Abstract

Higher order mode (HOM) damping is a crucial issue for the next generation of high-current energy recovery linacs (ERLs). Beam-induced HOMs can store sufficient energy in the superconducting RF (SRF) cavities, giving rise to beam instabilities and increasing the heat load at cryogenic temperatures. To limit these effects, using HOM couplers on the cutoff tubes of SRF cavities becomes crucial to absorb beam-induced wakefields consisting of all cavity eigenmodes. The study presented here focuses on a 5-cell 801.58 MHz elliptical SRF cavity designed for the multi-turn energy recovery linac PERLE (Powerful Energy Recovery Linac for Experiments). Several coaxial coupler designs are analyzed and optimized to enhance the damping of monopole and dipole HOMs of the 5-cell cavity. The broadband performance of HOM damping is also confirmed by the time-domain wakefield and the frequency-domain simulations. In addition, the thermal behavior of the HOM couplers is investigated. Abstract

The maximum achievable beam current in an Energy Recovery Linac (ERL) is often constrained by Beam Breakup (BBU) instability. Our previous research highlighted that filling patterns have a substantial impact on BBU instabilities in multi-pass ERLs. In this study, we extend our investigation to the 8-cavity model of the Powerful ERL for Experiment (PERLE). We evaluate its requirements for damping cavity Higher Order Modes (HOMs) and propose optimal filling patterns and bunch timing strategies. Our findings reveal a significant new insight: while filling patterns are crucial, the timing of bunches also plays a critical role in mitigating HOM beam loading and BBU instability. This previously underestimated factor is essential for effective BBU control. We estimated the PERLE threshold current using both analytical and numerical models, incorporating the designed PERLE HOM dampers. During manufacturing, HOM frequencies are expected to vary slightly, with an assumed RMS frequency jitter of 0.001 between cavities for the same HOM. Introducing this jitter into our models, we found that the dampers effectively suppressed BBU instability, achieving a threshold current an order of magnitude higher than the design requirement. Our results offer new insights into ERL BBU beam dynamics and have important implications for the design of future ERLs.

Abstract

The International Muon Collider Collaboration (IMCC) [1] was established in 2020 following the recommendations of the European Strategy for Particle Physics (ESPP) and the implementation of the European Strategy for Particle Physics-Accelerator R&D Roadmap by the Laboratory Directors Group [2], hereinafter referred to as the the European LDG roadmap. The Muon Collider Study (MuC) covers the accelerator complex, detectors and physics for a future muon collider. In 2023, European Commission support was obtained for a design study of a muon collider (MuCol) [3]. This project started on 1st March 2023, with work-packages aligned with the overall muon collider studies. In preparation of and during the 2021-22 U.S. Snowmass process, the muon collider project parameters, technical studies and physics performance studies were performed and presented in great detail. Recently, the P5 panel [4] in the U.S. recommended a muon collider R&D, proposed to join the IMCC and envisages that the U.S. should prepare to host a muon collider, calling this their "muon shot". Abstract

The $^{246}$Cm(n,$\gamma$) and $^{248}$Cm(n,$\gamma$) cross-sections have been measured at the Experimental Area 2 (EAR2) of the n_TOF facility at CERN with three C$_6$D$_6$ detectors. This measurement is part of a collective effort to improve the capture cross-section data for Minor Actinides (MAs), which are required to estimate the production and transmutation rates of these isotopes in light water reactors and innovative reactor systems. In particular, the neutron capture in $^{246}$Cm and $^{248}$Cm open the path for the formation of other Cm isotopes and heavier elements such as Bk and Cf and the knowledge of (n,$\gamma$) cross-sections of these Cm isotopes plays an important role in the transport, transmutation and storage of the spent nuclear fuel. The reactions $^{246}$Cm(n,$\gamma$) and $^{248}$Cm(n,$\gamma$) have been the two first capture measurements analyzed at n_TOF EAR2. Until this experiment and two recent measurements performed at J-PARC, there was only one set of data of the capture cross-sections of $^{246}$Cm and $^{248}$Cm, that was obtained in 1969 in an underground nuclear explosion experiment. In the measurement at n_TOF a total of 13 resonances of $^{246}$Cm between 4 and 400 eV and 5 of $^{248}$Cm between 7 and 100 eV have been identified and fitted. The radiative kernels obtained for $^{246}$Cm are compatible with JENDL-5, but some of them are not with JENDL-4, which has been adopted by JEFF-3.3 and ENDF/B-VIII.0. The radiative kernels obtained for the first three $^{248}$Cm resonances are compatible with JENDL-5, however, the other two are not compatible with any other evaluation and are 20% and 60% larger than JENDL-5.

