Abstract

The prevalence of malnutrition are 23-76% of ESRD patients undergoing HD, and 20-50% of them suffers inflammation. Nowadays, the “malnutrition-inflammation” duo is frequently found in HD patients.The aim of this Spanish multicenter study is to evaluate the effects of the Mid-Dilution HDF on the inflammatory-nutritional state, on some body composition and on the quality of life in the HD patients. The total number of patients expected 64; the preliminary analysis of 52 patient/3 months and 23 after 6months (the study will last 1 year), is presented. Methods: Patients undergoing standard HD treatment with High Flux dialyzers for 4 hours/three times a week passed to HDF Online MidDilution with OLPUR 220 filters at a reinfusion rate of 12l/h.The patients are classified by: age, gender, Charlson comorbidity index, dialysis vintage. The parameters analyzed each 3 months are: urea, β2microglobuline, albumin, pre-albumin, CRP, fibrinogen, IL6, IL10, leptin, adiponectin, neuropeptide Y, body composition by BIVA parameters and apetite and quality life surveys. RESULTS: Patients classification: • Age: 64.06±0.8 years; • Sex: 64% male; • Charlson-index: 3.97±1.66; • HD vintage 54.7±44.8 months.Significant decrease of β2microglobuline pre-dialysis from baseline 26,16 to 20,06 mg/L (p= 0,006) after 3 months and to 17,88smg/L (p= 0.09) after 6 months. xβ2microglobulineRR was 82.45 ±3.20 % and the URR was 79.56±3.51 %, which demonstrates a good removal of medium and small molecules. The Kt/V remained stable (>1,5). Albumin increased from 3.81 g/dL to 3.87 g/dL in e months and to 3.89 g/dL in the 6 months of evaluation. No significant differences in levels of pre-albumin (xbaseline 28 mg/dl). Corporal and BIVA parameters evolution These data shown an improvement of the body composition and water distribution. .Sig. improvements were seen in the appetite scale in the first 6 months (p= 0.09). The total Quality of Life, evaluated in 3 months by SF36, increased from 55,61 to 50,66 (p= 0,05); the physic from 50,28 to 55,92 (p= 0,036); the mental from 55,69 to 60,1 (p= 0,12).Cytokines:we found an increase in neuropeptide Y and IL10 and no significant changes in leptin and adiponectin with slight increase of IL6. CONCLUSIONS: 1-The preliminary results show that MidDilution provides a good removal of small and middle molecules, increases appetite by providing a proper balance of cytokines through stimulation of antiinflamatory ones and neuropeptide Y. 2-It provides an improvement of body composition. Finally MidDilution improves nutritional parameters which leads to a better quality of life, as well as physical and mental status.

Abstract

The effect of high-flux hemodialysis membranes on patient survival has not been unequivocally determined. In this prospective, randomized clinical trial, we enrolled 738 incident hemodialysis patients, stratified them by serum albumin < or = 4 and >4 g/dl, and assigned them to either low-flux or high-flux membranes. We followed patients for 3 to 7.5 yr. Kaplan-Meier survival analysis showed no significant difference between high-flux and low-flux membranes, and a Cox proportional hazards model concurred. Patients with serum albumin < or = 4 g/dl had significantly higher survival rates in the high-flux group compared with the low-flux group (P = 0.032). In addition, a secondary analysis revealed that high-flux membranes may significantly improve survival of patients with diabetes. Among those with serum albumin < or = 4 g/dl, slightly different effects among patients with and without diabetes suggested a potential interaction between diabetes status and low serum albumin in the reduction of risk conferred by high-flux membranes. In summary, we did not detect a significant survival benefit with either high-flux or low-flux membranes in the population overall, but the use of high-flux membranes conferred a significant survival benefit among patients with serum albumin < or = 4 g/dl. The apparent survival benefit among patients who have diabetes and are treated with high-flux membranes requires confirmation given the post hoc nature of our analysis.

34. Renal Tubular Transporter-Mediated Interactions of HIV Drugs: Implications for Patient Management

Abstract

Interactions of drugs with renal transporters can reduce the tubular secretion of endogenous products and affect drug pharmacokinetics, efficacy, and toxicity. This review aims to understand the clinical implications of renal transporter-mediated interactions of HIV drugs. These interactions have been fully investigated for nucleoside/nucleotide reverse transcriptase inhibitors, particularly tenofovir disoproxil fumarate, and for some of the newer agents, such as rilpivirine, dolutegravir, and cobicistat. Interactions may include competition, inhibition, or induction of transporters, and interference with renal active secretion of creatinine, the most commonly used marker of renal function. Drug-drug interactions may result in an increased risk of drug toxicity. This interaction is more likely to occur with the protease inhibitors, particularly ritonavir, due to the inhibitory effects of these drugs on specific transporters involved in renal excretion of other drugs. Interactions with the transport of creatinine have been identified with rilpivirine, dolutegravir, and cobicistat. While rilpivirine and dolutegravir inhibit mainly the renal transporter OCT2 in the basolateral membrane of the proximal tubular cell, cobicistat predominantly inhibits the renal transporter MATE1 in the luminal membrane. These interactions can cause mild-to-moderate increases in serum creatinine concentrations and moderate reductions in estimated glomerular filtration rate that do not translate into real decreases in glomerular filtration. To use these drugs safely, clinicians must correctly interpret changes upon initiation of therapy to differentiate these spurious elevations in serum creatinine from clinically significant toxicity. In this article we propose a set of recommendations for clinical use of antiretroviral drugs that interfere with creatinine renal transporters.

