Abstract: The IceCube Neutrino Observatory instruments about 1 km3 of deep, glacial ice at the geographic South Pole. It uses 5160 photomultipliers to detect Cherenkov light emitted by charged relativistic particles. An unexpected light propagation effect observed by the experiment is an anisotropic attenuation, which is aligned with the local flow direction of the ice. We examine birefringent light propagation through the polycrystalline ice microstructure as a possible explanation for this effect. The predictions of a first-principles model developed for this purpose, in particular curved light trajectories resulting from asymmetric diffusion, provide a qualitatively good match to the main features of the data. This in turn allows us to deduce ice crystal properties. Since the wavelength of the detected light is short compared to the crystal size, these crystal properties include not only the crystal orientation fabric, but also the average crystal size and shape, as a function of depth. By adding small empirical corrections to this first-principles model, a quantitatively accurate description of the optical properties of the IceCube glacial ice is obtained. In this paper, we present the experimental signature of ice optical anisotropy observed in IceCube light-emitting diode (LED) calibration data, the theory and parameterization of the birefringence effect, the fitting procedures of these parameterizations to experimental data, and the inferred crystal properties.

Abstract: Abstract Atmospheric muon neutrinos are produced by meson decays in cosmic-ray-induced air showers. The flux depends on meteorological quantities such as the air temperature, which affects the density of air. Competition between decay and re-interaction of those mesons in the first particle production generations gives rise to a higher neutrino flux when the air density in the stratosphere is lower, corresponding to a higher temperature. A measurement of a temperature dependence of the atmospheric $$\nu _{\mu }$$ ν μ flux provides a novel method for constraining hadronic interaction models of air showers. It is particularly sensitive to the production of kaons. Studying this temperature dependence for the first time requires a large sample of high-energy neutrinos as well as a detailed understanding of atmospheric properties. We report the significant ( $$> 10 \; \sigma $$ > 10 σ ) observation of a correlation between the rate of more than 260,000 neutrinos, detected by IceCube between 2012 and 2018, and atmospheric temperatures of the stratosphere, measured by the Atmospheric Infrared Sounder (AIRS) instrument aboard NASA’s AQUA satellite. For the observed 10 $$\%$$ % seasonal change of effective atmospheric temperature we measure a 3.5(3) $$\%$$ % change in the muon neutrino flux. This observed correlation deviates by about 2-3 standard deviations from the expected correlation of 4.3 $$\%$$ % as obtained from theoretical predictions under the assumption of various hadronic interaction models.

Abstract: The origin of high-energy galactic cosmic rays is yet to be understood, but some galactic cosmic-ray accelerators can accelerate cosmic rays up to PeV energies. The high-energy cosmic rays are expected to interact with the surrounding material or radiation, resulting in the production of gamma-rays and neutrinos. To optimize for the detection of such associated production of gamma-rays and neutrinos for a given source morphology and spectrum, a multimessenger analysis that combines gamma-rays and neutrinos is required. In this study, we use the Multi-Mission Maximum Likelihood framework with IceCube Maximum Likelihood Analysis software and HAWC Accelerated Likelihood to search for a correlation between 22 known gamma-ray sources from the third HAWC gamma-ray catalog and 14 yr of IceCube track-like data. No significant neutrino emission from the direction of the HAWC sources was found. We report the best-fit gamma-ray model and 90% CL neutrino flux limit from the 22 sources. From the neutrino flux limit, we conclude that, for five of the sources, the gamma-ray emission observed by HAWC cannot be produced purely from hadronic interactions. We report the limit for the fraction of gamma-rays produced by hadronic interactions for these five sources.

Abstract: Active galactic nuclei (AGN) are prime candidate sources of the high-energy, astrophysical neutrinos detected by IceCube. This is demonstrated by the real-time multimessenger detection of the blazar TXS 0506+056 and the recent evidence of neutrino emission from NGC 1068 from a separate time-averaged study. However, the production mechanism of the astrophysical neutrinos in AGN is not well established, which can be resolved via correlation studies with photon observations. For neutrinos produced due to photohadronic interactions in AGN, in addition to a correlation of neutrinos with high-energy photons, there would also be a correlation of neutrinos with photons emitted at radio wavelengths. In this work, we perform an in-depth stacking study of the correlation between 15 GHz radio observations of AGN reported in the MOJAVE XV catalog, and 10 yr of neutrino data from IceCube. We also use a time-dependent approach, which improves the statistical power of the stacking analysis. No significant correlation was found for both analyses, and upper limits are reported. When compared to the IceCube diffuse flux, at 100 TeV and for a spectral index of 2.5, the upper limits derived are ∼3% and ∼9% for the time-averaged and time-dependent cases, respectively.

Abstract: Neutrino flares in the sky are searched for in data collected by IceCube between 2011 and 2021 May. This data set contains cascade-like events originating from charged-current electron neutrino and tau neutrino interactions and all-flavor neutral-current interactions. IceCube’s previous all-sky searches for neutrino flares used data sets consisting of track-like events originating from charged-current muon neutrino interactions. The cascade data set is statistically independent of the track data sets, and while inferior in angular resolution, the low-background nature makes it competitive and complementary to previous searches. No statistically significant flare of neutrino emission was observed in an all-sky scan. Upper limits are calculated on neutrino flares of varying duration from 1 hr to 100 days. Furthermore, constraints on the contribution of these flares to the diffuse astrophysical neutrino flux are presented, showing that multiple unresolved transient sources may contribute to the diffuse astrophysical neutrino flux.

Abstract: We present the results of a search for 10–1000 GeV neutrinos from 2268 gamma-ray bursts (GRBs) over 8 yr of IceCube-DeepCore data. This work probes burst physics below the photosphere where electromagnetic radiation cannot escape. Neutrinos of tens of giga electronvolts are predicted in sub-photospheric collision of free-streaming neutrons with bulk-jet protons. In a first analysis, we searched for the most significant neutrino-GRB coincidence using six overlapping time windows centered on the prompt phase of each GRB. In a second analysis, we conducted a search for a group of GRBs, each individually too weak to be detectable, but potentially significant when combined. No evidence of neutrino emission is found for either analysis. The most significant neutrino coincidence is for Fermi-GBM GRB bn 140807500, with a p -value of 0.097 corrected for all trials. The binomial test used to search for a group of GRBs had a p -value of 0.65 after all trial corrections. The binomial test found a group consisting only of GRB bn 140807500 and no additional GRBs. The neutrino limits of this work complement those obtained by IceCube at tera electronvolt to peta electronvolt energies. We compare our findings for the large set of GRBs as well as GRB 221009A to the sub-photospheric neutron-proton collision model and find that GRB 221009A provides the most constraining limit on baryon loading. For a jet Lorentz factor of 300 (800), the baryon loading on GRB 221009A is lower than 3.85 (2.13) at a 90% confidence level.

