5. Drug-induced lupus erythematosus in childhood: Case-based review.

Background: Drug-induced lupus erythematosus (DILE) is the development of lupus-like syndrome following a drug exposure. DILE has been reported less frequently among children than adults. Methods: In this study, we present four children with DILE and similar published cases through a systematic literature review. Results: We report four children (three girls and one boy) who developed DILE associated with the use of topiramate, doxycycline, etanercept, and ethosuximide. Three of them were positive for anti-histone antibodies. In all patients, the drug was discontinued and symptoms resolved completely. The literature review revealed 48 articles describing 61 children with DILE. In the evaluation of 65 patients (our 4 patients and 61 patients from the literature), the most frequently reported drugs associated with DILE were ethosuximide (n = 13) and minocycline (n = 12). Fever (n = 33), arthralgia (n = 31), rash (n = 30), and arthritis (n = 29) were the most common clinical manifestations. Antinuclear antibody (ANA) was positive in 93.5% of patients and anti-histone antibodies were detected in 72.2% of the patients. As for treatment, the responsible drug was discontinued in all patients, and corticosteroids were initiated in 53.3%. Improvement was achieved in 92.0% of patients. Conclusion: For children presenting with SLE features, proper drug history is crucial since DILE may be more frequent than anticipated. An association of the relevant drug with the symptoms, and resolution of symptoms on drug withdrawal provides evidence for the diagnosis of DILE. [ABSTRACT FROM AUTHOR]

IgA vasculitis (IgAV) is a leukocytoclastic vasculitis affecting small vessels and is more common in children than in adults. In this study, we aimed to investigate the effects of oxidative stress in adult IgAV patients by Dynamic Thiol Disulfide Homeostasis (DTDH) and Ischemia Modified Albumin (IMA) levels. We also aimed to correlate the presence of inflammation detected by C-Reactive Protein (CRP) and Highly Sensitive C-Reactive Protein (hsCRP) with these oxidative stress parameters. Forty-one IgAV patients and forty-five controls were included in the study. Patients were classified as "active" or "in remission" according to the British Vasculitis Activity Score (BVAS). Measured parameters were compared between the groups. DTDH, IMA, CRP and hsCRP parameters were analyzed in both patient and control groups. According to our results, disulfide (p=0.047), index 1 (p<0.001), index 2 (p<0.001), IMA (p<0.001), CRP (p=0.005) and hsCRP (p=0.004) levels were significantly increased while native thiol (p<0.001), total thiol (p<0.001) and index 3 (p<0.001) levels were significantly decreased in IgAV patients. The results of this study show that oxidative stress is associated with IgAV disease and also with disease severity. Objectives Takayasu arteritis (TAK) is a large-vessel vasculitis rarely reported in children and infants. Most articles on paediatric TAK have not focused on infants. We present the largest case series of infantile TAK, aiming to identify its demographic and clinical characteristics and compare them with existing data on older children. Methods We conducted an international multicentre retrospective cohort study. Epidemiological and clinical data were collected from patients' charts from six rheumatology centres. All patients met both the EULAR/PReS 2008 criteria and the 1990 ACR/EULAR criteria and were diagnosed with TAK at age <5 years. Results Twelve patients were included (50% female). Median age of symptom onset was 11 months, with a diagnostic delay of 4 months. The most common symptoms at presentation were hypertension, blood pressure differences between limbs, and fever. The most commonly involved arteries were the abdominal aorta and renal artery. Medications included steroids, conventional and biologic DMARDs, and other immunosuppressive therapies. Half of the patients received biologic agents, of which infliximab had the highest complete remission rate (40%). Other medications resulting in complete remission were CYC (40%) and