4. Comparison of Home Blood Pressure Monitoring with and without Training: Does Adherence to the Recommended Instructions Overlook Hypertension?

Background: Home blood pressure monitoring (HBPM) is commonly used to diagnose hypertension (HT), with a diagnostic threshold of ≥135/85 mm Hg, the same as daytime ambulatory BP monitoring (ABPM). This study hypothesizes that training and adherence to HBPM guidelines will yield more accurate BP readings compared to ABPM. Methods: The study involved 129 patients with elevated office BP but no prior HT diagnosis. After a two-week observation period with lifestyle advice, HBPM was conducted over a week before and after structured measurement training, with ABPM performed in each period. Adherence to instructions post-training was also assessed. Results: Post-training, mean systolic and diastolic home BP values significantly decreased (from 128 ± 13.1/84.9 ± 8.2 to 122.6 ± 12.7/81.8 ± 7.8; P < 0.001 for both), while daytime ABPM values remained unchanged (131.7 ± 11.1/86.7 ± 9.3 before vs. 130.7 ± 11.7/85.9 ± 8.6 after; P = 0.185). Although HBPM values were consistently lower than ABPM values, the discrepancy grew post-training. The number of patients reaching the HT threshold via HBPM decreased significantly post-training [71 (55%) to 54 (41.9%); P = 0.006], whereas the number via daytime ABPM remained similar [82 (64.3%) vs. 84 (65.1%); P = 1.000]. Conclusion: Training and adherence to HBPM guidelines led to lower BP readings and fewer HT diagnoses. Contrary to the hypothesis, this method under "ideal conditions" underestimated HT prevalence when compared to daytime ABPM. Further studies with clinical endpoints are needed to refine HBPM methods and establish new BP thresholds for more accurate HT detection. [ABSTRACT FROM AUTHOR]

Cardiac masses are rare entities that can be challenging in clinical diagnosis and management. Cardiac masses can be detected incidentally in patients with an asymptomatic course or may cause systemic inflammation findings due to inflammatory cytokine release or symptoms such as shortness of breath, chest pain, syncope, sudden cardiac death, and mortality due to the location of the mass. Cardiac masses associated with systemic inflammatory disorders are uncommon in this disease group. This case report will present a case with an asymptomatic IgG4‐related left atrial mass detected in routine echocardiographic control imaging due to rheumatic valve disease. [ABSTRACT FROM AUTHOR]

12. Evaluation of the Transcatheter Aortic Valve Replacement Results in Patients with Borderline Aortic Annulus and the Impact of Under- or Oversizing the Valve.

Background: Optimal valve sizing provides improved results in transcatheter aortic valve replacement. Operators hesitate about the valve size when the annulus measurements fall into borderline area. Our purpose was to compare the results of borderline versus nonborderline annulus and to understand the impact of valve type and under or oversizing. Methods: Data from 338 consecutive transcatheter aortic valve replacement procedures were analyzed. The study population was divided into 2 groups as "borderline annulus" and "non-borderline annulus." Balloon expandable valves already have a grey zone definition. Similar to balloon expandable valves, annulus sizes that are within 15% above or below the upper or lower limit of a particular self-expandable valve size are defined as the "borderline annulus" for self-expandable valves. The borderline annulus group was also divided into 2 subgroups according to the smaller or larger valve selection as "undersizing" and "oversizing." Comparisons were made regarding the paravalvular leakage and residual transvalvular gradient. Results: Of these 338 patients, 102 (30.1%) had a borderline and 226 (69.9%) had a non-borderline annulus. Both the transvalvular gradient (17.81 ± 7.15 vs. 14.44 ± 6.27) and the frequency of paravalvular leakage (for mild, mild to moderate, and moderate, 40.2%, 11.8%, and 2.9% vs., 18.8%, 6.7%, and 0.4%, respectively) were significantly higher in the borderline annulus than the non-borderline annulus group (P < .001). There were no significant differences between the groups balloon expandable versus self-expandable valves and oversizing versus undersizing regarding the transvalvular gradient and paravalvular leakage in patients with borderline annulus (P > .05). Conclusion: Regardless of the valve type and oversizing or undersizing, borderline annulus is related to significantly higher transvalvular gradient and paravalvular leakage