Abstract

We present recent highlights from the most recent operation phases of Wendelstein 7-X, the most advanced stellarator in the world. Stable detachment with good particle exhaust, low impurity content, and energy confinement times exceeding 100 ms, have been maintained for tens of seconds. Pellet fueling allows for plasma phases with reduced ion-temperature-gradient turbulence, and during such phases, the overall confinement is so good (energy confinement times often exceeding 200 ms) that the attained density and temperature profiles would not have been possible in less optimized devices, since they would have had neoclassical transport losses exceeding the heating applied in W7-X. This provides proof that the reduction of neoclassical transport through magnetic field optimization is successful. W7-X plasmas generally show good impurity screening and high plasma purity, but there is evidence of longer impurity confinement times during turbulence-suppressed phases.

Abstract

It is well known that the step from a dense packing of colloidal beads to the inverted systems was important for the optimization of photonic crystal properties. Inverted opals made of high-refractive index semiconductors have attracted great attention due to their supreme optical features such as the occurrence of a photonic band-gap and because of an astonishing behavior in photocatalysis or for photovoltaics caused by so-called slow photons. It is much less known that photonic glasses, despite being disordered, exhibit unique optical properties too like random lasing or high-contrast structural colors. In analogy to opals and inverted opals, one can expect that inverted colloidal glasses may lead to an amplification of photonic properties as well or even to the emergence of unexpected features. An inverted photonic glass is characterized by a dense packing of monodisperse voids with colloidal dimensions without any long-range order. The preparation of inverse photonic glasses has rarely been reported by now and cases for materials composed of a semiconductor as a pore-wall material are unknown. The synthesis of porous carbon nitride (C3N4) with inverted colloidal glass structure is demonstrated here using a template approach. The formation of the template with glass-like order is achieved by analytical ultracentrifugation (AUZ) of size-selected silica colloids, followed by infiltration of a precursor sol, transformation to carbon nitride and the final removal of the template. The use of AUZ is particularly important because it even allows to use a mixture of differently sized template particles, which are gradually fractionated. Monoliths with optimized morphological features exhibiting a gradient porosity and highly accessible pores are obtained. The result are materials with a graded structure. What makes such functional gradient material interesting is, a dependence of the optical features on the position can be expected. In addition, the method presented here allows to synthesize materials with adjustable composition ranging from carbon over nitrogen-doped carbon to C3N4 with either graphitic or polymeric structure. Therefore, the optical band gap is highly adjustable and tunable with regards to the photonic properties, as confirmed by optical absorption and photoluminescence measurements.

Abstract

Wendelstein 7-X is the first comprehensively optimized stellarator aiming at good confinement with plasma parameters relevant to a future stellarator power plant. Plasma operation started in 2015 using a limiter configuration. After installing an uncooled magnetic island divertor, extending the energy limit from 4 to 80 MJ, operation continued in 2017. For this phase, the electron cyclotron resonance heating (ECRH) capability was extended to 7 MW, and hydrogen pellet injection was implemented. The enhancements resulted in the highest triple product (6.5 × 1019 keV m-3 s) achieved in a stellarator until now. Plasma conditions [Te(0) ≈ Ti(0) ≈ 3.8 keV, τE > 200 ms] already were in the stellarator reactor-relevant ion-root plasma transport regime. Stable operation above the 2nd harmonic ECRH X-mode cutoff was demonstrated, which is instrumental for achieving high plasma densities in Wendelstein 7-X. Further important developments include the confirmation of low intrinsic error fields, the observation of current-drive induced instabilities, and first fast ion heating and confinement experiments. The efficacy of the magnetic island divertor was instrumental in achieving high performance in Wendelstein 7-X. Symmetrization of the heat loads between the ten divertor modules could be achieved by external resonant magnetic fields. Full divertor power detachment facilitated the extension of high power plasmas significantly beyond the energy limit of 80 MJ.

10. Home study program: creating a patient-safe environment in a perioperative setting.

Abstract

As a result of new regulatory requirements and consumer demands for optimal health care, a focus on patient safety is even more important. In 2001, a corporate patient safety project was initiated by managers at one facility's six hospital-based perioperative suites. Although each suite had its own policies and procedures, no formal assessment of compliance with policies existed. A perioperative safety assessment was designed by resource management department consultants. The assessment included a tool, a questionnaire, and an assessment process. After being tested at one facility, perioperative safety assessments were performed in the organization's six ORs and two ambulatory surgery centers. 13. Mapping the terrain of disability and sexuality: from policy to practice

Abstract

This paper is based on a lecture held in Nagoya, Japan as part of a seminar on issues relating to disabled women, sexuality and sexual and reproductive health and rights (SRHR). I present preliminary results from the research project Sexual Citizenship and Disability: Implications for Theory, Practice and Policy, which focuses on sexual rights for people with mobility impairments. The data comprises policy analysis and interviews with organizations working with sexuality and disability issues in Sweden, the Netherlands, England and Australia. While this paper has a somewhat broader approach than the seminar in Nagoya, the themes are nevertheless relevant to disabled women. I give examples of policies and work by organizations around disabled women’s rights, sex education, sexual and reproductive health and rights (SRHR), and different kinds of sexual support services. The research demonstrates the variability of how sexual rights are understood and their culturally-specific nature. It shows how the personal is indeed political: states’ different policy approaches change the outcomes for disabled people in terms of support to explore and express their sexualities. The role of the disability movement, and which issues it takes up, is also influenced by the policy landscape. This highlights how some of the organizations inadvertently adapt to what is deemed as ‘policy-relevant’ and how sexual rights are often less a priority than other rights – especially in times of austerity.

