(1) Background: Conversational turns between parents and children contribute to the language development of children. This study aimed to examine parental language input during interactions with high numbers of conversational turns (focused interactions) and those with fewer turns (regular interactions) in children with hearing loss at home. (2) Methods: Twelve children (aged 18–47 months) with hearing loss and their parents participated. Each child wore a Language ENvironment Analysis system digital language processor for 2 days to record all conversations between the parent and child. Focused interactions were characterized by high conversational turns, while regular interactions were defined by median conversational turns. The quantity of language input was reflected by the number of words parents used during the interaction, and the quality was reflected by the mean length of parental utterances, the use of low- and high-level facilitative language techniques, lexical diversity, and the use of (de)contextualized talk. (3) Results: During focused interactions, parents exposed their children to more words than during regular interactions, while the opposite was found for lexical diversity. The quality of parental language input did not differ between the two types of interactions. Parental language input was associated with children's spontaneous language. In normal hearing (NH), the perception of the gender of a speaker is strongly affected by two anatomically related vocal characteristics: the fundamental frequency (F0), related to vocal pitch, and the vocal tract length (VTL), related to the height of the speaker. Previous studies on gender categorization in cochlear implant (CI) users found that performance was variable, with few CI users performing at the level of NH listeners. Data collected with recorded speech produced by multiple talkers suggests that CI users might rely more on F0 and less on VTL than NH listeners. However, because VTL cannot be accurately estimated from recordings, it is difficult to know how VTL contributes to gender categorization. In the present study, speech was synthesized to systematically vary F0, VTL, or both. Gender categorization was measured in CI users, as well as in NH participants listening to unprocessed (only synthesized) and vocoded (and synthesized) speech. Perceptual weights for F0 and VTL were derived from the performance data. With unprocessed speech, NH listeners used both cues (normalized perceptual weight: F0 = 3.76, VTL = 5.56). With vocoded speech, NH listeners still made use of both cues but less efficiently (normalized perceptual weight: F0 = 1.68, VTL = 0.63). CI users relied almost exclusively on F0 while VTL perception was profoundly impaired (normalized perceptual weight: F0 = 6.88, VTL = 0.59). As a result, CI users' gender categorization was abnormal compared to NH listeners. Future CI signal processing should aim to improve the transmission of both F0 cues and VTL cues, as a normal gender categorization may benefit speech understanding in competing talker situations. © 2014 Association for Research in Otolaryngology.

Background: It is assumed that speech comprehension deficits in background noise are caused by age-related or acquired hearing loss. Methods: We examined young, middle-aged, and older individuals with and without hearing threshold loss using pure-tone (PT) audiometry, short-pulsed distortion-product otoacoustic emissions (pDPOAEs), auditory brainstem responses (ABRs), auditory steady-state responses (ASSRs), speech comprehension (OLSA), and syllable discrimination in quiet and noise. Results: A noticeable decline of hearing sensitivity in extended high-frequency regions and its influence on low-frequency-induced ABRs was striking. When testing for differences in OLSA thresholds normalized for PT thresholds (PTTs), marked differences in speech comprehension ability exist not only in noise, but also in quiet, and they exist throughout the whole age range investigated. Listeners with poor speech comprehension in quiet exhibited a relatively lower pDPOAE and, thus, cochlear amplifier performance independent of PTT, smaller and delayed ABRs, and lower performance in vowel-phoneme discrimination below phase-locking limits (/o/-/u/). When OLSA was tested in noise, listeners with poor speech comprehension independent of PTT had larger pDPOAEs and, thus, cochlear amplifier performance, larger ASSR amplitudes, and higher uncomfortable loudness levels, all linked with lower performance of vowel-phoneme discrimination above the phase-locking limit (/i/-/y/). Conslusions: This study indicates that listening in noise in humans has a sizable disadvantage in envelope coding when basilar-membrane compression is compromised. Clearly, and in contrast to previous assumptions, both good and poor speech comprehension can exist independently of differences in PTTs and age, a phenomenon that urgently requires improved techniques to diagnose sound processing at stimulus onset in the clinical routine. BackgroundAmplitude modulation (AM) detection is a measure of temporal processing that has been correlated with cochlear implant (CI) users' speech understanding. For CI users, AM stimuli have been shown to be louder than steady-state (non-AM) stimuli presented at the same reference current level, suggesting that unwanted loudness cues might contribute to CI users' AM sensitivity as measured in a modulation detection task. In this paper, a new method is introduced to dynamically control unwanted AM loudness cues when adaptively measuring modulation detection thresholds (MDTs) in CI users.MethodsMDTs were adaptively measured in 9 CI subjects using a three-alternative, forced-choice procedure, with and without dynamic control of unwanted AM loudness cues. To control for AM loudness cues during the MDT task, the level of the steady-state (non-AM) stimuli was increased to match the loudness of the AM stimulus using a non-linear amplitude scaling function, which was obtained by first loudness-balancing non-AM stimuli to AM stimuli at various modulation depths. To further protect against unwanted loudness cues, ±0.75dB of level roving was also applied to all stimuli during the MDT task.ResultsAbsolute MDTs were generally poorer when unwanted AM loudness cues were controlled. However, the effects of modulation frequency and presentation level on modulation sensitivity were fundamentally unchanged by the availability of AM loudness cues.ConclusionsThe data suggest that the present method controlling for unwanted AM loudness cues might better represent CI users' MDTs, without changing fundamental effects of modulation frequency and presentation level on CI users' modulation sensitivity.