when compared to the non-borderline annulus in transcatheter aortic valve replacement. Background: Obesity is the main obstacle for metabolic control in patients with type 2 diabetes. Turkey has the highest prevalence of obesity and type 2 diabetes in Europe. The effect of obesity on the metabolic control, and the macro-and microvascular complications of patients are not apparent. Objectives: This nationwide survey aimed to investigate the prevalence of overweight and obesity among patients with type 2 diabetes and to search for the impact of obesity on the metabolic control of these patients. We also investigated the independent associates of obesity in patients with type 2 diabetes. Methods: We consecutively enrolled patients who were under follow-up for at least 1 year in 69 tertiary healthcare units in 37 cities. The demographic, anthropometric, and clinical data including medications were recorded. Patients were excluded if they were pregnant, younger than 18 years, had decompensated liver disease, psychiatric disorders interfering with cognition or compliance, had bariatric surgery, or were undergoing renal replacement therapy. Results: Only 10% of patients with type 2 diabetes (n = 4,648) had normal body mass indexes (BMI), while the others were affected by overweight (31%) or obesity (59%). Women had a significantly higher prevalence of obesity (53.4 vs. 40%) and severe obesity (16.6 vs. 3.3%). Significant associations were present between high BMI levels and lower education levels, intake of insulin, antihypertensives and statins, poor metabolic control, or the presence of microvascular complications. Age, gender, level of education, smoking, and physical inactivity were the independent associates of obesity in patients with type 2 diabetes. Conclusion: The TEMD Obesity Study shows that obesity is a major determinant of the poor metabolic control in patients with type 2 diabetes. These results underline the importance of prevention and management of obesity to improve health care in patients with type 2 diabetes. Background: Menopause is an important life stage for women, which can bring along sexual and cardiac problems. Increased heart rate variability is an indicator of parasympathetic activity and is associated with mental and physical health and life expectancy. This study aimed to evaluate the effect of sexual activity (only penile-vaginal intercourse but not masturbation or non-coital sex with a partner) on heart rate variability in healthy menopausal women. Methods: We evaluated 130 menopausal patients aged 45-60 years, without chronic disease. The average weekly sexual activity numbers remembered in the last 1 year were questioned. The patients were divided into 2 groups according to the presence of sexual activity. The sexually active group was divided into subgroups as 1 per week and 2 or more per week. Menopause Rating Scale was applied for menopausal symptoms. Heart rate variability was analyzed from the 24-hour electrocardiography Holter recording. Results: Heart rate variability parameters were higher in the sexually active group than in the sexually inactive group (mean of the standard deviations of all the NN intervals for each 5 min segment of a 24-hour heart rate variability recording: P = .004; root mean square of differences between adjacent normal RR intervals, expressed in ms: P = .001; number of NN intervals exceeding 50 milliseconds: P = .011; percentage of adjacent RR intervals with a difference of duration >50 ms: P = .009; low frequency: P = .011; high frequency: P = .008, low frequency/high frequency: P = .018). When assessed by multiple linear regression analysis by adjusting for age, body mass index, and menopause duration, the variables mean of the standard deviations of all the NN intervals for each 5 min segment of a 24-hour heart rate variability recording, root mean square of differences between adjacent normal RR intervals, expressed in ms, and low frequency were independently associated with the number of sexual activities per week (B = 2.89 ± 1.02, 95% CI = 0.866-4.91, P = .005; B = 4.57 ± 1.83, 95% CI = 0.94-8.2, P = .014; and B = 1174.9 ± 592.2, 95% CI = 2.9-2346.9, P = .049, respectively). Conclusion: In healthy menopausal women, continued sexual activity with penile-vaginal intercourse is associated with better health outcomes on cardiac autonomic function through higher heart rate variability, an index of parasympathetic activity. 19. Transcatheter Tricuspid Valve Replacement for Tricuspid Regurgitation: A Systematic Review and Meta-analysis.