The aim of this article is to update the 2010 recommendations on the evaluation and management of renal disease in human immunodeficiency virus (HIV)-infected patients. Renal function should be monitored in all HIV-infected patients. The basic renal work-up should include measurements of serum creatinine, estimated glomerular filtration rate by CKD-EPI, urine protein-to-creatinine ratio, and urinary sediment. Tubular function tests should include determination of serum phosphate levels and urine dipstick for glycosuria. In the absence of abnormal values, renal screening should be performed annually. In patients treated with tenofovir, or with risk factors for chronic kidney disease (CKD), more frequent renal screening is recommended. In order to prevent disease progression, potentially nephrotoxic antiretroviral drugs are not recommended in patients with CKD or risk factors for CKD. The document provides indications for renal biopsy and advises on the optimal time for referral of a patient to the nephrologist. The indications for and evaluation and management of dialysis and renal transplantation are also addressed.

Abstract

Background: Geographical differences in disease prevalence and mortality have been described in the general population and in chronic kidney disease patients in Europe. In this secondary analysis of the Membrane Permeability Outcome (MPO) study, we addressed differences in patient and treatment patterns, and whether these affect patient outcomes. Methods: Participating countries were grouped according to geographical location; thus study centers in France, Greece, Italy, Portugal and Spain were allocated to southern Europe (n=499), and those in all other countries (Belgium, Germany, Poland and Sweden) to northern Europe (n=148). Descriptive analysis of patient and treatment patterns at study start, as well as survival analysis, was performed. Abstract

Ante la alta prevalencia de la situación de malnutrición en pacientes en hemodiálisis y conociendo que ello implica un aumento de la tasa de infecciones, hospitalización y estancia lo que se traduce en un incremento de la morbimortalidad global, la Sociedad Española de Nefrología (SEN) y la Sociedad Española de Nutrición Parenteral y Enteral (SENPE) realizan un consenso sobre las indicaciones, contraindicaciones y límites de la Nutrición Parenteral Intra Diálisis (NPID). En este consenso se considera a la NPID como una alternativa válida a otros tipos de soporte nutricional cuando estos demuestran su ineficacia. Se sientan las bases del momento de intervención nutricional con NPID, su composición ideal, el tiempo de administración, los controles, las pautas de seguimiento y el momento en que este soporte nutricional debe de ser discontinuado. Given the high prevalence of the hyponutrition state among haemodialysis patients and knowing that this implies an increase in the rates of infection, hospitalisation and hospital stay, which translates into an increase in global morbid-mortality, the Spanish Society of Nephrology (SEN) and the Spanish Society of Parenteral and Enteral Nutrition (SENPE) have reached a consensus on the indications, contraindications, and limitations of Intra-Dialysis Parenteral Nutrition (IDPN.) This consensus considers IDPN as a valid alternative to other types of nutritional support when these show their lack of efficacy. The bases are set regarding the timing of nutritional intervention with IDPN, its ideal composition, the time of administration, its controls, follow-up schedules, and the time at which the nutritional support has to be discontinued.

Abstract

Renal dysfunction in the setting of HIV-1 infection and antiretroviral therapy is an increasingly more common event that has a significant impact on the clinical management of HIV-1-infected patients. The present review examines the epidemiology of renal dysfunction in HIV-1-infected patients, the spectrum of renal injury associated with HIV-1 infection and its treatment, and the various forms of presentation seen in daily clinical practice. As a practical contribution to the management of HIV-1-associated renal dysfunction, guidelines are proposed for assessing the most common kidney-related problems and their management in daily practice. Abstract

Sera samples from 113 llamas (Lama glama) and 127 alpacas (Lama pacos) from the IX and V Regions, respectively, of Chile were tested forToxoplasma gondiiantibodies. The modified agglutination test (MAT) was used in both species and titers 1:25 were considered diagnostically significant based in previously published data. Sera from 49 lamas (43.3%) and 15 alpacas (11.8%) were positive toT. gondii. Percentaje seropositivity in serum dilutions of 1:25, 1:50, 1:500 and 1:5000 was 17.6%; 7.9%; 14.1% and 3.5% in lamas and 0%; 2.3%; 0.7% and 8.6% in alpacas, respectively. The rather low prevalence in alpacas may be associated with geographical conditions, management practices or contacts with cats rather than different species susceptibility. As expected, older animals showed higher reactivity ofT. gondiithan young animals. &nbsp, Se obtuvieron 113 sueros de llamas y 127 de alpacas de criaderos ubicados en la IX y V Regiones del país respectivamente para determinar la presencia de infección deT. gondii. Para el análisis de los sueros se utilizó el “test” modificado de aglutinación (MAT) a diluciones 1:25; 1:50, 1:500; 1:5000. Del total de 113 sueros de llamas estudiados 49 (43,3%) de ellas presentaron anticuerpos contraT. gondii, mientras que en las alpacas 15 de las 127 (11,8%) fueron positivas. El número de animales positivos en las diluciones 1/25; 1/50; 1/500; 1/5000 fue de 20 (17,6%); 9 (7,9%); 16 (10,6%); 4 (3,5%) en las llamas, respectivamente. En las alpacas los valores fueron de 0 (0%); 3 (2,3%); 1 (0,7%); 11 (8,6%), respectivamente. Aunque no existen antecedentes clínicos de la infección en los establecimientos estudiados, su potencial implicancia en este tipo de patología debería ser considerada a base de los resultados del presente trabajo que demuestra que llamas y alpacas del centro-sur de Chile son reaccionantes a la infección porT. gondii. &nbsp