Single-channel modulation detection thresholds (MDTs) have been shown to predict cochlear implant (CI) users' speech performance. However, little is known about multi-channel modulation sensitivity. Two factors likely contribute to multichannel modulation sensitivity: multichannel loudness summation and the across-site variance in single-channel MDTs. In this study, single- and multi-channel MDTs were measured in 9 CI users at relatively low and high presentation levels and modulation frequencies. Single-channel MDTs were measured at widely spaced electrode locations, and these same channels were used for the multichannel stimuli. Multichannel MDTs were measured twice, with and without adjustment for multichannel loudness summation (i.e., at the same loudness as for the single-channel MDTs or louder). Results showed that the effect of presentation level and modulation frequency were similar for single- and multi-channel MDTs. Multichannel MDTs were significantly poorer than single-channel MDTs when the current levels of the multichannel stimuli were reduced to match the loudness of the single-channel stimuli. This suggests that, at equal loudness, single-channel measures may over-estimate CI users' multichannel modulation sensitivity. At equal loudness, there was no significant correlation between the amount of multichannel loudness summation and the deficit in multichannel MDTs, relative to the average single-channel MDT. With no loudness compensation, multichannel MDTs were significantly better than the best single-channel MDT. The across-site variance in single-channel MDTs varied substantially across subjects. However, the across-site variance was not correlated with the multichannel advantage over the best single channel. This suggests that CI listeners combined envelope information across channels instead of attending to the best channel.

Introduction: Underlying mechanisms of speech perception masked by background speakers, a common daily listening condition, are often investigated using various and lengthy psychophysical tests. The presence of a social agent, such as an interactive humanoid NAO robot, may help maintain engagement and attention. However, such robots potentially have limited sound quality or processing speed. Methods: As a first step toward the use of NAO in psychophysical testing of speech-in-speech perception, we compared normal-hearing young adults' performance when using the standard computer interface to that when using a NAO robot to introduce the test and present all corresponding stimuli. Target sentences were presented with colour and number keywords in the presence of competing masker speech at varying target-to-masker ratios. Sentences were produced by the same speaker, but voice differences between the target and masker were introduced using speech synthesis methods. To assess test performance, speech intelligibility and data collection duration were compared between the computer and NAO setups. Human-robot interaction was assessed using the Negative Attitude Toward Robot Scale (NARS) and quantification of behavioural cues (backchannels). Results: Speech intelligibility results showed functional similarity between the computer and NAO setups. Data collection durations were longer when using NAO. NARS results showed participants had a relatively positive attitude toward "situations of interactions" with robots prior to the experiment, but otherwise showed neutral attitudes toward the "social influence" of and "emotions in interaction" with robots. The presence of more positive backchannels when using NAO suggest higher engagement with the robot in comparison to the computer. Discussion: Overall, the study presents the potential of the NAO for presenting speech materials and collecting psychophysical measurements for speech-inspeech perception. [ABSTRACT FROM AUTHOR]

The just-noticeable differences (JNDs) of the voice cues of voice pitch (F0) and vocal-tract length (VTL) were measured in school-aged children with bilateral hearing aids and children and adults with normal hearing. The JNDs were larger for hearing-aided than normal-hearing children up to the age of 12 for F0 and into adulthood for all ages for VTL. Age was a significant factor for both groups for F0 JNDs, but only for the hearing-aided group for VTL JNDs. Age of maturation was later for F0 than VTL. Individual JNDs of the two groups largely overlapped for F0, but little for VTL. Hearing thresholds (unaided or aided, 500–400 Hz, overlapping with mid-range speech frequencies) did not correlate with the JNDs. However, extended low-frequency hearing thresholds (unaided, 125–250 Hz, overlapping with voice F0 ranges) correlated with the F0 JNDs. Hence, age and hearing status differentially interact with F0 and VTL perception, and VTL perception seems challenging for hearing-aided children. On the other hand, even children with profound hearing loss could do the task, indicating a hearing aid benefit for voice perception. Given the significant age effect and that for F0 the hearing-aided children seem to be catching up with age-typical development, voice cue perception may continue developing in hearing-aided children.
