Objective: It has been shown that the presence of fragmented QRS (fQRS) is associated with poor prognosis in many cardiovascular diseases and in patients with hypertrophic cardiomyopathy (HCM). However, no study has shown an association with the absolute risk score of sudden cardiac death. The aim of this study was to determine the relationship between QRS and the predicted risk score of sudden cardiac death at 5 years (HCM Risk-SCD) in HCM patients. Methods: In total, 115 consecutive HCM patients were included in this prospective observational study. The patients were divided into two groups according to the presence [fQRS(+) group (n=65)] or absence [fQRS(-) group (n=50)] of fQRS on a 12-lead electrocardiogram (ECG). Results: The HCM Risk-SCD (%) HCM Risk-SCD (>6%) values and some echocardiographic parameters, including ventricular extrasystole, ventricular tachycardia, cardiopulmonary resuscitation, implantable cardioverter defibrillator implantation, appropriate shock, and heart failure at the time of admission, were significantly higher in the fQRS(+) group than in the fQRS(-) group (all p<0.05). Both univariate and multivariate analyses revealed fQRS and New York Heart Association (NYHA) class as independent predictors of HCM Risk-SCD. In a receiver operating characteristic (ROC) curve analysis, an HCM Risk-SCD value of >4 was identified as an effective cut-off point in fQRS for HCM. An HCM Risk-SCD value of >4 yielded a sensitivity of 77% and a specificity of 76%. Conclusion: fQRS is determined to be an independent high-risk indicator of HCM Risk-SCD. Aim: The aim of the study is to evaluate extracorporeal life support system (ECLS) employed in neonates in pediatric cardiac intensive care unit.Material and Methods: Twenty-five neonates that required ECLS in between November 2010 and November 2015 were evaluated.Results: The median age was 12 days (range 3-28 days) and the median body weight was 3 kg (range 2.5-5 kg). Venoarterial ECLS was performed in all of the cases. Ascendan aorta-right atrial cannulation in 22 patients and neck cannulation in three patients were performed. The reason for ECLS was E-CPR in two patients, inability to wean from cardiopulmonary bypass (CPB) in seven patients, respiratory insufficiency and hypoxia in nine patients, low cardiac output (LCOS) in seven patients. Median duration of ECLS was four days (range 1-15). Hemorrhagic complications developed in 15, renal complications in 13, pulmonary complications in 12, infectious complications in 11, neurologic complications in three and mechanical complications in two of the patients. Weaning was successful in 15 of the patients. Eleven patients were successfully discharged.Conclusion: ECLS is an important treatment option that is performed successfully in many centers around the world to maintain life support in patients unresponsive to medical treatment. Objective: We evaluated autonomic behavior by examining heart rate variability (HRV) in the time domain and frequency domain in pediatric patients who underwent transcatheter closure of atrial septal defect (ASD). Methods: A prospective study design was used. Holter ECG was performed in a control group of 30 healthy subjects and a group of 47 patients who underwent transcatheter ASD closure. ECG was taken one day before, one day after, and six months after the procedure to evaluate changes in the time domain [SDNN, rMSSD, NN, pNN50(%), and SDANN] and frequency domain (VLF, LF, HF, VHF, and LF/HF) in the patient group. Student's t-test was used to evaluate changes prior to and after the procedure. Results: There were 28 females (60%) in the patient group and 21 females (70%) in the control group. The mean age and weight of the participants in the patient group were 9.61±4.72 years and 32.40±19.60 kg, respectively; the mean age and weight of the control subjects were 10.43±5.31 years and 32.83±13.00 kg, respectively. In both the time domain and frequency domain analyses, the patient group values were found to be lower than those in the control group prior to the procedure; the values in the patient group were found to approach the values in the control group following the procedure. By the sixth month, the values in the patient group reached the control levels with no statistically significant difference (SDNN: 145±0.84, 137.50±42.50; rMSSD: 72.18±48.22, 58.14±28.49; SDANN: 125.13±13.50, 122.40±41.06; VLF: 112.85±29.07, 114.41±98.39; LF: 50.40±24.09, 45.69±15.13; HF: 39.28±19.86, 44.29±13.14; VHF: 10.29±4.24, 9.99±6.47; LF/HF: 1.90±1.44, 1.24±0.81; p>0.05). Conclusion: The transcatheter closure of secundum ASDs was found to have a positive effect on HRV. Consequently, it may contribute to reduced mortality and morbidity. Thoracic and Cardiovascular Surgery Training and Research Hospital, Istanbul, Turkey Background Pericardial effusion (PE), atrial fibrillation (AF), and acute kidney injury (AKI) are commonly found after coronary artery surgery. These adverse events may also be interwoven in the postoperative period. In this prospective study, we investigated whether posterior pericardiotomy (PP) with intrapericardial tube positioned along the right atrium (pericardial space intervention) is effective in the prevention of these adverse events. Methods The patients were randomly distributed to the study and control groups. The study group consisted of patients with pericardial space intervention, whereas the control group consisted of patients without pericardial space interventions. In all patients, a straight tube was placed in the anterior mediastinum and an angled tube was placed into the left hemithorax. Results A total of 210 patients were studied: 107 in the control group and 103 in the study group. Statistically significant results were obtained in the amount of PE, cardiac tamponade, AF (p = 0.019), and AKI during the postoperative period, in favor of the study group. Length of hospital stay was significantly shorter in the study group (6.11 ± 2.31, p = 0.009). Conclusion The PP with intrapericardial tube approach is safe, easy, and effective in the prevention of PE, cardiac tamponade, and AF. The use of this approach may reduce the risk of developing AKI during the postoperative period. Besides, this technique also reduces the length of hospital stay. [ABSTRACT FROM AUTHOR]

