Abstract

IntroductionThe number of doctoral programs to train future public health leaders is critical in meeting the demand of global health challenges in the 21st century. Ten United States online public health doctoral programs accept only a fraction of interested learners.MethodsThis research examines the launch of the first online public health doctoral program, accredited by the Council on Education for Public Health, and compares nine similar programs that have followed in the ensuing 12 years.ResultsSurvey results highlight the demand by Master of Public Health degree holders for online public health doctoral programs; 84.11% of survey respondents indicated an interest in obtaining a doctoral degree.DiscussionIf we strive to answer the question posed by the Institute of Medicine in 2003, “Who will keep the public healthy?” then we need to provide education that is accessible, efficient and equitable for interested learners, most of whom are turned down by online public health doctoral programs that have limited capacity.

2. Success of Patient and Family Advisory Councils: The Importance of Metrics

Abstract

Hospital Patient and Family Advisory Councils have been around for over 40 years. Yet, their adoption is waning with only slightly more than 50% of hospitals fielding Councils, which are composed of patient and family volunteers, who provide feedback to hospital staff. Demonstrating the value of Councils is critically important to their success and sustainability. Hospitals can ensure Council success by adopting these initiatives: (1) Highlight the importance of measuring program impacts to Council liaisons and members, (2) Educate members and staff about the three types of impacts defined by the Agency for Healthcare Research Quality, (3) Describe the steps to ensure that the value of the Council is well documented, and (4) Provide tools and training for the Council and staff to conduct robust measurements and analysis. When Councils measure their impact, their value to hospitals and leadership will increase.

This study analyzed data from a survey distributed to teachers in an urban school district in New Jersey. The survey asked teachers about their past and current perceptions of, and practices with, the use of classroom technology and in particular one-to-one devices. Findings reveal teachers generally perceive technology as positive in teaching and learning, but also noted some limitations. The biggest takeaway from this study is that teachers believe they can improve both their technology skills and their methods of technology integration into instruction, provided they are given the necessary time to learn, use, and practice with technology and one-to-one devices.

5. Data-Driven Decision Making: An Holistic Approach to Assessment in Special Collections Repositories

Abstract

Objective – In an environment of shrinking budgets and reduced staffing, this study seeks to identify a comprehensive, integrated assessment strategy to better focus diminished resources within special collections repositories.Methods – This article presents the results of a single case study conducted in the Special and Digital Collections department at a university library. The department created an holistic assessment model, taking into account both public and technical services, to explore inter-related questions affecting both day-to-day operations as well as long-term, strategic priorities.Results – Data from a variety of assessment activities positively impacted the department’s practices, informing decisions made about staff skill sets, training, and scheduling; outreach activities; and prioritizing technical services. The results provide a comprehensive view of both patron and department needs, allowing for a wide variety of improvements and changes in staffing practices, all driven by data rather than anecdotal evidence.Conclusion – Although the data generated for this study is institutionally specific, the methodology is applicable to special collections departments at other institutions. A systemic, holistic approach to assessment in special collections departments enables the implementation of operational efficiencies. It also provides data that allows the department to document its value to university-wide stakeholders.

7. Use of One-to-One Devices in an Urban School District

Abstract

This is a qualitative mixed-methods study of an urban school district in New Jersey. A survey of teachers explored their past and current practices with regard to the use of classroom technology, in particular one-to-one devices; and an interview with the district supervisor for educational technology provided detailed information about district plans during and after the shutdown of schools in 2020, due to the COVID-19 pandemic. The pivot to remote learning during the shutdown forced teachers to learn new technological skills and helped them gain confidence in those skills. Many teachers continue to use these skills in the classroom since they returned to in-person teaching and learning. This study provided valuable insight into teachers' experiences and opinions on the use of technology, professional development, one-to-one devices, and helping students thrive in the post-pandemic era. Teachers emphasized the importance of relevant professional development, but the biggest takeaway from this study is that teachers are more than capable of improving both their technology skills and their methods of integration into instruction, provided they are given the necessary time: time to learn, time to practice, and time to give and receive help on their journey towards technological fluency. 8. Analysis of novel cellular responses and serological markers in colorectal cancer patients