Abstract: IceCube alert events are neutrinos with a moderate-to-high probability of having astrophysical origin. In this study, we analyze 11 yr of IceCube data and investigate 122 alert events and a selection of high-energy tracks detected between 2009 and the end of 2021. This high-energy event selection (alert events + high-energy tracks) has an average probability of ≥0.5 of being of astrophysical origin. We search for additional continuous and transient neutrino emission within the high-energy events’ error regions. We find no evidence for significant continuous neutrino emission from any of the alert event directions. The only locally significant neutrino emission is the transient emission associated with the blazar TXS 0506+056, with a local significance of 3 σ , which confirms previous IceCube studies. When correcting for 122 test positions, the global p -value is 0.156 and compatible with the background hypothesis. We constrain the total continuous flux emitted from all 122 test positions at 100 TeV to be below 1.2 × 10 ^−15 (TeV cm ^2 s) ^−1 at 90% confidence assuming an E ^−2 spectrum. This corresponds to 4.5% of IceCube’s astrophysical diffuse flux. Overall, we find no indication that alert events in general are linked to lower-energetic continuous or transient neutrino emission.

Abstract: The IceCube Neutrino Observatory has been continuously taking data to search for ${ \mathcal O }(0.5\mbox{--}10)$ s long neutrino bursts since 2007. Even if a Galactic core-collapse supernova is optically obscured or collapses to a black hole instead of exploding, it will be detectable via the ${ \mathcal O }(10)$ MeV neutrino burst emitted during the collapse. We discuss a search for such events covering the time between 2008 April 17 and 2019 December 31. Considering the average data taking and analysis uptime of 91.7% after all selection cuts, this is equivalent to 10.735 yr of continuous data taking. In order to test the most conservative neutrino production scenario, the selection cuts were optimized for a model based on an 8.8 solar mass progenitor collapsing to an O–Ne–Mg core. Conservative assumptions on the effects of neutrino oscillations in the exploding star were made. The final selection cut was set to ensure that the probability to detect such a supernova within the Milky Way exceeds 99%. No such neutrino burst was found in the data after performing a blind analysis. Hence, a 90% C.L. upper limit on the rate of core-collapse supernovae out to distances of ≈25 kpc was determined to be 0.23 yr ^−1 . For the more distant Magellanic Clouds, only high neutrino luminosity supernovae will be detectable by IceCube, unless external information on the burst time is available. We determined a model-independent limit by parameterizing the dependence on the neutrino luminosity and the energy spectrum.

Abstract: Core-collapse supernovae are a promising potential high-energy neutrino source class. We test for correlation between seven years of IceCube neutrino data and a catalog containing more than 1000 core-collapse supernovae of types IIn and IIP and a sample of stripped-envelope supernovae. We search both for neutrino emission from individual supernovae as well as for combined emission from the whole supernova sample, through a stacking analysis. No significant spatial or temporal correlation of neutrinos with the cataloged supernovae was found. All scenarios were tested against the background expectation and together yield an overall p -value of 93%; therefore, they show consistency with the background only. The derived upper limits on the total energy emitted in neutrinos are 1.7 × 10 ^48 erg for stripped-envelope supernovae, 2.8 × 10 ^48 erg for type IIP, and 1.3 × 10 ^49 erg for type IIn SNe, the latter disfavoring models with optimistic assumptions for neutrino production in interacting supernovae. We conclude that stripped-envelope supernovae and supernovae of type IIn do not contribute more than 14.6% and 33.9%, respectively, to the diffuse neutrino flux in the energy range of about [ 10 ^3 –10 ^5 ] GeV, assuming that the neutrino energy spectrum follows a power-law with an index of −2.5. Under the same assumption, we can only constrain the contribution of type IIP SNe to no more than 59.9%. Thus, core-collapse supernovae of types IIn and stripped-envelope supernovae can both be ruled out as the dominant source of the diffuse neutrino flux under the given assumptions.

Abstract: Gamma-ray bursts (GRBs) have long been considered a possible source of high-energy neutrinos. While no correlations have yet been detected between high-energy neutrinos and GRBs, the recent observation of GRB 221009A—the brightest GRB observed by Fermi-GBM to date and the first one to be observed above an energy of 10 TeV—provides a unique opportunity to test for hadronic emission. In this paper, we leverage the wide energy range of the IceCube Neutrino Observatory to search for neutrinos from GRB 221009A. We find no significant deviation from background expectation across event samples ranging from MeV to PeV energies, placing stringent upper limits on the neutrino emission from this source.

Abstract: We present a catalog of likely astrophysical neutrino track-like events from the IceCube Neutrino Observatory. IceCube began reporting likely astrophysical neutrinos in 2016, and this system was updated in 2019. The catalog presented here includes events that were reported in real time since 2019, as well as events identified in archival data samples starting from 2011. We report 275 neutrino events from two selection channels as the first entries in the catalog, the IceCube Event Catalog of Alert Tracks, which will see ongoing extensions with additional alerts. The Gold and Bronze alert channels respectively provide neutrino candidates with a 50% and 30% probability of being astrophysical, on average assuming an astrophysical neutrino power-law energy spectral index of 2.19. For each neutrino alert, we provide the reconstructed energy, direction, false-alarm rate, probability of being astrophysical in origin, and likelihood contours describing the spatial uncertainty in the alert's reconstructed location. We also investigate a directional correlation of these neutrino events with gamma-ray and X-ray catalogs, including 4FGL, 3HWC, TeVCat, and Swift-BAT.

Abstract: The LIGO/Virgo collaboration published the catalogs GWTC-1, GWTC-2.1, and GWTC-3 containing candidate gravitational-wave (GW) events detected during its runs O1, O2, and O3. These GW events can be possible sites of neutrino emission. In this paper, we present a search for neutrino counterparts of 90 GW candidates using IceCube DeepCore, the low-energy infill array of the IceCube Neutrino Observatory. The search is conducted using an unbinned maximum likelihood method, within a time window of 1000 s, and uses the spatial and timing information from the GW events. The neutrinos used for the search have energies ranging from a few GeV to several tens of TeV. We do not find any significant emission of neutrinos, and place upper limits on the flux and the isotropic-equivalent energy emitted in low-energy neutrinos. We also conduct a binomial test to search for source populations potentially contributing to neutrino emission. We report a nondetection of a significant neutrino-source population with this test.

