Background: Human adenovirus (HAdV) is common pathogens that cause various respiratory diseases. The genetic diversity of viruses caused by recombination is considered to be the main source of emerging outbreaks. The aim of this study is to explore the evolutionary relationship and recombination events of HAdV genome in respiratory tract infections in Jiangsu Province. Methods: Whole-genome sequencing (WGS) technology was used to sequence 66 patients with HAdV infection (37 patients with influenza-like illness (ILI) and 29 hospitalized patients with pneumonia) from Jiangsu Province. Epidemiological analysis was performed on hospitalized pneumonia and ILI patients infected with HAdV. Subsequently, phylogenetic, recombination, and nucleotide and amino acid identity analyses were performed. Results: Epidemiological analysis of patients undergoing WGS showed that 75.7% of ILI patients were infected with the HAdVB strain and 69.0% of hospitalized pneumonia patients were infected with the HAdVC strain. Moreover, the hospitalized pneumonia and ILI patients infected with HAdV were different in region and time. The strains of HAdVB3 and HAdVB7 genotypes were mainly infected in 2015 and 2017, and the strains of HAdVC1 and HAdVC2 genotypes were mainly infected in 2020. The results of histogram analysis showed that the HAdV strain mainly infected children under 5 years old. In addition, 36 novel recombinant strains were identified. The discovery of these recombinant strains may contribute to understanding the epidemiology of HAdV and research on related vaccines. Furthermore, the percentage of nucleotide and amino acid identities revealed a high level of genetic conservation within isolates from HAdVB3, HAdVB7, HAdVC1, HAdVC2 and HAdVC5 genotypes. Conclusion: The WGS analysis reveals the evolutionary relationships and recombination events of HAdV strains in Jiangsu Province, which is helpful to deepen the understanding of HAdV epidemiology and evolution. Aim: This study aimed to evaluate the cost‐effectiveness of hepatitis E vaccination strategies in chronic hepatitis B (CHB) patients. Methods: Based on the societal perspective, the cost‐effectiveness of three hepatitis E vaccination strategies—vaccination without screening, screening‐based vaccination, and no vaccination—among CHB patients was evaluated using a decision tree–Markov model, and incremental cost‐effectiveness ratios (ICERs) were calculated. Values for treatment costs and health utilities were estimated from a prior investigation on disease burden, and values for transition probabilities and vaccination‐related costs were obtained from previous studies and government agencies. Sensitivity analyses were undertaken for assessing model uncertainties. Results: It was estimated that CHB patients superinfected with hepatitis E virus (HEV) incurred significantly longer disease course, higher economic burden, and more health loss compared to those with HEV infection alone (all p < 0.05). The ICERs of vaccination without screening and screening‐based vaccination compared to no vaccination were 41,843.01 yuan/quality‐adjusted life year (QALY) and 29,147.32 yuan/QALY, respectively, both lower than China's per‐capita gross domestic product (GDP) in 2018. The screening‐based vaccination reduced the cost and gained more QALYs than vaccination without screening. One‐way sensitivity analyses revealed that vaccine price, vaccine protection rate, and decay rate of vaccine protection had the greatest impact on the cost‐effectiveness analysis. Probabilistic sensitivity analyses confirmed the base‐case results, and if the willingness‐to‐pay value reached per‐capita GDP, the probability that screening‐based vaccination would be cost‐effective was approaching 100%. Conclusions: The disease burden in CHB patients superinfected with HEV is relatively heavy in China, and the screening‐based hepatitis E vaccination strategy for CHB patients is the most cost‐effective option. A novel influenza A (H7N9) virus of avian origin emerged in eastern China in the spring of 2013. This virus causes severe disease in humans, including acute and often lethal respiratory failure. As of January 2014, 275 cases of H7N9-infected patients had been reported, highlighting the urgency of identifying biomarkers for predicting disease severity and fatal outcomes. Here, we show that plasma levels of angiotensin II, a major regulatory peptide of the renin-angiotensin system, are markedly elevated in H7N9 patients and are associated with disease progression. Moreover, the sustained high levels of angiotensin II in these patients are strongly correlated with mortality. The predictive value of angiotensin II is higher than that of C-reactive protein and some clinical parameters such as the PaO2/FiO2 ratio (partial pressure of arterial oxygen to the fraction of inspired oxygen). Our findings indicate that angiotensin II is a biomarker for lethality in flu infections.

Over-voltage and over-current protection are two forms of over-voltage and over-current protection sequences of intrinsic safety power supply. In order to study the influence of different protection sequence forms on safety performance of intrinsic safety power supply, an equivalent model of spark discharge of intrinsic safety power supply was established, and essential safety performance factors affected the selection of the protection sequences were determined. The protection performance of two protection sequences was compared and studied when happened over-voltage, over-current and over-voltage over-current. The research results show that in the design of intrinsic safety power supplies, the selection of over-voltage and over-current protection sequence is not random and arbitrary, but there are principles to follow: the type of over-voltage protection circuit determines the sequences of over-voltage and over-current protection of intrinsic safety power supplies. In design, the first step is determining the type of over-voltage protection circuit, if the type of over-voltage protection circuit is short circuit type, protection circuit of intrinsic safety power should be designed to protection sequences of over-current-over-voltage, otherwise it should be designed as over-voltage and over-current protection sequence, the protection circuit of the over-voltage and over-current protection sequence has a shorter turn-off time, the protection effect is better and the safety performance of the intrinsic safety power supply is higher.

Background: Recent seasonal epidemics of influenza have been caused by human influenza A viruses of the H1N1 and H3N2 subtypes and influenza B viruses. Annual vaccination is recommended to prevent infection; however, how annual influenza vaccination influences vaccine effectiveness is largely unknown. Methods: To investigate the impact of repeated vaccination on immune and protective effect, we performed a prospective seroepidemiologic study. Participants with or without prior vaccination (2018–2019) were enrolled during the 2019–2020 influenza season. Inactivated quadrivalent influenza vaccine (IIV4) was administered through the intramuscular route, and venous blood samples were collected regularly to test hemagglutination inhibition (HAI) titers. Results: The geometric mean titers and proportion with titers ≥40 against the influenza vaccine components peaked at 30 days post‐vaccination. At Day 30, the geometric mean titer and proportion with titers ≥40 in participants who had been previously vaccinated were higher for H3N2 but similar for both B lineages (Victoria and Yamagata) as compared with participants vaccinated for the first time. As for H1N1, the geometric mean titer was lower in repeated vaccinated participants, but the proportion with titers ≥40 was consistent in both groups. Conclusions: Repeated vaccination provides similar or enhanced protection as compared with single vaccination in first‐time vaccinees. [ABSTRACT FROM AUTHOR]
