INTRODUCTION The high potential of Microbeam Radiation Therapy (MRT) in improving tumor control whilst reducing side effects has been shown by numerous preclinical studies. MRT offers a widened therapeutic window by using the periodical spatial fractionation of synchrotron generated x-rays into an array of intense parallel microbeams. MRT now enters a clinical transfer phase. As proof of principle and cornerstone for the safe clinical transfer of MRT, we conducted a "first in dog" trial under clinical conditions. In this report, we evaluated whether a 3D conformal MRT can be safely delivered as exclusive radiosurgical treatment in animal patients. METHODS We irradiated a 17.5 kg French Bulldog for a spontaneous brain tumor (glioma suspected on MRI) with conformal high dose rate microbeam arrays (50µm-wide microbeams, replicated with a pitch of 400μm) of synchrotron-generated x-rays. The dose prescription adjusted a minimal cumulated valley dose of 2.8Gy to the PTV (CTV+1mm). Thus, each beam delivered 20-25Gy to the target as peak doses, and ∼1Gy as valley doses. RESULTS The treatment was successfully delivered. Clinical follow-up over 3 months showed a significant improvement of the dog's quality of life: the symptoms disappeared. MRI, performed 3 months post irradiation, revealed reduction in tumor size (-87.4%) and mass effect with normalization of the left lateral ventricle. CONCLUSION To our knowledge, this neuro-oncologic veterinary trial is the first 3D conformal synchrotron x-ray MRT treatment of a spontaneous intracranial tumor in a large animal. It is an essential last step towards the clinical transfer of MRT in the near future, to demonstrate the feasibility and safety of treating deep seated tumors using synchrotron-generated microbeams.

International audience; Simple Summary: We unveiled the potential of an innovative irradiation technique that ablates brain cancer while sparing normal tissues. Spatially fractionating the incident beam into arrays of micrometer-wide beamlets of X-rays (MRT for Microbeam Radiation Therapy) has led to significantly increased survival and tumor control in preclinical studies. Multiport MRT versus conventional irradiations, for the same background continuous dose, resulted in unexpectedly high equivalent biological effects in rats that have not been achieved with any other radiotherapeutic method. These hallmarks of multiport MRT, i.e., minimal impact on normal tissues and exceptional tumor control, may promote this method towards clinical applications, possibly increasing survival and improving long-term outcomes in neuro-oncology patients. Delivery of high-radiation doses to brain tumors via multiple arrays of synchrotron X-ray microbeams permits huge therapeutic advantages. Brain tumor (9LGS)-bearing and normal rats were irradiated using a conventional, homogeneous Broad Beam (BB), or Microbeam Radiation Therapy (MRT), then studied by behavioral tests, MRI, and histopathology. A valley dose of 10 Gy deposited between microbeams, delivered by a single port, improved tumor control and median survival time of tumor-bearing rats better than a BB isodose. An increased number of ports and an accumulated valley dose maintained at 10 Gy delayed tumor growth and improved survival. Histopathologically, cell death, vascular damage, and inflammatory response increased in tumors. At identical valley isodose, each additional MRT port extended survival, resulting in an exponential correlation between port numbers and animal lifespan (r(2) = 0.9928). A 10 Gy valley dose, in MRT mode, delivered through 5 ports, achieved the same survival as a 25 Gy BB irradiation because of tumor dose hot spots created by intersecting microbeams. Conversely, normal tissue damage remained minimal in all the single converging extratumoral arrays. Multiport MRT reached exceptional similar to 2.5-fold biological equivalent tumor doses. The unique normal tissue sparing and therapeutic index are eminent prerequisites for clinical translation.

In 2016 the South East European International Institute for Sustainable Technologies (SEEIIST) was proposed by Herwig Schopper and brought to the political level by Sanja Damjanovic, Minister of Science of Montenegro. In this framework two design studies have been completed by two groups of European experts: a "4th Generation Synchrotron Light Source for Science and Technology" (SRL) and a "Facility for Tumour Hadron Therapy and Biomedical Research" (HTR). A preliminary report was presented and discussed at the Workshop on "New International Research Facilities for South East Europe" held in January 2017 at ICTP (Trieste). In March 2018 the Steering Committee came unanimously to the conclusion that the first facility to be built should be the HTR. This report contains the HTR study, which was completed in July 2018; the Executive Summary has been written for the readers who are not interested in the details.

