Introducción: El Retinoblastoma es el cáncer intraocular más frecuente en los niños menores de 5 años. Está relacionado con mutaciones en el gen RB1, pudiendo ser esporádicas (mayoritariamente) o congénitas. El diagnostico se basa en la detección de los principales signos y síntomas de alarma. Es un tumor muy agresivo, por lo que el primer tratamiento empleado fue la enucleación. Con el paso de los años se fueron mejorando las técnicas, teniendo un amplio espectro de opciones terapéuticas como la quimioterapia sistémica intravenosa, la quimioterapia intraarterial oftálmica, la quimioterapia intravítrea y la radioterapia, entre otras. En los últimos años también se desarrolló la quimioterapia intravítrea de precisión, una técnica innovadora que busca una mayor supervivencia con menores efectos secundarios. Objetivos: Determinar la superioridad o no de la quimioterapia intravítrea de precisión en cuanto a efectividad y tolerancia, frente a los tratamientos clásicos en el tratamiento del retinoblastoma en los primeros 10 años de vida. Material y métodos: Se trata de una revisión sistemática desarrollada a partir de las bases de datos PubMed y Google Académico, siguiendo las indicaciones establecidas por el método PRISMA. Posteriormente se aplican filtros de inclusión y exclusión y se revisan los artículos para seleccionar los más adecuados. Resultados: Con la búsqueda se obtienen 102 documentos que, tras el cribado, se reducen a 30 que se analizan para, finalmente, incluir 6 en la revisión sistemática. En el análisis de los documentos se tuvo en cuenta la calidad y metodología empleadas y su contenido, buscando los que mejor se adaptasen a la temática a tratar. Conclusiones: La quimioterapia intravítrea de precisión (p-IVitC) promete ser una técnica que podría mejorar la eficacia y seguridad en el tratamiento del retinoblastoma, gracias a su mayor exactitud, con menores efectos adversos. Aun así, son necesarios más estudios acerca de esta técnica tan reciente, Introdución: O Retinoblastoma é o cancro intraocular máis frecuente nos nenos menores de 5 anos. Conclusións: A quimioterapia intravítrea de precisión (p-IVitC) promete ser unha técnica que podería mellorar a eficacia e seguridade no tratamento do retinoblastoma, grazas á súa maior exactitude, con menores efectos adversos. Aínda así, son necesarios máis estudos acerca desta técnica tan recente, Introduction: Retinoblastoma is the most frequent intraocular cancer in children under 5 years. It is related to mutations in the RB1 gene and can be sporadic (mostly) or congenital. Diagnosis is based on the detection of the main warning signs and symptoms. It is a very aggressive tumor, so the first treatment used was enucleation. Over the years, techniques have been improved, with a wide range of therapeutic options such as intravenous systemic chemotherapy, intra-arterial ophthalmic chemotherapy, intravitreal chemotherapy and radiotherapy, among others. In recent years, intravitreal precision chemotherapy has also been developed, an innovative technique that aims to achieve a longer survival with fewer side effects. Objectives: To determine the superiority or not of intravitreal precision chemotherapy in terms of effectiveness and tolerance compared to classic treatments in the treatment of retinoblastoma in the first 10 years of life. Material and methods: This is a systematic review developed from the PubMed and Google Scholar databases, following the indications established by the PRISMA method. Subsequently, inclusion and exclusion filters were applied and the articles were reviewed to select the most appropriate ones. Results: The search allowed to obtain 102 documents which, after screening, were reduced to 30 and analyzed to finally include 6 in the systematic review. In the analysis of the documents, the quality and methodology used and their content were taken into account, looking for those that best adapted to the subject. Conclusions: Precision intravitreal chemotherapy (p-IVitC) promises to be a technique that could improve efficacy and safety in the treatment of retinoblastoma, thanks to its greater accuracy, with fewer adverse effects. Even so, more studies are needed on this very recent technique

Purpose To evaluate the relationship between unilateral or bilateral criptorchidism, patient age, primary location of the gonad and modality of treatment with testicular volume and hormonal status at 18 years in patients diagnosed and treated for cryptorchidism during childhood. Materials and Methods Testicular volume, LH, FSH, and testosterone were evaluated in 143 young men at 18 years treated in childhood for unilateral (n=103) or bilateral (n=40) cryptorchidism. Results Unilateral cryptorchidism: Location of testis was prescrotal in 36 patients, inguinal in 52 and non-palpable in 15. The mean volume was 9.7 mL compared to 16.2 mL. for the spontaneously descended testicle in unilateral cryptorchidism. However, 22 patients who received HCG had a significantly bigger testis (11.8 mL.) than those treated with primary surgery (9.2 mL). The results showed a significant positive correlation between testicular volume and patient age at treatment. Bilateral cryptorchidism Location of testis was prescrotal in 34 cases, inguinal in 40 and 6 patients with non-palpable testicles. Mean volume at 18 years was 12.9 mL, greater than unilateral cryptorchid testis (9.7 mL) but smaller than healthy contralateral in unilateral cases (16.2 mL). There were significant differences in the testicular growth for bilateral patients with testicular descent after being treated with HCG (14.4 mL) in respect with those untreated (11.1 mL) or those who underwent primary surgery (11.4 mL). There was a significant positive correlation between the testicular volume and palpable (12.4 mL) or non-palpable testis (10.4 mL). There was a correlation between unilateral or bilateral cryptorchidism and levels of FSH. Conclusions Testicular volume and hormonal function at 18 years for patients diagnosed and treated for cryptorchidism during childhood are strongly influenced by whether the undescended testis was unilateral or bilateral. Location of the testes at diagnosis or age of initial treatment exerts no definite effect on testicular volume improvement or hormonal levels at 18 years of age.