Abstract

In recent years there have been significant advances in the treatment of cancer. This has been particularly aided by the discovery and exploitation of checkpoint inhibition; immunotherapy has the potential to induce sustained responses and increased survival in some patients. In solid tumours such as colorectal cancer (CRC), however, these therapies only benefit a small subgroup of patients. This is thought to be a result of poor tumour immunogenicity and low immune cell infiltration. In light of this, efforts are underway to identify new immunotherapeutic approaches to both alleviate tumour-mediated immunosuppression, and to increase anti-tumour immune responses. The Godkin/Gallimore research group has recently completed a clinical trial, “TroVax and Cyclophosphamide Treatment in Colorectal Cancer” (TaCTiCC), in which the ability of cyclophosphamide (CPM) and 5T4 cancer vaccine TroVax to induce immune responses in advanced metastatic CRC (mCRC) patients was assessed. Unexpectedly, these treatments were found to not only induce immune responses but were also associated with increased survival in a proportion of patients, therefore may represent new treatment options for mCRC patients. Given the rapid explosion in the number of available immunotherapies, it is vital that we understand which patients will benefit from treatment. This thesis investigates a panel of plasma proteins and serological/immunological markers to identify mCRC patients who responded to CPM and TroVax on the TaCTiCC trial; several potential biomarkers of response to both therapies have been identified. Plasma proteins were also assessed for their ability to identify earlier-stage and mCRC patients both from each other, and from healthy donors; there is a clear and significant difference in circulating plasma proteins between these groups. Finally, this thesis investigates T cell responses to 7 novel CRC tumour antigens recently identified within the Godkin/Gallimore group; responses to several antigens are reduced in CRC patients compared to healthy donors and may therefore represent useful immunotherapy targets. It is hoped that the findings described in this thesis will contribute to the improved identification of CRC, and to our understanding of factors associated with immunotherapy response. Moreover, the identification of novel CRC antigen targets may inform the development of improved immunotherapies.

12. Clinical Practice Patterns in Childhood Apraxia of Speech and Augmentative and Alternative Communication: Implications for Speech-Language Pathology Education

Purpose: The purpose of this study was to describe speech-language pathologists' (SLPs') clinical practice patterns related to childhood apraxia of speech (CAS) and augmentative and alternative communication (AAC). Method: A survey study was conducted with a total of 52 SLPs. Survey questions were related to SLPs' education, clinical experiences, and perceptions of CAS and AAC and implementation of AAC. Results: Most SLPs reported having had a dedicated AAC class during graduate school and are regularly pursuing continuing education in both CAS and AAC. SLPs reported varied clinical experiences, perceptions, and implementation of AAC in clients with CAS, with 86.54% reporting recommending AAC at some point to a client with CAS. The percentage of clients with CAS who were recommended AAC was significantly less for clinicians who pursued less continuing education in CAS ( p = .023). SLPs were significantly more likely to report comfort with AAC if they pursued continuing education in AAC ( p = .025) or had an AAC class in graduate school ( p = .007). Conclusions: SLPs' educational and clinical experience influence their clinical practice patterns related to clients with CAS and implementation of AAC. Based on the needs and skills of clients with CAS, graduate clinical education and continuing education courses regarding treatment of CAS should include information on AAC, particularly in the context of supporting receptive and expressive language skills, alongside speech development.

13. Structural variation in families with speech and language disorder provides a deeper understanding of the genetic basis for naturally acquired speech

Abstract

Childhood apraxia of speech (CAS) is a severe and rare form of speech sound disorder (SSD) with an estimated prevalence of 0.01-2%. CAS typically occurs in isolation (sporadic), but may segregate in families with broader speech and language deficits. We hypothesized that disruptive genetic changes may be involved in the etiology of CAS, and were resolvable by examination of whole genome sequence. We sequenced 27 families with a CAS child within the Cleveland Family Speech and Reading Study, examining 101 individuals in all. CAS subjects displayed errors on single word articulation (75%), multisyllabic real word repetition (93%), multisyllabic non-word repetition (82%), and oral motor function (82%); many also exhibited problems with fine and gross motor skills. We identified 17 genomic regions including 19 unique structural variants (SVs) present in children with CAS. Three variants were shared across families, but the rest were unique; some events were de novo. In four families, siblings with milder phenotypes co-inherited the same SVs, suggesting that some SVs display variable expressivity. In an independent sample, we replicated eight SVs using microarray technology and found that many of these SVs were present in children with milder forms of SSD. Bioinformatic examination of the deletions/duplications identified four SVs with substantial functional consequences (cytobands 2q24.3, 6p12.3-6p12.2, 11q23.2-11q23.3, and 16p11.2). Of these, the 16p11.2 deletion is the most well-established variant that causes a broad array of neurological features. These discoveries show that SVs are a heterogeneous, but prevalent cause of CAS, identifiable by standard genetic testing.

Prenatal cocaine exposure (PCE) has been associated with small but significant effects on language development in childhood and early adolescence. This study examined whether this association persists into later adolescence and what relationship language skills may have with reading proficiency in this population.Enrolled were 338 (167 with PCE, 171 with NCE or no cocaine exposure) 17-year-olds recruited at birth who, together with their current caregiver, were seen as part of a 17-year follow-up. Participants were given assessments of reading achievement (WIAT-III), receptive and expressive language (CELF-IV), and phonological processing (CTOPP). Relationships between PCE status and language outcomes were modeled using multiple linear regression controlling for environmental and caregiver factors, and other prenatal substance exposures.Adolescents with PCE scored lower in areas of phonological processing and reading related skills compared to adolescents with NCE. PCE by sex interactions were identified on language, memory and spoken language comprehension, with lower scores for girls with PCE compared to girls with NCE.These findings suggest the persistence of PCE's relationship with phonological awareness well into adolescence. PCE was also associated with lower scores on measures of skills related to reading ability, which may be a manifestation of the observed deficits in phonological processing.

Abstract

This paper describes the software changes made to the data processing and display system for HERMES III accelerator at the Simulation Technology Laboratory (STL) at Sandia National Laboratories, New Mexico. The HERMES III accelerator is a gamma ray simulator producing 100kRad[Si] dose per shot with a full width half max pulse duration of ~25 nanoseconds averaging six shots per day. For each accelerator test approximately 400 probe signals are recorded over approximately 65 digitizers. The original data processing system provided the operator a report summarizing the start of probe signal timings for groups of probes located within the power flow conductors. This timing information is indicative of power flow symmetry allowing the operator to make necessary adjustments prior to the next test. The report also provided data overlays concerning laser trigger light output, x-ray diode currents and x-ray source output. Power flow in the HERMES III accelerator is comprised of many circuit paths and detailed current and voltage information within these paths could provide a more thorough understanding of accelerator operation and performance, however this information was either not quickly available to the operators or the display of the data was not optimum. We expanded our data processing abilities to determine the current and voltage amplitudes throughout the power flow conductors and improved the data display abilities so data plots can be presented in a more organized fashion. We detail our efforts creating a software program capable of processing the ~ 400 probe signals together with an organized method for displaying the dozens of current and voltage probes. This process is implemented immediately after all digitizer data has been collected so the operator is provided timing and power flow information shortly after each accelerator shot.

This paper describes the hardware changes made to the triggering system of the HERMES III accelerator at Sandia National Laboratories, New Mexico. The HERMES III accelerator is a gamma ray simulator producing 100 kRad dose per shot with a full width half max pulse duration of approximately 25 nanoseconds and averaging six shots per day. For each accelerator test, approximately 400 probe signals are recorded over approximately 65 digitizers. The original digitizer trigger system employed numerous independent legacy signal generators resulting in non-referenceable digitizer time bases. We detail our efforts to reference the digitizer time bases together using a modular and scalable approach with commercial-off-the-shelf components. This upgraded trigger system presently measures a maximum digitizer trigger time spread of less than two nanoseconds across the 65+ digitizers. This document details the hardware changes, provides a summary of the accelerator charging process, presents “one-line” trigger system diagrams and summarizes the times of interest for a typical HERMES accelerator shot.

Abstract

The Hermes III accelerator at Sandia National Laboratories is a 20-cavity multi-stage linear induction voltage accelerator typically producing a 20-MV, 40-ns, 600-kA output pulse. Energy is initially stored in Marx banks that are each discharged into two intermediate store capacitors. Each of these capacitors are then switched with an SF6-insulated high voltage Rimfire gas switch into four parallel pulse forming lines that further condition the discharge pulse and deliver it to the induction cavities arrayed along the axis of the machine. Presently, a single 0.9-J KrF laser operating at 248 nm, the output of which is divided into twenty beams, is used to trigger the 20 rim-fire switches. As part of an upgrade to the accelerator, however, a new solid state laser triggering system is being designed to replace this system and provide additional capabilities for the accelerator. The laser triggering system will be made up of 10 discrete compact flash-lamp pumped, Q-switched Nd:YAG lasers (Tempest 300), each having an output energy of 40 mJ at a wavelength of 266 nm. As each laser will be responsible for triggering only two of the rim-fire switches, it becomes possible to shape the output pulse by varying the times at which the individual lasers fire. Overall reliability for the accelerator's operation with these new lasers will be increased, as well. The general layout of this new laser triggering system design will be presented, along with details pertaining to the triggering of the lasers and the optical beam paths.

Abstract

In 2009, the University of South Florida (USF) Libraries received from the organization Waging Peace a collection of materials which included transcripts of interviews with female genocide survivors and original children's drawings depicting the atrocities of genocide in Darfur. These materials are particularly significant in that they were used as evidence of genocide in Darfur in the International Criminal Court. The children's drawings have been digitized by USF and are an important element in the development of a web portal for the new Holocaust & Genocide Studies Center. These drawings also inspired the performance piece What the Heart Remembers: The Women and Children of Darfur staged by the USF Theatre and Dance Department. This article focuses on the library's acquisition and digitization of these materials, the ongoing efforts to develop a web presence, faculty outreach, and the multidisciplinary collaboration between the library and other USF departments in this project.

This article examines how special collections are presented on the Web, the use of LibGuides for special collections, and how the use of LibGuides is facilitating the goal to provide information in a meaningful way by Special and Digital Collections at the University of South Florida Tampa Library.

The flexible API allowed SDC to embed digital content, enable user comments, advertise events, and measure the impact of various services.Practical implicationsLibGuides provides an intuitive, simple, and cost‐effective method to create dynamic and frequently updated guides to special collections.Originality/valueThis is the first paper to explore the use of LibGuides in a special collections repository.

Abstract

The National Lung Screening Trial (NLST) is a randomized multicenter study comparing low-dose helical computed tomography (CT) with chest radiography in the screening of older current and former heavy smokers for early detection of lung cancer, which is the leading cause of cancer-related death in the United States. Five-year survival rates approach 70% with surgical resection of stage IA disease; however, more than 75% of individuals have incurable locally advanced or metastatic disease, the latter having a 5-year survival of less than 5%. It is plausible that treatment should be more effective and the likelihood of death decreased if asymptomatic lung cancer is detected through screening early enough in its preclinical phase. For these reasons, there is intense interest and intuitive appeal in lung cancer screening with low-dose CT. The use of survival as the determinant of screening effectiveness is, however, confounded by the well-described biases of lead time, length, and overdiagnosis. Despite previous attempts, no test has been shown to reduce lung cancer mortality, an endpoint that circumvents screening biases and provides a definitive measure of benefit when assessed in a randomized controlled trial that enables comparison of mortality rates between screened individuals and a control group that does not undergo the screening intervention of interest. The NLST is such a trial. The rationale for and design of the NLST are presented.

Abstract

Declining reference statistics, diminishing human resources, and the desire to be more proactive and embedded in academic departments, prompted the University of South Florida Library to create a taskforce for re-envisioning reference services. The taskforce was charged with examining the staffing patterns at the desk and developing recommendations to give librarians greater flexibility and to better respond to the information-seeking needs of users.These recommendations were based on statistics of desk usage, collected with the newly adapted online tool Desk Tracker, and structured interviews with library administrators. The taskforce was interested in how these stakeholders use quantitative data in decision-making.

Abstract

This paper summarizes the recent changes to the hardware and software systems associated with the diagnostic digitizer triggers and their data acquisition control program at the Saturn pulsed power facility at Sandia National Laboratories. The main screen room contains approximately 70 digitizing scopes for monitoring 6 voltage or current probes along each of its 36 energy storage and transmission lines. Each module locates probes along its length in an identical fashion so that by comparing the timing of similar probes on each line, a measure of the power flow time symmetry can be obtained. The probes from each line are recorded in scope groupings, each containing 36 channels. In numerous cases, the archived group time bases were not correctly quantified with respect to the other scope groupings. This was problematic when comparing signals from different scope groupings. The source of the errors was found to be how the scope groupings were triggered as well as a software error associated with two models of digitizers. We have implemented changes so that all scope groupings are time tied to a common reference.

Abstract

The Sandia National Laboratories SPHINX accelerator is used to study the response of electronics to pulsed x-ray and electron environments. The system consists of a Marx generator and an oil-insulated pulse-forming line with self-closing oil switches. SPHINX has a peak load voltage of 2 MV and an adjustable pulse width ranging from 3 to 10 ns. The previous pulsed-power system had reliability and triggering issues with the Marx generator and subsequent undesired variations in voltage output. SPHINX was upgraded to a new Marx-generator system that has solved many of the voltage-output fluctuation and timing issues. The new Marx generator uses recently developed low-inductance 100-kV capacitors and 200-kV spark-gap switches. This paper provides an overview of SPHINX while capturing in detail the design, characterization, and comparative performance of the new Marx generator.

David Krasner: In surveying contemporary London theatre, New York Times critic Ben Brantley reported that the Tricycle Theatre hadinaugurated a season of African-American plays with the commandingly titled but obscure Walk Hard, Talk Loud, a play by Abram Hill from the early1940's. Abram who? The name meant nothing to me, but Abram Hill (1910–1986) was a founder and director of the American Negro Theater in New York (1940–1951) and a playwright, it seems, of considerable verve.3That Abram Hill and the American Negro Theatre—the most important black theatre company during the mid-twentieth century—has flown below the radar is indicative of how much work still needs to be accomplished.

Abstract

Relatively limited contemporary information is available about the magnitude of, and factors associated with, the metabolic syndrome in adult men and women. The purpose of our observational study was to describe the prevalence and predictors of the metabolic syndrome in a sample of employed adults attending a worksite cardiovascular screening program. The study sample consisted of 871 men and women between the ages of 21 and 77 years from 6 locations of the parent company. These individuals attended an employer-sponsored cardiovascular screening and wellness program during 2003. A standardized questionnaire was administered to all study participants and a number of different coronary risk factors were measured. Approximately 27% of the study sample was classified as having the metabolic syndrome. Men, persons with a history of hypertension, heart disease, or stroke, sedentary individuals, and those with an increased heart rate and higher levels of C-reactive protein were associated with presence of the metabolic syndrome. A relatively similar risk factor profile was noted in persons without a self-reported history of prior cardiovascular disease. The results of our cross-sectional observational study suggest that the prevalence of the metabolic syndrome is considerable. A number of demographic, comorbid, and other factors are associated with this syndrome. Increased attention to the metabolic syndrome, and modification of predisposing factors, remains of considerable public health and clinical importance.