Abstract

We present recent highlights from the most recent operation phases of Wendelstein 7-X, the most advanced stellarator in the world. Stable detachment with good particle exhaust, low impurity content, and energy confinement times exceeding 100 ms, have been maintained for tens of seconds. Pellet fueling allows for plasma phases with reduced ion-temperature-gradient turbulence, and during such phases, the overall confinement is so good (energy confinement times often exceeding 200 ms) that the attained density and temperature profiles would not have been possible in less optimized devices, since they would have had neoclassical transport losses exceeding the heating applied in W7-X. This provides proof that the reduction of neoclassical transport through magnetic field optimization is successful. W7-X plasmas generally show good impurity screening and high plasma purity, but there is evidence of longer impurity confinement times during turbulence-suppressed phases. 16. On the interfacial charge transfer between solid and liquid Li + electrolytes.

Abstract

The Li + ion transfer between a solid and a liquid Li + electrolyte has been investigated by DC polarisation techniques. The current density i is measured as a function of the electrochemical potential drop Δ[small mu, Greek, tilde] Li + at the interface, using a liquid electrolyte with different Li + concentrations. The subject of this experimental study is the interface between the solid electrolyte Ta-substituted lithium lanthanum zirconate (Li 6.6 La 3 Zr 1.6 Ta 0.4 O 12 ) and a liquid electrolyte consisting of LiPF 6 dissolved in ethylene carbonate/dimethyl carbonate (1 : 1). The functional course of i vs. Δ[small mu, Greek, tilde] Li + can be described by a serial connection between a constant ohmic resistance R slei and a current dependent thermally activated ion transfer process. For the present solid-liquid electrolyte interface the areal resistance R slei of the surface layer is independent of the Li + concentration in the liquid electrolyte. At room temperature a value of about 300 Ω cm 2 is found. The constant ohmic resistance R slei can be attributed to a surface layer on the solid electrolyte with a (relatively) low conductivity (solid-liquid electrolyte interphase). The low conducting surface layer is formed by degradation reactions with the liquid electrolyte. R slei is considerably increased if a small amount (ppm) of water is added to the liquid electrolyte. The thermally activated ionic transfer process obeys a Butler-Volmer like behaviour, resulting in an exchange current density i 0 depending on the Li + concentration in the liquid electrolyte by a power-law. At a Li + concentration of 1 mol l -1 a value of 53.1 μA cm -2 is found. A charge transfer coefficient α of ∼0.44 is measured. The finding of a superposed constant ohmic resistance due to a solid-liquid electrolyte interphase and a current dependent thermally activated ion transfer process is confirmed by the results of two former experimental studies from the literature, performing AC measurements/EIS.

Abstract

Purpose: The purpose of this study was to compare planimetric aortic valve area (AVA) measurements from 256-slice CT to those derived from transesophageal echocardiography (TEE) and cardiac catheterization in high-risk subjects with known high-grade calcified aortic stenosis., Methods and Materials: The study included 26 subjects (10 males, mean age: 79±6; range, 61-88 years). All subjects were clinically referred for aortic valve imaging prior to percutaneous aortic valve replacement from April 2008 to March 2009. Two radiologists, blinded to the results of TEE and cardiac catheterization, independently selected the systolic cardiac phase of maximum aortic valve area and independently performed manual CT AVA planimetry for all subjects. Repeated AVA measurements were made to establish CT intra- and interobserver repeatability. In addition, the image quality of the aortic valve was rated by both observers. Aortic valve calcification was also quantified., Results: All 26 subjects had a high-grade aortic valve stenosis (systolic opening area <1.0 cm(2)) via CT-based planimetry, with a mean AVA of 0.62±0.18. In four subjects, TEE planimetry was precluded due to severe aortic valve calcification, but CT-planimetry was successfully performed with a mean AVA of 0.46±0.23 cm(2). Mean aortic valve calcium mass score was 563.8±526.2 mg. Aortic valve area by CT was not correlated with aortic valve calcium mass score. A bias and limits of agreement among CT and TEE, CT and cardiac catheterization, and TEE and cardiac catheterization were -0.07 [-0.37 to 0.24], 0.03 [-0.49 to 0.55], 0.12 [-0.39 to 0.63]cm(2), respectively. Differences in AVA among CT and TEE or cardiac catheterization did not differ systematically over the range of measurements and were not correlated with aortic valve calcium mass score., Conclusion: Planimetric aortic valve area measurements from 256-slice CT agree well with those derived from TEE and cardiac catheterization in high-risk subjects with known high-grade calcified aortic stenosis., (Copyright © 2010 Elsevier Ireland Ltd. Abstract

