Background Skin appendageal tumors (SATs) comprise a wide and diverse variety of neoplasms that exhibit histogenetic differentiation towards one or more of the cutaneous adnexal structures. Objective The goal of this research was to investigate the incidence, clinical and histopathological profile of patients with SATs registered at a single pathology institute over a 3-year period. Methods This cross-sectional descriptive study was realized at the Martin Biopsy Center from August 2019 to July 2022. A retrospective analysis of all consecutive skin tumors histologically diagnosed during the study period was performed. The SATs have been extracted from that file and analyzed in details. Results A total of 80 cases of SATs have been identified in a set of over 10,000 various skin tumors. They were obtained from 80 individuals (40 males, 40 females) in the age range of 10 to 89 years (mean 54.1 yrs.). The most common site affected was the head and neck (55/80; 68.75%), followed by limbs (15/80; 18.75%) and trunk (10/80; 12.5%). The majority (60%; 48/80) of SATs were of hair follicle origin, followed by the tumors derived from sweat glands (37.5%; 30/80). The sebaceous tumors represented only two cases (2.5%). All but one case comprised benign lesions. The most common types were trichoepithelioma, pilomatrixoma, poroma and hidrocystoma. The only case of malignancy (1.2%) was hidradenocarcinoma. Conclusion SATs represent less than 1% of all cutaneous neoplasms. Compared to global literature data we have observed a higher age of patients, a very low incidence of sebaceous tumors, and a minimum proportion of malignant lesions. The discrepancies regarding the clinicopathologic features of SATs found in many published studies are partly due to inconsistent approach to classification of this peculiar tumor group. Rhinophyma is a rare skin disease characterized by a disfiguring enlargement of the nose due to proliferation of sebaceous glands and surrounding soft tissue. It is generally considered to be a final stage of untreated rosacea. This report describes a 79-year-old man manifested with a giant rhinophyma measuring 7cm in diameter. It grew for many years but the patient had not sought any dermatologic intervention. The lesion was completely removed by using sharp scalpel incision. Although rhinophyma is not a life-threatening condition, it can sometimes reach a huge size, with a negative effect on quality of life. Such cases are usually a consequence of patient's delay in seeking medical advice. In more pronounced lesions, a simple surgical removal under tumescent anestesia is an optimal treatment modality. Spindle cell/pleomorphic lipoma (SCL/PL) is a rare histologic subtype of lipoma. It occurs in a characteristic clinical settings, arising mainly in middle-aged and older men in subcutaneous tissue of the posterior neck, upper back, and shoulders (shawl region). In women, it much more comonly develops outside the typical shawl location. The author describes a 66-year-old man with a subcutaneous tumor mass in the left back side of the neck. It consisted of bland spindle cell population situated in a fibromyxoid stroma containing thick collagen bundles. Scattered pleomorphic and multinucleated floret-like giant cells were also found. The neoplastic cells showed CD34-positive/S100-negative immunophenotype. A diagnosis of a SCL/PL was established. Subcutaneous SCL/PL is an unfamiliar tumor entity to most dermatologists. Despite the worrisome name it is a clearly benign lesion with no tendency to recur after surgical treatment. Background Cytokeratin (CK) gene expression in the skin reflects the type of epithelium, the differentiation state of epithelial cells and is subjected to striking modulation upon various challenges including carcinogenesis. Objective The aim of the study was to evaluate an immunohistochemical expression status of CK17, CK19 and epithelial cell adhesion molecule EpCAM (BerEP4) in basal cell carcinoma (BCC) and adjacent epithelial structures of the skin. Material and Methods Biopsy samples from 20 cases of BCC were investigated. All were stained for CK17 (clone E3), CK19 (clone RCK108) and EpCAM (clone BerEP4) by immunohistochemistry. Results In normal skin, all three markers were constantly positive in the hair follicle components. They were typically negative in the interfollicular epidermis, but the epidermis overlying the tumor and at the edges of tumor mass expressed CK17 in the vast majority of cases. In BCC tissue, the CK17, CK19 and BerEP4 were positive in 100% (20/20), 40% (8/20), and 100% (20/20) of the cases. Conclusion BCC constantly express CK17 and BerEP4 and is variably positive for CK19. Since all three markers are strongly positive in the hair follicle but not in the interfollicular epidermis, the staining pattern of CK17, CK19 and BerEP4 in BCC may provide an evidence to the suggestion that the tumor differentiation is toward follicular germinative epithelium. A biopsy diagnosis of conventional (common) type of benign fibrous histiocytoma (BFH) is usually easy. However, there are many other histopathologic variants, that may cause difficulties to make an accurate diagnosis. The author describes a 53-year old women, who presented with a protuberated ulcerated skin tumor arising on the left shoulder. It consisted of a highly cellular mass of proliferating spindle-shaped cells with a fascicular growth pattern, that was confined to dermis. The cells were relatively uniform with a slightly increased mitotic rate and proliferation activity. Immunohistochemically, the tumor expressed vimentin, Factor XIIIa and alpha-SMA, while it was negative for CK5/6, CK7, BerEP4, p63, EMA, S-100, melan A, HMB-45, MSA, h-caldesmon and CD34. A diagnosis of ulcerated cellular BFH was made. Although BFH is a very frequent cutaneous tumor, its less common cellular variant may result in diagnostic pitfalls. The purpose of this paper was to point out that even such „banal" and prognostically favourable cutaneous lesion sometimes requires a complex differential diagnostic approach in biopsy practice. Background: Several registries of idiopathic pulmonary fibrosis (IPF) have been established to better understand its natural history, though their size and duration of follow-up are limited. Here, we describe the large European MultiPartner IPF Registry (EMPIRE) and validate predictors of long-term survival in IPF.Methods: The multinational prospective EMPIRE registry enrolled IPF patients from 48 sites in 10 Central and Eastern European countries since 2014. Survival from IPF diagnosis until death was estimated, accounting for left-truncation. The Cox proportional hazards regression model was used to estimate adjusted hazard ratios (HR) of death for prognostic factors, using restricted cubic splines to fit continuous factors.Results: The cohort included 1620 patients (mean age at diagnosis 67.6 years, 71% male, 63% smoking history), including 75% enrolled within 6 months of diagnosis. Median survival was 4.5 years, with 45% surviving 5 years post-diagnosis. Compared with GAP stage I, mortality was higher with GAP stages II (HR 2.9; 95% CI: 2.3-3.7) and III (HR 4.0; 95% CI: 2.8-5.7) while, with redefined cut-offs, the corresponding HRs were 2.7 (95% CI: 1.8-4.0) and 5.8 (95% CI: 4.0-8.3) respectively. Mortality was higher with concurrent pulmonary hypertension (HR 2.0; 95% CI: 1.5-2.9) and lung cancer (HR 2.6; 95% CI: 1.3-4.9).Conclusions: EMPIRE, one of the largest long-term registries of patients with IPF, provides a more accurate confirmation of prognostic factors and co-morbidities on longer term five-year mortality. It also suggests that some fine-tuning of the indices for mortality may provide a more accurate long-term prognostic profile for these patients. [ABSTRACT FROM AUTHOR]