Background: The present data aim to evaluate the feasibility of the orthotopic transcatheter tricuspid valve replacement devices, echocardiographic, functional improvements, and mortality rates following replacement in patients with significant tricuspid valve regurgitation. Methods: We systematically searched for the studies evaluating the efficacy and safety of transcatheter tricuspid valve replacement for significant tricuspid valve regurgitation. The efficacy and safety outcomes were the improvements in New York Heart Association functional class, 6-minute walking distance, all-cause death, and periprocedural and long-term complications. In addition, a random-effect meta-analysis was performed comparing outcomes before and after transcatheter tricuspid valve replacement. Results: Nine studies with 321 patients were included. The mean age was 75.8 years, and the mean European System for Cardiac Operative Risk Evaluation II score was 8.2% (95% CI: 6.1 to 10.3). Severe, massive, and torrential tricuspid valve regurgitation was diagnosed in 95% of patients (95% CI: 89% to 98%), and 83% (95% CI: 73% to 90%) of patients were in New York Heart Association functional class III or IV. At a weighted mean follow-up of 122 days, New York Heart Association functional class (risk ratio = 0.20; 95% CI: 0.11 to 0.35; P < .001) and 6-minute walking distance (mean difference = 91.1 m; 95% CI: 37.3 to 144.9 m; P < .001) significantly improved, and similarly, the prevalence of severe or greater tricuspid valve regurgitation was significantly reduced after transcatheter tricuspid valve replacement (baseline risk ratio = 0.19; 95% CI: 0.10 to 0.36; P < .001). In total, 28 patients (10%; 95% CI: 6% to 17%) had died. Pooled analyses demonstrated non-significant differences in hospital and 30-day mortality and >30-day mortality than predicted operative mortality (risk ratio = 1.03; 95% CI: 0.41 to 2.59; P = .95, risk ratio = 1.39; 95% CI: 0.69 to 2.81; P = .35, respectively). Conclusion: Transcatheter tricuspid valve replacement could be an emerging treatment option for patients with severe tricuspid regurgitation who are not eligible for transcatheter repair or surgical replacement because of high surgical risk and poor prognosis. - Abstract
In December 2019, in the city of Wuhan, in the Hubei province of China, treatment-resistant cases of pneumonia emerged and spread rapidly for reasons unknown. A new strain of coronavirus (severe acute respiratory syndrome coronavirus-2 [SARS-CoV-2]) was identified and caused the first pandemic of the 21st century. The virus was officially detected in our country on March 11, 2020, and the number of cases increased rapidly; the virus was isolated in 670 patients within 10 days. The rapid increase in the number of patients has required our physicians to learn to protect both the public and themselves when treating patients with this highly infectious disease. The group most affected by the outbreak and with the highest mortality rate is elderly patients with known cardiovascular disease. Therefore, it is necessary for cardiology specialists to take an active role in combating the epidemic. The aim of this article is to make a brief assessment of current information regarding the management of cardiovascular patients affected by COVID-19 and to provide practical suggestions to cardiology specialists about problems and questions they have frequently encountered. / Aralık 2019’da, Çin’in Hubei eyaletindeki Wuhan şehrinde, nedeni bilinmeyen, tedaviye dirençli pnömoni olguları ile ortaya çıkan ve devamında hızla yayılarak XXI. yüzyılın ilk pandemisine neden olan yeni koronavirüsün (severe acute respiratory syndrome koranavirüs-2 [SARS-CoV-2]) 11 Mart 2020 tarihinde ülkemizde de resmi olarak saptanmasının ardından olgu sayısı hızla artmış ve 10 gün içerisinde 670 hastada virüs izole edilmiştir. Objective: The management of severe functional mitral regurgitation (FMR) in patients with heart failure (HF) and low ejection fraction is controversial, but percutaneous transcatheter procedures are promising. In this retrospective analysis, we aimed to assess the efficacy of the Carillon Mitral Contour System in patients with "inoperable" severe FMR. Methods: Seventy three patients (mean age 66.89, range 31-90 years) with congestive heart failure (CHF), severe FMR, and reduced ejection fraction (<35%) who underwent Carillon device implantation were examined. The study group consisted of patients with successfully implanted devices whereas the control group comprised patients in whom the device could not be deployed. The primary endpoint was combined allcause mortality and first hospitalization for HF (whichever came first). Results: The median (Q1, Q3) follow-up was 31 (11-49) months. The device was deployed successfully in 50 patients (implant group) and not in 23 patients (non-implant group). Both the primary endpoint and all-cause mortality were lower in the "implant" group, but the differences were not significant. The median to primary endpoint was 21 [95% confidence interval (CI) 8.8-33.2] and six (95% CI 0.1-11.9) months for the implant group and the non-implant group, respectively (p=0.078). Conclusion: Carillon Mitral Contour System implantation is a safe procedure and results in the reduction of all-cause mortality and combined endpoint of mortality and hospitalizations for HF in inoperable patients with severe FMR and low ejection fraction, although the difference did not meet the significance level. It is therefore essential to know the overall cardiovascular risk and reveal the predictors of metabolic control in Turkish adults with diabetes mellitus. METHODS: A nationwide, multicenter survey consecutively enrolled patients who were under follow up for at least a year. Optimal control was defined as HbA1c 7%, home arterial blood pressure (ABP) 135/85 mmHg, or LDL-C 100 mg/dL. Achieving all parameters indicated triple metabolic control. RESULTS: HbA1c levels of patients (n = 5211) were 8.6 ± 1.9% (71 ± 22 mmol/mol) and 7.7 ± 1.7% (61 ± 19 mmol/mol), in Type 1 and Type 2 diabetes, respectively. Glycemic control was achieved in 15.3% and 40.2%, and triple metabolic control was achieved in 5.5% and 10.1%, respectively. Only 1.5% of patients met all the criteria of being non-obese, non-smoker, exercising, and under triple metabolic control. Low education level was a significant predictor of poor glycemic control in both groups. CONCLUSIONS: Few patients with Type 2, and even fewer with Type 1 diabetes have optimal metabolic control in Turkey. TEMD study will provide evidence-based information to policy makers to focus more on the quality and sustainability of diabetes care in order to reduce the national burden of the disease.

