Abstract

BackgroundThe Tenosynovial giant cell tumor Observational Platform Project (TOPP) registry is an international prospective study that -previously described the impact of diffuse-type tenosynovial giant cell tumour (D-TGCT) on patient-reported outcomes (PROs) from a baseline snapshot. This analysis describes the impact of D-TGCT at 2-year follow-up based on treatment strategies.Material and methodsTOPP was conducted at 12 sites (EU: 10; US: 2). Captured PRO measurements assessed at baseline, 1-year, and 2-year follow-ups were Brief Pain Inventory (BPI), Pain Interference, BPI Pain Severity, Worst Pain, EQ-5D-5L, Worst Stiffness, and -Patient-Reported Outcomes Measurement Information System. Treatment interventions were no current/planned treatment (Off-Treatment) and systemic treatment/surgery (On-Treatment).ResultsA total of 176 patients (mean age: 43.5 years) were included in the full analysis set. For patients without active treatment strategy -(Off-Treatment) at baseline (n = 79), BPI Pain Interference (1.00 vs. 2.86) and BPI Pain Severity scores (1.50 vs. 3.00) were numerically favorable in patients remaining Off-Treatment compared with those who switched to an active treatment strategy at year 1. From 1-year to 2-year -follow-ups, patients who remained Off-Treatment had better BPI Pain Interference (0.57 vs. 2.57) and Worst Pain (2.0 vs. 4.5) scores compared with patients who switched to an alternative treatment strategy. In addition, EQ-5D VAS scores (80.0 vs. 65.0) were higher in patients who remained -Off-Treatment between 1-year and 2-year follow-ups compared with patients who changed treatment strategy. For patients receiving systemic treatment at baseline, numerically favorable scores were seen in patients remaining on systemic therapy at 1-year follow-up: BPI Pain Interference (2.79 vs. 5.93), BPI Pain Severity (3.63 vs. 6.38), Worst Pain (4.5 vs. 7.5), and Worst Stiffness (4.0 vs. 7.5). From 1-year to 2-year follow-up, EQ-5D VAS scores (77.5 vs. 65.0) were higher in patients who changed from systemic treatment to a different treatment strategy.ConclusionThese findings highlight the impact D-TGCT has on patient quality of life, and how treatment strategies may be influenced by these outcome measures. Abstract

Background and objectivesDiffuse-tenosynovial giant cell tumor (D-TGCT) is a rare, locally aggressive, typically benign neoplasm affecting mainly large joints, representing a wide clinical spectrum. We provide a picture of the treatment journey of D-TGCT patients as a 2-year observational follow-up.MethodsThe TGCT Observational Platform Project registry was a multinational, multicenter, prospective observational study at tertiary sarcoma centers spanning seven European countries and two US sites. Histologically confirmed D-TGCT patients were categorized as either those who remained on initial treatment strategy (determined at baseline visit) or those who changed treatment strategy with specific changes documented (e.g., systemic treatment to surgery) at the 1-year and/or 2-year follow-up visits.ResultsA total of 176 patients were assessed, mean diagnosis age was 38.4 (SD ± 14.6) years; most patients had a knee tumor (120/176, 68.2%). For the 2-year observation period, most patients (75.5%) remained on the baseline treatment strategy throughout, 54/79 patients (68.4%) remained no treatment, 30/45 patients (66.7%) remained systemic treatment, 39/39 patients (100%) remained surgery. Those who changed treatment strategy utilized multimodal treatment options.ConclusionsThis is the first prospectively collected analysis to describe D-TGCT patient treatments over an extended follow-up and demonstrates the need for multidisciplinary teams to determine an optimal treatment strategy.