MTX (38%). Invasive procedures were required for 58% of patients. The most common complications were cardiac (50%), stroke (42%), and serious infections (33%). No patients died. Conclusion This study presents the largest series of infantile TAK. Compared with other reported series on older children, infants with TAK have more severe disease and were more likely to receive biologic agents, develop complications, and require invasive interventions. Aim: In this study conducted in a single tertiary care center, it was aimed to examine the sociodemographic, clinical, biochemical, microbiological and follow-up results of cellulitis patients. Materials and Methods: Demographic characteristics, presence of predisposing factors, cellulitis involvement area, attack characteristics, hospitalization status/duration, agents used in treatment and their durations, tissue/wound/blood culture, biochemistry and ultrasonography characteristics of the patients were examined and the results were compared by grouping the patients according to first attack/recurrent attack, inpatient/outpatient treatment, presence of concomitant diabetes and obesity. Results: In the recurrent attack group, the frequency of diabetes, obesity, venous insufficiency, lymphedema and onychomycosis were significantly higher than in the initial attack group (p values; <0.001, <0.001, 0.025, <0.001, 0.002, respectively). The prevalence of obesity, diabetes mellitus and changing the initial antibiotic was significantly higher in the inpatient group compared to the outpatient group (p values; <0.001, 0.004, <0.001, respectively). Conclusion: Diabetes, obesity, venous insufficiency, lymphedema, and conditions that impair skin integrity are important predisposing factors for both the development and recurrence of cellulitis. Objectives: Rice body (RB) formation is an uncommon inflammatory process seen in systemic disorders. In this study, we aimed to assess characteristic features of RBs in pediatric patients. Method: We retrospectively evaluated pediatric patients who underwent joint/extremity magnetic resonance imaging. A systematic literature review was conducted for articles including children with RBs. Results: We found 24 patients (median age 6.1 years; F/M = 2.4) with RBs [23 with juvenile idiopathic arthritis (JIA) and one with arthralgia]. The most prevalent location for RBs was the knee joint (75%). RBs were most frequently seen as diffuse multiple millimetric structures. In three out of five patients with follow-up magnetic resonance imaging, resolution or regression of RBs was observed without surgical intervention. Our literature search identified 13 pediatric patients with RBs. Most (84.6%) had JIA, and the knee joint (71.4%) was the most commonly affected joint. Surgery was preferred in our 3 patients (12.5%) and 10 literature patients (83.3%) in the treatment. Conclusion: Our results showed that RBs were most commonly detected in the knee joint, and most cases were secondary to JIA. Although surgery is used as a treatment option, we observed that RBs can occasionally disappear during follow-up without surgical intervention. [ABSTRACT FROM AUTHOR]

Cogan's syndrome (CS) is a rare inflammatory disease characterized by interstitial keratitis or uveitis, vestibular impairment, and progressive hearing loss, commonly bilateral. Although glucocorticoids are fundamental treatment options, in most cases, hearing loss gradually worsens. Herein we report 2 pediatric cases of CS who were treated with corticosteroids and methotrexate. One patient had a cochlear implant, and the hearing of the other patient improved with treatment. Also, a systematic literature review was conducted for articles including pediatric CS patients. In the literature, 34 articles describing 44 pediatric patients with CS were identified. Sudden hearing loss (95.3%) and ocular symptoms (92.5%) were the most common manifestations in these patients. Also, aortic involvement was present in 19.5% of patients in the literature. Otorhinolaryngologists, ophthalmologists, and pediatricians should collaborate to diagnose and manage CS to prevent progressive hearing loss and eye involvement. [ABSTRACT FROM AUTHOR]