Objective: To determine whether a mandatory seasonal influenza vaccination program will increase vaccination rates among healthcare personnel (HCP) and affiliated physicians., Setting: MedStar Health is a not-for-profit regional healthcare organization that includes 9 hospitals with approximately 25,000 HCP and approximately 4,000 affiliated physicians. HCP describes any person employed by MedStar Health., Methods: With previous vaccination rates parallel to reported national rates of 54% among HCP, MedStar Health introduced a mandatory seasonal influenza vaccination program promulgated during the 2009-2010 influenza season. HCP and affiliated physicians were given an opportunity to apply for medical or religious exemptions. Noncompliant HCP were terminated. Noncompliant physicians had their privileges administratively suspended for the influenza season., Results: HCP compliance (vaccinated and exempt) was 99.9%. The influenza vaccination rate among HCP was 98.5%. There were 338 approved medical exemptions and 18 approved religious exemptions. Only 0.01% of HCP (9 full-time, 2 part-time, and 17 per diem employees) were terminated. Overall, 93% of the affiliated physicians were vaccinated; 7 religious and 99 medical exemptions were granted. In total, 149 physicians (4%) had their admitting privileges suspended during the influenza season., Conclusion: A mandatory influenza vaccination program achieves high rates of vaccination among HCP and affiliated physicians.

Abstract

Background: The aim of this study was to evaluate the Navibase navigation system for ear, nose, and throat (ENT) surgery. A new methodology for evaluating surgical and human factors is developed., Patients and Methods: The evaluation is based on 102 ENT surgical applications, including 89 cases of functional endoscopic sinus surgery (FESS). The evaluation of surgical and human factors was performed by seven ENT surgeons. To evaluate surgical performance, level of quality (LOQ) in the 89 cases of FESS was determined, comparing the surgeon's own impressions with those of the navigation system on a scale from 0 to 100 and further comparing them with clinical results. Intraoperative changes in surgical strategy were documented. The human factors of total confidence (trust), situation awareness, skill set requirement and workload shift were recorded as level of reliance (LOR)., Results: The maximum deviation amounted to 1.93 mm. Averaging the quality of information resulted in an LOQ of 63.59. Every second application of the navigation system (47.9%) led to a change in surgical strategy. Total confidence showed a positive evaluation of 3.35 points in LOR., Conclusion: Application-relevant information relevant to the application beyond only technical details permits comparison with other assisting systems.

Abstract

The aim of this study is the evaluation of a navigation system (NaviBase) for ENT surgery. For this purpose, a new methodology for the evaluation of surgical and ergonomic system properties has been developed. The practicability of the evaluation instruments will be examined using the example of the overall assessment of the system in comparison with the current surgical standard and with other systems using clinical efficiency criteria. The evaluation is based on 102 ENT surgical applications; of these, 89 were functional endoscopic sinus surgeries (FESS). The evaluation of surgical and ergonomic performance factors was performed by seven ENT surgeons. To evaluate surgical system properties, the Level of Quality (LOQ) in 89 cases of the FESS was determined. It compares the existing information of the surgeon with that of the navigation system on a scale of 0 to 100 and with a mean value of 50 and places it in a relationship to the clinical impact. The intraoperative change of the planned surgical strategy (Change of Surgical Strategy) was documented. The ergonomic factors of the system with the categories of Overall Confidence (Trust), awareness of the situation (Situation Awareness), influence on the operating team, requirements for specific skills (Skill Set Requirement), and cognitive load (Workload Shift) were recorded for all surgical procedures as Level of Reliance (LOR). In the evaluation of the surgical system properties, an average evaluation of the quality of the information, as an LOQ of 63.59, resulted. Every second application of the navigation system (47.9%), on average, led to a change in the surgical strategy. An extension/enhancement of the indication of the endonasal approach through the use of the navigation system was shown in 7 of 102 (6.8%) cases. The completion of the resection in the FESS was rated by 74% of group I and 11% of group II as better in comparison with the standard approach. Total confidence shows a positive evaluation of 3.35 in the LOR. To supplement the evaluation of the navigation system, the technical parameters were included. The maximum deviation, Amax, of the displayed position of the reference value amounted to 1.93 mm. The average deviation was at 1.29 mm with an SD above all values, sd, of 0.29. The subsequent economic evaluation resulted in an effective average extra expenditure of time of 1.35 minutes per case. The overall evaluation of the system imparts application-relevant information beyond the technical details and permits comparability between different assistance systems.

Abstract

As a result of new regulatory requirements and consumer demands for optimal health care, a focus on patient safety is even more important. In 2001, a corporate patient safety project was initiated by managers at one facility's six hospital-based perioperative suites. Although each suite had its own policies and procedures, no formal assessment of compliance with policies existed. A perioperative safety assessment was designed by resource management department consultants. The assessment included a tool, a questionnaire, and an assessment process. After being tested at one facility, perioperative safety assessments were performed in the organization's six ORs and two ambulatory surgery centers. Best practices then were shared across the system.