Purpose There are different clinical cardiac applications of dual source computed tomography ( DSCT). Here, we aimed to compare the DSCT with the transthoracic echocardiography ( TTE) for evaluating the Wilkins score and planimetric mitral valve area ( MVA) of a rheumatic stenotic mitral valve. Materials and Methods We prospectively evaluated mitral valvular structure and function in 31 patients with known mitral stenosis undergoing electrocardiogram-gated, second-generation DSCT, in our heart center for different indications. Mitral valve was evaluated using Wilkins score, and also, the planimetric MVA was assessed. Results We found a significant difference between MVAs determined by DSCT (average 1.42 ± 0.44 cm2) and MVAs determined by TTE (average 1.35 ± 0.43 cm2; difference 0.07 ± 0.16 cm2; P = 0.018). Linear regression analysis revealed a good correlation between the two techniques (r = 0.934; P < 0.0001). The limits of agreement for DSCT and TTE in the Bland-Altman analysis were ±0.31 cm2. DSCT using TTE as the reference enabled good discrimination between mild and moderate-to-severe stenosis and had an area under the ROC curve of 0.967 ( CI 0.912-1.023; P < 0.0001). Wilkins scores obtained by DSCT (7.51 ± 1.17, range 5-10) and TTE (8.16 ± 1.27, range 6-10) had a moderate correlation (r = 0.686; P < 0.0001). Conclusion We found that planimetric MVA measurements assessed by DSCT were closely correlated with MVA calculations by TTE. The moderate correlation was observed for the Wilkins score. We investigated the association between the preoperative logistic clinical syntax score (log CSS), saphenous vein graft (SVG) patency, and major adverse cardiac and cerebrovascular events (MACCEs) after coronary artery bypass surgery (CABG). Of 1875 patients undergoing isolated CABG between 2009 and 2011, 267 patients, who later underwent coronary angiography, were included in the study. The primary end point was at least 1 graft occlusion on the follow-up coronary angiogram. The secondary end point was a composite of MACCE. In multivariate analysis, log CSS was found as a strong predictor of SVG failure (odds ratio [OR] 0.66, 95% confidence interval [CI]: 0.46-0.94, P = .02; and OR: 2.21, 95% CI: 1.02-4.75, P = .04, respectively): log CSS was also associated with MACCE (P = .001 and P < .001, respectively). The addition of clinical parameters to the anatomical SYNTAX score, termed as “log CSS”, augmented the accuracy and reliability of the prediction of SVG failure and MACCE in patients undergoing CABG. The article presents a case study of a 75-year-old female patient with a history of hypertension and coronary artery disease (CAD). He was given valve placement where a stent was implanted in his left anterior descending artery. The article discusses transcatheter aortic valve implantation (TAVI) as a therapeutic option for patients with symptomatic, degenerative aortic stenosis (AS).

