Methods Sixty-six children aged > 5-16 years presented with NE were enrolled in the study. Children with urinary tract anatomical abnormalities or infection, intellectual disabilities, genetic syndromes, psychological issues, and diabetes mellitus were excluded. They were clinically examined, scored using sleep clinical record score (SCR), and subjected for full-night polysomnogram (PSG). Children with obstructive apnea/hypopnea index (AHI) [greater than or equal to] 2 episodes/hour (h) were considered as OSA. Results Fifty-four children (81.8% of the recruited children) aged 8.3 ± 2.8 years agreed to undergo PSG as 68.5% had OSA with median obstructive AHI of 6.1 (3.7-13.2) episodes/h, median oxygen saturation of 97% and nadir of 88%. Thirty-three percent were obese with significantly higher AHI [7.0 (3.7-12.4) vs. 2.4 (1.3-6.1) episodes/h; p = 0.023]. SCR score correlated significantly with AHI (r.sup.2 = 0.462, p = 0.001) with 91% sensitivity in detecting OSA [greater than or equal to] 5 episodes/h. Nasal obstruction, adenoid/adult facial phenotype, and arched palate were associated with OSA (p < 0.05). Conclusion NE is commonly associated with OSA especially in obese children. Nasal obstruction, abnormal facial phenotype, and high-arched palate were common risk factors.

Abstract

Fundarced es una fundación creada en el 2017 como una entidad sin ánimo de lucro que busca la formación en deportes, arte, cultura y educación. Tiene como objetivo favorecer la educación integral para niños, niñas, jóvenes, adultos y adultos mayores de la localidad de USME a partir de la recreación, con el fin de mitigar el riesgo psicosocial en el que puede estar inmersa dicha población y presenta a sus participantes diferentes herramientas para el buen manejo del tiempo libre, favoreciendo en la construcción de distintos proyectos innovadores para ser propuestos frente a la plataforma SECOP II y así, poder licitar con ellos y de esta forma brindarle a la fundación un aporte significativo, generando un impacto positivo a nivel distrital. Esta entidad sin ánimo de lucro pretende organizar su esquema financiero, pues la realidad actual es compleja para sus fundadores, se propone en primera instancia una dinámica interesante y organizada llamada “Plan padrinos” donde personas naturales apasionadas de los procesos de formación integral enmarcados en el deporte, la cultura, la recreación, la música , la danza y el cuidado ambiental realizan un aporte mensual que corresponde a la inscripción de un niño, niña o adolescente en los diferentes programas. Dicha inscripción permitirá que haya un respaldo económico para entrenadores, administrativos y voluntarios, También servirá como inversión para compra de materiales, pago de alquileres y gastos varios que aparecen normalmente al trabajar con determinado target group. Rhinitis is an extremely common clinical condition in children [...], Objective. Although rhinitis is extremely frequent in children, methods for assessing the severity of nasal inflammation produce results with wide variability and hence weak clinical significance. We designed this epidemiologic investigation to define the clinical usefulness of assessing nasal cellularity in children. Methods. We studied 183 of 203 eligible unselected schoolchildren who were aged 9 to 11 years and whose parents gave informed consent and completed a questionnaire on the history of atopic and respiratory symptoms. In all children, nasal swabs were obtained from both nostrils and eluted in saline and slides were prepared from cytospin preparations for staining and white cell counts. Children also underwent determination of nasal volume, skin prick tests with 7 common local allergens, flow volume curves, and nitric oxide measurement in expired air. Blood samples were drawn for the measurement of total immunoglobulin E, eosinophil percentage, and detection of Chlamydia pneumoniae antibodies. C pneumoniae DNA was also sought in eluates from nasal swabs. The percentage, standard deviations, and percentiles of the various nasal white cell populations were determined. Results. No correlation of the percentage of these cells was found with the history of allergies or respiratory disease or with functional or laboratory finding. Repeat nasal swabs obtained 1 month after the initial examination in 31 children (20 with neutrophils higher and 11 lower than 14%) in 77.4% of the cases confirmed the previous (high or normal) result. Twelve of the 16 eligible children with persistently high nasal neutrophil counts completed a 15-day cycle of intranasal flunisolide therapy (200 µg twice a day). Therapy significantly reduced nasal neutrophil percentage and increased nasal volume. Conclusions. Increased nasal neutrophils, although related neither to the clinical history nor to laboratory variables, are a common important finding in children. Abstract

Finger-sucking in early childhood can induce morphologic changes in the oropharynx and upper airways, which could arise even in children without reported oral breathing. The aim of this study was to compare cephalometric findings in children with and without finger sucking habit with respect to oral breathing.Fifty-six children aged 4 to 12 years (28 with diagnosis of finger sucking and 28 controls) treated at the Port Colombia Dental Clinic underwent cephalometric radiographs, from which morphologic (n=11, whole skull), upper airways (n=10), hyoid bone (n=3) and postural (n=5) measurements were performed. The unpaired t-test was used for comparison between groups.Both groups had similar age and gender distribution (7.9 yrs ± 2.9; M/F: 14/14). Patients with finger-sucking habit had a higher hard palate length from the anterior nasal spine to the posterior nasal spine (finger-sucking subjects: 50.18 mm; controls: 46.91 mm; p = 0.0001) and distance from the epiglottic vallecula to the posterior pharyngeal wall (finger-sucking subjects: 15.55 mm; controls: 13.36 mm; p = 0.0231) than control subjects. Patients with finger-sucking habits also had a shorter distance from the posterior nasal spine to the adenoids (14.91 mm vs. 17.82 mm; p = 0.0173), wider cranial-cervical angles (105.64 mm vs. 101.6 mm; p = 0.05) and lower hyoid bone positioning (14.55 mm vs. 11.82 mm; p = 0.0125) than controls.Finger-sucking habit is associated with characteristic cephalometric changes even in children without oral breathing, especially at the hyoid bone and postural measurements.

41. Asthma Control Test and Bronchial Challenge with Exercise in Pediatric Asthma

Abstract

Background: Poor asthma control can lead to exercise-induced bronchoconstriction (EIB), but the relationship between subjective disease control and EIB is unclear. No studies have compared asthma control test (ACT) scores of children with those of their parents regarding EIB. We assessed whether ACT scores predict the occurrence of EIB in two age groups. We also evaluated ACT scores and objective measures as explanatory variables for airway response to exercise. Methods: Patients (71 aged <12 years, 93 aged ≥12 years) and their parents completed an ACT questionnaire separately. Current therapy, skin prick testing and spirometry at baseline and after exercise were assessed. EIB was defined as a fall in FEV1 of at least 12% from baseline. Sensitivity and specificity for cut-off values of ACT scores predictive of EIB were plotted, and the Area Under Curve (AUC) was described. Results: Atopy and current therapy were similarly frequent. EIB was observed in 23.9% of children aged <12 years and in 33.3% aged ≥12 years. EIB occurrence in subjects previously scored as having full control (25), partial control (20‐24) and no control (<20) varied according to the age group and responder. Percentages of EIB cases increased as ACT scores decreased in children aged ≥12 years alone (child ACT scores, 25: 21.9%, 20-24: 31.1%, <20: 62.5%, p=0.017). Plots for ACT scores as predictors of EIB yielded low non‐significant AUC values in children aged <12 years; by contrast, moderate AUC values emerged in children aged ≥12 years (child: 0.67, p=0.007; parent: 0.69, p=0.002). Sensitivity of ACT scores below 20 as a predictor of EIB was low in older children (child: 32.3%, parent: 22.6%), whereas specificity was high (child: 90.3%, parent: 93.5%). Multiple regression analysis with percent fall in FEV1 as dependent variable included FEV1/FVC%, ACT child score and gender in the prediction model ( r=0.42, p=0.000).
