17. Clinical and Radiographic Outcomes of Modified Posterior Atlantoaxial Temporary Fixation with Preservation of Semispinalis Cervicis: A Comparative Study.

Abstract

Study Design: Retrospective study. Objective: Current studies suggested that the posterior cervical deep muscles should be preserved during the atlantoaxial posterior approach. This study aimed to modify the conventional temporary posterior fixation by preserving the semispinalis cervicis and to evaluate the radiographic and clinical outcomes compared with the conventional technique. Methods: Applied anatomical and imaging study were assessed prior to the clinical studies. Thirty patients with odontoid fracture were enrolled. 12 patients underwent modified operation with preservation of semispinalis cervicis (Group P-SSC), and 18 patients underwent conventional approach with detachment of the semispinalis cervicis (Group D-SSC) as controls. Clinical and radiological assessments were evaluated postoperatively, including activities of daily living (ADL), axial symptom scoring system (Ass), cervical range of motion (ROM), and the average atrophy rate of muscles. Results: All patients experienced fracture healed within 10 months after temporary fixation. At early follow-up, ADL accompanying extension and flexion were significantly different between the two groups. The Ass scores in the Group P-SSC were constantly higher than that in Group D-SSC. Flexion-Extension ROM in Group P-SSC was significantly larger than that of Group D-SSC. The average atrophy rate at the C3 and C5 level in Group P-SSC was 7.62±3.38% and 7.55±3.89%, repectivcly, while 15.67±5.97% and 8.56±4.76% in Group D-SSC, respectively. Significant differences in the atrophy rate were found between groups at C3 level. Conclusions: Compared with the conventional technique, the modified surgical technique with preservation of semispinalis cervicis was effective in improving axial symptoms, decreasing the atrophy rate of muscles, and increasing the cervical ROM. Abstract

Unmanned aerial vehicle(UAV) often flies in close proximity to terrain. The expansion in the number of low-altitude flight operations, along with increased operator workload, have made controlled flight into terrain the number one cause of loss of UAV. This paper is to give an overview of the Automatic Ground Collision Avoidance System (Auto-GCAS) program attempt to provide a system to eliminate that cause of UAV loss. The Auto-GCAS utilizes a digital terrain system with a terrain referenced navigation algorithm to locate the aircraft spatially with respect to the terrain. The terrain database around the aircraft is scanned, and a terrain profile is created. This paper presents a new compression algorithm called bubble terrain compression algorithm for Auto-GCAS of UAV. In this paper, optimal trajectory is considered as a minimax optimal control problem, which is solved using direct transcription of the continuous optimal control problem. Within a very general framework for solving such problems, we transform the non-smooth cost function into a constrained nonlinear programming problem. In the formulation, we solve for optimal collision avoidance manoeuvres. To ensure smooth derivatives of general two dimensional terrain, it is approximated using the optimal trajectories with disturbances in the initial conditions. Simulation results show that the UAV with the proposed methodology successfully achieves autonomous recovery maneuvers in MATLAB environment.

42. Comparative studies of thermogels in preventing post-operative adhesions and corresponding mechanisms

Abstract

Post-surgical peritoneal adhesions constitute a classic problem in surgery, and thus anti-adhesion materials are much required. In this study, a series of polyester–PEG–polyester triblock copolymers with different biodegradable polyester compositions were synthesized, their properties were examined, and the in vivoefficacies as anti-adhesion biomaterials were evaluated in a comparative way for the first time. These samples not only exhibited various morphologies in the bulk state, but also possessed different stabilities in the sol state. All the polymer aqueous solutions with appropriate compositions and concentrations underwent sol–gel transitions with increase of temperature and formed semi-solid hydrogels at body temperature. The efficacy of PEG/polyester thermogels (25 wt%) for preventing post-operative abdominal adhesions was investigated and compared in a rabbit model of sidewall defect-bowel abrasion. Different efficacies of anti-adhesion were observed, possible mechanisms were discussed, and the importance of viscoelasticity was suggested for the first time. These results illustrated that appropriate properties of PEG/polyester thermogels including viscoelastic matrix, hydrophilic surface and moderate in vivopersistence played crucial roles in enabling an effective device to prevent post-surgical peritoneal adhesions.

Abstract

Excessive apoptosis and inflammatory responses of nucleus pulposus (NP) cells induced by oxidative stress contribute to intervertebral disc degeneration (IVDD). Though some microRNAs are associated with IVDD, the specific microRNA that can mediate apoptotic and inflammatory responses of NP cells induced by oxidative stress synchronously still needs further identification. Here, we find that microRNA-623 (miR-623) is downregulated in IVDD and its expression is regulated by hypoxia-inducible factor-1 α (HIF-1 α ) under oxidative stress conditions. Mechanistically, HIF-1 α is observed to promote miR-623 expression by directly binding to its promoter region (-1,994/-1,987 bp). Functionally, miR-623 is found to work as an intermediator in alleviating apoptosis and inflammatory responses of NP cells induced by oxidative stress via regulating thioredoxin-interacting protein ( TXNIP ) expression by directly targeting its 3'-untranslated region (3'-UTR).