Objectives The lower extremity venous wall thickness (VWT) of Behçet's disease (BD) patients was reported to be significantly increased in adults, suggesting its use for the support of BD diagnosis. This prospective study aimed to investigate the lower extremity VWT in childhood-onset definite and incomplete BD patients and compare it to healthy age-matched controls. Methods Paediatric patients classified with BD according to the 2015 international paediatric BD criteria in our centre were included in the study. Intima-media thickness of the lower extremity veins to evaluate VWT was measured by ultrasonography, including common femoral vein (CFV), femoral vein (FV), vena saphena magna, vena saphena parva and popliteal vein (PV). Results In this cross-sectional study, VWT was measured in 35 patients (63% male) and 27 healthy controls (55% male). Thirteen (37%) of 35 patients met the criteria for the diagnosis of BD. The remaining 22 (63%) had incomplete BD and met two criteria. The median VWT values of both definite and incomplete BD patients were significantly higher than the control group in all veins on both sides. Regarding the best cut-off values of VWT for all lower extremity veins, the sensitivity rates were between 63% and 86%, while specificity rates were between 71% and 100%. Conclusion Increased VWT was present not only in BD patients with vascular involvement but also in those without. We suggest that VWT may be a new criterion in supporting the diagnosis of childhood BD both in definite and incomplete BD patients. [ABSTRACT FROM AUTHOR]

A child-friendly tourism enterprise includes (1) physical facilities and services for children, and (2) employees’ attitudes and behaviors that support children’s development and happy holiday experience. In this study, we focus on the first pillar. This study aims to reveal the physical facilities and services for children in hotels that appeal to the family market. The information on the web pages of the top 24 family hotels that have been awarded in the family category by TripAdvisor, a world-renowned travel review site in 2020 was subjected to content analysis. Findings demonstrated that kids-related amenities in hotels are related to activity areas, activities, baby equipment, room options, kids tab on website, price options, food and beverage options, health and safety and room with toys. Based on the findings, suggestions were given to hotel businesses that appeal to the family market on issues that should be considered in child-related practices and marketing communications., Çocuk dostu turizm işletmesi, (1) çocuklara yönelik fiziksel olanaklar ve hizmetler ile (2) çalışanların çocukların gelişimini ve mutlu bir tatil deneyimini destekleyen tutum ve davranışlarını içermektedir. Bulgular, otellerde çocuklara yönelik olanakların aktivite alanları, aktiviteler, bebek ekipmanları, oda seçenekleri, web sitesindeki çocuklar sekmesi, fiyat seçenekleri, yiyecek ve içecek seçenekleri, sağlık-güvenlik ve oyuncaklı oda ile ilgili olduğunu göstermiştir. Elde edilen bulgulara dayalı olarak, aile pazarına hitap eden otel işletmelerine çocuklarla ilgili uygulamalarda ve pazarlama iletişiminde dikkat edilmesi gereken hususlara ilişkin önerilerde bulunulmuştur.

Objectives: Multisystem inflammatory syndrome in children (MIS-C) is a rare but severe condition associated with coronavirus disease 2019. Here we aimed to raise awareness for the symptoms of MIS-C in patients with rheumatic diseases, emphasizing the challenges of the differential features. Methods: We retrospectively evaluated the demographic and clinical characteristics, laboratory and imaging findings, treatments, and outcomes of six MIS-C patients with previous rheumatic disease. Results: Three of the patients had familial Mediterranean fever (FMF), one had juvenile dermatomyositis, one had systemic juvenile idiopathic arthritis (JIA), and another patient had oligoarticular JIA. All FMF patients presented with fever and abdominal pain, two also had chest pain. The patient with systemic JIA presented with fever, rash, and myalgia. All patients had