Türkiye’de KV risk faktörlerinin son durumu ve yıllar içindeki değişimini değerlendirmek amacıyla epidemiyolojik çalışmaların sistematik derleme ve meta-analizi planlandı. Bu makalede hipertansiyon (HT) ve kan basıncına ait sonuçlar açıklanmaktadır. Yöntemler: Son 15 yılda Türkiye’de yapılan epidemiyolojik çalışmalar Ovid Medline, Web of Science Core Collection ve ULAKBİM veritabanlarında tarandı. HT prevalansı ile ilgili bilgi veren 7 (n=73218), kan basıncı bilgisini veren 8 (n=75879) çalışma bulundu. Çalışmalar arasındaki heterojenite yüksekti. 2003-2012 yılları arasında yapılmış olan bu çalışmaların yanlılık riski düşük olanlarının meta-analizinde HT kaba prevalansının kadınlarda erkeklere göre daha fazla olduğu, kan basıncı düzeylerinin her iki grupta benzer olduğu, hem HT prevalansında hem de kan basıncı değerlerinde azalma olduğu, buna rağmen hipertansif kişi sayısının 15 milyon civarında sabit kaldığı, kontrolsüz hipertansiyonu olanların sayısının azalmakla birlikte 11 milyon kişi düzeyinde olduğu saptandı. Sonuç: HT sıklığı ve kan basıncı değerlerinde olumlu değişmeler olmasına rağmen HT halen önemli bir halk sağlığı problemi olmaya devam etmektedir., Objective: Cardiovascular risc factors may show significant changes over the years. A systematic review and meta-analysis of epidemiological studies conducted in Turkey was performed to assess the latest profile and temporal changes in cardiovascular risk factors. Presented here are the data on hypertension (HT) and blood pressure (BP). Methods: Ovid Medline, the Web of Science Core Collection, and the Turkish Academic Network and Information Center (ULAKBIM) were searched for epidemiological studies conducted in Turkey during the last 15 years. in addition, the web pages of the Ministry of Health, the Turkish Statistical Institute, and associations of cardiology, nephrology, and endocrinology were searched for appropriate studies. Regional studies were excluded. the studies included were assessed with a bias score developed by our team, then categorized as having a low risk or a high risk of bias. the crude values of HT prevalence and BP were pooled using a random effects model. Meta-regression was performed to explain heterogeneity and to assess temporal changes. Results: the agreement between the 2 authors on the selection and bias scoring of the studies was perfect (Kappa ?0.95). There were 7 (n=73218) studies providing HT prevalence data, and 8 (n=75879) studies with BP data. the heterogeneity between the studies was high. Meta-analysis of the studies with a low risk of bias indicated that the crude prevalence of HT is higher in women, but that BP levels were similar in both sexes. the HT prevalence and BP value decreased between 2003 and 2012; however, the number of hypertensives stabilized at approximately 15 million, and the number of uncontrolled hypertensives, despite some decrease, was around 11 million. Conclusion: Despite some improvement, HT is still an important public health problem in Turkey

