Abstract

Ultra‐long‐term electroencephalographic (EEG) registration using minimally invasive low‐channel devices is an emerging technology to assess sporadic seizure events. Highly sensitive automatic seizure detection algorithms are needed for semiautomatic evaluation of these prolonged recordings. We describe the design and validation of a deep neural network for two‐channel seizure detection. The model is trained using EEG recordings from 590 patients in a publicly available seizure database. These recordings are based on the full 10–20 electrode system and include seizure annotations created by reviews of the full set of EEG channels. Validation was performed using 48 scalp EEG recordings from an independent epilepsy center and consensus seizure annotations from three neurologists. For each patient, a three‐electrode subgroup (two channels with a common reference) of the full montage was selected for validation of the two‐channel model. Mean sensitivity across patients of 88.8% and false positive rate across patients of 12.9/day were achieved. The proposed training approach is of great practical relevance, because true recordings from low‐channel devices are currently available only in small numbers, and the generation of gold standard seizure annotations in two EEG channels is often difficult. The study demonstrates that automatic seizure detection based on two‐channel EEG data is feasible and review of ultra‐long‐term recordings can be made efficient and effective. [ABSTRACT FROM AUTHOR]

32. Harmonization of Product Classifications: A Consistent Time Series of Economic Trade Activities.

Abstract

Firm-product data provide information for various research questions in international trade, industrial or innovation economics. However, working with these data requires harmonizing product classifications consistently over time to avoid internal validity issues. Harmonization is required because classification systems like the EU classifications combined nomenclature (CN) for goods or the Prodcom for the production of manufactured goods undergo several changes. We have addressed this problem and developed an approach to harmonize product codes. This approach tracks product codes from 1995 to 2022 for CN and 2001 to 2021 for Prodcom. Additional years can be conveniently added. We provide the harmonized product codes for CN and Prodcom in the selected period's last (or first) year. Our approach is summarized in an open-source R package so that researchers can consistently track product codes for their selected period. We demonstrate the importance of harmonization using the micro-level trade data for Croatia as a case study. Abstract

Cerebral cavernous malformations (CCMs) are well-defined, mostly singular lesions present in 0.4-0.9% of the population. Epileptic seizures are the most frequent symptom in patients with CCMs and have a great impact on social function and quality of life. However, patients with CCM-related epilepsy (CRE) who undergo surgical resection achieve postoperative seizure freedom in only about 75% of cases. This is frequently because insufficient efforts are made to adequately define and resect the epileptogenic zone. The Surgical Task Force of the Commission on Therapeutics of the International League Against Epilepsy (ILAE) and invited experts reviewed the pertinent literature on CRE. Definitions of definitive and probable CRE are suggested, and recommendations regarding the diagnostic evaluation and etiology-specific management of patients with CRE are made. Prospective trials are needed to determine when and how surgery should be done and to define the relations of the hemosiderin rim to the epileptogenic zone.

Abstract

Die klinische Bedeutung der automatischen Detektion von epileptischen Anfällen besteht in der Anfallsdokumentation zur objektiven Evaluation von Epilepsietherapien, zur Prävention des „sudden unexpected death in epilepsy“ (SUDEP), zur Vermeidung von anfallsassoziierten Verletzungen, zur Warnung der Patienten vor bevorstehenden Anfällen und zur Entwicklung von anfallsgetriggerten, bedarfsgesteuerten Therapieformen. Die automatische Anfallsdetektion kann durch Analyse des EEGs (Oberflächen-EEG, intrakranielles EEG, subkutanes EEG), von motorischen Entäußerungen während epileptischer Anfälle (Oberflächenelektromyographie, Accelerometrie, Videodetektionssystem, Matratzensensoren) sowie von physiologischen autonomen Parametern (Herz- und Atemfrequenz, Sauerstoffsättigung, Schweißsekretion, Körpertemperatur) erfolgen. Bestimmte Parameter können ausschließlich oder besonders gut bestimmte Anfallstypen erfassen, während andere Anfallsformen nicht detektiert werden können. Jedenfalls gibt es keine One-fits-all-Lösung. Deshalb kommen zunehmend multimodale Anfallsdetektionssysteme (Erfassung mehrerer komplementärer Parameter) zum Einsatz, die individuell auf den Patienten und dessen Anfälle abgestimmt werden können. Derzeit wird der Gebrauch von klinisch validierten Geräten für die automatische Detektion von generalisierten tonisch-klonischen Anfällen und von fokalen zu bilateral tonisch-klonischen Anfällen empfohlen, insbesondere bei nicht supervidierten Patienten, bei denen ein Alarm zu einer raschen Intervention führen kann.