Abstract: The Galactic plane, harboring a diffuse neutrino flux, is a particularly interesting target in which to study potential cosmic-ray acceleration sites. Recent gamma-ray observations by HAWC and LHAASO have presented evidence for multiple Galactic sources that exhibit a spatially extended morphology and have energy spectra continuing beyond 100 TeV. A fraction of such emission could be produced by interactions of accelerated hadronic cosmic rays, resulting in an excess of high-energy neutrinos clustered near these regions. Using 10 years of IceCube data comprising track-like events that originate from charged-current muon neutrino interactions, we perform a dedicated search for extended neutrino sources in the Galaxy. We find no evidence for time-integrated neutrino emission from the potential extended sources studied in the Galactic plane. The most significant location, at 2.6 σ post-trials, is a 1.°7 sized region coincident with the unidentified TeV gamma-ray source 3HWC J1951+266. We provide strong constraints on hadronic emission from several regions in the galaxy.

Abstract: The IceCube Neutrino Observatory sends realtime neutrino alerts with a high probability of being astrophysical in origin. We present a new method to correlate these events and possible candidate sources using 2089 blazars from the Fermi-LAT 4LAC-DR2 catalog and with 3413 active galactic nuclei (AGNs) from the Radio Fundamental Catalog. Abstract: The understanding of novae, the thermonuclear eruptions on the surfaces of white dwarf stars in binaries, has recently undergone a major paradigm shift. Though the bolometric luminosity of novae was long thought to arise directly from photons supplied by the thermonuclear runaway, recent gigaelectronvolt (GeV) gamma-ray observations have supported the notion that a significant portion of the luminosity could come from radiative shocks. More recently, observations of novae have lent evidence that these shocks are acceleration sites for hadrons for at least some types of novae. In this scenario, a flux of neutrinos may accompany the observed gamma rays. As the gamma rays from most novae have only been observed up to a few GeV, novae have previously not been considered as targets for neutrino telescopes, which are most sensitive at and above teraelectronvolt (TeV) energies. Here, we present the first search for neutrinos from novae with energies between a few GeV and 10 TeV using IceCube-DeepCore, a densely instrumented region of the IceCube Neutrino Observatory with a reduced energy threshold. We search both for a correlation between gamma-ray and neutrino emission as well as between optical and neutrino emission from novae. We find no evidence for neutrino emission from the novae considered in this analysis and set upper limits for all gamma-ray detected novae.

Abstract: This paper presents the results of a search for neutrinos that are spatially and temporally coincident with 22 unique, nonrepeating fast radio bursts (FRBs) and one repeating FRB (FRB 121102). FRBs are a rapidly growing class of Galactic and extragalactic astrophysical objects that are considered a potential source of high-energy neutrinos. The IceCube Neutrino Observatory’s previous FRB analyses have solely used track events. This search utilizes seven years of IceCube cascade events which are statistically independent of track events. This event selection allows probing of a longer range of extended timescales due to the low background rate. No statistically significant clustering of neutrinos was observed. Upper limits are set on the time-integrated neutrino flux emitted by FRBs for a range of extended time windows.

22. Farm Exposure as a Differential Risk Factor in ANCA-Associated Vasculitis.

Abstract: OBJECTIVE:To investigate the association of farm exposure and the development of ANCA-associated vasculitis (AAV). METHODS:One hundred eighty-nine well defined patients with AAV (n = 119 with granulomatosis with polyangiitis [GPA], n = 48 with microscopic polyangiitis [MPA], n = 22 patients with eosinophilic granulomatosis with polyangiitis [EGPA]) and 190 controls (n = 119 patients with rheumatoid arthritis, n = 71 with large vessel vasculitis) were interrogated using a structured questionnaire. Factors investigated were occupation, farm exposure, contact to different livestock, participation in harvesting, residence next to a farm, MRSA status, and contact to domestic pets at disease onset or ever before. The odds ratio (OR) and 95% confidence interval [95%CI] were calculated for each item. RESULTS:Univariate analysis revealed a strong association of AAV with regular farm exposure; OR 3.44 [95%CI 1.43-8.27]. AAV was also associated with regular contact to cattle 4.30 (1.43-8.27), pigs 2.75 (1.12-6.75) and MRSA carriage 3.38 (1.11-10.3). This association was stronger in the subgroup of GPA patients. OR in this group for farm exposure was 4.97; [2.02-12.2], for cattle 6.71 [95% CI 2.19-20.7], for pigs 4.34 [1.75-10.9], and MRSA carriage 5.06 [1.62-15.8]). There was no significant association of MPA or EGPA with these parameters. CONCLUSION:A significant association between farm exposure or farm animal exposure and AAV especially in the subgroup of patients with GPA has been identified. This suggests that these entities are distinct and have different triggers for the immune process.

Abstract: Cellular therapies require methods for noninvasive visualization of transplanted cells. Micron-sized iron oxide particles (MPIOs) generate a strong contrast in magnetic resonance imaging (MRI) and are therefore ideally suited as an intracellular contrast agent to image cells under clinical conditions. However, MPIOs were previously not applicable for clinical use. Here, we present the development and evaluation of silica-based micron-sized iron oxide particles (sMPIOs) with a functionalizable particle surface. Particles with magnetite content of >40% were composed using the sol-gel process. The particle surfaces were covered with COOH groups. Fluorescein, poly-l-lysine (PLL), and streptavidin (SA) were covalently attached. Monodisperse sMPIOs had an average size of 1.18 μm and an iron content of about 1.0 pg Fe/particle. Particle uptake, toxicity, and imaging studies were performed using HuH7 cells and human and rat hepatocytes. sMPIOs enabled rapid cellular labeling within 4 h of incubation; PLL-modified particles had the highest uptake. In T2*-weighted 3.0 T MRI, the detection threshold in agarose was 1,000 labeled cells, whereas in T1-weighted LAVA sequences, at least 10,000 cells were necessary to induce sufficient contrast. Labeling was stable and had no adverse effects on labeled cells. Silica is a biocompatible material that has been approved for clinical use. sMPIOs could therefore be suitable for future clinical applications in cellular MRI, especially in settings that require strong cellular contrast. Moreover, the particle surface provides the opportunity to create multifunctional particles for targeted delivery and diagnostics.

