Objectif : Les tumeurs rétropéritonéales regroupent l’ensemble des tumeurs primitives qui naissent dans l’espace rétro et sous-péritonéal. L’imagerie occupe une place prépondérante dans le diagnostic et le suivi de ces tumeurs. Le but de notre travail était d’étudier la concordance radio-histologique des tumeurs rétropéritonéales de l’enfant. Matériel et Méthodes : Il s’agissait d’une étude transversale réalisée sur 09 ans (Février 2010 à Avril 2019) portant sur 132 enfants colligés dans les services d’Imagerie médicale et d’Oncopédiatrie de l’Hôpital Aristide Le Dantec (HALD). Les patients avaient été explorés par échographie et tomodensitométrie. Tous les patients avaient bénéficié du même protocole au scanner. Résultats : Plusieurs types histologiques étaient observés : Le néphroblastome (83%), le carcinome rénal à cellules claires (4%), le sarcome rénal à cellules claires (4%), le néphrome mésoblastique congénital (1%), le néphrome kystique (2%), l’angiomyolipome (1%), le neuroblastome (9%) et le lymphome de Burkitt (1%). Le néphroblastome était la tumeur la plus fréquente. Le neuroblastome était la tumeur qui donnait le plus de métastases. La sensibilité de l’échographie était de 100% pour le néphroblastome et le lymphome. La sensibilité du scanner était de 100% pour toutes les tumeurs sauf le sarcome rénal à cellules claires. Une concordance radio-histologique était observée dans 75% des cas. Conclusion : Les tumeurs rétropéritonéales sont largement dominées par le néphroblastome pour lequel le couple échographie-scanner possède une sensibilité de 100%. English title & abstract Iconographic review and radiohistological concordance of retroperitoneal tumors in children Objective : Retroperitoneal tumors include all the primary tumors that arise in the retro and sub peritoneal space. Imaging plays a major role in the diagnosis and monitoring of these tumors. The aim of our work was to study the radio-histological concordance of retroperitoneal tumors in children. Material and Methods: This was a cross-sectional study carried out over 09 years (February 2010 to April 2019) involving 132 children collected in the medical imaging and pediatric oncopediatrics departments of Aristide Le Dantec Hospital (HALD). The patients had been explored by ultrasound and computed tomography. All the patients had benefited from the same protocol on the scanner. Results: Several histological types were observed: Nephroblastoma (83%), renal clear cell carcinoma (4%), renal clear cell sarcoma (4%), congenital mesoblastic nephroma (1%), cystic nephroma (2%), angiomyolipoma (1%), neuroblastoma (9%) and Burkitt's lymphoma (1%). Nephroblastoma was the most common tumor. Neuroblastoma was the tumor that gave the most metastases. The sensitivity of the ultrasound was 100% for nephroblastoma and lymphoma. The sensitivity of the CT scan was 100% for all tumors except renal clear cell sarcoma. A radio-histological agreement was observed in 75% of cases. Conclusion: Retroperitoneal tumors are largely dominated by nephroblastoma for which the ultrasound-scanner pair has a sensitivity of 100%. Keywords: Imaging, tumor

Objectifs : Étudier l’impact de la protrusion prostatique intra vésicale sur les troubles urinaires dans une population de patients âgés de plus de 40 ans au service de radiologie générale d’un hôpital de niveau 3 en Afrique sub-saharienne. Matériel et Méthodes : Il s’agissait d’une étude prospective, descriptive, transversale réalisée sur une période de 09 mois allant de mars 2019 à novembre 2019. Nous avons inclus, tous les patients ayant une protrusion prostatique intra vésicale liée à une hypertrophie bénigne de la prostate et ayant donné leur consentement pour participer à l’étude. Nous avons colligé 40 dossiers. L’âge moyen était de 66,43 ans avec des extrêmes de 49 et 90 ans. L’analyse statistique de nos résultats a été réalisée avec les logiciels SPSS 20.0. Résultats : L’échographie a trouvé un volume prostatique entre 25-50 ml chez 18 patients (45%), ≥ 50-100 ml chez 16 patients (40%) et ≥ 100 ml chez 6 patients (15%). Un indice de protrusion prostatique grade 3 était observé chez 35 patients (87,5%). Cinq patients (12,5%) avaient un IPP de grade 2. Aucun cas d’indice de protrusion prostatique de grade 1 n’a été observé. Chez 14 patients, nous avons trouvé une vessie de lutte avec un épaississement pariétal diffus. Aucun cas de retentissement rénal n’a été observé. Un patient hypertendu mal suivi présentait une dédifférenciation cortico-médullaire. L’analyse comparative des moyennes n’a pas noté d’association entre l’IPP et le VP (P- value 0 ,176). Une association significative a été trouvée entre l’IPP et l’International Prostate Symptom Score (P-value = 0,006) et entre l’indice de protrusion prostatique et le Q max (P-value =0 ,035). Conclusion : L’indice de protrusion prostatique est un bon marqueur de la sévérité des symptômes urinaires. Il doit être évalué lors des examens échographiques chez les sujets âgés présentant un tableau de prostatisme. Objectives: To study the impact of intravesical prostatic protrusion on urinary disorders in a population of patients aged over 40 years in the general radiology department of a level 3 hospital in sub-Saharan Africa. Material and Methods: This was a prospective, descriptive, cross-sectional study conducted over a 09-month period from March 2019 to November 2019. We included, all patients with intravesical prostatic protrusion related to BPH who gave their consent to participate in the study. We collected 40 records. The mean age was 66.43 years with extremes of 49 and 90 years. Statistical analysis of our results was performed with SPSS 20.0 software. Results: Ultrasound found prostatic volume between 25-50 ml in 18 patients (45%), ≥ 50-100 ml in 16 patients (40%) and ≥ 100 ml in 6 patients (15%). A grade 3 prostatic protrusion index was observed in 35 patients (87.5%). Five patients (12.5%) had a grade 2 protrusion prostatic index. No cases of grade 1 PPI were observed. In 14 patients, we found a struggle bladder with diffuse parietal thickening. No cases of renal involvement were observed. One poorly followed hypertensive patient had cortico-medullary dedifferentiation. Comparative analysis of means did not find an association between protrusion prostatic index and Prostate volume (P-value 0 .176). A significant association was found between prostatic protrusion index and the international prostate symptom score (P-value = 0.006) and between protrusion prostatic index and Q max (P-value =0 .035). Conclusion: The prostatic protrusion index is a good marker of the severity of urinary symptoms. It should be evaluated during ultrasound examinations in elderly subjects presenting with a prostatic picture.

Objectifs : partager notre expérience sur le scanner cervical pour la suspicion clinique du syndrome d’Eagle.Matériels et méthodes : revue iconographique sur une période de 03 ans allant du 01 aout 2015 au 01 août 2018 incluant 43 patients, reçus dans le service de radiologie et imagerie médicale de l’hôpital régional de Diourbel dans un contexte de suspicion de syndrome d’Eagle chez qui nous avons réalisé une TDM cervicale sans et avec injection de produit de contraste. Les paramètres étudiés étaient la longueur du processus styloïde (normale inférieure à 25 mm), la topographie, le type selon la classification de Langlais et la présence de complications vasculaires. Les données ont été analysées avec les logiciels SPSS 24 et Excel 2016.Résultats : L’âge moyen était de 38 ans et le sex-ratio de 13,3 en faveur des femmes. Les résultats de la TDM étaient anormaux chez 30 patients (70%). La longueur moyenne des élongations du processus styloïde était de 37,9 mm. Elle était bilatérale chez 23 patients (77 %), unilatérale droite chez 4 patients (13%) et unilatérale gauche chez 3 patients (10%). Le type 1 selon la classification de Langlais était trouvé chez 21 patients (70%) suivi du type 3 chez 8 patients (27%). Aucune complication vasculaire n’a été retrouvée.Conclusion : la tomodensitométrie a permis de poser le diagnostic de syndrome d’Eagle et de préciser le type. Objectives: share our experience on the cervical scanner for the clinical suspicion of Eagle