7. The psychiatric symptoms, diagnoses and care needs of 100 mentally handicapped patients

Abstract

In the last decade there has been a substantial decrease in the number of mentally handicapped individuals placed in long-term care in hospitals. This is largely due to deinstitutionalization and a move to provide community care for many of these individuals. In order to understand the link between mental handicap and psychiatric illness, and the need for inpatient versus community residential long-term placement of these patients, 100 patients were randomly selected from a hospital population. Nursing staff were asked to report on behavioral problems during the week prior to the study, each patient underwent a psychiatric interview, and health care and residential care needs were identified from professional evaluations. Patients included 55 men and 45 women with a mean age of 43.2 years. Average hospital stay was 24.4 years. Sixty were receiving psychiatric medication. Fifty-nine patients were rated as having at least one pathological behavior problem, and 80 were psychiatrically diagnosed. Only 30 percent were rated to be in need of long-term care in a hospital, but 85 percent were in need of specialist care. All but one was rated as needing residential care, but for only 31 was this judged to be a mental handicap hospital. Most of these displayed more severe psychiatric disorders than the rest. Statistically, it is recommended that there be 30 long-term hospital beds available per 100,000 population in order to properly care for those who require long-term hospital placement. The rest can be cared for in residential care programs in the community and in less restrictive environments when possible. Staff availability in the community is an important prerequisite, however. Abstract

Background: PDSAFE is an individually-tailored, physiotherapist-delivered, balance, strength and strategy training programme aimed at preventing falls among people with Parkinson's. We evaluated the cost-effectiveness of PDSAFE compared with usual care for people with Parkinson's at higher risk of falling, from a UK National Health Service and Personal Social Service perspective.Methods: Resource use and quality of life data (EQ-5D-3L) were collected from 238 participants randomised to the PDSAFE intervention and 236 participants randomised to control, at baseline, 3 months, 6 months (primary outcome), and 12 months. Adjusted cost and quality-adjusted life-years (QALYs) were estimated using generalised linear models and uncertainty estimated using a non-parametric bootstrap.Results: Over 6 months, the PDSAFE intervention was associated with an incremental cost of £925 (95% CI £428 to £1422) and a very small and statistically insignificant QALY gain of 0.008 (95% CI - 0.006 to 0.021). The resulting incremental cost-effectiveness ratio (ICER) was £120,659 per QALY and the probability of the intervention being cost-effective at a UK threshold of £30,000/QALY was less than 1%. The ICER varied substantially across subgroups although no subgroup had an ICER lower than the £30,000 threshold. The result was sensitive to the time horizon with the ICER reducing to £55,176 per QALY when adopting a 12-month time horizon and assuming a sustained treatment effect on QoL, nevertheless, the intervention was still not cost-effective according to the current UK threshold.Conclusions: Evidence from this trial suggests that the PDSAFE intervention is unlikely to be cost-effective at 6 months. The 12-month analysis suggested that the intervention became closer to being cost-effective if quality of life effects were sustained beyond the intervention period, however this would require confirmation. Further research, including qualitative studies, should be conducted to better understand the treatment effect of physiotherapy and its impact on quality of life in people with Parkinson's given existing mixed evidence on this topic.Trial Registration: ISRCTN48152791. Abstract

This report presents an assessment of the efficiency and power density limitations of thermophotovoltaic (TPV) energy conversion systems for both ideal (radiative limited) and practical (defect-limited) systems. Thermodynamics is integrated into the unique physics of TPV conversion, and used to define the intrinsic tradeoff between power density and efficiency. The results of the analysis reveal that the selection of diode band gap sets a limit on achievable efficiency well below the traditional Carnot level. In addition it is shown that filter performance dominates diode performance in any practical TPV system and determines the optimum band gap for a given radiator temperature. It is demonstrated that for a given radiator temperature, lower band-gap diodes enable both higher efficiency and power density when spectral control limitations are included. The goal of this work is to provide a better understanding of the basic system limitations that will enable successful long-term development of TPV energy conversion technology. © 2001 American Institute of Physics.