elevated inflammatory markers and high d-dimer levels. Chest imaging of two FMF patients showed infiltrations compatible with pneumonia. One FMF patient had mildly decreased systolic functions with a shortening fraction of 48% in his echocardiography. Intravenous immunoglobulin and methylprednisolone were administered to all patients. Anakinra was given to four patients. Conclusions: Clinical and laboratory signs of MIS-C may overlap with the findings of various rheumatic diseases, and this may cause a delay in diagnosis. Objectives: Gout is a common and easily treated disease characterized by the accumulation of monosodium urate crystals in both joints and other tissues. Monosodium urate crystals are the main stimulants for initiating and maintaining an inflammatory response. Oxidative stress is also an early change in gout pathogenesis together with inflammation. This study aimed to investigate the presence of oxidative stress in gout together with thiol–disulfide homeostasis and ischemia-modified albumin (IMA) levels. We aimed to compare these parameters with inflammation marker C-reactive protein (CRP) and high-sensitivity C-reactive protein (hsCRP). Methods: Levels of native thiol, total thiol, disulfide, IMA, CRP, and hsCRP were detected in patients with gout (n=50) and healthy subjects (n=50). Student’s t-test and the Mann-Whitney U test were used for istatistical analysis. Results: Native thiol, total thiol, and index 3 (native thiol/total thiol×100) were significantly lower in the patient group, while disulfide, index 1 (disulfide/native thiol×100), index 2 disulfide/total thiol×100), IMA, CRP, and hsCRP were significantly higher. In addition, elevation in native thiol, total thiol, and disulfide levels was detected as disease duration increased. Conclusion: The present study has shown the role of oxidant damage in gout disease. Additional studies are needed to identify sources of oxidative stress in gout. [ABSTRACT FROM AUTHOR]

İnflamatuvar bağırsak hastalıkları (İBH), gastrointestinal sistemin multifaktöriyel kronik idiyopatik inflamasyonu ile karakterize bir grup multisistemik hastalıktır. Crohn hastalığı, ülseratif kolit ve sınıflanamayan kolit formları mevcuttur. Aynı zamanda yaş gruplarına göre de sınıflandırılmakta olup yaşa göre hastalık seyri değişkenlik göstermektedir.Bu çalışmaya Hacettepe Üniversitesi Çocuk Gastroenteroloji, Hepatoloji ve Beslenme Bilim Dalı'nda 2000-2017 yılları arasında İBH tanısıyla izlenmekte olan 92 hasta dahil edildi. Hastalar ülseratif kolit veya Crohn hastalığı tanısına göre ve semptom başlama yaşının 6 yaştan önce veya sonra başlamasına göre gruplandırılarak değerlendirildi. Semptom başlangıcı 6 yaştan önce olanlar erken başlangıçlı İBH olarak adlandırıldı. Hastaların 57'si (%61,9) Crohn hastalığı, 33'ü (%35,8) ülseratif kolit ve 2'si (%2,2) sınıflanamayan kolit tanısı ile izlenmekte idi. Semptom başlama yaşı Crohn hastalığında ülseratif kolite göre daha düşüktü (6,95,6 yıl/9,65,0 yıl). Semptom başlama ile tanı arasında geçen süre 0,1 ile 13,8 yıl arasında değişmekte idi. Crohn hastalığında semptom başlangıcı ile tanı arasında geçen süre daha uzundu (1,42,0 yıl/1,11,9 yıl). Crohn hastalığında en sık başvuru yakınmaları karın ağrısı ve ishal iken, ülseratif kolitte kanlı dışkılama idi. Laboratuvar tetkiklerinde en sık dışkıda gizli kan pozitifliği, anemi ve CRP yüksekliği görüldü. Crohn hastalığında eritrosit sedimentasyon hızı ve CRP ülseratif kolite göre daha yüksekti. On sekiz hastada akut faz reaktanları normal sınırlardaydı. Özefagogastroduodenoskopide en sık midede tutulum saptandı. Kolonoskopide ülseratif kolit hastalarının tümünde anormal bulgu saptanmış olup en sık pankolit (%54,5) mevcuttu. Beş (%15,1) hastada `backwash ileit` saptandı. Crohn hastalarında en sık ileokolonik tutulum (%45,6) görüldü. Yedi hastada kolon normalken terminal ileumda tutulum vardı. Histopatolojik değerlendirmede en sık rastlanan anormal bulgu aktif kolitti. Tedavide en sık 5-ASA, steroid ve azatiyopürin kullanıldı. Konvansiyonel tedaviye cevap vermeyen 10 hastaya anti-TNF tedavisi verildi. İmmünolojik değerlendirme yapılan 48 hastanın 20'sinde (%41,7) immün