Settings: Outpatients referred at Pain Unit of San Vincenzo Hospital in Taormina (Italy), between September 2014 and January 2016. Subjects: Eleven FBSS patients diagnosed with neuropathic pain using the Douleur ­Neuropathique 4 questionnaire and suffering from moderate to severe chronic refractory pain, and undergoing treatment with SCS and a combination of THC/CBD for 12 consecutive months. Materials and methods: All the included patients discontinued previous unsuccessful therapy at least 2 months before the beginning of the cannabinoid therapy, with the exception of the SCS that was continued. Patients received a fixed dosage of cannabinoid agonists (THC/CBD) that could be increased subjective to pain control response. A Brief Pain Inventory questionnaire was administered to measure pain and its interference with characteristic dimensions of feelings and functions. The duration of treatment with SCS and THC/CBD combination was 12 months. Results: Effective pain management as compared to baseline result was achieved in all the cases studied. The positive effect of cannabinoid agonists on refractory pain was maintained during the entire duration of treatment with minimal dosage titration. Abstract

Abstract Background Multicomponent interventions based on a comprehensive geriatric assessment (CGA) could promote active aging and improve health status in older people with Noncommunicable Chronic Diseases (NCDs), but conflicting evidences are available. Aim To evaluate the efficacy of a CGA-based multicomponent personalized preventive program (PPP) in reducing unplanned hospitalization rates during 12-month follow-up in community-dwelling older people with NCDs. Materials and methods In this randomized clinical trial (RCT), 1216 older adults recruited by 33 general practitioners (GPs) will be randomly allocated to intervention group (IG) or usual care control group (CG). The IG will receive a multicomponent PPP developed on the findings of the CGA-based Multidimensional Prognostic Index short-form (Brief-MPI), including structured interventions to improve functional, physical, cognitive, and nutritional status, to monitor NCDs and vaccinations, and to prevent social isolation. Participants in the CG will receive usual care. Brief-MPI, resilience, and health-related quality of life will be assessed after 6 and 12 months. Moreover, saliva samples will be collected at baseline in IG to measure biomarkers of oxidative stress, inflammatory cytokines, and oral microbiome. Expected results The CGA-based PPP might reduce unplanned hospitalization rates and potentially institutionalization rates, emergency department (ED) and unplanned GP visits, and mortality. Further outcomes explored in the IG will be the adherence to PPP, resilience, health-related quality of life, and multidimensional frailty as assessed by the Brief-MPI. Conclusions Results will suggest whether the CGA-based multicomponent PPP is able to improve specific outcomes in a primary care setting. 12. Sarcopenic obesity and health outcomes: An umbrella review of systematic reviews with meta‐analysis

