial artists compared to age and gender matched healthy sedentary controls., Methods: Ten martial artists (54.0 ± 2.0 years), who practice Soo Bahk Do (SBD), a Korean martial art, and ten sedentary subjects (54.7 ± 1.8 years) for a total of twenty subjects took part in this cross-sectional study. Arterial stiffness was assessed in all subjects using pulse wave velocity (PWV), a recognized index of arterial stiffness. Flexibility of the trunk and hamstring were also measured. The independent variables were the martial artists and matched sedentary controls. The dependent variables were PWV and flexibility., Results: There were significant differences, between the SBD practitioners and sedentary controls, in PWV (P = 0.004), in trunk flexibility (P= 0.002), and in hamstring length (P= 0.003)., Conclusion: The middle-aged martial artists were more flexible in their trunk and hamstrings and had less arterial stiffness compared to the healthy sedentary controls. The flexibility component of martial art training or flexibility exercises in general may be considered as a possible intervention to reduce the effects of aging on arterial stiffness.

We present an optical scheme to encode and decode 2 bits of information into different orbital angular momentum (OAM) states of a paraxial optical beam. Our device generates the four light angular momentum states of order ±2 and ±4 by spin-to-orbital angular momentum conversion in a triangular optical loop arrangement. The switching among the four OAM states is obtained by changing the polarization state of the circulating beam by two quarter-wave plates, and the 2 bit information is transferred to the beam OAM exploiting a single q plate. The polarization of the exit beam is left free for an additional 1 bit of information. The switching among the different OAM states can be as fast as a few nanoseconds, if suitable electro-optical cells are used. This may be particularly useful in communication systems based on light OAM.

In this paper we show that an optical setup based on a polarizing Sagnac interferometer combined with a Dove prism can be used as a convenient general-purpose tool for the generation, detection and sorting of spin-orbit states of light. This device can work both in the classical and in the quantum single-photon regime, provides higher sorting efficiency and extinction ratio than usual hologram-fiber combinations, and shows much higher stability and ease of alignment than Mach-Zehnder interferometer setups. To demonstrate the full potential of this setup, we also report some demonstrative experiments of several possible applications of this setup.

The orbital angular momentum of photons, being defined in an infinite-dimensional discrete Hilbert space, offers a promising resource for high-dimensional quantum information protocols in quantum optics. The biggest obstacle to its wider use is presently represented by the limited set of tools available for its control and manipulation. Here, we introduce and test experimentally a series of simple optical schemes for the coherent transfer of quantum information from the polarization to the orbital angular momentum of single photons and vice versa. All our schemes exploit a newly developed optical device, the so-called "q-plate", which enables the manipulation of the photon orbital angular momentum driven by the polarization degree of freedom. By stacking several q-plates in a suitable sequence, one can also have access to higher-order angular momentum subspaces. In particular, we demonstrate the control of the orbital angular momentum m degree of freedom within the subspaces of |m| = 2h and |m| = 4h per photon.

Objectives: Significant hypotension after carotid endarterectomy (CEA) and carotid angioplasty with stenting (CAS) has been correlated with adverse outcomes. The objective of this study was to determine risk factors that predict hypotension after patients undergo CEA and CAS., Methods: The review included 1474 CEA patients and 157 CAS patients who underwent procedures from 2002 to 2008. Specific patient characteristics, such as comorbid diseases, degree of carotid stenosis, presence of neurologic symptoms, and preprocedure medications, were assessed. Also reviewed were specific postprocedural clinical outcomes, including hypotension requiring pressors, myocardial infarction, stroke, death, and hospital length of stay., Results: The incidence of clinically significant hypotension was 12.6% in CEA patients and 35% in CAS patients (P < .001). Clinically significant hypotension was correlated with increased postprocedural myocardial infarction (2.1% vs 0.5%, P = .022), increased mortality (2.1% vs 0.1%, P < .001), and length of stay >2 days (46.3% vs 27.4%, P = .01). Hypotension was not associated with increased postprocedural strokes (0.8% vs 0.6%, P = .75) or recurrent neurologic symptoms (0.4% vs 0.3%, P = .55). Preoperative nitrate use predicted a greater incidence of postprocedural hypotension (P = .043). A history of tobacco use was correlated with postprocedure hypotension (P = .033). Preprocedural strokes, the use of calcium channel blockers, beta-blockers, angiotensin-converting enzyme inhibitors, prior myocardial infarction, degree of preprocedural carotid stenosis, type of stent, previous ipsilateral and contralateral interventions, and female gender did not correlate with postprocedural hypotension (P >.05)., Conclusions: Postprocedural hypotension occurs more commonly with CAS than CEA and is associated with increased postprocedural myocardial infarction and length of stay, and death. Nitrates and tobacco use predict a higher incidence of postprocedural hypotension. High-risk patients should be aggressively managed to prevent the increased morbidity and mortality due to postprocedural hypotension.