The article presents a case study of a 10-year-old boy presented with severe "tearing" type chest pain. He was transferred to intensive care unit immediately and transesophageal echocardiography (TEE) was done which showed dissection flap above the sinotubular junction. It also discusses acute dissection, which refers to an unusual complication of ascending aortic aneurysm which occurs during childhood.

The article presents a case study of a 48-year-old man who was evaluated for incidence of paravalvular leak (PVL) and underwent coronary artery bypass and mitral valve replacement surgery in past. It reveals that Paravalvular regurgitant jet flow adjacent to the appendix was localized with transesophageal echocardiography imaging. It also reveals that transapical transcatheter PVL closure was planned because of continuing congestive heart failure symptoms.

The article presents early and short term results of percutaneous pulmonary valve implantation (PPVI) from a medical center in Turkey. The center had performed 10 implantations since October 2010 to June 2012 using Edwards Sapien transcatheter heart valves from Edwards Lifesciences LLC. It was found that right ventricular (RV) pressure decreased in all patients.

A response by Mehmet Kata et al. to a letter to the editor about their article "The Preventive Effects of Posterior Pericardiotomy With Intrapericardial Tube on the Development of Pericardial Effusion (PE), Atrial Fibrillation (AF), and Acute Kidney Injury (AKI) After Coronary Artery Surgery: A Prospective, Randomized, Controlled Trial" in a 2015 issue is presented.

Objective: The main purpose of this study is to analyze the unit cost of different angiography services given by the angiography department of Istanbul Mehmet Akif Ersoy Cardiothoracic Surgery Hospital. Material and Method: Due to the fact that the existing cost system of this hospital and also the angiography unit is unsatisfactory to calculate the indirect unit costs of different services given by the service departments, a more contemporary approach named Activity Based Costing System was applied to the adult angiography unit. After many interviews, observations and data collection processes, unit costs of fifteen different angiography services were calculated. In this study the unit cost calculation was done according to the cost data belonging to May 2010. Result: The unit costs that have been calculated by activitiy based costing system was compared to the package costs determined by the Social Security Institution named shortly "SUT." It can easily be observed that there are significant differences between the calculated costs and the package SUT costs. For example, although the operation process and duration of percutaneous transluminal coronary angioplasty (service number is 7) and percutaneous ballon valvoplasty, mitral valve (service number 9) differs significantly, the unit package SUT costs of these two services are equal. Therefore, it can be concluded that the costs obtained by this study is more accurate than the current package costs because this study considers the processes and the time consumed by the services. This comparison has a vital importance to see the usage of the resources of the hospitals and helps the hospital management to allocate these resources more accurately. Conclusion: This pilot study is expected to be an example of a more accurate cost calculation tool to the other service departments of the hospital and also to the other state hospitals. The wide application of activity based costing system applied to the whole institution is also expected to increase the efficiency and effectiveness of the hospital management. Kabuki syndrome is a rare congenital malformation syndrome characterized by mental retardation, skeletal deformities, auditory dysfunction, cardiac defects, and distinctive facial appearance. Although complex cardiovascular malformations present in early childhood, rarely, atrioventricular septal defects may also present in young adults. Presently described is case of a 22-year-old female with KS who presented with ostium secundum atrial septal defect with deficient rim and idiopathic thrombocytopenic purpura. In this case, minimally invasive robotic surgery was preferred for closure of atrial septal defect.