Abstract

Abstract Many studies support the idea that sarcopenic obesity (SO) could be considered a potential risk factor for negative health outcomes. These results have been inconsistent, and no umbrella reviews exist regarding this topic. Several databases until November 2023 were searched for systematic reviews with meta‐analysis of observational studies (cross‐sectional, case–control and prospective). For each association, random‐effects summary effect sizes with correspondent 95% confidence intervals (CIs) were evaluated using the GRADE tool. Among the 213 papers initially screened, nine systematic reviews with meta‐analysis were included, for a total of 384 710 participants. In cross‐sectional and case–control studies, 30 different outcomes were analysed, and 18 were statistically significant. In any population addressed in cross‐sectional and case–control studies, compared with non‐SO, SO increased the prevalence of cognitive impairment (k = 3; odds ratio [OR] = 3.46; 95% CI: 2.24–5.32; high certainty of evidence), coronary artery disease (k = 2; OR = 2.48; 95% CI: 1.85–3.31) and dyslipidaemia (k = 3; OR = 2.50; 95% CI: 1.51–4.15). When compared with sarcopenia or obesity, the results were conflicting. In prospective studies, the association between SO—compared with non‐SO—and other negative outcomes was supported by low/very low certainty of evidence and limited to a few conditions. Besides, no comparison with sarcopenia or obesity was provided. Finally, only a few studies have considered muscle function/physical performance in the diagnostic workup. SO could be considered a risk factor only for a few conditions, with the literature mainly based on cross‐sectional and case–control studies. Future studies with clear definitions of SO are needed for quantifying the importance of SO—particularly when compared with the presence of only sarcopenia or obesity—and the weight of muscle function/physical performance in its definition.

Purpose: The mesoderm specific transcription (MEST) gene is a paternally expressed imprinted gene that appears to play a role in embryo survival. The latest meta-analysis on MEST methylation pattern in spermatozoa of infertile patients found higher methylation in spermatozoa from infertile patients than fertile controls. To provide an updated and comprehensive systematic review and meta-analysis on the MEST gene methylation pattern in patients with abnormal sperm parameters compared to men with normal parameters. Materials and Methods: This meta-analysis was registered in PROSPERO (CRD42023397056) and performed following the MOOSE guidelines for Meta-analyses and Systematic Reviews of Observational Studies and the Preferred Reporting Items for Systematic Reviews and Meta-Analysis Protocols (PRISMA-P). Only original articles evaluating MEST gene methylation in spermatozoa from patients with infertility or abnormalities in one or more sperm parameters compared to fertile or normozoospermic men were included. Results: Of 354 abstracts evaluated for eligibility, only 6 studies were included in the quantitative synthesis, involving a total of 301 patients and 163 controls. Abstract

In several elementary particle scenarios, self-dual fields emerge as fundamental degrees of freedom. Some examples are the $D = 2$ chiral boson, $D = 10$ Type IIB supergravity, and $D = 6$ chiral tensor multiplet theory. For those models, a novel variational principle has been proposed in the work of Ashoke Sen. The coupling to supergravity of self-dual models in that new framework is rather peculiar to guarantee the decoupling of unphysical degrees of freedom. We generalize this technique to the self-dual super Maxwell gauge theory in $D = 4$ Euclidean spacetime both in the component formalism and the superspace. We use the geometric tools of rheonomy and integral forms since they are very powerful geometrical techniques for the extension to supergravity. We show the equivalence between the two formulations by choosing a different integral form defined using a Picture Changing Operator. That leads to a meaningful action functional for the variational equations. In addition, we couple the model to a non-dynamical gravitino to extend the analysis slightly beyond the free case. Abstract

