Abstract

BackgroundPatient access to clinical information represents a means to improve the transparency and delivery of health care as well as interactions between patients and health care providers. We examine the movement toward augmenting patient access to clinical information using technology. Our analysis focuses on “Blue Button,” a tool that many health care organizations are implementing as part of their Web-based patient portals. ObjectiveWe present a framework for evaluating the effects that technology-assisted access to clinical information may have on stakeholder experiences, processes of care, and health outcomes. MethodsA case study of the United States Department of Veterans Affairs' (VA) efforts to make increasing amounts of clinical information available to patients through Blue Button. Drawing on established collaborative relationships with researchers, clinicians, and operational partners who are engaged in the VA’s ongoing implementation and evaluation efforts related to Blue Button, we assessed existing evidence and organizational practices through key informant interviews, review of documents and other available materials, and an environmental scan of published literature and the websites of other health care organizations. ResultsTechnology-assisted access to clinical information represents a significant advance for VA patients and marks a significant change for the VA as an organization. Evaluations of Blue Button should (1) consider both processes of care and outcomes, (2) clearly define constructs of focus, (3) examine influencing factors related to the patient population and clinical context, and (4) identify potential unintended consequences. ConclusionsThe proposed framework can serve as a roadmap to guide subsequent research and evaluation of technology-assisted patient access to clinical information. To that end, we offer a series of related recommendations.

Abstract

ObjectiveTo describe how a partnered evaluation of the Whole Health (WH) system of care-comprised of the WH pathway, clinical care, and well-being programs-produced patient outcomes findings, which informed Veterans Health Administration (VA) policy and system change.Data sourcesElectronic health records (EHR)-based cohort of 1,368,413 patients and a longitudinal survey of Veterans receiving care at 18 WH pilot medical centers.Study designIn partnership with VA operations, we focused the evaluation on the impact of WH services utilization on Veterans' (1) use of opioids and (2) care experiences, care engagement, and well-being. Outcomes were compared between Veterans who did and did not use WH services identified from the EHR.Data collectionPharmacy records and WH service data were obtained from the VA EHR, including WH coaching, peer-led groups, personal health planning, and complementary, integrative health therapies. We surveyed veterans at baseline and 6 months to measure patient-reported outcomes.Principal findingsOpioid use decreased 23% (31.5-6.5) to 38% (60.3-14.4) among WH users depending on level of WH use compared to a secular 11% (12.0-9.9) decrease among Veterans using Conventional Care. Compared to Conventional Care users, WH users reported greater improvements in perceptions of care (SMD = 0.138), engagement in health care (SMD = 0.118) and self-care (SMD = 0.1), life meaning and purpose (SMD = 0.152), pain (SMD = 0.025), and perceived stress (SMD = 0.191).ConclusionsEvidence developed through this partnership yielded key VA policy changes to increase Veteran access to WH services. Findings formed the foundation of a congressionally mandated report in response to the Comprehensive Addiction and Recovery Act, highlighting the value of WH and complementary, integrative health and well-being programs for Veterans with pain. Findings subsequently informed issuance of an Executive Decision Memo mandating the integration of WH into mental health and primary care across VA, now one lane of modernization for VA.

Abstract

IntroductionThe COVID-19 pandemic brought an urgent need to discover novel effective therapeutics for patients hospitalised with severe COVID-19. The Investigation of Serial studies to Predict Your Therapeutic Response with Imaging And moLecular Analysis (ISPY COVID-19 trial) was designed and implemented in early 2020 to evaluate investigational agents rapidly and simultaneously on a phase 2 adaptive platform. This manuscript outlines the design, rationale, implementation and challenges of the ISPY COVID-19 trial during the first phase of trial activity from April 2020 until December 2021.Methods and analysisThe ISPY COVID-19 Trial is a multicentre open-label phase 2 platform trial in the USA designed to evaluate therapeutics that may have a large effect on improving outcomes from severe COVID-19. The ISPY COVID-19 Trial network includes academic and community hospitals with significant geographical diversity across the country. Enrolled patients are randomised to receive one of up to four investigational agents or a control and are evaluated for a family of two primary outcomes-time to recovery and mortality. The statistical design uses a Bayesian model with 'stopping' and 'graduation' criteria designed to efficiently discard ineffective therapies and graduate promising agents for definitive efficacy trials. Each investigational agent arm enrols to a maximum of 125 patients per arm and is compared with concurrent controls. As of December 2021, 11 investigational agent arms had been activated, and 8 arms were complete. Enrolment and adaptation of the trial design are ongoing.Ethics and disseminationISPY COVID-19 operates under a central institutional review board via Wake Forest School of Medicine IRB00066805. Data generated from this trial will be reported in peer-reviewed medical journals.Trial registration numberNCT04488081.

Abstract

We are in the midst of a technological revolution that is providing new insights into human biology and cancer. In this era of big data, we are amassing large amounts of information that is transforming how we approach cancer treatment and prevention. Enactment of the Cancer Moonshot within the 21st Century Cures Act in the USA arrived at a propitious moment in the advancement of knowledge, providing nearly US$2 billion of funding for cancer research and precision medicine. In 2016, the Blue Ribbon Panel (BRP) set out a roadmap of recommendations designed to exploit new advances in cancer diagnosis, prevention, and treatment. Those recommendations provided a high-level view of how to accelerate the conversion of new scientific discoveries into effective treatments and prevention for cancer. The US National Cancer Institute is already implementing some of those recommendations. As experts in the priority areas identified by the BRP, we bolster those recommendations to implement this important scientific roadmap. In this Commission, we examine the BRP recommendations in greater detail and expand the discussion to include additional priority areas, including surgical oncology, radiation oncology, imaging, health systems and health disparities, regulation and financing, population science, and oncopolicy. We prioritise areas of research in the USA that we believe would accelerate efforts to benefit patients with cancer. Finally, we hope the recommendations in this report will facilitate new international collaborations to further enhance global efforts in cancer control.

31. Local perceptions of insecurity and reassurance strategies : a socio-spatial perspective

Abstract

This thesis develops an understanding of the social factors and spatial dynamics informing public perceptions of (in)security and how diverse social groups interpret endeavours by public authorities to provide reassurance and manage crime and disorder in two areas of Leeds. It incorporates insights from urban geography, geo-spatial analysis and contemporary social theory, combining social research methods to capture the nuanced and lived experiences informing perceptions of (in)security in public space. The empirical findings are based on 22 focus group interviews with 121 residents of various ethnic and age groupings, leading to the development of local perceptual maps, and ten one-to-one interviews with long-standing residents. These data are embedded within detailed area profiles, including insights from interviews with 16 professionals involved in the delivery of reassurance. The findings suggest that perceptions of insecurity are not constant, universal or widespread but have particular temporal, social and spatial dimensions, principally influenced by a lack of mutual trust, social tensions and negative area reputations. The thesis highlights the contested nature of neighbourhood safety, whereby processes of diversity, social identity and 'ownership' shape perceptions of social interactions in particular locations as unpredictable and potentially volatile (e.g. parks and housing estates). The physical character and appearance of public spaces emerge as secondary factors in residents' assessments of their personal safety. The findings indicate the importance of social processes informing insecurity and, especially, the issue of social trust, for professionals involved in the delivery of reassurance. The thesis raises concerns about the widespread assumptions underpinning certain visible public reassurance strategies; specifically, their paradoxical capacity to offer reassurance and simultaneously undermine a community's collective sense of well-being, whilst potentially engendering neighbourhood stigma. It therefore identifies challenges for formal agencies seeking to reassure diverse publics. More generally, it maps the contours and implications of a socio-spatial perspective for the study of crime and insecurity.

33. The role of HLA-DP in renal transplantation

Abstract

Introduction: HLA-DP (DP) is a human Major Histocompatibility Complex class II molecule which presents peptide to the immune system. In the context of kidney transplantation, DP mismatches (mm) can be recognised as allogeneic, demonstrated by the detection of DP specific antibodies (abs). IFN-y is produced during the inflammatory response following kidney transplantation and induces cell surface expression of HLA-c1ass II molecules, providing non-self target molecules for the binding of abs and subsequent graft damage. This study aimed to address the hypotheses that DP mm affect kidney transplant (tx) survival, DP abs are produced. against mismatched donor antigens and are clinically relevant, DP expression can be upregulated in cell types involved in tx rejection and binding of abs to class II molecules leads to the activation of cell signalling pathways which contribute to the mechanism of chronic rejection (CR). Methods: 147 recipients of txs with zero mms at HLA-A, B and DR (000 txs) were DPB1 typed and 127 were DPA1 typed. The effect of mismatching at both loci was investigated using Kaplan-Meier survival analysis and multivariate Cox proportional hazards analysis. 123 of the recipients were tested for class II and DP abs using FlowPRA® assays (One Lambda) and their clinical significance analysed. HLA-DPA1 and B1 gene expression was investigated in IFN-y treated Human Microvascular Endothelial Cells (HMEC) and HK-2 cells (a renal epithelial cell line) using quantitative real-time reverse transcriptase PCR (q r-t RT-PCR). The effect of IFN-y treatment on HMEC DP cell surface expression was determined using flow cytometry. IFN-y-treated HMEC were investigated for cell activation in response to HLA ab binding using a plate fluorimetry method to detect changes in intracellular calcium levels ([Ca2+]i). HMEC protein expression in response to DP ab binding was investigated using an antibody array (Sigma). Results: DPA1 mm had no effect on graft survival whereas an effect of DPB1 mm was demonstrated in retransplant patients using Cox regression analysis (p = 0.04). DP abs were detected in 11 of 40 (27.5%) patients with class II abs. DP abs were frequently found in patients who had had more than one transplant (7/11) and in pre-ODD tx samples (10/11). Six of the 11 recipients had no DP mms at their 000 tx. DP abs were produced in response to previous transplant or pregnancy. The DP response was restimulated by repeat DP mm and in one case was implicated in tx rejection and in another with tx rejection and graft loss. DP gene and cell surface expression increased in response to IFN-y treatment. Cell activation measured by [Ca2+]i increase was observed after class I and class II ab binding. DP ab binding to HMEC led to increased expression of a variety of proteins inclUding those involved in apoptosis, cell cycle and signal transduction. Summary and Conclusion: This study has shown that DP does have a role in renal transplantation. DPB1 mms affect survival of re-transplants. DP abs are produced and reactivated in response to repeat DP mm and were implicated in graft loss. DP gene and cell surface expression was induced on epithelial and endothelial cell lines and DP ab binding led to cell activation. DP ligation on HMEC also led to increased expression of proteins previously shown to be involved in class I ab-mediated cell signalling pathways in endothelial cells. The results of this study suggest that all kidney tx candidates who have had a preVious transplant or pregnancy should be tested for DP abs in order to inform donor selection and patient management.

Abstract

The International Cancer Genome Consortium (ICGC) was launched to coordinate large-scale cancer genome studies in tumours from 50 different cancer types and/or subtypes that are of clinical and societal importance across the globe. Systematic studies of more than 25,000 cancer genomes at the genomic, epigenomic and transcriptomic levels will reveal the repertoire of oncogenic mutations, uncover traces of the mutagenic influences, define clinically relevant subtypes for prognosis and therapeutic management, and enable the development of new cancer therapies.
