Abstract

Inhibitors of the mechanistic target of rapamycin (mTOR) are currently used to treat advanced metastatic breast cancer. However, whether an aggressive phenotype is sustained through adaptation or resistance to mTOR inhibition remains unknown. Here, complementary studies in human tumors, cancer models and cell lines reveal transcriptional reprogramming that supports metastasis in response to mTOR inhibition. This cancer feature is driven by EVI1 and SOX9. EVI1 functionally cooperates with and positively regulates SOX9, and promotes the transcriptional upregulation of key mTOR pathway components (REHB and RAPTOR) and of lung metastasis mediators (FSCN1 and SPARC). The expression of EVI1 and SOX9 is associated with stem cell-like and metastasis signatures, and their depletion impairs the metastatic potential of breast cancer cells. These results establish the mechanistic link between resistance to mTOR inhibition and cancer metastatic potential, thus enhancing our understanding of mTOR targeting failure.

Abstract

La caza marina fue una actividad esencial en el modo de vida de los habitantes del litoral del desierto de Atacama, al norte de Chile. Un pilar de su estructura social que tenía como base material los medios técnicos para realizarla, como las embarcaciones y su sofisticado dispositivo de arponaje, descansando en este último la posibilidad de capturar y arrastrar hacia tierra firme presas que superaban sustancialmente el tamaño y peso del cazador. En la actualidad muy poco se sabe del arponaje desde la arqueología. Por esto ahondaremos en su morfología, composición, cualidades técnicas, diversidad instrumental y cronología a partir de una muestra de 129 cabezales de arpón de distintos períodos de la prehistoria (6500 cal AP a momentos coloniales). Comenzaremos definiendo conceptualmente la función del arpón, seguido de un primer ejercicio tipológico que ordene la multiplicidad de formas y soluciones a nivel mundial para ubicar los ejemplares analizados. En un segundo ejercicio tipológico clasificaremos la diversidad de expresiones locales, diferenciándolas materialmente y situándolas en la secuencia cronológica. Solo después de esta necesaria etapa clasificatoria podremos aproximarnos a su tecnología de arponaje, el rol de los cazadores como sujetos sociales y el papel de la caza en la sociedad litoral.

Abstract

The recovery of motor function after suffering a brain injury is still poorly understood. There are complex interactions between motor learning and motor recovery principles, which guide neural reorganization and allow the reacquisition and restitution of goal-oriented motor patterns (Maier et al. in Front. Syst. Neurosci. 13:74 (2019) [1]). This paper, reviews several studies and applications for rehabilitation and impairment assessment. Were we capitalize on the interaction between motor learning and motor recovery principles and investigate the manipulation of visuomotor feedback for the delivery of optimal neurorehabilitation and diagnostics for motor deficits after stroke and cannabis addiction.

21. Intraoperative Scintigraphy With Portable Gamma Camera for the Localization of Interaortocaval Paraganglioma

Abstract

A 15-year-old adolescent girl diagnosed of interaortocaval paraganglioma with a positive 123I-MIGB SPECT/CT and 1 unsuccessful prior surgery was operated on with the assistance of a handheld gamma camera. Once the lesion was located and removed, 2 images were taken, one of the surgical field (without 123I-MIGB uptake) and another of the tumor ex vivo (with high 123I-MIGB uptake), confirming that the lesion had been satisfactorily excised. This case highlights the use of a portable gamma camera as a useful tool to locate this rare tumor, with a SPECT/CT positive for 123I-MIGB and a difficult anatomical location suspected. 22. Effectiveness and efficiency of a special program to reduce the bariatric surgery waiting list at a tertiary hospital

