Complete benign biliary stenosis is a difficult-to-treat condition in which the common endoscopic instruments fail. Surgery is the standard treatment method with high morbidity and mortality rates. The magnetic compression anastomosis (MCA) technique is an emergent procedure allowing endoscopic treatment possible in such patients. While MCA application has not reached a standard yet, herein, we present the treatment of an 8-year-old girl with total biliary obstruction in Roux-en-Y hepaticojejunostomy anastomosis with magnetic compression anastomosis using a pediatric colonoscope. Giriş: Yoğun bakım ünitelerine (YBÜ) kabul edilen hastaların önemli bir kısmını enfeksiyon şüphesi olan hastalar oluşturmaktadır. Bu hastaların erken fark edilebilmesi ve prognozlarının belirlenmesi için Modifiye Erken Uyarı Skoru (MEUS), SIRS, SOFA ve qSOFA skorları kullanılmaktadır. Bu skorların, hastanın yoğun bakım ihtiyacı gelişmeden önce farklı zaman dilimlerine ait prognoz öngörü çalışmaları kısıtlıdır. Amaç: Çalışmamızda enfeksiyon şüphesi olan hastaların yoğun bakım yatışından 48, 24 ve 8 saat önceki ve yoğun bakım kabulü sırasındaki skor değerlerinin mortalite öngörülerinin değerlendirilmesi amaçlandı. Gereç-Yöntem: Çalışmaya 01.01.2018-31.05.2018 tarihleri arasında Hacettepe Üniversitesi Hastaneleri iç hastalıkları YBÜ, onkoloji YBÜ ve akut bakım ünitesine (1. basamak YBÜ eşdeğeri) acil veya yataklı servislerden kabul edilen ve YBÜ kabulü öncesindeki 48 saatlik periyotta enfeksiyon şüphesi bulunan 120 hasta dahil edildi. YBÜ kabulü öncesine ait vital bulgular hasta dosyasından retrospektif olarak, YBÜ yatışı esnasındaki bulgular ise prospektif olarak kaydedildi. Bulgular: Çalışmaya alınan 120 hastanın %44,2'si erkek idi. Ortalama yaş 66,3 ± 17,1 yıl olarak hesaplandı. Hastaların %75,8'i acil servisten, %24,2'si yataklı servislerden YBÜ'ye kabul edildi. Ortalama APACHE-II skoru 19,1 ± 8,0 bulundu. YBÜ kabulünden 48 saat önce hesaplanan SIRS >=2 ve SOFA >=2 olması ile hastane mortalitesi riskinde (%95 güven aralığı) sırasıyla 7,6 (1,5--38,0) ve 13,2 (2,3--74,3) kat; 28 günlük mortalite riskinde ise sırasıyla 11,9 (1,9--73,9) ve 16,2 (1,8--142,5) kat artış görüldü. 24 saat önce ölçülen skorlardan SOFA >=2 ve qSOFA >=2 olması ile hastane mortalitesi riskinde sırasıyla 14,2 (2,5--80,6) ve 2,9 (1,0-8,4); SIRS >=2 ve SOFA >=2 olması ile 28 günlük mortalite riskinde sırasıyla 6,8 (1,3--36,9) ve 5,5 (1,1--27,5) kat artış görüldü. 8 saat önce gözlenen SOFA >=2 ve qSOFA >=2 olması ile hastane mortalitesi riskinde sırasıyla 18,3 (2,2--151,1) ve 3,9 (1,4--11,0) kat; 28 günlük mortalite riskinde ise sırasıyla 9,0 (1,1--72,4) ve 3,5 (1,2--10,1) kat artış görüldü. YBÜ kabulü öncesine ait en yüksek skorların eğri altında kalan alan değerleri hastane mortalitesine göre değerlendirildiğinde SOFA için 0,80 (0,72-0,89), MEUS için 0,66 (0,54-0,76), qSOFA için 0,63 (0,51-0,74) ve SIRS için 0,61 (0,49-0,73) bulundu. Skor eşik değerlerine ait duyarlılık ve özgüllük verileri Tablo.1'de görüldüğü gibi hesaplandı. Sonuç: Hastanede yatan ve enfeksiyon şüphesi ile YBÜ izlem ihtiyacı gelişmiş olan hastalarda mortalite öngörmede YBÜ kabulünden 48 saat öncesinde duyarlılığı en yüksek SOFA, özgüllüğü en yüksek qSOFA skorudur. Background/aim: In this study, we aimed to assess the cancer risk among patients with periodontal disease., Materials and Methods: Patients diagnosed with periodontal diseases at Hacettepe University between 2007 and 2012 were included and data on the diagnosis of any cancer after periodontal disease were collected from patient files. The age- and sex-standardized incidence rates (SIRs) were calculated using Turkish National Cancer Registry 2013 data., Results: A total of 5199 patients were included. Median follow-up was 7.2 years. Patients with periodontal diseases had 17% increased risk of cancer compared with the expected counts for the corresponding age and sex groups (SIR: 1.17; 95% CI: 1.04–1.3, P = 0.006). The increased cancer risk was statistically significant in women (SIR: 1.24; 95% CI: 1.05–1.45, P = 0.008) but not in men. Among women with periodontal disease, the risks of breast cancer (SIR: 2.19) and head and neck cancer (SIR: 4.71) were significantly increased. Among men, the risks of prostate cancer (SIR: 1.84), head and neck cancer (SIR: 3.55), and hematological cancers (SIR: 1.76) were significantly increased., Conclusion: This study showed that periodontal diseases were associated with increased risk of several cancers.