International audience; Among over 100 proton therapy centres worldwide in operation or under construction, French proton therapy is coming to full maturity with the recent opening of the Nice (1991, upgrade in 2016) and Caen (2018) facilities next to the Orsay (1991, upgrade in 2010) centre. Proton therapy is a national priority for children and young adults in all three centres. The patient-related activity of the three French centres is coordinated via the Protonshare portal to optimise referral by type of indication and available expertise in coordination with the French society of radiation oncology SFRO and French radiotherapy centres. The centres are recognised by the French Health Care excellence initiative, promoted by the ministry of Foreign Affairs. The three centres collaborate structurally in terms of clinical research and are engaged at the international level in the participation to European databases and research initiatives. Concerted actions are now also promoted in preclinical research via the Radiotransnet network. Ongoing French developments in proton therapy are well presented in international hadron therapy meetings, including European Proton Therapy Network and Particle Therapy Cooperative Oncology Group. Proton therapy teaching in France is offered at several levels and is open to colleagues from all radiation oncology centres, so that they are fully informed, involved and trained to facility recognition of possible indications and thereby to contribute to appropriate patient referral. This close collaboration between all actors in French radiation oncology facilitates the work to demonstrate the required level of medical and scientific evidence for current and emerging indications for particle therapy. Based on that, the future might entail a possible creation of more proton therapy facilities in France.; Il existe plus de 100 centres de proton thérapie dans le monde, et trois en France. La protonthérapie française est en phase de structuration et de montée en charge, avec l’ouverture récente des centres de Nice (2016) et Caen (2018) en plus du centre d’Orsay (2010) pour les indications corps entier. La prise en charge des enfants et adolescents-jeunes adultes est une priorité nationale partagée par les trois centres. L’action des trois centres français est coordonnée en termes d’adressage via le portail Protonshare sous l’égide de la Société Française de radiothérapie oncologique (SFRO). Les trois centres ont aussi une dynamique coordonnée d’offre de soin à des patients étrangers dans le cadre du réseau French Health Care du ministère des Affaires étrangères. Ils sont inscrits dans une démarche nationale et internationale (en particulier, en Europe, via l’European Proton Therapy Network) de participation à des essais thérapeutiques et des bases de données. Des actions concertées sont désormais favorisées en recherche préclinique via le réseau Radiotransnet. Dans ses développements, la protonthérapie française est bien représentée dans les congrès internationaux d’hadronthérapie. L’enseignement de la protonthérapie en France est proposé à plusieurs niveaux et ouvert aux centres photons, potentiellement adresseurs. La participation de l’ensemble des acteurs de la radiothérapie française est ainsi encouragée pour favoriser la participation de tous à la construction du niveau de preuve médicoscientifique requis pour une éventuelle extension future du parc de protonthérapie français.

Under the umbrella of the European Network for Light Ion Therapy (ENLIGHT), the project on Union of Light Ion Centers in Europe (ULICE), which was funded by the European Commission (EC/FP7), was carried out from 2009 to 2014. Besides the two pillars on Transnational Access (TNA) and Networking Activities (NA), six work packages formed the pillar on Joint Research Activities (JRA). The current manuscript focuses on the objectives and results achieved within these research work packages: “Clinical Research Infrastructure”, “Biologically Based Expert System for Individualized Patient Allocation”, “Ion Therapy for Intra-Fractional Moving Targets”, “Adaptive Treatment Planning for Ion Radiotherapy”, “Carbon Ion Gantry”, “Common Database and Grid Infrastructures for Improving Access to Research Infrastructures”. The objectives and main achievements are summarized. References to either publications or open access deliverables from the five year project work are given. Overall, carbon ion radiotherapy is still not as mature as photon or proton radiotherapy. Achieved results and open questions are reflected and discussed in the context of the current status of carbon ion therapy and particle and photon beam therapy. Most research topics covered in the ULICE JRA pillar are topical. Future research activities can build upon these ULICE results. Together with the continuous increase in the number of particle therapy centers in the last years ULICE results and proposals may contribute to the further growth of the overall particle therapy field as foreseen with ENLIGHT and new joint initiatives such as the European Particle Therapy Network (EPTN) within the overall radiotherapy community.

To enhance evidence-based introduction of particle therapy in Europe, one of the work packages within the European Proton Therapy network (EPTN) will focus on uniform data registration and defining methodological criteria for phase I, II and III clinical trials. The main objective of EPTN WP1 is to establish a uniform prospective data registration program for all patients treated with particle therapy in Europe. This will be supported by EORTC through existing and new additional QA-platforms and IT-infrastructures for data collection with different formats. In addition, EPTN WP1, to enhance high quality clinical trials, EPTN-WP1 will define the requirements for high quality clinical trials and set up an infrastructure for methodological support.