yetmezlik saptandı. Dördüne kök hücre transplantasyonu yapıldı. Transplantasyon sonrası 2 hastanın kliniği düzeldi ve tedavileri kesildi. Semptom başlangıcı 6 yaşın altında olan 36 (%39,1) hasta mevcuttu. Altı yaş altındaki hastalarda Crohn hastalığı daha sıktı (%75,0/%53,6). İlk semptomdan tanıya kadar geçen süre 6 yaş altı grupta daha uzundu (2,02,7 yıl/0,91,0 yıl). Altı yaş altında en sık yakınma ishal (%80,6) iken 6 yaş üzerinde yakınma karın ağrısı (%87,5) idi. Crohn hastalığında izole üst GİS tutulumu 6 yaş altında hiçbir hastada görülmedi. Ülseratif kolitte her iki yaşta en sık pankolit görülmüş olup 6 yaş altında proktit veya sol kolon tutulumu gözlenmedi. Altı yaş altında büyüme geriliği, komplikasyonlar ve dirençli hastalık daha sıktı.Anahtar Kelimeler: İnflamatuvar bağırsak hastalıkları, Çok erken başlangıçlı inflamatuvar bağırsak hastalıkları, Ülseratif kolit, Crohn hastalığı Inflammatory bowel disease (IBD) is a group of multisystemic diseases characterized by multifactorial chronic idiopathic intestinal inflammation. Inflammatory bowel disease encompasses the diagnoses which include Crohn's disease, ulcerative colitis and unclassified colitis. It is also classified according to age groups and disease characteristics change between different age groups. This study included 92 patients who were diagnosed with IBD between 2000-2017 in the Department of Child Gastroenterology, Hepatology and Nutrition, Hacettepe University. Patients were divided into groups according to the diagnosis of ulcerative colitis or Crohn's disease and the age at symptom onset before or after 6 years of age. The patient who's age at symptom onset is before 6 years, defined as very early onset IBD. 57 patients (61,9%) were diagnosed with Crohn's disease, 33 (35,8%) with ulcerative colitis and 2 (2,2%) unclassified colitis. The mean age at symptom onset was earlier in Crohn's disease than in ulcerative colitis (6,95,6 years/9,65 years). The time between symptom onset and diagnosis was longer in Crohn's disease than in ulcerative colitis (1,42,0 years/1,11,9 years). Abdominal pain and diarrhea were the most common symptoms in Crohn's disease while bloody stool was in ulcerative colitis. Occult blood in stool, anemia and elevated C-reactive protein levels were the most common laboratory results. Erythrocyte sedimentation rate and C-reactive protein level were higher in Crohn's disease. Acute phase reactants were within normal limits in 18 (19,6%) patients. The most abnormal finding was detected on gastric mucosa in esophagogastroduodenoscopy. All patients with ulcerative colitis had abnormal colonoscopy findings and pancolitis was the most common finding (54,5%). `Backwash ileitis` was detected in 5 (15,1%) patients. Ileocolonic involvement was the most common finding (45,6%) in Crohn's disease. Seven patients had terminal ileum involvement while colon was normal. The most common abnormal finding in histopathological evaluation was active colitis. Corticosteroids, 5-aminosalicylate and azathiopurine were used mostly as therapeutic agents whereas 10 patients treated with anti-TNF drugs. Immunodeficiency was detected in 20 of the 48 patients (41,7%) who underwent immunologic evaluation and 4 of them underwent stem cell transplantation. Two of them had reached clinical remission after stem cell transplantation and all treatment was with-drawn. There were 36 patients (39,1%) who were under 6 years of age at symptom onset. In which Crohn disease was much more common (%75,0/%53,6) and the mean time between symptom onset and diagnosis was longer (2,0 ± 2,7 years/0,91,0 years). The most common complaint was diarrhea (80,6%) in very early onset IBD patients whereas it was abdominal pain (87,5%) in the older onset group. Isolated upper gastrointestinal system involvement in Crohn's disease was not seen in very early onset IBD patients. Ulcerative colitis mostly gone with pancolitis in both ages and no proctitis or left colon involvement was seen in very early onset group. Araştırmanın temel amacı MEB'in düzenlediği Yüz Temel Eser listesindeki yerel masalların, Hayat Bilgisi dersi öğretim programlarında öğrencilere