Abstract: Die Fallserie des Schlaflabors der Vestischen Kinder- und Jugendklinik Datteln umfasste in dem Zeitraum 1995–2015 64 Patienten mit Narkolepsie (32 Jungen, 32 Madchen, Lebensalter [Median] 12,5 Jahre). Wahrend in dem Zeitraum von 1995 bis 2010 (15 Jahre) bei 5 Patienten die Diagnose Narkolepsie gestellt worden war, kam es von 2011 bis 2015 (5 Jahre) zu einem Anstieg auf 59 Narkolepsiepatienten. Die Analyse des Beschwerdebeginns zeigte den Anstieg bereits ab 2008. Das Intervall zwischen Beschwerdebeginn und Diagnosestellung betrug 9,5 Monate (Median). Alle Patienten wiesen das Hauptsymptom Hypersomnie auf, davon 53 % eine Kataplexie, 3,1 % eine Schlaflahmung und 1,6 % hypnagoge Halluzinationen. In 65,6 % der Falle erfolgte die klinische Manifestation „plotzlich“, in 34,4 % „schleichend“. Eine positive Familienanamnese wurde bei 15,6 % der Betroffenen gefunden. In 9,4 % der Falle war eine H1N1-Impfung (Pandemrix®) vorausgegangen. Neben den klinisch-anamnestischen Daten der Fallserie werden die Ergebnisse der Polysomnographie (PSG), des multiplen Schlaflatenztests (MSLT) und der Labordiagnostik („Human-leukocyte-antigen“[HLA]Marker, Hypocretinkonzentration im Liquor cerebrospinalis) dargestellt. Die Fallserie zeigt das Problem der unvollstandigen Auspragung der typischen Narkolepsiesymptome und -befunde im Kindes- und Jugendalter. Daher sollte die PSG-Diagnostik bei „unklarer Hypersomnie“ wiederholt werden, wenn die klinische Symptomatik weiterbesteht.

Abstract: Summary Introduction Although sleep disturbances in disabled children are of central clinical importance, there is little research on that topic. There are no data available on frequency, severity or aetiology of sleep disturbances and related symptoms in this specific patient group. Objective To review the current state of research and outline future research objectives. Methods We searched international scientific databases for relevant publications from 1980-2009. From all papers qualifying for further analysis we retrieved systematic information on sample characteristics, sleep assessment tools and their test quality criteria, and core findings. Results 61 publications including 4392 patients were categorized as "mixed" (reporting on heterogeneous diagnoses), or "specified" papers (specific diagnoses) based on international classification of diseases (ICD) 10 classification. To assess sleep disturbances, most authors relied on subjective instruments with poor psychometric quality. Mean prevalence of sleep disturbances was 67% (76%,"mixed" group; 65%, "specified" group). In children suffering severe global cerebral injury, the prevalence of sleep disturbances was even higher (>90%). The most frequent symptoms were insomnia and sleep-related respiratory disorders. Some of these symptoms were closely associated with specific medical syndromes. Conclusion There is an urgent need for sleep disturbance assessment tools evaluated for the patient group of interest. By use of validated assessment tools, patient factors, which may be crucial in causing sleep disturbances, may be investigated and appropriate treatment strategies may be developed.

Abstract: The prevalence of obstructive sleep apnea syndrome (OSAS) is clearly increased in adults with polycystic ovarian syndrome (PCOS), whereas OSAS does not seem to be frequent in adolescents with PCOS, pointing towards the fact that some patients with PCOS develop OSAS in the further course of the disease. We therefore aimed to analyze the changes of polysomnographic variables in obese adolescents with PCOS in a longitudinal analysis.Fifteen adolescents with PCOS (age 15.3 years ± 1.2, BMI 32.9 kg/m(2) ± 6.4, SDS-BMI 2.5 ± 0.8) underwent overnight 12-channel polysomnography at baseline and after a mean duration of 28 ± 6 months (age 17.8 years ± 1.1, BMI 32.7 kg/m(2) ± 7.0, SDS-BMI 2.1 ± 0.9). After performing the initial polysomnography, we treated hyperandrogenemia and insulin resistance in the study group. We determined parameters of body weight/body composition, parameters of glucose metabolism, and serum androgens in all patients at baseline and follow-up. At follow-up, we compared the polysomnographic variables of the study group to those of healthy female adults.The polysomnographic variables, the parameters of body weight/body composition, and the parameters of glucose metabolism in the study group did not change significantly during the observation period. The serum levels of total testosterone and sex hormone binding globulin increased significantly, whereas free androgen index decreased significantly. At follow-up, the polysomnographic variables of the study group did not differ from those of healthy female adults.OSAS does not seem to develop in adolescents with PCOS being treated for hyperandrogenism and insulin resistance. The pathogenesis of OSAS in PCOS needs to be examined in larger controlled studies.

Abstract: We aimed to determine the differences in polysomnographic variables between obese adolescents with polycystic ovarian syndrome (PCOS) with and without metabolic syndrome, as the prevalence of obstructive sleep apnea syndrome (OSAS) is increased in adults with PCOS, OSAS has been regarded as a manifestation of the metabolic syndrome, and the prevalence of metabolic syndrome is increased in patients with PCOS.Fourteen obese adolescents with PCOS and metabolic syndrome [15.7 years ± 1.9, body mass index (BMI) 36.2 kg/m(2) ± 6.2], 14 obese adolescents with PCOS without metabolic syndrome (15.7 years ± 1.1, BMI 33.8 kg/m(2) ± 6.2), 19 healthy, obese adolescents without PCOS or metabolic syndrome (15.3 years ± 1.0, BMI 34.4 kg/m(2) ± 6.5), and 14 healthy, normal-weight adolescents (15.4 years ± 0.7, BMI 21.1 kg/m(2) ± 2.2) underwent polysomnography to compare transcutaneous arterial oxygen saturation (Sat O(2)), apnea index (AI), hypopnea index (HI), apnea-hypopnea index (AHI), the absolute number of obstructive apneas (NOA), percentage sleep stages 3 and 4 of non REM-sleep (stages 3 and 4), percentage of rapid eye movement (REM) sleep (%REM), sleep-onset latency, and sleep efficiency.We found no differences among the four groups concerning AI, HI, AHI, NOA, and stages 3 and 4. Significant differences among the groups were found regarding Sat O(2) (P = 0.04), %REM (P = 0.03), sleep-onset latency (P = 0.002), and sleep efficiency (P = 0.01).Weight status, PCOS, and metabolic syndrome do not seem to have significant effects on respiratory polysomnographic variables in adolescent girls with PCOS, suggesting that the pathomechanisms leading to OSAS in patients with PCOS develop in the later course of the disease.