- Abstract
The primary tumor side is emerging as a prognostic factor for patients with liver metastatic colorectal cancer (mCRC). In a combined analysis of data from 2 randomized studies, the addition of selective internal radiation therapy to first-line chemotherapy in patients with mCRC was associated with statistically and clinically significant overall survival gains for patients with a right-sided primary tumor. Background(s): The primary tumor side is emerging as a major prognostic factor for patients with metastatic colorectal cancer (mCRC). We examined the survival data from 2 randomized studies to determine whether the outcomes differ between patients with mCRC with right-sided primary (RSP) tumors and those with left-sided primary (LSP) tumors after selective internal radiation therapy (SIRT) plus mFOLFOX6 (folinic acid [leucovorin], 5-fluorouracil, oxaliplatin) chemotherapy, versus chemotherapy alone. Patients and Methods: Separate and combined analyses were performed on the data from the SIRFLOX and FOXFIRE global trials, which compared chemotherapy plus SIRT with chemotherapy alone for patients with mCRC liver metastases. The primary tumor side data were prospectively collected. The principal outcome measure was overall survival (OS) stratified by treatment and primary tumor side. Result(s): In the combined analysis of all 739 patients enrolled, SIRT had no effect on OS (median OS, 24.3 vs. 24.6 months; hazard ratio [HR], 1.021; P =.810). For the 179 patients (24.2%) with a RSP tumor, OS was improved with the addition of SIRT (median, 22.0 vs. 17.1 months HR, 0.641; P =.008). The addition of SIRT was not associated with a significant difference in OS among the 540 patients with a LSP tumor (median, 24.6 vs. 26.6 months; HR, 1.120; P =.264). A test of treatment interaction by primary tumor side was statistically significant for RSP and SIRT (P =.002). - Abstract
The Union of Light Ion Centers in Europe (ULICE) program addressed the need for uniting scientific results for carbon-ion radiation therapy obtained by several institutions worldwide in different fields of excellence, and translating them into a real benefit to the community. Particularly, the concepts for dose/volume parameters developed in photon radiotherapy cannot be extrapolated to high linear energy transfer particles. The ULICE-WP2 taskforce included radiation oncologists involved in carbon-ion radiation therapy and International Commission on Radiation Units and Measurements, radiation biologists, expert physicists in the fields of carbon-ion radiation therapy, microdosimetry, biological modeling and image-guided radiotherapy. Consensual reports emerged from multiple discussions within both the restricted group and the wider ULICE community. Public deliverables were produced and disseminated to the European Commission. Here we highlight the disparity in practices between treating centers, then address the main topics to finally elaborate specific recommendations. Although it appears relatively simple to add geometrical margins around the clinical target volume to obtain the planning target volume as performed in photon radiotherapy, this procedure is not appropriate for carbon-ion radiation therapy. Due to the variation of the radiation quality in depth, there is no generic relative biological effectiveness value for carbon-ions outside of an isolated point, for a given fractionation and specific experimental conditions. Absorbed dose and "equieffective dose" for specified conditions must always be reported. This work contributed to the development of standard operating procedures for carbon-ion radiation therapy clinical trials. These procedures are now being applied, particularly in the first phase III international, multicenter trial (PHRC Étoile).

- Abstract
Under the umbrella of the European Network for Light Ion Therapy (ENLIGHT), the project on Union of Light Ion Centers in Europe (ULICE), which was funded by the European Commission (EC/FP7), was carried out from 2009 to 2014. Besides the two pillars on Transnational Access (TNA) and Networking Activities (NA), six work packages formed the pillar on Joint Research Activities (JRA). The current manuscript focuses on the objectives and results achieved within these research work packages: "Clinical Research Infrastructure", "Biologically Based Expert System for Individualized Patient Allocation", "Ion Therapy for Intra-Fractional Moving Targets", "Adaptive Treatment Planning for Ion Radiotherapy", "Carbon Ion Gantry", "Common Database and Grid Infrastructures for Improving Access to Research Infrastructures". The objectives and main achievements are summarized. References to either publications or open access deliverables from the five year project work are given. Overall, carbon ion radiotherapy is still not as mature as photon or proton radiotherapy. Achieved results and open questions are reflected and discussed in the context of the current status of carbon ion therapy and particle and photon beam therapy. Most research topics covered in the ULICE JRA pillar are topical. Future research activities can build upon these ULICE results. Together with the continuous increase in the number of particle therapy centers in the last years ULICE results and proposals may contribute to the further growth of the overall particle therapy field as foreseen with ENLIGHT and new joint initiatives such as the European Particle Therapy Network (EPTN) within the overall radiotherapy community.