Abstract

BackgroundTenosynovial giant cell tumor (TGCT) is a rare, locally aggressive neoplasm arising from the synovium of joints, bursae, and tendon sheaths affecting small and large joints. It represents a wide spectrum ranging from minimally symptomatic to massively debilitating. Most findings to date are mainly from small, retrospective case series, and thus the morbidity and actual impact of this rare disease remain to be elucidated. This study prospectively explores the management of TGCT in tertiary sarcoma centers.MethodsThe TGCT Observational Platform Project registry was a multinational, multicenter, prospective observational study involving 12 tertiary sarcoma centers in 7 European countries, and 2 US sites. This study enrolled for 2 years all consecutive ≥ 18 years old patients, with histologically diagnosed primary or recurrent cases of diffuse-type TGCT. Patient demographic and clinical characteristics were collected at baseline and every 6 months for 24 months. Quality of life questionnaires (PROMIS-PF and EQ-5D) were also administered at the same time-points. Here we report baseline patient characteristics.Results166 patients were enrolled between November 2016 and March 2019. Baseline characteristics were: mean age 44 years (mean age at disease onset: 39 years), 139/166 (83.7%) had prior treatment, 71/166 patients (42.8%) had ≥ 1 recurrence after treatment of their primary tumor, 76/136 (55.9%) visited a medical specialist ≥ 5 times, 66/116 (56.9%) missed work in the 24 months prior to baseline, and 17/166 (11.6%) changed employment status or retired prematurely due to disease burden. Prior treatment consisted of surgery (i.e., arthroscopic, open synovectomy) (128/166; 77.1%) and systemic treatments (52/166; 31.3%) with imatinib (19/52; 36.5%) or pexidartinib (27/52; 51.9%). Treatment strategies at baseline visits consisted mainly of watchful waiting (81/166; 48.8%), surgery (41/166; 24.7%), or targeted systemic therapy (37/166; 22.3%). Patients indicated for treatment reported more impairment compared to patients indicated for watchful waiting: worst stiffness NRS 5.16/3.44, worst pain NRS 6.13/5.03, PROMIS-PF 39.48/43.85, and EQ-5D VAS 66.54/71.85.ConclusionThis study confirms that diffuse-type TGCT can highly impact quality of life. A prospective observational registry in rare disease is feasible and can be a tool to collect curated-population reflective data in orphan diseases. Abstract

Non-Line-of-Sight (NLOS) imaging aims at recovering the 3D geometry of objects that are hidden from the direct line of sight. In the past, this method has suffered from the weak available multibounce signal limiting scene size, capture speed, and reconstruction quality. While algorithms capable of reconstructing scenes at several frames per second have been demonstrated, real-time NLOS video has only been demonstrated for retro-reflective objects where the NLOS signal strength is enhanced by 4 orders of magnitude or more. Furthermore, it has also been noted that the signal-to-noise ratio of reconstructions in NLOS methods drops quickly with distance and past reconstructions, therefore, have been limited to small scenes with depths of few meters. Actual models of noise and resolution in the scene have been simplistic, ignoring many of the complexities of the problem. We show that SPAD (Single-Photon Avalanche Diode) array detectors with a total of just 28 pixels combined with a specifically extended Phasor Field reconstruction algorithm can reconstruct live real-time videos of non-retro-reflective NLOS scenes. We provide an analysis of the Signal-to-Noise-Ratio (SNR) of our reconstructions and show that for our method it is possible to reconstruct the scene such that SNR, motion blur, angular resolution, and depth resolution are all independent of scene size suggesting that reconstruction of very large scenes may be possible. In the future, the light efficiency for NLOS imaging systems can be improved further by adding more pixels to the sensor array.

Abstract

Epilepsy is a common neurological disorder characterized by recurrent seizures accompanied by excessive synchronous brain activity. The process of structural and functional brain alterations leading to increased seizure susceptibility and eventually spontaneous seizures is called epileptogenesis (EPG) and can span months or even years. Detecting and monitoring the progression of EPG could allow for targeted early interventions that could slow down disease progression or even halt its development. Here, we propose an approach for staging EPG using deep neural networks and identify potential electroencephalography (EEG) biomarkers to distinguish different phases of EPG. Specifically, continuous intracranial EEG recordings were collected from a rodent model where epilepsy is induced by electrical perforant pathway stimulation (PPS). A deep neural network (DNN) is trained to distinguish EEG signals from before stimulation (baseline), shortly after the PPS and long after the PPS but before the first spontaneous seizure (FSS). Experimental results show that our proposed method can classify EEG signals from the three phases with an average area under the curve (AUC) of 0.93, 0.89, and 0.86. To the best of our knowledge, this represents the first successful attempt to stage EPG prior to the FSS using DNNs.

