6. Essure®-sterilisatieveertjes. Een literatuuronderzoek

Abstract

RIVM rapport:Essure® is een implantaat om vrouwen te steriliseren. Kleine metalen veertjes worden in de eileiders gezet, waardoor ze dichtgroeien. Sommige vrouwen met Essure® melden gezondheidsklachten. Bijvoorbeeld pijn, vermoeidheid, hevige bloedingen en geheugenverlies. Soms zijn de klachten zo ernstig dat de gynaecoloog het implantaat verwijdert. In Nederland hebben sinds 2002 naar schatting 30.000 vrouwen dit implantaat gekregen. Sinds juli 2017 wordt Essure® niet meer in Nederland verkocht. Het RIVM onderzocht of metalen die uit Essure® kunnen vrijkomen mogelijk schadelijk zijn voor de gezondheid. Gekeken is wat hierover in de wetenschappelijk literatuur bekend is en zijn experts en interne deskundigen bevraagd. Het is onwaarschijnlijk dat deze metalen schadelijk zijn voor de gezondheid, concludeert het RIVM. Veel implantaten, zoals heupimplantaten en stents, zijn net als Essure® gemaakt van metalen. Als metaal in het lichaam wordt gebracht, is het normaal dat daar metaaldeeltjes uitkomen. Doordat Essure® een klein implantaat is, gaat het om kleine hoeveelheden metalen. Verder blijkt dat er weinig wetenschappelijk onderzoek naar is gedaan. De literatuur die er is, wijst er niet op dat deze metalen schadelijke effecten voor de gezondheid kunnen hebben. Grotere studies met een goede studieopzet zijn nodig om een verband tussen metalen uit Essure® en deze gezondheidsklachten echt uit te sluiten. Het RIVM heeft ook gekeken naar de mogelijkheden om Essure® te verwijderen en of daarmee de klachten verminderen. De klachten blijken in dezelfde mate te verminderen bij de verschillende operatietechnieken om Essure® te verwijderen. Volgens experts is er daarom geen reden om de behandeling in Nederland aan te passen. In Nederland worden bij voorkeur beide eileiders met de Essure®-implantaten weggehaald. Dat is naar schatting bij ruim 4000 vrouwen in Nederland gedaan. Dit onderzoek is in opdracht van de Inspectie Gezondheidszorg en Jeugd (IGJ) uitgevoerd., Essure® is an implant used for female sterilisation. Small metal coils are placed in each of the fallopian tubes to induce closure of the fallopian tubes. Some women with Essure® have reported health complaints such as pain, fatigue, heavy bleeding, and memory loss. Sometimes the complaints are serious in such a way that a gynaecologist removes the implants. Since 2002, an estimated 30,000 women have received this implant in the Netherlands. Since July 2017, Essure® has not been sold in the Netherlands. RIVM has investigated if metals released from Essure® could be harmful to women's health. It reviewed existing scientific literature and consulted experts and in-house specialists on the subject. It is unlikely that these metals are harmful to women's health, concludes RIVM. Many implants, like hip implants and stents, are made out of metals, like Essure®. The introduction of metals into the body typically results in the release of metal particles. As Essure® is a small implant, the amounts of metals released are minimal. Additionally, the investigation highlighted a lack of extensive scientific research in this area. The available literature does not indicate that these metals could be harmful to health. Larger studies with robust study designs are needed to definitively rule out a connection between metals released from Essure® and the reported health complaints. The RIVM investigated the possibilities of removing Essure® and whether this reduces the health complaints. The health complaints decrease to the same extent with the various surgical techniques to remove Essure®. According to experts, there is no reason to modify the current treatment approach. In the Netherlands, it is the preference to remove both fallopian tubes with the Essure® implants. In the Netherlands an estimated 4000 women have had Essure® removed. This investigation was commissioned by the Health and Youth Care Inspectorate.

7. Influence of copper source and dietary inclusion level on growth performance of weaned pigs and expression of trace element related genes in the small intestine

Abstract

Copper is routinely supplemented to weanling pig diets at concentrations above nutritional requirements to enhance growth performance. We hypothesised that this effect depends on the source of Cu and its dietary concentration. We tested this in weaned pigs (26 d of age) over a 35-d period using a 2 × 3 factorial arrangement with two Cu-sources (CuSO4 and Cu2O, monovalent copper oxide, CoRouge®) and three supplementary dietary Cu-levels (15, 80 and 160 mg Cu/kg) as respective factors. Increasing Cu level linearly increased (P < 0.001) final BW and daily gain. These effects tended (P = 0.09) to be greater with Cu2O than CuSO4. Feed conversion ratio decreased linearly (P < 0.001) with increasing dietary Cu content, independent of Cu source. Plasma Cu, Zn and Fe levels were unaffected, whereas liver Cu content increased quadratically (P < 0.001) with increasing dietary Cu content, with a larger increase (P < 0.001) with CuSO4 than Cu2O. Bile Cu content increased quadratically (P = 0.025) with increasing Cu content, irrespective of Cu source. RT-qPCR analysis revealed that increasing Cu content quadratically (P = 0.009) increased duodenal but not ileal metallothionein 1A (MT1A) mRNA, with greater effect (P = 0.010) of CuSO4. Regardless of the Cu source, increasing Cu dose linearly increased (P = 0.006) duodenal DMT1/SLC11A2 mRNA but decreased ZIP4/SLC39A4 mRNA in duodenum (P < 0.001) and ileum (P < 0.005). ZnT10/SLC30A10 mRNA was significantly (P = 0.021) and numerically (P = 0.061) greater with Cu2O compared to CuSO4, in duodenum and ileum, respectively. Copper content quadratically modulated duodenal but not ileal transferrin receptor (P = 0.029) and ferric reductase CYBRD1 mRNA (P = 0.022). In hypothalamus, high Cu dose (P = 0.024) and Cu2O as source (P = 0.028) reduced corticotropin-releasing hormone (CRH) mRNA. Abstract

Item does not contain fulltext BACKGROUND: Present theory is that uncomplicated and complicated appendicitis are different entities. Recent studies suggest it is safe to delay surgery in patients with uncomplicated appendicitis. We hypothesize that patients with complicated appendicitis are at higher risk for postoperative complications when surgery is delayed. METHODS: Data was used from the multicenter, prospective SNAPSHOT appendicitis study of 1975 patients undergoing surgery for suspected appendicitis. Adult patients (≥ 18 years) who underwent appendectomy for appendicitis were included in this study. The primary outcome was the difference in postoperative complications between patients with complicated appendicitis who were operated within and after 8 h after hospital presentation. Secondary outcomes were the incidence of both uncomplicated and complicated appendicitis in relationship to delay of appendectomy. Follow-up was 30 days. A multivariable analysis was performed. RESULTS: Of 1341 adult patients with appendicitis, 34.3% had complicated appendicitis. In patients with complicated appendicitis, 22.8% developed a postoperative complication compared to 8.2% for uncomplicated appendicitis (P 8 h) increased the complication rate in patients with complicated appendicitis (28.1%) compared to surgery within 8 h (18.3%; P = 0.01). Multivariate analysis showed a delay in surgery as an independent predictor for a postoperative complication in patients with complicated appendicitis (OR 1.71; 95%CI 1.01-2.68, P = 0.02). CONCLUSION: In-hospital delay of surgery (> 8 h) in patients with complicated appendicitis is associated with a higher risk of a postoperative complication. It is important that we recognize and treat these patients early.

Abstract

Purpose For the diagnosis of acute appendicitis, the combination of clinical and laboratory variables achieves high diagnostic accuracy. Nevertheless, appendicitis can present with normal laboratory tests of inflammation. The aim of this study was to investigate the incidence of normal inflammatory markers in patients operated for acute appendicitis. Methods This is an analysis of data from a prospective, multicentre SNAPSHOT cohort study of patients with suspected acute appendicitis. Only patients with histopathologically proven acute appendicitis were included. Adult patients with acute appendicitis and normal preoperative inflammatory markers were explored further in terms of abdominal complaints, preoperative imaging results and intraoperative assessment of the degree of inflammation and compared to those with elevated inflammatory markers. Results Between June and July 2014, 1303 adult patients with histopathologically proven acute appendicitis were included. In only 23 of 1303 patients (1.8%) with proven appendicitis, both preoperative white blood cell count and C-reactive protein levels were normal. Migration of pain was reported less frequently in patients with normal inflammatory markers compared to those with elevated inflammatory marker levels (17.4% versus 43.0%, p = 0.01). Characteristics like fever, duration of symptoms and localized peritonitis were comparable. Only 4 patients with normal inflammatory markers (0.3% overall) had complicated appendicitis at histopathological evaluation. Conclusion Combined normal WBC and CRP levels are seen in about 2 per 100 patients with confirmed acute appendicitis and can, although rarely, be found in patients with complicated appendicitis.

Abstract

Introduction In patients treated with an appendectomy for acute appendicitis, the specimen is generally sent for histological evaluation. In an era of increasing non-operative treatment for acute appendicitis, it is important to know the incidence, the diagnostic accuracy, and treatment consequences of appendicular neoplasms that are found in acute appendicitis. We hypothesize that pre- and intra-operative parameters might predict an appendicular neoplasm. Methods Data was used from our previous prospective observational cohort study. All patients undergoing surgery for suspected acute appendicitis were included. The primary outcome was the incidence of appendicular neoplasms in patients operated for acute appendicitis. Secondary outcomes were pre-operative diagnostics and imaging outcomes, intra-operative surgical judgment, and postoperative management and outcome. Possible predictors of an appendicular neoplasm were identified and used in multivariable logistic regression. Patients with an appendicular neoplasm were followed for 3 years after initial appendectomy. Results A total of 1975 patients underwent surgery for suspected acute appendicitis and in 98.3% (1941/1975) the appendix was removed. In 1.5% (30/1941) of these patients, an appendicular neoplasm was found. Among the malignant neoplasms, the majority were grade 1 neuroendocrine tumors (NET) in 65% (13/20). On pre-operative imaging, there was no suspicion of malignancy. In three cases, there was an intra-operative suspicion of malignancy. Multivariable analysis showed only age as an independent predictor for appendicular neoplasms. No recurrent or new malignancy was found during follow-up. Discussion The incidence of appendicular neoplasm in patients undergoing an acute appendectomy is very low and clinical risk factors could not be identified.

Abstract

Sommige behandelingen in schoonheidssalons brengen risico's met zich mee, waardoor bijvoorbeeld roodheid, zwelling of littekenvorming kan ontstaan. Het is onduidelijk of en hoe vaak dergelijke effecten optreden. Volgens veldpartijen uit de schoonheidsbranche ontstaan risico's vooral door onbekwaam handelen van de behandelaar. Maatregelen om nadelige effecten van risicovolle behandelingen te beperken zouden dan ook vooral gericht moeten zijn op het handelen en/of de opleiding van de behandelaar. Om te kunnen bepalen of maatregelen daadwerkelijk nodig zijn, is het nodig dat nadelige effecten worden gemeld of geregistreerd. Dit blijkt uit een inventarisatie van het RIVM, waarin is bekeken welke behandelingen in schoonheidssalons risicovol zijn en wat de risico's inhouden. De Inspectie Gezondheidszorg en Jeugd (IGJ) wil daar inzicht in krijgen, omdat zij sinds de introductie van de Wet kwaliteit, klachten en geschillen zorg (Wkkgz) in 2016 verantwoordelijk is voor het toezicht op schoonheidssalons. Risicovolle behandelingen in schoonheidssalons zijn behandelingen die door de huid heen gaan of de huid (al dan niet bedoeld) beschadigen, of de samenhang van het huidweefsel verbreken. Voorbeelden zijn behandelingen die huidcellen chemisch (peelings) of mechanisch (zoals microdermabrasie) verwijderen of die door de huid heen gaan (bijvoorbeeld microneedling). De meeste nadelige effecten ontstaan kort na de behandeling (binnen 7 dagen) en houden kort aan (minder dan 7 dagen). Nadelige effecten die langer dan een maand aanhouden zijn bijvoorbeeld littekenvorming en veranderingen in de pigmentatie van de huid, zoals overmatige pigmentvorming. Nadelige effecten die kunnen optreden, zijn in grote lijnen terug te voeren op kenmerken van de behandeling (zoals diepte en frequentie), kenmerken van de cliënt (bijvoorbeeld huidtype), en bekwaamheid van de behandelaar (zoals kennis en ervaring). Het risico van behandelingen hangt grofweg samen met de diepte van de beschadiging van de huid die bij de behandeling wordt aangericht, en de mate van controle die de behandelaar hierover heeft.