kazandırılması öngörülen değerlerin aktarılmasında ne gibi katkısının olabileceğini ortaya koymaktır. Bunun yanı sıra Hayat Bilgisi ders kitaplarında da bu değerlerin nasıl ele alındığını ortaya koymak ikinci bir amacımızdır. Bu bağlamda incelenen kitaplardan hangisinin hangi değeri daha iyi kazandıracağı noktasında öğretmenlerde, eğitimcilerde ve ebeveynlerde fikir oluşturması amaçlanmaktadır. Ders kitapları ve masal kitapları incelenerek değerler eğitiminde bu kitaplardan yararlanılıp, yararlanılamayacağı üzerinde fikir geliştirilmeye çalışılmıştır.Araştırmanın birinci bölümünde içerik analizi yöntemi kullanılarak Hayat Bilgisi ders kitapları incelenmiş, ikinci bölüm ise betimsel bir çalışma olduğundan, nitel araştırma tekniklerinden genel tarama modeli kullanılmıştır.Sonuç olarak incelenen ders kitaplarının Hayat Bilgisi öğretim programında yer alan bütün değerleri kapsadığı ancak bu değerlerin dışında kalan birçok değeri barındırmadığı tespit edilmiştir. Bu nedenle ders kitapları Hayat Bilgisi öğretim programında yer alan değerleri aktarmada yeterli olmakla beraber bunların dışında kalan değerleri aktarmakta yetersiz olmaktadır.MEB'in 2004 yılında tavsiye ettiği Yüz Temel Eser'deki yerel masallarda ise Hayat Bilgisi öğretim programında yer alan değerlerin tamamının etkili bir şekilde işlenmediği tespit edilmiştir. Bazı değerlerin sıklıkla yer aldığı, bazı değerlerin ise bir-iki örnekle sınırlı kaldığı görülmüştür. Bu nedenle özellikle az yer alan değerlerin geliştirilmesinde Yüz Temel Eser listesinde yer alan masal kitaplarıyla sınırlı kalınmamalıdır.Bunun yanı sıra Yüz Temel Eser listesinde yer alan masal kitaplarında, Hayat Bilgisi öğretim programında yer almayan birçok faklı değerin yer aldığı tespit edilmiştir. The main aim of this research is to disclose whether, and if so, how the stories in 'a hundred main books' which are compiled by MONE in 2004 can contribute to transmitting students the values that are identified in the humanities subject curriculum. Furthermore, it aims at revealing how such values are taken into consideration in the humanities textbooks. Therefore, it is presumed that teachers, practitioners and parents will have an idea about which textbooks promote which specific value or values. By examining the textbooks and the story books this study has sought out whether these books can be utilised in values education.In the first part of the research, content analysis has been used in examining and analysing the textbooks. In the second part, as this was a descriptive study, the general examination model, which is a qualitative research method, has been used.Overall, it has been found out that the humanities subject textbooks that were examined in this study only include the values identified in the social studies curriculum. Therefore, whilst they seem to be sufficient in transmitting the values targeted in the humanities curriculum, they are not sufficient enough to transmit students any other values. In the stories that are included in 'a hundred main books', it is seen that the targeted values are not taken into account in an efficient way either. In some books only a number of values are engaged with whilst others are not given any considerable thought. Therefore, it is suggested that in transmitting certain values these books should not be seen as enough. Furthermore, it has been found out that the story books in the series of 'a hundred main books' include many different values than the humanities curriculum. Ancak ekonominin lokomotifi olarak nitelendirilen bu işletmelerin, gerek çıraklıktan yetişme veya pratik eğitimini almış, gerekse baba mesleği olarak işini sürdüren kişilerce yönetilmesi bazı yönetim sorunlarının yanında pek çok yapısal sorunları da beraberinde getirmektedir. KOBİ’lerin ekonomi için arz ettikleri öneme paralel olarak bir takım yapısal sorunlarının varlığı üzerinde durulması gereken bir konudur.Bu çalışmanın amacı, Şırnak ilinde faaliyet gösteren KOBİ’lerin demografik özelliklerini belirleyerek onların yapısal sorunlarını ortaya çıkarmaktır. Analiz sonuçlarına göre söz konusu bölgedeki KOBİ’ler için çözüm önerileri sunulmaya çalışılmıştır.