Abstract

Epilepsy is one of the most common neurological disorders, affecting about 1% of the population at all ages. Detecting the development of epilepsy, i.e., epileptogenesis (EPG), before any seizures occur could allow for early interventions and potentially more effective treatments. Here, we investigate if modern machine learning (ML) techniques can detect EPG from intra-cranial electroencephalography (EEG) recordings prior to the occurrence of any seizures. For this we use a rodent model of epilepsy where EPG is triggered by electrical stimulation of the brain. We propose a ML framework for EPG identification, which combines a deep convolutional neural network (CNN) with a prediction aggregation method to obtain the final classification decision. Specifically, the neural network is trained to distinguish five second segments of EEG recordings taken from either the pre-stimulation period or the post-stimulation period. Due to the gradual development of epilepsy, there is enormous overlap of the EEG patterns before and after the stimulation. Hence, a prediction aggregation process is introduced, which pools predictions over a longer period. By aggregating predictions over one hour, our approach achieves an area under the curve (AUC) of 0.99 on the EPG detection task. 25. Phasor field waves: A statistical treatment for the case of a partially coherent optical carrier

Abstract

This paper presents a statistical treatment of phasor fields (P-fields) - a wave-like quantity denoting the slow temporal variations in time-averaged irradiance (which was recently introduced to model and describe non-line-of-sight (NLoS) imaging as well as imaging through diffuse or scattering apertures) - and quantifies the magnitude of a spurious signal which emerges due to a partial spatial coherence of the underlying optical carrier. This spurious signal is not described by the Huygens-like P-field imaging integral which assumes optical incoherence as a necessary condition to describe P-field imaging completely (as was shown by Reza etal. recently). In this paper, we estimate the relationship between the expected magnitude of this spurious signal and the degree of partial roughness within the P-field imaging system. The treatment allows us to determine the accuracy of the estimate provided by the P-field integral for varying degrees of partial coherence and allows to define a P-field signal-to-noise ratio as a figure-of-merit for the case of a partially coherent optical carrier. Abstract

Recently, an optical meta concept called the Phasor Field (P-Field) was proposed that yields great quality in the reconstruction of hidden objects imaged by non-line-of-sight (NLOS) imaging. It is based on virtual sinusoidal modulation of the light with frequencies in the MHz range. Phasor Field propagation was shown to be described by the Rayleigh-Sommerfeld diffraction integral. We extend this concept and stress the analogy between electric field and Phasor Field. We introduce Phasor Field optical elements and present experiments demonstrating the validity of the approach. Abstract

Given its non-invasive nature, there is increasing interest in the use of transcutaneous vagus nerve stimulation (tVNS) across basic, translational and clinical research. Contemporaneously, tVNS can be achieved by stimulating either the auricular branch or the cervical bundle of the vagus nerve, referred to as transcutaneous auricular vagus nerve stimulation(VNS) and transcutaneous cervical VNS, respectively. In order to advance the field in a systematic manner, studies using these technologies need to adequately report sufficient methodological detail to enable comparison of results between studies, replication of studies, as well as enhancing study participant safety. We systematically reviewed the existing tVNS literature to evaluate current reporting practices. Based on this review, and consensus among participating authors, we propose a set of minimal reporting items to guide future tVNS studies. The suggested items address specific technical aspects of the device and stimulation parameters. We also cover general recommendations including inclusion and exclusion criteria for participants, outcome parameters and the detailed reporting of side effects. Furthermore, we review strategies used to identify the optimal stimulation parameters for a given research setting and summarize ongoing developments in animal research with potential implications for the application of tVNS in humans. Finally, we discuss the potential of tVNS in future research as well as the associated challenges across several disciplines in research and clinical practice.

Abstract

It is proved that the space of differential forms with weak exterior and co-derivative, is compactly embedded into the space of square integrable differential forms. Mixed boundary conditions on weak Lipschitz domains are considered. Furthermore, canonical applications such as Maxwell estimates, Helmholtz decompositions and a static solution theory are proved.