OBJECTIVE: To clarify the role of peritoneography in assessing the patency of processus vaginalis (PV) in pediatric patients diagnosed with cryptorchidism. MATERIALS AND METHODS: We designed a prospective clinical trial to evaluate the patency of PV in boys presenting cryptorchidism. Herniography was performed in 310 prepubertal boys. Data about the morphology of PV was compared with operative findings in those surgically treated patients. Retractile and ectopic testes were excluded from the study. RESULTS: Of the 376 undescended testes (310 patients), 281 cases were associated with an obliterated PV. Herniography revealed 95 cases of open PV in cryptorchid boys. The 244 normally descended testes had associated patent processus vaginalis in only 31 cases. CONCLUSIONS: Herniography is the most relevant procedure for accurate diagnosis of persistent PV. The persistence of PV was significantly more frequent when the position of the testes is more cranial. The incidence of an open PV decreases with age.

Abstract Tailgut cyst, or retrorectal cystic hamartoma, is a rare congenital lesion found in the presacral space. The lession has been infrequently reported in medical literature. It is most common in middle-aged women and is rare in children. We report a case of a tailgut cyst mimicking a rectal duplication in an 8-year-old child. Excision and histological examination of the mass confirmed the tailgut cyst.

Análisis retrospectivo de una cohorte de niños menores de 15 años con dolor abdominal agudo a quienes se realizó una ecografía por sospecha de apendicitis, con el objetivo de determinar la validez y confiabilidad diagnósticas de la ecografía. La ecografía diagnosticó apendicitis en 143 niños. Se realizaron 154 apendicectomías. El porcentaje de perforación apendicular fue del 25% y de apendicectomía negativa del 4,5%. La ecografía presentó una sensibilidad del 85%, especificidad del 90%, exactitud diagnóstica del 88% y Coeficiente Kappa de 0,8. La asociación de la ecografía a la clínica y laboratorio incrementó el área bajo la curva a 0,92. Concluimos que la ecografía es una prueba válida y fiable y su rendimiento fue superior a la clínica y al laboratorio en el diagnóstico de apendicitis.

Resumen: La ingestión de cáusticos representa un grave problema médico-social por las consecuencias devastadoras e irreversibles que puede producir en el tracto digestivo superior. En Iberoamérica no se han publicado datos fidedignos sobre la incidencia o la prevalencia de lesiones inducidas por cáusticos. La información disponible sobre la presentación clínica, diagnóstico, tratamiento y pronóstico se basa en series retrospectivas de casos y, de hecho, su manejo clínico se sustenta en muchos casos fundamentalmente en la opinión de expertos. Recientemente como una iniciativa de la Sociedad Latinoamericana de Gastroenterología, Hepatología y Nutrición Pediátrica (SLAGHNP) y con la co laboración de colegas de la Sociedad Española de Gastroenterología, Hepatología y Nutrición Pediá trica (SEGHNP), hemos diseñado una Guía de Práctica Clínica (GPC) la cual incluye una serie de enunciados y recomendaciones dirigidos a optimizar la atención a los pacientes y que se basan en la revisión sistemática de la evidencia. En dos (2) manuscritos sucesivos nos hemos enfocado primero, en los aspectos fisiopatológicos y de diagnóstico clínico-endoscópico de la esofagitis cáustica en niños (1a. Parte) y en segundo lugar, en los aspectos más relevantes del tratamiento (2a. Parte). Esperamos esta guía se convierta en una herramienta útil para el clínico en el difícil proceso de toma de decisio nes a la hora de evaluar un paciente posterior a la ingesta de una sustancia cáustica. Abstract: Caustic ingestion represents a serious social-medical problem due to the devastating and irreversible consequences it can produce in the upper digestive tract. In Ibero-America, there are no published reliable data on the incidence or prevalence of caustic-induced injuries, and most of the available information on clinical presentation, diagnosis, treatment, and prognosis is based on retrospective clinical series and, indeed, its clinical management is often based primarily on expert opinion. Re cently as an initiative of the Latin American Society for Pediatric Gastroenterology, Hepatology and Nutrition (LASPGHAN) and with the cooperation of the Spanish Society for Pediatric Gastroente rology, Hepatology and Nutrition (SEGHNP), we have designed a Clinical Practice Guideline that include a series of statements and recommendations aimed at optimizing patient medical care which is based on the systematic review of evidence. