Abstract

Despite notable scientific and medical advances, broader political, socioeconomic and behavioural factors continue to undercut the response to the COVID-19 pandemic . Here we convened, as part of this Delphi study, a diverse, multidisciplinary panel of 386 academic, health, non-governmental organization, government and other experts in COVID-19 response from 112 countries and territories to recommend specific actions to end this persistent global threat to public health. The panel developed a set of 41 consensus statements and 57 recommendations to governments, health systems, industry and other key stakeholders across six domains: communication; health systems; vaccination; prevention; treatment and care; and inequities. In the wake of nearly three years of fragmented global and national responses, it is instructive to note that three of the highest-ranked recommendations call for the adoption of whole-of-society and whole-of-government approaches , while maintaining proven prevention measures using a vaccines-plus approach that employs a range of public health and financial support measures to complement vaccination. Other recommendations with at least 99% combined agreement advise governments and other stakeholders to improve communication, rebuild public trust and engage communities in the management of pandemic responses. The findings of the study, which have been further endorsed by 184 organizations globally, include points of unanimous agreement, as well as six recommendations with >5% disagreement, that provide health and social policy actions to address inadequacies in the pandemic response and help to bring this public health threat to an end.

Abstract

There has been a wide range of responses by governments to the COVID-19 (coronavirus) pandemic. Building on Oxford COVID-19 Government Response Tracker (OxCGRT), this report shows how the stringency of government responses to COVID-19 index can be used to better understand how governments are seeking to deter COVID-19 in the context of federalism. To do so, this report refines the stringency index introduced by OxCGRT and shows how it can be used to track federal and state-level government responses across states and time in Brazil. After introducing the refined version of this index (CGRT-BRFED), this report focuses on the social distancing policies adopted by the 26 states and the federal district of the Brazilian federation. Specifically, this report details the school, work, and public gathering closure policies that have been enacted and also describes the duration of these actions at the state-level in Brazil. The report highlights that there is significant heterogeneity across Brazil in the Social Distancing Stringency (SDS) Index of the CGRT-BRFED, and presents some preliminary findings related to the level of the social distancing stringency index and the rates of COVID-19 infection across states in Brazil. In future updates, the remaining indicators will be updated and a more detailed report of federal and state-level government policies will be analyzed so that we may be able to assess the complete set of policies that are being enacted as part of a stringent response to the pandemic.

Investigated differences in views and involvements between two cohorts of first generation Cuban-American immigrants. Interviews with immigrants indicated that the first wave left between 1959-1979 mainly for political reasons and publicly opposes travel to Cuba. The second wave immigrated largely for economic reasons and is enmeshed in transnational ties that are unwittingly doing more to transform Cuba than first wave isolationism. (Contains references.) Abstract

This article discusses the epidemic situation of Covid-19 in Brazil, in the face of the emergence of a new strain called P.1, which is more transmissible and may be associated with reinfection. Given the collapse of hospital care in Manaus in January 2021 and the results of three recent preprints, each that reports increased transmissibility of the P.1 variant, we propose some urgent measures. Genomic surveillance based on multi-step diagnostics, starting with RT-PCR type tests and up to sequencing, should be established. Efforts to identify reinfections associated with this variant and the update of its definition in protocols should be prioritized, and studies on the efficacy of currently available vaccines in Brazil concerning the new variant should be conducted. We also propose improving the Brazilian health surveillance system such that genomic surveillance is coordinated and thereby better able to respond to future emergencies in a more timely fashion. We call on the public agents involved in health surveillance to share data and information regarding the epidemic in a clear, fast and transparent way. Finally, we propose a greater engagement in inter-institutional cooperation of all those involved in the response and production of knowledge about the pandemic in our country. RESUMO: Este artigo discute a situação epidêmica da COVID-19 no Brasil diante do aparecimento de uma nova linhagem, chamada P.1, mais transmissível e com possível reinfecção associada. Tendo em vista o colapso do atendimento hospitalar em Manaus em janeiro de 2021 e os resultados de três preprints recentes, dos quais todos encontraram maior transmissibilidade da variante P.1, propomos algumas ações urgentes: o estabelecimento de uma vigilância genômica baseada em diagnóstico em múltiplos passos, iniciando com os testes do tipo transcrição reversa seguida de reação em cadeia da polimerase (RT-PCR) até o sequenciamento; um esforço imediato na identificação de reinfecções associadas à nova variante, com a atualização dos protocolos de definição; e estudos sobre a eficácia das vacinas disponíveis no Brasil na vigência da nova variante. Propomos, ademais, o aprimoramento do sistema de vigilância em saúde brasileiro para que seja articulado com a vigilância genômica, de forma a responder mais oportunamente a emergências futuras. Chamamos os agentes públicos implicados na vigilância em saúde para que compartilhem dados e informações referentes à epidemia de forma clara, rápida e transparente. Finalmente propomos maior engajamento na cooperação interinstitucional de todos os envolvidos na resposta e produção de conhecimento sobre a pandemia em nosso país.

Abstract

The two paradigms for analyzing immigrant experiences, 'assimilationist' and 'transnationalist,' leave unanalyzed important differences in immigrant adaptation rooted in different historical generational experiences. This article analyzes the importance of a historically grounded generational frame of analysis. It captures differences in views and involvements between two cohorts of first generation emigres. Empirically, the study focuses on different Cuban-American cohort crossborder ties. The first cohort, comprised of emigres who left between 1959 and 1979 primarily for political reasons, publicly oppose travel to Cuba because they believe it helps sustain a regime they wish to bring to heel. The second cohort, who emigrated largely for economic reasons, is enmeshed in transnational ties that, paradoxically, are unwittingly doing more to transform Cuba than first wave isolationism. The cohort comparison is based on interviews with emigres in Union City, New Jersey and Miami, Florida.