Abstract

INTRODUCTION: Bariatric surgery is one of the most common surgical practices in Spain. However, this procedure currently has longest delay on surgical waiting lists (SWL). We have developed a special surgical program that aims to reduce this waiting list and to assess the economic and clinical repercussions in a high-volume bariatric surgery unit. METHODS: A three-month prospective study was carried out comparing outcomes, results and perioperative resources consumed for 45 patients who underwent bariatric surgery. The patients were divided into 2 groups: patients who underwent the standard procedure in the operating room, and patients treated in the special program. Epidemiological, healthcare and economic factors were taken into account. RESULTS: Two homogeneous groups of patients were operated on, successfully reducing the SWL. Morbidity was similar in both groups and the average cost of the surgeries performed was 5,331.40; in the standard group, the cost was 5,372.50±798.10, and the cost of the special program group was 5,290.30±685.10, with no significant differences. CONCLUSIONS: In hospitals with a high volume of bariatric surgery, it is feasible to incorporate special surgical programs that are able to reduce surgical waiting lists, while maintaining quality criteria and without incurring a greater expense to the healthcare system.

26. [Extraovarian granulosa cell tumor with FOXL2 mutation. Morphological and immunohistochemical differential diagnosis]

Abstract

Extraovarian granulosa cell tumor is a very uncommon tumor and the identification of a recurrent mutation in FOXL2 may be used as another diagnostic tool along with the classical morphological and immunohistochemical findings. Here, we report a new case of extraovarian granulosa cell tumor in a 57 years old female patient presented with a sub-hepatic mass and abdominal pain. Histopathological examination of the excised mass showed features of adult-type granulosa cell tumor with alpha-inhibin, calretinin, WT1, S100, CD99 and progesterone receptor immunoreactivity. A FOXL2 mutation was detected on molecular biology study. A final diagnosis was an extraovarian adult-type granulosa cell tumor. 27. Extensive colonic pneumatosis. Conservative or Surgical approach?

Abstract

Intestinal pneumatosis is a radiologic finding that consists of the presence of air in the intestinal wall. The etiology is extensive and can range from a benign entity to one that compromises the patients life. It is important to know and recognize the differences between them as the surgical management will depend on it. We report the case of an 82-year-old male diagnosed with a metastatic castration resistant prostate carcinoma, under chemotherapy with disease progression in treatment with high doses of intravenous morphics. The patient reported abdominal distension with constipation and no vomiting in the emergency department and a distended and tympanic abdomen, without generalized peritonitis was identified during physical examination. A blood analysis showed lactate levels of 0.9 mmol/L and a PCR of 4.2 mg/L without leukocytosis. An extensive colonic pneumatosis with minimum pneumoperitoneum without free fluid was confirmed by a computed tomography (CT) scan (Fig 1). A conservative approach with intravenous antibiotic and clinical surveillance was decided due to the fact that the patient was clinically stable without generalized peritonitis nor pathological findings in the supplementary blood tests. The clinical evolution was uneventful and the patient was discharged from hospital De la Serna et al. opted for a conservative approach for a patient with stage IV lung adenocarcinoma under chemotherapy treatment, who presented asymptomatic intestinal pneumatosis and subsequently made a full recovery. We also believe that the use of some chemotherapeutic agents may cause an increase in the permeability of the intestinal capillaries, allowing air to flow into the intestinal walls. Thus, resulting in this radiologic image, which is more a radiological finding than a disease. We think that a conservative initial attitude should be recommended in hemodynamically stable cases, without peritonitis and non-pathological blood analysis.

Abstract

The Large Hadron Collider (LHC) is one of the largest scientific instruments ever built. Since opening up anew energy frontier for exploration in 2010, it has gathered a global user community of about 9000 scientists working in fundamental particle physics and the physics of hadronic matter at extreme temperature and density. To sustain and extend its discovery potential, the LHC will need a major upgrade in the 2020s. This will increase its instantaneous luminosity (rate of collisions) by a factor of five beyond the original design valueand the integrated luminosity (totalnumber of collisions) by a factor ten. The LHC is already a highly complexand exquisitely optimised machine so this upgrade must be carefully conceived and will require new infrastructures(underground and on surface)and over a decade to implement. The new configuration, known as High Luminosity LHC (HL-LHC), relies on a number of key innovations that push accelerator technology beyond its present limits. Among these are cutting-edge 11–12Tesla superconducting magnets, compact superconducting cavities for beam rotation with ultra-precise phase control, new technology and physical processes for beam collimation and 100 metre-long high-power superconducting links with negligible energy dissipation, all of which required several years of dedicated R&D; effort on a global international level. The present document describes the technologies and components that will be used to realise the projectand is intended to serve as the basis for the detailed engineering design of the HL-LHC.