Çünkü örgüte bağlılıkları yüksek olan iş görenler örgütün amaç ve değerlerini benimsemekte, işini iyi yapmak için gönüllü bir çaba içerisine girmekte ve bunun dışında iş gören devrinin azalmasını da sağlamaktadır. İşgörenin bağlılığına etki eden çeşitli faktörler bulunmakta olup, araştırmamızda birincil etkiye sahip olduğu varsayılan örgütsel adalet ile ilişkisi incelenmektedir.Bu çalışmanın amacı iş görenlerin, örgütsel adalet algılamalarının örgütsel bağlılık ve alt boyutları arasındaki ilişkiyi ve ilişkinin yönünü belirlemek veya incelemektir. Bu doğrultuda, çalışma üç bölüme ayrılmış ve ilk bölümde örgütsel adalet ile ilişkili kavramlar değerlendirilmiş ve bu kavramla ilgili geliştirilen teoriler ve alt boyutları olan dağıtımsal, prosedürel ve etkileşimsel adalet boyutları teorik perspektiften değerlendirilmiştir. ikinci bölümde örgütsel bağlılık ile ilişkili kavramlara yer verilmiştir ve bağlılık ile ilgili teoriler ve alt boyutlar olarak duygusal bağlılık, devam bağlılığı ve normatif bağlılık kavramlarına yer verilmiştir. Çalışmamızın üçüncü ve son bölümünde ise, örgütsel adalet ve örgütsel bağlılık arasındaki ilişki uygulamalı olarak incelenmiş ve devamında konu ile ilgili Hatay ili Organize Sanayi Bölgesinde faaliyette bulunan tekstil sektörü çalışanlarına uygulanan anket bulgularına yer verilmiştir. Literatürde konu ile ilgili araştırmalar bulunmakla birlikte tekstil sektöründe daha önce uygulamalı bir çalışma yapılmadığı tespit edilmiştir. Knowing that there are lots of factors affecting employees commitment, in this dissertation, the relationship between organizational commitment and organizational justice, which is assumed the primary influence on the issue is studied.The aim of this study is spesifing the relationship between commitment, justice and dimension of employees. In this direction, the study is devided into three parts. At the first part, the notions related to the organizational justice are examined and the theories developed about the organizational justice is studied in theorical perspective with the distributional, prosedural and interactional sub-dimensions of the justice. 33. Polyarticular juvenile idiopathic arthritis has a distinct co-inhibitor receptor profile.

Objectives: Juvenile Idiopathic Arthritis (JIA) is the most common rheumatic disease of childhood; the pathogenesis is associated with T cell activation. T cell activation can be counter-balanced by signals generated by inhibitory receptors (IRs) such as CTLA-4, PD-1, LAG-3, and TIM-3. Here, we identify the role of IRs in the pathogenesis of different JIA subtypes., Methods: In total, we included 67 oligoarticular JIA, 12 IgM-RF negative polyarticular JIA, 17 enthesitis related arthritis, 11 systemic JIA patients and 10 healthy controls. We collected plasma (and synovial fluid) samples from the patients either at the onset or during a flare of their disease. We measured the soluble levels of co-IRs (IL-2Rα, 4-1BB, CD86, TGF-β1, CTLA-4, PD-L1, PD-1, TIM-3, LAG- 3, Galectin-9) by cytometric bead array kits and their cellular expression (PD-1, CTLA-4, TIM-3, LAG-3) by flow cytometry. We compared the plasma levels and cellular expressions of different co-IRs within different JIA subgroups., Results: The polyarticular-JIA group was different from the three other examined JIA subgroups, having higher levels of plasma sCTLA-4(p< 0.001), sPD-1(p< 0.05), and s4-1BB(p< 0.05) when compared with the other JIA subgroups and healthy controls. We analyzed the cellular surface expression of different co-IRs on the PBMCs of different JIA subtypes. Similar to plasma levels, both the percentage(p< 0.05) and the MFI (mean fluorescence intensity) (p< 0.01) of CTLA4 expression were higher in the poly-JIA subgroup., Conclusion: This is the first report studying the expression profile of different co-IRs in different subtypes of JIA. Polyarticular JIA patients had a different co-IR profile, having more CTLA-4, PD-1 and 4-1BB in their plasma than the other subtypes of JIA., (© The Author(s) 2024. 34. Transitioning from pediatric to adult-oriented healthcare in rheumatology: the pediatric side of the coin.

