According to official surveys, the number of adolescents with mental health problems in Poland is increasing, which is also typical of other European countries. At the same time, programmes are being introduced to secure infrastructure and adequate therapeutic space by expanding or creating new, architecturally friendly centres. The comfort of the stay created by appropriate spatial conditions must be developed with the participation of the users (patients, staff and visitors). A qualitative study was carried out among staff, based on the structured interview method. The aim of the study was to diagnose the elements of architectural space that support health. It was hypothesised that the architecture of health care facilities could promote mental health. Using advances in environmental psychology, evidence-based design, and elements such as architectural form, spatial-functional layout and the surrounding environment were considered. The results of the study identified features of a user-friendly environment, creating a set of recommendations based on the research-by-design method. Consequently, the recommendations were implemented into the conceptual design. Their validity was proven using the example of the design of a health centre for young people in Klawa, Poland. Abstract The purpose of our study was to elucidate the association between obstetric and psychological factors and fear of childbirth (FOC) during the third trimester of pregnancy and to identify women at risk of severe FOC in Poland. An additional goal of the study was to verify the Polish version of the Wijma Delivery Expectancy Questionnaire (W-DEQ) and to establish its psychometric characteristics. Cross-sectional study with a total of 359 women recruited during routine visits to an antenatal clinic in Poland during the third trimester (≥ 27 weeks gestation). The survey included obstetric details (parity, obstetric history and preferred mode of delivery), and standardized psychological measures: the W-DEQ (fear of childbirth) and the EPDS (depressive symptoms). We demonstrated the satisfactory psychometric properties of the Polish version of the W-DEQ. Our findings confirm the one-factor structure found by the authors of the original version of the scale. A greater FOC was reported by women with unplanned pregnancies, women whose preferred mode of delivery was a cesarean section, and women who had previously undergone psychiatric treatment. The risk factors for severe FOC were depression, unplanned pregnancy or parity, and disagreement with the birth plan proposed by the obstetrician. The W-DEQ is a widely used, valid instrument for the assessment of FOC in pregnant women and can be used in Poland. Findings support the key role of obstetric and psychological variables in predicting fear of childbirth.

Preterm premature rupture of membranes (pPROM) before 22–24 weeks of gestation is considered previable and poses a high risk to the chances of successful delivery. Multicystic dysplastic kidney (MCDK) is the most common form of dysplasia of this organ, usually affecting 1 kidney. A prenatal suspicion of MCDK requires escalation of the clinical process to a reference center qualified and equipped for further diagnosis and treatment. We present a unique case of prenatal pPROM at 19 weeks of gestation in a pregnancy complicated by fetal MCDK as an interesting obstetric and neonatal patient story. We have not found any published evidence on the impact of coexisting fetal congenital malformations on a pregnancy affected by previable pPROM. The aim of this study is to highlight the importance of a multidisciplinary team approach in the management of previable pPROM and the difficulties of decision-making involving both health professionals and the mother. Initially, medical management focuses on the safety of the mother, and as the fetus reaches viability, the focus shifts to the high standard of care for the child. The final successful outcome and follow-up was the effect of coordinated prenatal and postnatal medical care. [ABSTRACT FROM AUTHOR]