The optical "spin-orbit" coupling occurring in a suitably patterned nonuniform birefringent plate known as a "q plate" allows entangling the polarization of a single photon with its orbital angular momentum (OAM). This process, in turn, can be exploited for building a bidirectional "spin-OAM interface," capable of transposing the quantum information from the spin to the OAM degree of freedom of photons and vice versa. Here, we experimentally demonstrate this process by single-photon quantum tomographic analysis. Moreover, we show that two-photon quantum correlations such as those resulting from coalescence interference can be successfully transferred into the OAM degree of freedom.

We calculated the Fresnel paraxial propagator in a birefringent plate having topological charge q at its center, named "q-plate." We studied the change of the beam transverse profile when it traverses the plate. An analytical closed form of the beam profile propagating in the q-plate can be found for many important specific input beam profiles. We paid particular attention to the plate having a topological unit charge and found that if small losses due to reflection, absorption, and scattering are neglected, the plate can convert the photon spin into orbital angular momentum with up to 100% efficiency provided the thickness of the plate is less than the Rayleigh range of the incident beam.

We present a novel family of paraxial optical beams having a confluent hypergeometric transverse profile, which we name hypergeometric Gauss modes of type-II (HyGG-II). These modes are eigenmodes of the photon orbital angular momentum and they have the lowest beam divergence in the waist of HyGG-II among all known finite power paraxial modes families. We propose to exploit this feature of HyGG-II modes for generating, after suitable focusing, a "light needle" having record properties in terms of size and aspect ratio, possibly useful for near-field optics applications.

Liquid crystals interacting with light represent a unique class of soft-matter systems that exhibit various generic nonlinear behaviors, including chaotic rotational dynamics. Despite several experimental observations, complex nematic liquid crystal director rotations in presence of spin and orbital angular momentum transfer processes were left unexplained. We present a self-consistent three-dimensional model able to describe the previous experimental observations, accounting for the dependence on the incident beam intensity, polarization, finite size and shape. More generally, our model is able to describe quantitatively the dynamics of, and beyond, the optical Fréedericksz transition under realistic experimental conditions almost three decades after its experimental discovery.

Recently important efforts have been dedicated to the realization of a new kind of photonic crystals, known as photonic quasicrystals, in which the lack of the translational symmetry is compensated by rotational symmetries not achievable by the conventional periodic crystals. Here we show a novel approach to their fabrication based on the use of a programmable Spatial Light Modulator encoding Computer-Generated Holograms. Using this single beam technique we fabricated Penrose-tiled structures possessing rotational symmetry up to 23-fold, and a two-dimensional Thue-Morse structure, which is an aperiodic structure not achievable by multiple beam holography.

We studied a novel family of paraxial laser beams forming an overcomplete yet nonorthogonal set of modes. These modes have a singular phase profile and are eigenfunctions of the photon orbital angular momentum. The intensity profile is characterized by a single brilliant ring with the singularity at its center, where the field amplitude vanishes. The complex amplitude is proportional to the degenerate (confluent) hypergeometric function, and therefore we term such beams hypergeometric-Gaussian (HyGG) modes. Unlike the recently introduced hypergeometric modes [Opt. Lett. 32, 742 (2007)], the HyGG modes carry a finite power and have been generated in this work with a liquid-crystal spatial light modulator. We briefly consider some subfamilies of the HyGG modes as the modified Bessel Gaussian modes, the modified exponential Gaussian modes, and the modified Laguerre-Gaussian modes.