syndrome. Materials and methods: iconographic review over a period of 03 years from 01 August 2015 to 01 August 2018 including 43 patients, received in the radiology and medical imaging department of the regional hospital of Diourbel in a context of suspicion of Eagle syndrome in whom we performed a cervical CT scan without and with injection of contrast agent. The parameters studied were the length of the styloid process (normal less than 25 mm), topography, type according to the Langlais classification and the presence of vascular complications. The data were analysed using SPSS 24 and Excel 2016 software. Results: The average age was 38 and the sex ratio was 13.3 in favour of women. CT scan results were abnormal in 30 patients (70%).The average length of the styloid process elongations was 37.9 mm. It was bilateral in 23 patients (77%), unilateral right in 4 patients (13%) and unilateral left in 3 patients (10%). Type 1 according to the Langlais classification was found in 21 patients (70%) followed by type 3 in 8 patients (27%). No vascular complications were found. Conclusion: The CT scan made it possible to diagnose Eagle syndrome and to specify the type.

Objectifs : Rechercher les étiologies des DAANT chez l’enfant, évaluer la sensibilité des différentes techniques d’imagerie et proposer une stratégie d’exploration des DAANT.Matériels et méthodes : Etude transversale descriptive portant sur 59 dossiers d’enfants adressés au service de radiologie de l’HRSL pour exploration d’une douleur abdominale aigue non traumatique sur une période de 5 mois allant du 20 février 2020 au 20 juillet 2020. Nous avions inclus tous les patients d’âge inférieur ou égal à 15 ans qui avaient bénéficié d’un examen d’imagerie motivé par une douleur abdominale aigue et avaient eu un diagnostic final de certitude retenu après traitement médical et/ou chirurgical. Les paramètres étudiés étaient le sexe, l’âge, la topographie de la douleur, les signes cliniques associés, les données biologiques, le diagnostic clinique, le diagnostic de l’imagerie et le diagnostic final retenu. Les résultats de l’imagerie ont été comparés aux données cliniques et au diagnostic final retenu. Une saisie simple des textes et des tableaux a été faites sur Excel et Word de Microsoft Office 2016. Le test exact de Fisher a également été appliqué avec un seuil de significativité fixé à 5%.Résultats : L’âge moyen de la série était de 10 ans avec des extrêmes de 45 jours et de 15 ans (âge médian de 11 ans) et une prédominance du sexe masculin (genre-ratio de 1,36). La douleur abdominale était diffuse dans 11,9% des cas et localisée au niveau de la FID dans 49,1% des cas. Les signes cliniques associés étaient dominés par la fièvre (42%). Les signes biologiques étaient dominés par l’hyperleucocytose. Tous les patients avaient bénéficié d’une imagerie médicale. L’échographie était réalisée seule chez 79,7% des patients. La TDM était réalisée seule chez 8,5% et couplée à l’échographie chez 11,8% des patients. Les étiologies de DAANT étaient dominées par la pathologie appendiculaire (54,2%), suivie de suspicion d’adénolymphite mésentérique (16,9%), des abcès hépatiques (6,8%) et des IIA (5,1%). La sensibilité globale du diagnostic clinique initial par rapport au diagnostic établi par l’imagerie était de 50%. La sensibilité globale de l’imagerie par rapport au diagnostic final retenu était de 98,3%. La sensibilité de l’échographie par rapport au diagnostic retenu final était de 100%. La sensibilité du scanner par rapport au diagnostic retenu final était de 91,7% et sa sensibilité par rapport aux diagnostics finaux d’appendicite simple ou compliquée et de l’abcès hépatique était de 100%.Conclusion : Les étiologies de douleur abdominales aigues non traumatiques de l’enfant sont dominées par la pathologie appendiculaire. L’échographie est très sensible permettant une orientation diagnostique et une prise en charge adéquate dans l’immense majorité des cas. Objectives: Research the etiologies of acute non-traumatic abdominal pain (ANTAP) in children, assess the sensitivity of different imaging modalities and propose an