Abstract: We aimed to determine the impact of insulin resistance and hyperandrogenemia on polysomnographic variables in obese adolescents with polycystic ovarian syndrome (PCOS), as studies in adults with PCOS suggest that parameters of glucose metabolism and serum androgens are related to respiratory polysomnographic variables (RPV), and the symptoms of PCOS usually begin around menarche.We divided our study group of obese adolescents with PCOS according to HOMA-index and in a second analysis according to free androgen index (FAI). Study group A consisted of 14 girls with HOMA-index4, study group B of 17 girls with HOMA-index4. Study group C consisted of 19 girls with FAI10, and study group D of 18 girls with FAI10. The control group for both analyses consisted of 19 healthy obese adolescents without PCOS. All girls underwent overnight 12-channel polysomnography.In both analyses, we found no differences between the groups concerning the RPV. Study group B demonstrated a significantly lower percentage of REM-sleep than the control group (p = 0.02). Study group D demonstrated a significantly lower percentage sleep stages 3 and 4 of non-REM-sleep than study group C and the controls (p = 0.008). Study group D demonstrated significantly lower sleep efficiency than the controls (p = 0.03).Insulin resistance and hyperandrogenemia do not seem to have a significant impact on RPV in obese adolescents with PCOS. Differences in sleep architecture found between patients with PCOS and controls, however, are possibly influenced by insulin resistance and/or serum androgens.

We found no differences between the two groups concerning the respiratory indices, percentage sleep Stages 2, 3 and 4 of NREM sleep, TIB and sleep-onset latency. The girls with PCOS differed significantly from the controls regarding TST, WASO, TWT, sleep efficiency, percentage Stage 1 of NREM sleep and percentage of REM sleep. We found a weak significant correlation between insulin resistance and apnoea index and between insulin resistance and apnoea–hypopnoea index. Concerning the respiratory variables, adolescents with PCOS do not seem to differ from healthy controls; however, there seem to be differences concerning sleep architecture.

Methods: Sixteen obese and 17 extremely obese adolescents with PCOS, 18 normal-weight, 17 obese, and 13 extremely obese controls underwent polysomnonotgraphy to compare mean transcutaneous arterial oxygen saturation (Sat O2), apnea-index (AI), hypopnea- index (HI), apnea-hypopnea index (AHI), the absolute number of obstructive apneas (NOA), percentage sleep stages 3 and 4 of non REM-sleep (stages 3 & 4), percentage of REM-sleep (% REM), sleep-onset latency, and sleep efficiency. Results: We found no significant differences between the groups concerning AI, HI, AHI, NOA, and stages 3 & 4. Significant differnotences between the groups were found regarding Sat O2, % REM, sleep-onset latency, and sleep efficiency. Conclusions: Concerning the respiratory variables, adolescents with PCOS do not seem to differ from healthy controls regardless of weight status, but there seem to be differences in sleep architecture.

Abstract: Folgendes sollte bei Kindern mit Achondroplasie geklart werden: Haufigkeit anamnestisch erfassbarer Symptome (tages- und schlafbezogene Symptomatik) von schlafbezogenen Atmungsstorungen (SBAS), Beziehung zu erhohter Sterblichkeit, pathophysiologische Mechanismen sowie polysomnographische Formen und Schweregrade von SBAS. Es wurde eine systematische Literaturrecherche durchgefuhrt. Ausgewertet wurden 16 Publikationen aus den Jahren 1984 bis 2009: 2 Fallberichte, 8 klinische Fallserien, 1 Fragebodenstudie mit Referenzkollektiv, 5 physiologische, neuroradiologische bzw. molekulargenetische Studien, davon 3 mit Referenzkollektiven. Populationsbezogene epidemiologische Studien lagen nicht vor. Mehrere mit Achondroplasie assoziierte Faktoren (Mittelgesichtshypoplasie, muskulare Hypotonie, anatomische Veranderungen des Thorax und des kraniozervikalen Ubergangs) bedingen, dass diese Kinder als Gruppe ein erhohtes Risiko fur SBAS und chronische respiratorische Insuffizienz aufweisen. Im 1. Lebensjahr besteht ein erhohtes Risiko fur plotzlichen Tod, z. T. bedingt durch eine Hirnstammkompression. Im Laufe der Kindheit nimmt das Risiko der Kompression ab, und es kommt zu einer Verbesserung der Lungenfunktion. Trotz vieler Publikationen zu den Themen „SBAS bei Achondroplasie“, „prognostische Bedeutung von SBAS“ und „Pathophysiologie der SBAS bei Achondroplasie“, ist die Datenbasis limitiert. Die Resultate beziehen sich uberwiegend auf kleine Fallserien und sind durch Selektionsbias bzw. Recall Bias verzerrt. Aufgrund des hohen Risikos fur Atmungskomplikationen bei Achondroplasie erscheint es sinnvoll, alle Kinder in den ersten Lebensjahren polysomnographisch zu untersuchen.

Abstract: BACKGROUND: The appearance of autoimmunity in tumor patients seems to be associated with a better prognosis. The aim of this retrospective study was to detect autoimmunity under adjuvant treatment with pegylated interferon-alpha-2b and to determine a correlation between autoimmune induction and survival. PATIENTS AND METHODS: To detect autoimmunity clinical and serological parameters were examined in 25 patients with R0-resected histological proven stage Ib-IV melanoma before and every three months during an 18-month adjuvant treatment with 2 μg/kg pegylated interferon-alpha-2b given once a week. RESULTS: Under treatment 16 of 25 patients (64 %) developed autoantibodies. There was no significant difference regarding survival data in patients with or without induction of autoantibodies (31,6 versus 31,9 months). However, it is remarkable that all 6 patients developing a clinically significant autoimmune thyroiditis and/or specific antinuclear antibodies were still alive 36 months after treatment beginning, with two of these patients being in complete remission from stage IV melanoma, compared to a mean survival time of 26,73 months for the remaining patient group with physiological irrelevant positive antibodies (n = 10/16). CONCLUSIONS: In our cohort, the number of patients who developed autoimmunity under pegylated interferon-alpha-2b is significantly higher as published data of non-pegylated interferon-alpha. Nevertheless, the significance of specific autoimmune induction as a prognostic indicator for a disease controlling antitumor immune response still needs to be confirmed.