Background: The $^{14}$N(n,p)$^{14}$C reaction is of interest in neutron capture therapy, where nitrogen-related dose is the main component due to low-energy neutrons, and in astrophysics, where 14N acts as a neutron poison in the s-process. Several discrepancies remain between the existing data obtained in partial energy ranges: thermal energy, keV region and resonance region. Purpose: Measuring the 14N(n,p)14C cross section from thermal to the resonance region in a single measurement for the first time, including characterization of the first resonances, and providing calculations of Maxwellian averaged cross sections (MACS). Method: Time-of-flight technique. Experimental Area 2 (EAR-2) of the neutron time-of-flight (n_TOF) facility at CERN. $^{10}$B(n,${\alpha}$)$^7$Li and $^{235}$U(n,f) reactions as references. Two detection systems running simultaneously, one on-beam and another off-beam. Description of the resonances with the R-matrix code sammy. Results: The cross section has been measured from sub-thermal energy to 800 keV resolving the two first resonances (at 492.7 and 644 keV). A thermal cross-section (1.809$\pm$0.045 b) lower than the two most recent measurements by slightly more than one standard deviation, but in line with the ENDF/B-VIII.0 and JEFF-3.3 evaluations has been obtained. A 1/v energy dependence of the cross section has been confirmed up to tens of keV neutron energy. The low energy tail of the first resonance at 492.7 keV is lower than suggested by evaluated values, while the overall resonance strength agrees with evaluations. Conclusions: Our measurement has allowed to determine the $^{14}$N(n,p) cross-section over a wide energy range for the first time. We have obtained cross-sections with high accuracy (2.5 %) from sub-thermal energy to 800 keV and used these data to calculate the MACS for kT = 5 to kT = 100 keV., Comment: 18 pages, 15 figures, 4 tables

Abstract

With the aim of measuring the $^{235}$U(n,f) cross section at the n\_TOF facility at CERN over a wide neutron energy range, a detection system consisting of two fission detectors and three detectors for neutron flux determination was realized. The neutron flux detectors are Recoil Proton Telescopes (RPT), based on scintillators and solid state detectors, conceived to detect recoil protons from the neutron-proton elastic scattering reaction. This system, along with a fission chamber and an array of parallel plate avalanche counters for fission event detection, was installed for the measurement at the n\_TOF facility in 2018, at CERN. An overview of the performances of two RPTs - especially developed for this measurement - and of the parallel plate avalanche counters are described in this article. In particular, the characterization in terms of detection efficiency by Monte Carlo simulations and response to neutron beam, the study of the background, dead time correction and characterization of the samples, are reported. The results of the present investigation show that the performances of these detectors are suitable for accurate measurements of fission reaction cross sections in the range from 10 to 450~MeV.

Background: The role of main work during the life course in predicting frailty, a typical geriatric syndrome, is still largely unknown. Therefore, with this research, we aimed to investigate the potential association between the main work done during the life with frailty and pre-frailty among participants 60 years and older of the UK Biobank study. Methods: Frailty and pre-frailty presence were ascertained using a model including 5 indicators (weakness, slowness, weight loss, low physical activity, and exhaustion); the main employment status was ascertained using self-reported information. The association between frailty and main work was explored using an ordinal logistic regression model and reported as odds ratios (ORs) with their 95 % confidence intervals (CIs). Results: The final sample comprised a total of 50,447 individuals (mean age: 64.2 years, females: 50.2 %). Abstract

OBJECTIVES Programmatic assessment approaches can be extended to the design of allied health professions training, to enhance the learning of trainees. The Australasian College of Physical Scientists and Engineers in Medicine worked with assessment specialists at the Australian Council for Educational Research and Amplexa Consulting, to revise their medical physics and radiopharmaceutical science training programs. One of the central aims of the revisions was to produce a training program that provides standardized training support to their registrars throughout the 3 years, better supporting their registrars to successfully complete the program in the time frame through providing timely and constructive feedback on the registrar's progression. METHODS We used the principles of programmatic assessment to revise the assessment methods and progression decisions in the three training programs. RESULTS We revised the 3-year training programs for diagnostic imaging medical physics, radiation oncology medical physics and radiopharmaceutical science in Australia and New Zealand, incorporating clear stages of training and associated progression points. CONCLUSIONS We discuss the advantages and difficulties that have arisen with this implementation. We found 5 key elements necessary for implementing programmatic assessment in these specialized contexts: embracing blurred boundaries between assessment of and for learning, adapting the approach to each specialized context, change management, engaging subject matter experts, and clear communication to registrars/trainees.

