Abstract

To detect potential concern about severe acute hepatitis in children, we conducted a survey among 50 ERN RARE-LIVER centres. By 26 April 2022, 34 centres, including 25 transplant centres, reported an estimated median of 3-5, 0-2 and 3-5 cases in 2021, 2020 and 2019 and a mean of 2 (range: 0-8) cases between January and April 2022 (mean in 10 large liver transplant centres: 3). Twelve centres reported suspicion of an increase, but no rise. Following a report by the United Kingdom (UK) on 5 April 2022 on the occurrence of cases of severe acute hepatitis in children aged 16 years or under, the World Health Organization (WHO) raised concerns about the possibility of an epidemic [1,2]. By 21 April, 169 possible or confirmed cases were reported fulfilling the WHO case definition [3]. The cause of the hepatitis is unknown but a link to a virus infection has been suggested due to the epidemiological pattern of cases [4,5]. The hepatitis can progress to paediatric acute liver failure (pALF) necessitating urgent liver transplantation to avoid multi-organ failure [6]. We intended to assess whether a rise in incidence of severe acute hepatitis or pALF could be observed between 1 January and 26 April 2022 in comparison to previous years, within the European Reference Network on Hepatological Diseases (ERN RARE-LIVER) [7].

Abstract

Background & Aims: Bile salt export pump (BSEP) deficiency frequently necessitates liver transplantation in childhood. In contrast to two predicted protein truncating mutations (PPTMs), homozygous p.D482G or p.E297G mutations are associated with relatively mild phenotypes, responsive to surgical interruption of the enterohepatic circulation (siEHC). The phenotype of patients with a compound heterozygous genotype of one p.D482G or p.E297G mutation and one PPTM has remained unclear. We aimed to assess their genotype-phenotype relationship. Methods: From the NAPPED database, we selected patients with homozygous p.D482G or p.E297G mutations (BSEP1/1; n = 31), with one p.D482G or p.E297G, and one PPTM (BSEP1/3; n = 30), and with two PPTMs (BSEP3/3; n = 77). We compared clinical presentation, native liver survival (NLS), and the effect of siEHC on NLS. Results: The groups had a similar median age at presentation (0.7-1.3 years). Overall NLS at age 10 years was 21% in BSEP1/3 vs. 75% in BSEP1/1 and 23% in BSEP3/3 (p <0.001). Without siEHC, NLS in the BSEP1/3 group was similar to that in BSEP3/3, but considerably lower than in BSEP1/1 (at age 10 years: 38%, 30%, and 71%, respectively; p = 0.003). After siEHC, BSEP1/3 and BSEP3/3 were associated with similarly low NLS, while NLS was much higher in BSEP1/1 (10 years after siEHC, 27%, 14%, and 92%, respectively; p <0.001). Conclusions: Individuals with BSEP deficiency with one p.E297G or p.D482G mutation and one PPTM have a similarly severe disease course and low responsiveness to siEHC as those with two PPTMs. This identifies a considerable subgroup of patients who are unlikely to benefit from interruption of the enterohepatic circulation by either surgical or ileal bile acid transporter inhibitor treatment. Abstract

(1) Background: Post-transplant lymphoproliferative disease (PTLD) is a significant complication of solid organ transplantation (SOT). However, there is lack of consensus in PTLD management. Our aim was to establish a present benchmark for comparison between international centers and between various organ transplant systems and modalities; (2) Methods: A cross-sectional questionnaire of relevant PTLD practices in pediatric transplantation was sent to multidisciplinary teams from 17 European center members of ERN TransplantChild to evaluate the centers’ approach strategies for diagnosis and treatment and how current practices impact a cross-sectional series of PTLD cases; (3) Results: A total of 34 SOT programs from 13 European centers participated. The decision to start preemptive treatment and its guidance was based on both EBV viremia monitoring plus additional laboratory methods and clinical assessment (61%). Among treatment modalities the most common initial practice at diagnosis was to reduce the immunosuppression (61%). A total of 126 PTLD cases were reported during the period 2012-2016. According to their histopathological classification, monomorphic lesions were the most frequent (46%). Graft rejection after PTLD remission was 33%. Of the total cases diagnosed with PTLD, 88% survived; (4) Conclusions: There is still no consensus on prevention and treatment of PTLD, which implies the need to generate evidence. This might successively allow the development of clinical guidelines.

Abstract

Wendelstein 7-X (W7-X) is a fully optimized low-shear stellarator and shall demonstrate the reactor potential of this fusion plant. It is presently under construction at the Greifswald Branch Institute of IPP. The superconducting magnet system will allow continuous operation, limited only by the plasma exhaust system whose capacity is designed for 30 min full power operation. The Wendelstein 7-X (W7-X) coils and structures are part of the largest superconducting fusion device being constructed at present. They represent a technical challenge at industrial level and the need for proven techniques and manufacturing processes in accordance to the highest quality standards. The production of these components requires a management of monitoring for quality and tests. The coil system consists of 20 planar and 50 non-planar coils. They are supported by a pentagonal 10 m diameter, 2.5 m high coil support structure (CSS). The CSS is divided into five modules. Each module consists of two equal half modules. The manufacturing status of the CSS and the main project management and technical challenges will be presented. The lessons learned in the large scale production of this difficult kind of support structure will be presented as relevant experience for the realization of similar systems for future fusion devices, such as ITER.

Scopo del presente lavoro, primo del genere per la Sardegna, è quello di mettere a disposizione non solo degli speleologi, ma anche di tutti coloro che intendono o intenderanno occuparsi delle grotte naturali e artificiali della nostra Isola, quanto è stato scritto sull’argomento. Si è trattato di una lunga ricerca, durata oltre sei anni, nel corso dei quali abbiamo selezionato e catalogato opere monografiche, articoli e saggi, consultando libri, bollettini, riviste e quotidiani alla ricerca non solo di lavori d’interesse prettamente speleologico, ma anche archeologico, biologico, naturalistico, geografico, geologico, paleontologico etc. Tutte le pubblicazioni reperite, in cui si descrivono o si citano grotte sarde, sono state catalogate in una banca dati Access, nella quale sono state raccolte informazioni sui tipi d’argomenti trattati (archeologia, geologia, biospeleologia etc.). Questo ha consentito di fare complesse interrogazioni al database con combinazioni d’informazioni geografiche di ciascuna grotta (numero catastale, comune, nome), con gli argomenti trattati negli articoli catalogati. Chiaramente il libro, di prossima pubblicazione, non è certo destinato a un grosso pubblico, ma ad una stretta cerchia di persone che, per passione (speleologi, escursionisti) o per lavoro (archeologi, geologi, biologi, etc.), sono e saranno interessate ad avere un’ampia guida bibliografica a supporto delle loro attività e ricerche.

Abstract

Karyomorphology and relationships between three genera within Orchidinae (Orchidaceae). In the present contribution, karyomorphology, cytological heterochromatin amounts, and chromosomal C-bands distribution, are used as landmarks to study relationships among three genera of the family Orchidaceae, namely, Anacamptis, Ophrys and Serapias. The obtained data suggest a possible pathway of karyotype evolution in which the genus Anacamptis is the one from which both Ophrys and Serapias evolved; the differences in karyomorphology and, possibly, evolutionary rates are discussed in terms of species adaptation.