Objectives: The transition of adolescents and young adults (AYAs) from pediatric to adult-oriented healthcare may be affected by many factors, including the personal and cultural settings. We aimed to analyze the transition readiness and the factors affecting the transition success in rheumatology., Methods: Patients older than 12 years were included in this prospective study. All filled out the Transition Readiness Assessment Questionnaire (TRAQ) 5.0. AYAs were phone-interviewed after their transfer to adult-oriented healthcare. Drug adherence was evaluated with 4-item Morisky Medication Adherence Scale (MMAS-4). AYAs rated their transitional care experience with visual analogue scale (VAS 0-10; 0, the worst; 10, the best)., Results: A total of 504 TRAQs were filled out by 406 patients (F/M = 1.5). The total TRAQ score was positively correlated with age and higher in the forms filled out by girls than boys (4.2 vs 4.0, respectively; p= 0.005). The transition was successful for 78 (83.9%) out of 93 patients transferred to adult-oriented healthcare. The VAS for the transition process was lower and the post-transfer MMAS-4 score was worse (8 vs 9, p= 0.030 and 3 vs 4, p= 0.020; respectively) in patients whose transition was not successful when compared with the successfully-transitioned ones. The best-performing TRAQ cut-off value was >4.0 for predicting transfer readiness in rheumatology., Conclusion: A TRAQ score of > 4 could be used while deciding about the transfer readiness of AYAs in rheumatology. Improving the AYAs' experience of the transition process and closely monitoring medication adherence during transition are essential for a successful transition., (© The Author(s) 2024. Objectives: Familial Mediterranean fever (FMF) is characterized by febrile polyserositis attacks. Menstruation could be a trigger for attacks. We aimed to analyze the features of adolescent FMF patients with menstruation-associated attacks and propose a management algorithm., Methods: All female FMF patients who had menarche and visited the Pediatric Rheumatology Unit between January-December 2022, were included into this study. Demographics, general characteristics, and the features of menstrual cycle and FMF attacks were noted., Results: A total of 151 female FMF patients were included. Thirty-five (23.2%) had menstruation-associated attacks. Fever and arthritis were less frequent during the menstruation-associated attacks than the attacks not associated with menstruation in these patients (65.7% vs 88.6%, p= 0.01 and 2.9% vs 20%, p= 0.04; respectively). Patients with menstruation-associated FMF attacks were younger at symptom onset and diagnosis (2.5 vs 5 years, p= 0.004 and 4 vs 7 years, p= 0.01; respectively), had a higher rate of dysmenorrhea (74.3% vs 38.8%, p< 0.001, respectively) and higher pre- and post-menarche attack frequency (4 vs 2 and 10 vs 0, respectively; p< 0.001 for both) than patients whose attacks were not associated with menstruation. The interventions for menstruation-associated attacks included initiating colchicine, increasing the dose of colchicine, switching from coated to compressed colchicine tablets or anti-interleukin 1 drugs, and on-demand non-steroidal anti-inflammatory drugs, on-demand glucocorticoids, and on-demand anakinra. On-demand therapies were beneficial in controlling menstruation-associated attacks., Conclusions: This is the largest cohort of adolescent FMF patients with menstruation-associated attacks. Severe FMF may cause tendency to this association. On-demand therapies could be preferred in the management., (© The Author(s) 2024.