Abstract: Die vorgelegte Darstellung soll eine These stutzen, nach der sich die polysomnographisch objektivierbaren Atmungsphanomene des gesunden, ausgetragenen Sauglings parallel zur Reifung von Schlafarchitektur und Atmungsregulation entwickeln. Es wird dargestellt,wie im Laufe des ersten Lebensjahres unregelmasiges Atmen als Aquivalent aktiven Schlafes und paradoxe Atmung abnehmen, wahrend regelmasiges Atmen und koordinierte Atmung – die Entsprechungen von ruhigem Schlaf – in gleichem Mase zunehmen. Mit zunehmender Reifung des Organismus schwindet das Atemmuster der periodischen Atmung, wie auch Haufigkeit und Auspragung der verschiedenartigen Atempausen zuruckgehen. Qualitative und quantitative Abweichungen von als altersnormal erfasstem Atmungsverhalten finden sich im ersten Lebensjahr in signifikantem Ausmas bei nicht gesunden und bei unreif geborenen Kindern sowie bei Sauglingen, die zu epidemiologischen Risikogruppen des plotzlichen Sauglingstodes gehoren. So wurde das Atmungsverhalten von Sauglingen im Schlaf insbesondere von Fruhgeburtlichkeit, von Rauchen und Drogenkonsum der Mutter wahrend der Schwangerschaft und von Luftwegsinfekten zum Zeitpunkt der Untersuchung beeinflusst. Auch prospektiv untersuchte spatere SID-Opfer wiesen hinsichtlich ihrer Atmung Besonderheiten auf.

Abstract: Hypersomnolence is the leading symptom of non-restorative sleep. Complaints of the child’s restless, irritative or aggressive behaviour may be prominent, masking the problem of sleepiness. Assessing the significance of daytime sleepiness in pediatric patients may be difficult with respect to the child’s age, ranging from the 1-day-old newborn infant to the 18-years-old adolescent. Physiological developmental changes of the individual degree of wakefulness have to be distinguished from pathological sleepiness in patients with hypersomnic and insomnic states. Thus, the Pediatric Group of the German Sleep Society (DGSM) has elaborated an algorithm providing a rational pathway with respect to the International Classification of Sleep Disorders (ICSD) as well as an economic use of resources.

Abstract: Respiratory instability as well as tissue damage by free radicals (oxidative stress) have been hypothesized to play a role in cases of sudden and unexpected infant death in the first year of life. The ratio of the oxidized/reduced form of redox compounds in the circulation could be used as a marker of oxidative stress. Therefore, the sleep apnoea rate and redox status of coenzyme Q10 (CoQ10) (percentage of the oxidized form in total CoQ10) were measured in a population of clinically healthy infants in their first year of life in order to study whether a physiological parameter of respiratory instability is related to a biochemical parameter of oxidative stress. Between May and December 1999, 323 infants in the first year of life were referred to a paediatric sleep laboratory. Sleep apnoea rate, periodic breathing and parameters of oxygenation (SaO2 and TcPO2) were calculated based on polysomnographic recordings. The CoQ10 redox status was calculated based on high-pressure liquid chromatographic (HPLC) analysis. Statistical analysis showed an age-dependent decrease in apnoea rate (r=−0.38) and CoQ10 redox status (r=−0.40). An increased CoQ10 redox status (median: 16.6%; range: 7.3–29.7%) was found in infants with high apnoea rates above the 90th percentile of a reference group in comparison with infants with apnoea rates below the 90th percentile of a reference group (median: 10.4%; range: 5.1–20.4%; P=0.031). These findings may indicate that high apnoea rates are accompanied by increased formation of free radicals in clinically healthy infants in the first year of life.

Abstract: Das Phanomen des veranderten Bewusstseins im Schlaf hat in verschiedenen kulturgeschichtlichen Epochen entsprechend den Erkenntnismitteln der jeweiligen Zeit zu unterschiedlichen Deutungen gefuhrt. Am Beispiel des Schlafes sollten die geistesgeschichtlichen Wandlungen eines Alltagsphanomens dargestellt werden. Im Mythos wurde ein regelhafter Ablauf in der Natur auf ein gottliches Urgeschehen zuruckgefuhrt. Ein Erkenntnisprozess auf der Basis analytischen Denkens fand im Mythos nicht statt. Die im Zeitalter der Aufklarung vorgenommene scharfe Trennung von Subjekt und Objekt wurde von der romantischen Bewegung, die am Ende des 18. Jahrhunderts einsetzte, als ein Verlust der Ganzheit, der Einheit von Mensch und Natur empfunden. Im Sinne romantischer Chiffren standen “Nacht”, “Schlaf” und “Traum” fur Projektionsraume, in denen die ersehnte Ganzheit wiederhergestellt sein sollte. Die romantische Konzeption zielte auf eine Umdeutung der Realitat. Die Einlosung romatischer Postulate gelang allenfalls in Kunstwerken. Die Entwicklung der modernen Schlafforschung auf der Basis der Naturwissenschaften hatte zwei wesentliche Voraussetzungen: die Entdeckung der Zusammenhange zwischen Gehirn und Schlaf und die Methode der Elektroenzephalographie. Moderne Forschung entwickelt Modelle der Natur auf der Basis experimentell kontrollierter Beobachtungen. Auf diese Weise wurde das Wissen uber den Schlaf erheblich erweitert. Demnach ist der Schlaf ein dynamischer Vorgang, der durch “innere Uhren” gesteuert wird. Seine interne Regulation ist Ausdruck eines kontinuierlichen Prozesses, der uber Schlafen und Wachen hinweg verlauft.

