Acute Bacterial Parotitis (ABP) is an infection seen in very early and old ages. The infection can be prevented by sufficient fluid support, mouth hygiene, oral feeding and avoiding multiple medication use. Parotis ultrasonography can be used for diagnosis. Acute Bacterial Parotitis can be treated in a short time and free of problems with early diagnosis and appropriate antibiotics. Mortality can be seen in cases which are late diagnosed and which are resistant to antibiotic treatment. This article presents four case reports over the age of 65 with a diagnosis of ABP, who are monitored in the intensive care. It is thought that the first case developed ABP because of multiple medication use, liquid restriction and oral feeding difficulty, the second case because of oral feeding difficulty, the third case because of bad mouth hygiene, dementia and not getting enough liquid and the fourth case because of oral feeding difficulty. If the cases with the stated risk factors are carefully followed, intensive care hospitalization period will not be prolonged and mortality rates will not increase. Copyright © 2012 TBDHD.

Paraneoplastik nörolojik sendrom (PNS), kanserli hastalarda ortaya çıkan, önemli bir kısmının otoimmün kökenli mekanizmalarla oluştuğu kabul edilen nörolojik tablolardır. Patogenez için günümüzde en geçerli olan görüş, altta yatan tümör ile sinir sistemi arasında olan benzer antijenik özellikler sonucu gelişen bir otoimmun yanıtın PNS'ların ortaya çıkışından sorumlu olduğudur. Bu yazıda, subakut başlayıp paraneoplastik serebellar dejenerasyon, paraneoplastik Lambert-Eaton myastenik sendrom ve paraneoplastik polinöropati tanısıyla izlenen, tetkiklerinde tiroid foliküler tümör tespit edilen bir olgu sunulmuştur., Patients with paraneoplastic neurological syndromes (PNS) have a variety of neurological symptoms that are considered to be triggered by autoimmune mechanisms. The current valid opinion for the pathogenesis is that an autoimmune response caused by similar antigenic properties in the underlying tumor and the nervous system is responsible for the emergence of PNS. We present a case a patient with follicular thyroid cancer who had a combination of symptoms due to paraneoplastic cerebellar degeneration, Lambert-Eaton myasthenic syndrome and paraneoplastic polyneuropathy.

Erdheim-Chester disease is a rare, non-Langerhans form of systemic histiocytosis of unknown etiology. The disease affects multiple organ systems, including musculoskeletal, cardiac, pulmonary, gastrointestinal, and central nervous systems, producing protean manifestations. Neurological manifestations are less frequent. We present a 44-year-old patient with Erdheim-Chester disease who had bilateral carotid artery occlusion and cavernous sinus infiltration.

Thrombolytic treatment is the most effective and commonly used method into firs 4,5 hours of acute ischemic stroke and massive pulmonary embolism. It is known that thrombolytic treatment is definitely contraindicated in cases who had an ischemic stroke into last three months. In this paper, it was reported that thrombolytic treatment had given for pulmonary embolism which developed one week after stroke in a case with stroke due to middle cerebral artery occlusion. Here, we presented a case which is rarely seen and required difficulty in deciding.

Aims of the study were: 1. to establish the prevalence of CD44 protein expression in human astrocytomas; 2. to compare the distribution of the extracellular matrix in these tumors; 3. to investigate the relation between CD 44, the extracellular matrix proteins and the histological grade of the tumor. CD44, Type IV Collagen (Col IV), Laminin (LN), Fibronectin (FN), and Tenascin (TN) expression were detected by immunohistochemistry in formalin fixed paraffin embedded tissue samples of 52 astrocytic tumors: 35 glioblastomas (GB), 7 Anaplastic astrocytomas (AA) and 10 astrocytomas (A). The localization of Col IV was observed in the basement membrane of the vessel walls in most of the astrocytomas (88.4%) with a similar pattern obtained with LN staining. 7 of 10 A (70%), 2 of 7 AA (28%) and 9 of 35 GB (25.7%) showed LN positivity. There was a negative correlation between LN expression and tumor grade (p=0.03). FN was either localized in the basement membrane or showed thick multi-layered immunoreactivity of the vessel walls. FN expression was seen in 6 A (60%), 4 AA (57%) and all of 35 GB (100%). The FN distribution was not uniform and its staining intensity showed decrease in GB. 3A (30%), 3 AA (42%), 27 GB (77.1%) showed TN expression in the vessel walls and in some tumor cells of 19 GBs. Tuberculous meningitis is the most severe manifestation of extrapulmonary tuberculosis, with high mortality and morbidity rates. It usually presents with headache, fever, consciousness disturbances, and convulsions. Involvement of the eighth cranial nerve is rarely reported in patients with tuberculous meningitis. There are only a few cases in the literature who presented with unilateral sensorineural hearing loss, and the relationship between sensorineural hearing loss and tuberculous meningitis remains unclear. Acute deafness due to bilateral sensorineural hearing loss has not been reported in the literature previously. We present a 64-year-old man who presented with an acute-onset bilateral sensorineural hearing loss associated with tuberculous meningitis.