Amaç: Sigara kullanımı Türkiye’deki en önemli sağlık sorunlarından ve önlenebilir ölüm nedenlerinden birisidir. Son 10 yılda bu sorunun çözümüne yönelik pek çok yasal önlemler alınmıştır. Bu çalışmada ülkemizde son 15 yılda gerçekleştirilen epidemiyolojik çalışmaların sistematik derleme ve meta-analizi yapılarak sigara kullanımı prevalansının belirlenmesi amaçlandı. Yöntemler: Çalışma amacına uygun epidemiyolojik araştırmalar Ovid Medline, Web of Science Core Collection ve ULAKBİM veritabanları ile Sağlık Bakanlığı ve Türkiye İstatistik Kurumunun web sayfaları üzerinden tarandı. Analize alınan araştırmalar Türkiye’yi temsil etme derecesi ve ölçümlerin standardizasyonu dikkate alınarak kendi geliştirdiğimiz yanlılık (bias) skoru ile değerlendirildi. Rastgele etkiler modeli ile meta-analiz ve meta-regresyon yapıldı. Bulgular: Sigara kullanımı ile ilgili verisi olan 10 epidemiyolojik çalışmaya (n=122383) ulaşıldı. Kadın (n=49524) ve erkekler (n=37684) için ayrı olarak sigara kullanma verisi bu çalışmaların sekizinden elde edilebildi. Tümü yanlılık riski açısından düşük olarak değerlendirilen bu çalışmaların meta-analizinde arada bir sigara kullanan grup dahil edildiğinde sigara kullanımı prevalansı tüm grupta %30.5, kadınlarda %15.7, erkeklerde ise %46.1 bulundu. Meta-regresyon analizinde kaba (crude) prevalans değerlerinin zamanla değişimi anlamlı olmasa da, yaş kategorilerine göre prevalans değerlerini veren 3 çalışmanın verileri yaşa göre standardize edildiğinde 2003– 2012 yılları arasında sigara kullanım sıklığının arada bir kullananlar dahil edildiğinde %6.8 birim (%20.2 oranında), hariç tutulduğunda %8.4 birim (%26.3 oranında) azaldığı görüldü. Sonuç: Sigara tüketiminin azaltılmasına yönelik alınan pek çok önleme rağmen 15 yaş üzerindeki Türk toplumunun yaklaşık üçte biri, erkeklerin ise yaklaşık yarıya yakını sigara kullanmaktadır. Sigarayla mücadele programlarına devam edilmesi ülkemiz açısından oldukça önemlidir., bjective: Smoking is one of the most important public health problems and preventable causes of mortality in Turkey. Methods: Ovid Medline, the Web of Science Core Collection, and the Turkish Academic Network and Information Center (ULAKBIM), as well as the websites of the Ministry of Health and the Turkish Statistical Institute were searched for the appropriate epidemiological studies. Studies included in the analysis were evaluated by a selfdeveloped bias score regarding their potential to represent Turkey and standardization of measurements. the meta-analysis and metaregression analysis were performed using a random effects model. Results: Ten epidemiological studies (n=122383) that included data about smoking were found. Eight of them (all with low bias score) included separate data about the smoking habit of women (n=49524) and men (n=37684). the smoking prevalence was determined to be 30.5% for the whole group, 15.7% for women, and 46.1% for men, when occasional smokers were included. Although the change observed in crude prevalence values over time was not statistically significant, when the data of the 3 studies that gave prevalence values according to age categories were standardized according to age, the incidence of smoking between 2003 and 2012 decreased 6.8% (20.2%) when occasional smokers were included and 8.4% (26.3%) when they are excluded. Conclusion: Despite implementation of major healthcare policies and some success in decreasing rate of smoking, one-third of the Turkish population aged over 15 years and nearly half of the men are smokers. It is essential to continue and to strengthen measures to combat smoking.

