22. Successful treatment of ongoing intestinal allograft rejection permits recovery of graft structure and function

Abstract

Acute rejection episodes often complicate clinical small bowel transplantation, which prompted us to investigate whether such episodes can be reversed and the intestinal graft salvaged. Inbred Lewis rats that received fully allogeneic Brown-Norway small bowel allografts were treated with eyelosporin A (10 mg/kg) for 5 days, and the drug was then discontinued. Clinical and histologic signs of acute rejection developed, and the animals were subsequently treated with FK 506 (2 mg/kg) on days 14, 16, and 18. Survival was significantly prolonged ( 201.7[+ or -]46.8 days) when compared with animals that were not administered FK 506 (16.5[+ or -]0.8 days) or allograft recipients that received no immunosuppressive therapy (10.8[+ or -]0.7 days). The histologic changes and functional impairment due to rejection that were observed prior to the start of the FK 506-therapy were reversed. However, biopsy specimens of all animals exhibited features of chronic rejection. This study provides evidence that acute rejection of intestinal allografts can be successfully treated with a short course of FK 506.

Abstract

When transitional cell carcinoma (TCC) of the bladder invades the local muscle, the five-year survival rate drops to roughly 50 percent, even when the disease has remained confined to the bladder. This survival rate has not changed appreciably in 20 years, despite refinements in chemotherapy and other aspects of cancer care. A retrospective review was conducted of nine male patients with advanced transitional cell carcinoma of the bladder. These patients received neoadjuvant therapy with methotrexate, vinblastine, doxorubicin, and cisplatin (M-VAC) prior to surgical removal of the bladder, prostate gland, and pelvic lymph nodes. The surgically removed tissue was evaluated by a pathologist to see how it had responded to the chemotherapy. Two patients showed a complete response and an additional four showed a partial response. A clinical trial to compare the survival rates of patients treated with surgery alone versus those with M-VAC and surgery is currently underway and should be reported in the near future. 25. An Immunostaining Panel for Diagnosis of Malignancy in Mucinous Tumors of the Pancreas

Abstract

Background.--The diagnosis of malignancy in pancreatic mucinous cystic tumors depends on demonstrating invasion that may be focal and require extensive sectioning. Objective.--To explore markers that may indicate malignant potential in mucinous cystic tumors. Design.--Routinely processed sections from resected specimens of 12 normal pancreata, 14 pancreata with chronic pancreatitis, 9 mucinous cystic tumors, and 30 invasive adenocarcinomas were immunostained with antibodies to p53, HER.2/neu, epithelial growth factor receptor (EGFR), transforming growth factor [Alpha] (TGF-[Alpha], and Ki-67. Results.--Expression of p53, HER.2/neu, and Ki-67 was significantly more frequent in mucinous tumors than in normal pancreatic tissue and chronic pancreatitis tissue (P = .0003 to .05). Strong expression (more than one third of cells positive) and strong intensity (2+ and 3+) of staining of p53 and EGFR were seen only in carcinomas. Coexpression of p53/HER-2/neu and EGFR/HER.2/neu and a frequency of Ki-67+ nuclei of greater than 5% of cells discriminated between mucinous tumors and normal pancreatic tissue and chronic pancreatitis tissue, p53 expression was significantly more frequent in carcinomas than in mucinous tumor (P = .0326). Coexpression of p53/EGFR discriminated between mucinous tumors and carcinomas; however, TGF-[Alpha] was not discriminative. Conclusions.--The immunostaining panel of p53, HER2/neu, Ki-67, and EGFR can be helpful in indicating malignant potential in mucinous tumors of pancreas in routine pathology practice. Abstract

* Background.--Expression of cytokeratins 7 (CK7) and 20 (CK20) may help distinguish the site of origin for metastatic carcinomas. Little is known regarding their expression in biliary tract and pancreatic carcinomas. Our aim was to study the expression of CK7 and CK20 in these tumors. Design.--Fifty-three carcinomas of the extrahepatic bile ducts (n = 8), ampulla of Vater (n = 7), gallbladder (n = 11), and pancreas (n = 27), were retrieved from the surgical pathology files of the University of Massachusetts Medical Center. Formalin-fixed, paraffin-embedded sections were immunostained with mouse monoclonal antibodies to CK7 and CK20 using an avidin-biotin immuno-peroxidase technique with microwave antigen retrieval. The percentage of cells positive for each antibody was assessed on a scale of 0 to 3 (0, [is less than] 10%; 1+, 10% to 50%; 2+, 51% to 90%; 3+, [is greater than] 90%). Results.--The majority of carcinomas in all groups were positive for CK7 (CK7+) and negative for CK20 (CK20-). Of the CK7+ tumors, the majority of tumors in each group were 3+ positive. Conclusions.--(1) Carcinomas of the extrahepatic biliary tract and pancreas are strongly positive for CK7 and negative for CK20 and can be included in the differential diagnosis of other carcinomas with this profile in metastatic sites. (2) The CK7/CK20 immunostaining profile will not identify the site of origin for tumors with extensive growth in the porta hepatis region. Abstract

Systemic sclerosis, also known as scleroderma, is a rare autoimmune disease affecting connective tissue and characterized by thickening of the skin due to deposits of collagen, a fibrous protein. One of the most serious complications resulting from scleroderma is kidney or renal failure, which usually occurs in patients who also have high blood pressure or hypertension. However, a number of studies describe scleroderma patients having renal failure without hypertension. Over a 33 year period 131 patients who experienced scleroderma related kidney failure were followed. Eleven percent of this group had normal blood pressure or were normotensive. Researchers attempted to isolate factors that would cause a normotensive scleroderma patient to develop renal failure. Their analysis revealed that these patients had significantly higher incidences than the hypertensive patients of various diseases: asthma (80 percent versus 33 percent of hypertensives); small blood vessel diseases (90 percent versus 38 percent); and an abnormal decrease in blood platelets or thrombocytopenia (83 percent versus 21 percent). Another significant finding in the medical histories of the normotensive scleroderma patients who developed renal failure was the fact that 64 percent of them had been administered maximum doses of corticosteroids during the two months prior to the onset of kidney failure, compared to only 16 percent of the hypertensive scleroderma patients. Since both scleroderma and corticosteroid medications can cause injury to circulatory tissue or vascular endothelium, it seems likely that high doses of corticosteroids were a contributing factor to the onset of kidney failure. It is recommended that only low doses of corticosteroids be prescribed for patients with scleroderma who are at high risk for developing kidney failure.