Abstract

Functional hypothalamic amenorrhea (FHA) is a common cause of amenorrhea and chronic anovulation in adolescent girls and young women, diagnosed after excluding other organic causes. It is commonly associated with calorie restriction, excessive physical exercise, and psychosocial stress. These stressors alter the pulsatile secretion of gonadotropin-releasing hormone, leading to a chronic condition of hypoestrogenism and significant health consequences. Recent evidence has highlighted a genetic predisposition to FHA that could explain interindividual variability in stress response. Indeed, not all women experience FHA in response to stress. Rare variants in genes associated with idiopathic hypogonadotropic hypogonadism have been identified in women with FHA, suggesting that these mutations may contribute to an increased susceptibility of women to the trigger of stress exposure. FHA appears today as a complex disease resulting from the combination of genetic predisposition, environmental factors, and epigenetic changes. Furthermore, the genetic background of FHA allows for the hypothesis of a male counterpart. Despite the paucity of data, preliminary findings indicate that an equivalent condition of FHA exists in men, warranting further investigation. This narrative review aims to summarize the recent genetic evidence contributing to the pathophysiology of FHA and to raise awareness on a possible male counterpart.

Abstract

Abstract Remdesivir (RDV) was the first Food and Drug Administration (FDA)-approved medication for COVID-19, with discordant data on efficacy in reducing mortality risk and disease progression. In the context of a dynamic and rapidly changing pandemic landscape, the utilization of real-world evidence is of utmost importance. The objective of this study is to evaluate the impact of RDV on patients who have been admitted to two university referral hospitals in Italy due to COVID-19. All patients older than 18 years and hospitalized at two different universities (Bari and Palermo) were enrolled in this study. To minimize the effect of potential confounders, we used propensity score matching with one case (Remdesivir) and one control that never experienced this kind of intervention during hospitalization. Mortality was the primary outcome of our investigation, and it was recorded using death certificates and/or medical records. Severe COVID-19 was defined as admission to the intensive care unit or a qSOFAscore ≥ 2 or CURB65scores ≥ 3. This study aimed to evaluate the ability of a specific set of RNAs, including long ncRNAs (lncRNAs), microRNAs (miRNAs), and mRNAs, to discriminate between PCa and the non-neoplastic condition benign prostatic hyperplasia (BPH). After selecting by literature mining the most relevant RNAs differentially expressed in biofluids from PCa patients, we evaluated their discriminatory power in samples of unfiltered urine from 50 PCa and 50 BPH patients using both real-time PCR and droplet digital PCR (ddPCR). Additionally, we also optimized a protocol for urine sample manipulation and RNA extraction. This two-way validation study allowed us to establish that miRNAs (i.e., miR-27b-3p, miR-574-3p, miR-30a-5p, and miR-125b-5p) are more efficient biomarkers for PCa compared to long RNAs (mRNAs and lncRNAs) (e.g., PCA3, PCAT18, and KLK3), as their dysregulation was consistently reported in the whole urine of patients with PCa compared to those with BPH in a statistically significant manner regardless of the quantification methodology performed. Moreover, a significant increase in diagnostic performance was observed when molecular signatures composed of different miRNAs were considered. Hence, the abovementioned circulating ncRNAs represent excellent potential non-invasive biomarkers in urine capable of effectively distinguishing individuals with PCa from those with BPH, potentially reducing cancer overdiagnosis.

49. An Exploration into Early Childhood Physical Literacy Programs: A Systematic Literature Review

Abstract

This review explored the impact of physical literacy programs designed to engage two- to five-year-old preschool children. The Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) was used. Six EBSCO host databases were searched for the period 2011 to April 2021 using the search terms "physical literacy," "early childhood," and "impact." Articles were excluded if physical literacy was not the focal intervention. The final data set consisted of seven-peer reviewed articles meeting the eligibility criteria and quality assessment for this review. Three themes were created using Braun and Clark's (2006) approach to thematic analysis: Holistic benefits of physical literacy, Barriers to physical literacy and Education begins at home. Early childhood physical literacy programs provide holistic benefits for children; however, further research is needed in an Australian context. Families and community members working in the early childhood sector could benefit from further education and training to improve physical literacy awareness.