Abstract: Bei Kindern mit obstruktivem Schlaf-Apnoe-Syndrom besteht haufig eine Hyperplasie der Adenoide und Tonsillen, so das sich ein Ansatzpunkt fur eine chirurgische Therapie ergibt. Wir berichten uber unsere Erfahrungen mit der pra- und postoperativen polysomnographischen Diagnostik in einem Zeitraum von 10 Jahren. Von 1989 bis 1998 wurden im Schlaflabor der Vestischen Kinderklinik Datteln, einem regionalen Zentrum fur schlafbezogene Storungen des Kindesalters, polysomnographische Schlafuntersuchungen bei 226 Kindern mit klinisch-anamnestischen Hinweisen auf OSAS durchgefuhrt. 62 (27%) Patienten erfullten die Kriterien fur die Aufnahme in die retrospektive Analyse (Polygraphie vor Therapie mit Bestatigung der Diagnose OSAS, chirurgische Therapie, postoperative polygraphische Kontrolle). Die Fallserie umfaste 38 Jungen und 24 Madchen im Alter von 0,3 bis 14,3 Jahren (Median 4,5 J.). Schnarchen und Mundatmung (100 %), Atempausen im Schlaf (81%), gestorter Schlaf (77%) und die subjektive Angabe “schweres Atmen” (53%) waren haufige anamnestische Symptome, die in der Regel uber einen langen Zeitraum bestanden. Die praoperative padiatrische und HNO-arztliche Diagnostik ergab bei 15 (24%) Patienten hyperplastische Adenoide, bei 17 (28%) hyperplastische Tonsillen und bei 30 (48%) eine Hyperplasie der Adenoide und Tonsillen. Bei 19 der 62 Patienten (31%) bestanden komplexe Krankheitsbilder. Es wurden bei 17 Patienten (28%) eine Adenotomie, bei 17 (28%) eine Tonsillektomie, bei 28 (45%) eine Adenotonsillektomie durchgefuhrt. Zusatzlich wurden bei 9 Patienten weitere Operationsverfahren durchgefuhrt. Die Polysomnographie zeigte postoperativ in der Gruppe eine signifikante Verbesserung der kardiorespiratorischen Parameter. Bei Kindern mit OSAS in Verbindung mit einer Hyperplasie des lymphatischen Rachenrings stellen Adenotomie und Tonsillektomie effektive Therapieverfahren dar. Die Indikationsstellung setzt eine sorgfaltige padiatrische und HNO-arztliche Diagnostik voraus, die die exakte Beschreibung des anatomischen und funktionellen Status zum Ziel hat. Der Schweregrad der Funktionsstorungen kann mit Hilfe der Polysomnographie quantifiziert werden.

46. Erkrankungen mit Schlafstörungen: Hypersomnie

Abstract: Verschiedene Krankheitsbilder konnen im Kindes- und Jugendalter mit dem Leitsymptom Hypersomnie einhergehen. Ausgehend von den physiologischen Grundlagen der Schlafregulation und den fur das Kindesalter geltenden Referenzwerten verschiedener Schlafparameter wird das methodische Vorgehen zur Abklarung von Hypersomniezustanden anhand von Kasuistiken dargestellt. Die Schlafmenge pro 24 Stunden nimmt im Mittel von 16 Stunden bei Neugeborenen auf 11 Stunden bei 5jahrigen ab. Die Verteilung der Schlafphasen innerhalb des 24-Stunden-Tages last ab der zweiten Halfte des ersten Lebensjahres eine Synchronisierung durch ausere Zeitgeber, erkennen. Die Haufigkeit kurzer Schlafperioden am Tage nimmt bis zum Alter von 5 Jahren deutlich ab. Hypersomnie kann Ausdruck einer primaren Storung der Schlaf-Wach-Regulation sein. Dies wird durch die Kasuistiken eines 7jahrigen Madchens mit Narkolepsie und eines Sauglings mit Prader-Willi Syndrom veranschaulicht. Zahlreiche Erkrankungen konnen eine sekundare Storung des Schlaf-Wach-Rhythmus zur Folge haben. Dies wird durch zwei weitere Kasuistiken illustriert. Bei einer 13jahrigen Patientin, die akut mit vermehrtem Schlafbedurfnis, Kopfschmerzen und Miktionsstorungen erkrankte, wurde als Ursache der Hypersomnie eine Entmarkungsenzephalitis diagnostiziert. Beim obstruktiven Schlaf-Apnoe-Syndrom kann es infolge der Schlaffragmentation zu einem vermehrten Schlafbedurfnis kommen. Hypersomnie bei Kindern erfordert eine sorgfaltige Abklarung, bei der eine Vielzahl von Differentialdiagnosen zu berucksichtigen ist. Ein auf der Basis einer polygraphischen Schlafuntersuchung erstelltes Hypnogramm kann dabei richtungweisende Befunde ergeben.

47. Polysomnographie bei aufmerksamkeitsgestörten und hyperaktiven Kindern (Attention Deficit Hyperactivity Disorder, ADHD)

Abstract: Bei aufmerksamkeitsgestorten und hyperaktiven Kindern ergeben sich anamnestisch hauf ge Klagen uber Schlafstorungen dieser Patienten. Dies gab den Anlas, den Schlaf von Patienten mit Aufmerksamkeitsdefizit-Hyperaktivitat-Syndrom (Attention Deficit Hypractivity Disorder, ADHD) zu untersuchen. Von Oktober 1996 bis Oktober 1998 wurden polygraphische Schlafuntersuchungen an 22 Patienten mit ADHD (20 Jungen, 2 Madchen, Lobensalter 8±2 Jahre) und 11 Vergleichskindern ohne ADHD (8 Jungen, 3 Madchen, Lobensalter 7,2±3,5 Jahre) durchgefuhrt. Bei 6 ADHD-Patienten wurde eine medikamentos Therapie mit Methylphenidat durchgefuhrt. Bei den Kindern der Vergleichsgruppe wurdie Polysomnographie zur Abklarung von Parasomnien durchgefuhrt. Die polygraphisch Schlafuntersuchung umfaste die Registrierung kardiorespiratorischer und neurophysiologischer Parameter. Auf der Grundlage von EEG, EOG und EMG wurde visuell die Klassilzierung der Schlafstadien unter Berucksichtigung der Richtlinien von Rechtschaffen un-Kales durchgefuhrt. Bei Kindern mit ADHD, die keine medikamentose Therapie erhielte wurden verschiedene Muster des gestorten Schlafes gefunden: Einschlaf- und Durchschla probleme sowie gestorte Schlafzyklen. Als Gruppe wiesen sie im Vergleich zu gesunde Gleichaltrigen eine verminderte Schlafzeit und eine verminderte Schlafeffizienz auf. Die Unterschiede zu Vergleichskindern konnten bei ADHD-Patienten, die mit Methylphenid behandelt wurden, nicht nachgewiesen werden. Bei Kindern mit ADHD gibt es Hinwei auf eine Neurotransmitterstorung im Bereich des „limitierten Kapazitatskontrollsystem des Gehirns. Diese im Wachen manifeste Storung kann durch zentral wirksame indirek Sympatomimetika beeinflust werden. Fur die Schlafregulation ist das Aktivitatsverhaltraminerger und cholinerger Neuronenpopulationen im Hirnstamm von Bedeutung. Dies Aktivitatsverhaltnis konnte bei Kindern mit ADHD verandert sein und zu verschieden Mustern des gestorten Schlafs bei diesen Kindern fuhren. Veranderungen objektiver Schlaparameter im Sinne einer verminderten Schlafqualitat sind mit dem ADHD assoziiert, jedoch liegt kein einheitliches Muster des gestorten Schlafes vor. Unter der medikamentos Therapie mit zentral wirksamen Stimulantien zur Beeinflussung der Aufmerksamkeitssirung kommt es auch zu einer Verbesserung der Schlafqualitat.