© 2019 The Author(s). Background: It seems that lumen-apposing metal stents (LAMS) are displacing plastic stents in the therapy of pancreatic-fluid collection in walled-off necrosis (WON). To date, there is no quality of evidence to recommend LAMS as the standard treatment in the management of WON. The theoretical benefit of LAMS over plastic stents needs to be proven. Methods/design: This is a randomized controlled, multicenter, prospective clinical trial with two parallel groups, without masking. One-hundred and fourteen patients with WON will undergo endoscopic ultrasound (EUS)-guided transmural draining in nine tertiary hospitals in Spain and will be randomized to the LAMS or plastic-stent group. The primary endpoint is the short-term (4 weeks) clinical success determined by the reduction of the collection (to < 50% or < 5 cm in size), along with clinical improvement. Secondary endpoints: long-term (4 months) clinical success (total resolution or 5 cm), procedure duration, level of difficulty, safety, and recurrences. Discussion: The PROMETHEUS trial has been designed to determine whether LAMS are superior to plastic stents in EUS-guided transmural drainage of WON. Abstract

International audience; Marine hunting was an essential activity in the way of life of the Atacama Desert littoral inhabitants, at northern Chile. A pillar in their social structure that was based in the material means to realize it, as boats and their sophisticated harpoon device, resting in the last the possibility of capture and drag mainland preys that substantially outweigh the hunter. Currently the knowledge of harpooning is scarce in archaeology. For this reason we will delve in their morphology, composition, technical qualities, instrumental diversity, and chronology from a 129 harpoon heads sample from different prehistoric periods (6500 cal BP to colony). We will begin conceptually defining the harpoon function, followed by a first typological exercise to order the multiplicity of forms and solutions in a global scale to place the analyzed specimens. In a second typological exercise we will classify the local expression diversity, differentiating them materially and placing them chronologically. Only after this necessary classificatory stage we can approach to their harpoon technology, the hunter role as a social subject, and the position of hunting in the littoral society.; La caza marina fue una actividad esencial en el modo de vida de los habitantes del litoral del desierto de Atacama, al norte de Chile. Un pilar de su estructura social que tenía como base material los medios técnicos para realizarla, como las embarcaciones y su sofisticado dispositivo de arponaje, descansando en este último la posibilidad de capturar y arrastrar hacia tierra firme presas que superaban sustancialmente el tamaño y peso del cazador. En la actualidad muy poco se sabe del arponaje desde la arqueología. Por esto ahondaremos en su morfología, composición, cualidades técnicas, diversidad instrumental y cronología a partir de una muestra de 129 cabezales de arpón de distintos períodos de la prehistoria (6500 cal AP a momentos coloniales). Comenzaremos definiendo conceptualmente la función del arpón, seguido de un primer ejercicio tipológico que ordene la multiplicidad de formas y soluciones a nivel mundial para ubicar los ejemplares analizados. En un segundo ejercicio tipológico clasificaremos la diversidad de expresiones locales, diferenciándolas materialmente y situándolas en la secuencia cronológica. Solo después de esta necesaria etapa clasificatoria podremos aproximarnos a su tecnología de arponaje, el rol de los cazadores como sujetos sociales y el papel de la caza en la sociedad litoral.

Treatment for the DLBCL-RS was initiated in 8/12 cases, being 92% death at data cut-off date (median follow-up 6.5 months from DLBCL-RS diagnosis, IQR 2.4-9.7). Three cases were concomitantly diagnosed with CLL and DLBCL-RS; the median time of transformation in the remaining 9/12 was 76 months (IQR 51-112), and all of them had received at least 1 therapeutic line for the CLL (median 2).