exploration strategy for ANTAPMaterials and Methods: Prospective and descriptive study of 59 cases of children referred to the radiology department of the Saint-Louis hospital for exploration of acute nontraumatic abdominal pain over a period of 5 months from February 20, 2020 to July 20, 2020. We included all patients under 15 years of age who had undergone imaging for acute abdominal pain and had a definitive final diagnosis retained after medical and / or surgical treatment. The parameters studied were sex, age, pain topography, associated clinical signs, laboratory data, clinical diagnosis, imaging diagnosis and final diagnosis retained. The imaging results were compared with clinical data and the final diagnosis. Simple entry of texts and tables was done on Excel and Word from Microsoft Office 2016. Fisher's exact test was also applied with a significance level set at 5%.Results: The mean age of the series was 10 years with extremes of 45 days and 15 years (median age of 11 years) and predominance of male (gender-ratio of 1.36). Abdominal pain was diffuse in 11.9% of cases and localized at the level of the iliac fossa in 49.1% of cases. The associated clinical signs were dominated by fever (42%). Laboratory signs were dominated by leukocytosis. All patients had received medical imaging. Ultrasound was performed alone in 79.7% of patients. CT was performed alone in 8.5% and combined with ultrasound in 11.8% of patients. The etiologies of ANTAP were dominated by appendicular pathology (54.2%), followed by suspicion of mesenteric lymphadenitis (16.9%), hepatic abscesses (6.8%) and AII (5.1%). The overall sensitivity of the initial clinical diagnosis compared to the diagnosis established by imaging was 50%. The overall sensitivity of the imaging compared to the final diagnosis was 98.3%. The sensitivity of the ultrasound to the final diagnosis was 100%. The sensitivity of the CT scan compared to the final diagnosis was 91.7% and its sensitivity compared to the final diagnoses of simple or complicated appendicitis and liver abscess was 100%.Conclusion: The etiologies of acute non-traumatic abdominal pain in children are dominated in children by appendicular pathology. Ultrasound is very sensitive allowing diagnostic guidance and adequate management in the vast majority of cases.

Objectifs : rechercher les étiologies des lombalgies à la tomodensitométrie.Matériels et méthode : il s’agissait d’une étude transversale, prospective sur une période de 04 mois (07 Avril au 07 Aout 2018) incluant 67 patients (43 hommes et 24 femmes). L’âge moyen était de 49,58 ans. Les paramètres étudiés étaient les anomalies osseuses, discales et des parties molles non discales, analysées avec les logiciels SPSS 22.0 et CS-ENTRY 7.0 avec une p-value significative de 0,02.Résultats : les examens TDM étaient anormaux dans 97% des cas. on notait une étroitesse canalaire était notée chez 26,9% des patients, une arthrose corporéale chez 43,3%, une arthrose des articulaires postérieures chez 44,8% et une lyse isthmique chez 4,5% des patients. Une anomalie discale était trouvée chez 94% des patients, dominée par la hernie discale (61,9%) suivie du vide discal (23,8%) et du bombement global (14,3%). Ces anomalies discales siégeaient à l’étage L4-L5 dans 41,2 % et un conflit disco-radiculaire était noté chez 46 patients soit 69,7 %, le siège de ce conflit était canalaire dans 73,9%. Une hypertrophie des ligaments jaunes et une raréfaction de la graisse épidurale étaient notées chez 7 patients soit 10,4% des cas.Conclusion : la tomodensitométrie avait permis de diagnostiquer les différentes pathologies responsables de lombalgie chez 97% des patients, dominées par les anomalies discales. Objectives: to investigate the etiologies of low back pain on CT scan.Materials and method: it was a cross-sectional, prospective study over a period of 04 months (07 April to 07 August 2018) including 67 patients (43 men and 24 women). The mean age was 49.58 years. The parameters studied were bone, disc and non-discal soft tissue abnormalities, analysed with SPSS 22.0 and CS-ENTRY 7.0 software with a