Objective: It has been reported that women receive fewer preventive recommendations regarding pharmacological treatment, lifestyle modifications, and cardiac rehabilitation compared with men who have a similar risk profile. This study was an investigation of the impact of gender on cardiovascular risk profile and secondary prevention measures for coronary artery disease (CAD) in the Turkish population. Methods: Statistical analyses were based on the European Action on Secondary and Primary Prevention through Intervention to Reduce Events (EUROASPIRE)-IV cross-sectional survey data obtained from 17 centers in Turkey. Male and female patients, aged 18 to 80 years, who were hospitalized for a first or recurrent coronary event (coronary artery bypass graft, percutaneous coronary intervention, acute myocardial infarction, or acute myocardial ischemia) were eligible. Results: A total of 88 (19.7%) females and 358 males (80.3%) were included. At the time of the index event, the females were significantly older (p=0.003) and had received less formal education (p0.05). Conclusion: Achievement of ideal body weight, fasting blood glucose and blood pressure targets was lower in women despite similar reported medication use. This highlights the importance of the implementation of lifestyle measures and adherence to medications in women., Amaç: Benzer risk profiline sahip erkekler ile kıyaslandığında, kadınlara farmakolojik tedavi, yaşam tarzı değişiklikleri ve kardiyak rehabilitasyon açısından koruyucu önerilerde daha az bulunulduğu bilinmektedir. Bu çalışmada, Türk popülasyonunda kardiyovasküler risk profili ve ikincil korunma ölçütleri üzerine cinsiyetin etkisinin araştırılması hedeflenmiştir. Yöntemler: İstatistiksel analiz Türkiye’de 17 merkezden elde edilen EUROASPIRE-IV (European Action on Secondary and Primary Prevention - EA-IV) kesitsel araştırma verilerine dayanarak gerçekleştirildi. İlk veya tekrarlayan koroner olay (koroner arter baypas greft, perkütan koroner girişim, akut miyokart enfarktüsü ya da akut miyokart iskemisi) nedeniyle hastaneye yatırılan 18–80 yaş aralığındaki kadın ve erkek hastalar çalışma kapsamında incelendi. Bulgular: Bu çalışmaya 88 kadın (%19.7) ve 358 erkek (%80.3) dahil edildi. İlk koroner olayda, kadınların daha yaşlı (p=0.003) ve daha az eğitimli (p0.05). Sonuç: Benzer ilaç kullanım oranlarına rağmen, ideal vücut ağırlığı, açlık kan şekeri ve kan basıncı değerlerine ulaşma oranı kadınlarda daha düşük saptanmıştır. Bu bulgu, kadınlarda yaşam tarzı değişiklikleri ve ilaç tedavisine uyumun önemine vurgu yapmaktadır.

Amaç: Gerek gelişmiş gerekse gelişmekte olan ülkelerde metabolik sendrom sıklığı ülkeler arasında büyük değişkenlik göstermektedir. Ülkemizde son 10–15 yılda metabolik sendrom sıklığı ile ilgili önemli çalışmalar yapılmıştır. Bu yayında Türkiye genelini örnekleyen metabolik sendrom sıklığı ile ilgili çalışmaların meta-analizi yapılmıştır. Yöntemler: Son 15 yılda Türkiye’de yapılan ve metabolik sendrom sıklığı hakkında bilgi veren epidemiyolojik çalışmalar Ovid Medline, Web of Science Core Collection ve ULAKBİM veritabanlarında tarandı. Ayrıca Sağlık Bakanlığı, Türkiye İstatistik Kurumu ve ilgili meslek örgütlerinin web siteleri konu ile ilgili araştırmalar açısından incelendi. Analize alınan araştırmalar kendi geliştirdiğimiz bir yanlılık (bias) skoru ile değerlendirilerek yanlılık riski yüksek ve düşük olarak iki gruba ayrıldı. Meta-analizde rastgele etkiler modeli kullanıldı. Prevalans değerlerindeki heterojeniteyi ve zamansal değişimi değerlendirmek üzere lineer ve non-lineer meta-regresyon yapıldı. Bulgular: ATP III tanımına göre metabolik sendrom ile ilgili bilgi veren 4 çalışmaya (n=34893) ulaşıldı. Hem makalelerin seçiminde hem de seçilen makalelerin yanlılık açısından skorlanmasında gözlemciler arasındaki uyum mükemmel düzeyde bulundu. Rastgele etkiler modeli ile yapılan meta-analizde metabolik sendrom prevalansı tüm grupta %32.9 (%95 CI 30.2–35.6), kadınlarda %38.3 (%95 GA 35.8–40.8), erkeklerde %26.8 (%95 GA 23.4–30.3) olarak hesaplandı. Uluslararası Diyabet Federasyonu’nun tanımına göre metabolik sendrom prevalansı ile ilgili bilgi veren 2 çalışma vardı (n=8568). Bu çalışmaların meta-analizinde rastgele etkiler modelinde metabolik sendrom prevalansı tüm grupta %43.3 (%95 GA 41.9–44.7), kadınlarda %50.4 (%95 GA 49.0–51.9), erkeklerde ise %35.4 (%95 GA 32.5–38.5) bulundu. Sonuç: Ülkemizde metabolik sendrom sıklığı, özellikle kadınlarda çok yüksektir ve her dört erkekten birinde ve her üç kadından birinde metabolik sendrom vardır. Metabolik