This report describes a study in growing pigs to determine the influence of dietary calcium content and phytase inclusion on digestion, absorption, retention and excretion of calcium and phosphorus, degradation of dietary phytase and released isomers, the absorption from different gut segments, mRNA expression of transporters and other proteins involved in the digestive tract and the kidney, and potential regulatory mechanisms.

37. New insights into porcine/poultry calcium/phosphorus metabolism and nutrition

Abstract

Improving dietary phosphorus (P) absorption and retention has been intensively studied in pig and poultry due to the low digestibility of inositol phosphate (IP), global mineral P scarcity and increasing public concerns over environment pollution. The main aim of this thesis was to provide novel insights into calcium (Ca) and P absorption, metabolism and retention in pigs and poultry.The impact of dietary Ca content with or without microbial phytase supplementation on Ca and P absorption/solubilization in different digestive tract segments was studied in pigs. Increasing dietary Ca content reduced apparent P digestibility in all digestive tract posterior to stomach, and this impact was greater for the phytase supplemented diets in the distal ileum. Moreover, incremental dietary Ca content reduced P solubility, accompanied with a reduced IP6 degradation in the distal ileum. Thus dietary Ca reduced P absorption via reducing IP6 degradation and precipitation of P, which was more pronounced in microbial phytase included diets. Furthermore, a high dietary Ca content reduced mRNA expression of Ca-related transporters in the colon and jejunum, while it increased mRNA expression of Ca-related claudins in the jejunum. These results indicated that increasing dietary Ca supply switched Ca absorption from transcellular to paracellular pathway. In addition to that, dietary Ca reduced mRNA expression of Ca-related transporters and claudins in the kidney, suggesting that kidney played a central role in Ca and P homeostasis.In contrast to pigs, the negative impact of dietary Ca on P absorption was less pronounced in broilers fed diets supplemented with microbial phytase. Microbial phytase supplementation also increased Ca absorption particularly in the low-Ca diets. However, this positive impact was insufficient to restore the negative effect of Ca deficiency on growth performance or tibia breaking strength. Abstract

Since 2002 synthetic mesh implants are used to treat patients with pelvic organ prolapse. Because of serious complaints, measures were taken in the Netherlands in 2011-2012. Since then mesh products are only implanted if alternative treatments such as physiotherapy, a vaginal ring and an operation using the body's own tissue were not effective to treat pelvic organ prolapse. In addition, mesh implantation may only take place in a limited number of specialized centers by well-trained recognized specialists. This is because mesh implantation requires experience and precision. International scientific literature was examined by National Institute for Public Health and the Environment (RIVM) to determine complications that can occur one year or longer after mesh implantation. Complications that were observed in literature were: pain, mesh exposure and erosion, incontinence and pain during intercourse. In addition, a prolapse can recur, for example in another area then where the mesh was implanted. The complication rates varied widely in the literature. Additionally, data on the duration and severity of a complication was limited. This variation and the limited data can partially be attributed to the lack of an unambiguous, international inventory of complications. There is a lot of attention for mesh implants in the international media. Complaints reported to the Dutch Health and Youth Care Inspectorate between 2009 and 2012 demonstrated the occurrence of serious complications. For these reasons, RIVM is calling for a standardized guideline with universal definitions to facilitate the reporting of the complications of mesh implants for pelvic organ prolapse. In the meantime, newly developed mesh implants entered the market, that are expected to have less complications. In this literature study, identified complications were primarily associated with products that are no longer available on the Dutch market.