significant p-value of 0.02.Results: CT scans were abnormal in 97% of cases. Narrow lumbal canal was noted in 26.9% of patients, corporal osteoarthritis in 43.3%, osteoarthritis of the posterior joints in 44.8% and isthmic lysis in 4.5% of patients. A disc anomaly was found in 94% of patients, dominated by disc herniation (61.9%) followed by disc emptying (23.8%) and overall bulging (14.3%). These disc abnormalities were seated in the L4-L5 stage in 41.2% and a disco-radicular conflict was noted in 46 patients (69.7%), the seat of this conflict was medullary canal in 73.9%. Hypertrophy of the yellow ligaments and thinning of the epidural fat was noted in 7 patients, i.e. 10.4% of cases.Conclusion: CT scan had allowed to diagnose the different pathologies responsible for low back pain in 97% of the patients, dominated by disc anomalies.

Bacterial pneumonias are acute lower respiratory infections related to an inflammation of the pulmonary parenchyma determining a radio-clinical syndrome of lobar or segmental condensation [1]. Frequent reasons for hospitalization in pediatric settings, their distribution is worldwide with a high prevalence in Africa. Streptococcus Pneumoniae (STO.P) or Pneumococcus first infectious cause of pediatric mortality is responsible for 15% of deaths before 5 years. In Africa, it is the cause of 21% of deaths, 28% if we include the neonatal period, or nearly a third of deaths. A real public health problem, pneumococcal pneumonia is the subject of numerous guidelines from the World Health Organization (WHO). The emergence of strains of Pneumococcus with reduced sensitivity to penicillin, the difficulty of their biological objectification and the co-detection of several pathogens in the same sample complicate their management [2]. Radiography, an available and inexpensive technique, makes it possible to diagnose parenchymal involvement, obtain an etiological orientation, guide antibiotic therapy, ensure follow-up, and thus avoid complications. The general objective of this work is to evaluate the role of standard radiography in the management of bacterial pneumopathies in children. The specific objectives are: − To list the pathogens involved in the different age groups after the introduction of the pneumococcal vaccine in the Extended Vaccination Program (EVP) of Senegal, − To describe the radiological syndromes characteristic of these pathogenic agents. The prognosis of bacterial pneumopathies in children being severe, it is imperative to make an early diagnosis. Unfortunately, non-specific clinical signs, late and random bacteriological results do not allow a good etiological orientation. The fine study of radiological syndromes, confronted with epidemiological and clinical data, has the merit of guiding probabilistic antibiotic therapy and avoiding complications.

Objective: The aim of this study was to describe the stages of learning Kohonen's self-organizing maps applied to scintigraphy imaging in order to perform classification for medical diagnostic aid. Method: To achieve these goals, the neurons, arranged on a regular grid, are connected to each other by a neighbor relationship, which creates the topology of the map. The input layer consisted of pixels from the scintigraphy images. Results: During the iteration rounds of learning, we have seen a deployment of neurons on the nodes of the map that becomes more and more important. And it is the same for the winning neurons. After 750 iterations, the Davies Bouldin index attests to the end of the training with a quantization error that goes from 0.175 at the beginning of the training to 0.0225 at the end of the training. After this study, we find that neurons 41, 62, 121, 101 and 145 have captured most of the data with a peak uptake achieved by neuron 41 which has captured 1048 data. This individualizes the class of high intensities undoubtedly corresponding to metastatic hyperfixations. Conclusion: This innovative method could undoubtedly be integrated into the link in the chain highlighting periarticular metastases in developing countries, most of which do not have a SPECT-CT.