sendromu önlemeye yönelik çalışmalar yapılması gerekmektedir., Objective: the incidence of metabolic syndrome varies greatly in developed and developing countries. in the last 10 to 15 years, important studies have been performed examining the incidence of metabolic syndrome in Turkey. This article is a meta-analysis of the studies that investigated the incidence of metabolic syndrome this country. Methods: Epidemiological studies from the last 15 years that provided information about the incidence of metabolic syndrome in Turkey were screened in the Ovid Medline, the Web of Science Core Collection, and the Turkish Academic Network and Information Center (ULAKBIM) databases. in addition, web pages of the Ministry of Health, the Turkish Statistical Institute, and the associations of the related medical discipilnes were screened for appropriate studies. A bias tool that is developed by our team was used to categorize the included studies as having low- or high risk of bias. A random effects model was used in the meta-analysis. Linear and nonlinear meta-regression analyses were performed to evaluate heterogeneity and temporal variations in prevalence rates. Results: Four studies (n=34893) were identified that provided information about the status of metabolic syndrome in Turkey using the Adult Treatment Panel III definition. There was excellent interobserver agreement regarding the selection of studies and the scoring of the selected research with respect to bias. the metaanalysis revealed a prevalence of 32.9% (95% confidence interval [CI]: 30.2–35) overall, 38.3% (95% CI: 35.8–40.8) in women, and 26.8% (95% CI: 23.4–30.3) in men. Two studies (n=8568) provided data according to the International Diabetes Federation definition of metabolic syndrome. the random effects model used in the metaanalysis of these studies demonstrated a prevalence of metabolic syndrome of 43.3% (95% CI: 41.9–44.7) overall, 50.4% (95% CI: 49.0–51.9) in women, and 35.4% (95% CI: 32.5–38.5) in men. Conclusion: in Turkey, the incidence of metabolic syndrome is 1 in every 4 men, and 1 in every 3 women. the rate is particularly high among women. Studies designed to prevent the development of metabolic syndrome are needed.

Background/Aim: Myocardial infarction with non-obstructive coronary arteries (MINOCA) is an increasingly recognized entity. Recent studies have shown that MINOCA is not a benign syndrome, with younger MINOCA patients having outcomes comparable to their myocardial infarction with obstructive coronary artery disease (MI-CAD) counterparts. In this study, we will describe the demographic, clinical and angiographic characteristics of MINOCA patients in our hospital. Methods: In this retrospective cohort study, all patients who underwent coronary angiography with the diagnosis of acute coronary syndrome during September 2016-April 2019 were screened and those with MINOCA were detected. We described the demographic, clinical, and angiographic characteristics of MINOCA patients and compared the etiologic and pathophysiological mechanisms. Results: A total of 3855 patients with acute coronary syndrome were screened and 155 were diagnosed with MINOCA, with a total prevalence of 4.02%. Among them, 48.4% were female and the overall mean age was 55.04 (13.57) years. Plaque disruption was the most common cause of MINOCA (48.4%), which was followed by microvascular dysfunction and slow flow (9.7%). We compared plaque disruption and other causes to find that age (58.31 (13.76) vs 51.89 (12.68) P=0.003), hypertension (37 (48.7%) vs 25 (31.6%) P=0.034), prior coronary artery disease history (16 (21.1%) vs 2 (2.5%) P=0.001) and creatinine clearance (67.35 (IQR: 25.8) vs 74.0 (IQR: 28.58) P=0.009) were higher in patients with plaque disruption than those without. Conclusions: MINOCA is a diagnosis of exclusion with numerous potential causes. The etiological and pathophysiological mechanisms of plaque disruption are different from other causes of MINOCA and the correct treatment approach determines the prognosis. [ABSTRACT FROM AUTHOR]

The article focuses on the case of a 69-year-old male patient who developed coronary aneurysms in the proximal and distal parts of a sirolimus-eluting stent (DES) implanted in the left anterior descending (LAD) artery. It is discussed that the aneurysms were detected 7 days after the stent implantation, highlighting the rare occurrence of early-period coronary aneurysm formation after DES placement and emphasizing the importance of considering potential complications in such cases.