48. Intercorrelation of epidemiological and polysomnographical risk factors of sudden infant death

Abstract: Epidemiologische Erhebungen weisen Rauchen der Eltern und Stillverzicht als Risikofaktoren des plotzlichen Sauglingstodes aus. Aus klinisch-physiologischer Sciht gelten obstruktive Schlafapnoen und periodische Atmung als Risikomarker. So galt es, zu uberprufen, ob die genannten klinische-physiologischen Faktoren in einer abhangigen Beziehung zu den epidemiologischen stunden. Bei 302 Sauglingen wurden der Risikostatus erfast und eine Polysomnographische Untersuchung durchgefuhrt. Mit dem statistischen Verfahren der logistischen Regression wurden die moglichen Interkorrelationen uberpruft. Starkes Rauchen der Mutter wahrend der Schwangerschaft (≥10 Zigaretten/Tag) erwies sich als entscheidende Einflusgrose fur das Auftreten obstruktiver Schlafapnoen bei den Sauglingen. Kinder vielrauchender Mutter hatten im ersten Lebensjahr im Vergleich zu Kindern nicht rauchender Mutter ein etwa doppelt so hohes Risiko fur obstruktive Atempausen. Die Starke dieser Interkorrelation war altersabhangig, sie war im Alter von 14 bis 42 Tagen besonders hoch (O.R. 8.6, 95% CI 2.0;36.1, p=0.003). Das Auftreten periodischer Atmung wahrend mindestens 1% der Meszeit war bei gestillten Kindern von der 7. bis zur 10. Lebenswoche besonders haufig (O.R. 8.0, 95% CI 2.9;22.3, p=0.0001). Unabhangig vom Alter trat bei weiblichen Sauglingen periodische Atmung deutlich haufiger auf, als bei mannlichen (O.R. 2.3, 95% CI 1.4;3.9, p=0.002). Wir folgern, das starkes Rauchen der Schwangeren das Auftreten obstruktiver Apnoen bei ihren Kindern gerade fur den Zeitraum begunstigt, fur den die Epidemiologie den starksten Einflus solchen Verhaltens auf die SID-Inzidenz erweist. Die Auspragung periodischer Atmung wird von elterlichem Rauchen nicht verstarkt. Demgegenuber ist periodische Atmung positiv mit epidemiologischen Faktoren korreliert, die mit eher niedrigem SID-Risiko assoziiert sind. Dieser Befund stutzt die Auffassung, das periodische Atmung bei gesunden, reifgeborenen Sauglingen kein pathologisches Atemmuster darstellt.

49. Akute Osteomyelitis im Kindesalter

Abstract: Fragestellung: In der fruhen Krankheitsphase der Osteomyelitis ist die Sicherung der Diagnose schwierig, da charakteristische radiologische Befunde erst nach einigen Tagen auftreten. Daher wurde die Aussagekraft von Sonographie, Skelettszintigraphie und Kernspintomographie bei klinischer Manifestation einer akuten Osteomyelitis uberpruft. Methode: In den Jahren von 1980–1997 wurden 138 Patienten unter der Verdachtsdiagnose Osteomyelitis stationar aufgenommen, bei denen die Symptome weniger als 5 Tage bestanden. Die Aussagekraft von Sonographie, Skelettszintigraphie und Kernspintomographie wurde bei 66 Patienten mit gesicherter Osteomyelitis und 27 Patienten mit sicherem Ausschlus der Diagnose analysiert. Ergebnisse: Die Sonographie zeichnete sich durch eine grose Spezifitat (93%) und einen grosen positiv pradiktiven Wert (92%) bei geringer Sensitivitat (60%) und geringem negativ pradiktiven Wert (61%) aus. Die Skelettszintigraphie besas eine Sensitivitat von 80%, eine Spezifitat von 78%, einen negativ pradiktiven Wert von 68% und einen positiv pradiktiven Wert von 87%. Die groste Spezifitat (100%) wurde durch die Kernspintomographie erzielt (Sensitivitat 90%). Schlusfolgerung: Sonographien konnen die Verdachtsdiagnose weitgehend sichern, wahrend ein Ausschlus der Diagnose Osteomyelitis nicht moglich ist. Die Skelettszintigraphie eignet sich weder zur Sicherung noch zum Ausschlus der Verdachtsdiagnose. Differentialdiagnosen konnen v.a. durch die Kernspintomographie abgegrenzt werden. Da jedes bildgebende Verfahren Fehlinterpretationen zulast, kann die Diagnose einer Osteomyelitis nur durch Synopse aus klinischem Befund, Labordaten, Erregernachweis, Verlauf und bildgebender Diagnostik gestellt werden.

50. Lowe-Syndrom

Abstract: Die Kardinalsymptome des okulozerebrorenalen Syndroms (Lowe-Syndrom) sind kongenitale Katarakt, zerebrale Entwicklungsstorung, muskulare Hypotonie sowie tubulare Funktionsstorungen der Niere. Die Pathophysiologie ist unklar, vermutet wird eine Storung im Inositolphosphatstoffwechsel in Folge von Mutationen des auf dem X-Chromosom liegenden Gens. Wir berichten uber einen 4 Jahre alten Jungen, bei dem aufgrund der Familienanamnese, einer kongenitalen Katarakt, einer auffalligen Facies, einer muskularen Hypotonie, einer Entwicklungsverzogerung und einer tubularen Azidose die Diagnose eines Lowe-Syndroms gestellt wurde. Die augenarztliche Untersuchung der Mutter zeigte Linsentrubungen als Hinweis auf einen Konduktorinnenstatus. Die auf DNA-Ebene durchgefuhrte Mutationsanalyse ergab eine G-A-Transition an der Exon-Intron-Schnittstelle des vorletzten Exons im Gen. Diese Veranderung wurde bei der Mutter in heterozygoter Form nachgewiesen. Diskussion: Die Mutation, die aufgrund der besonderen Lokalisation neben dem Austausch einer Aminosaure ebenso das fehlerhafte Herausschneiden des Exons oder eine Verschiebung des Leserasters hervorrufen kann, ist die molekulargenetische Ursache der klinischen Auspragung des Lowe-Syndroms bei unserem Patienten.
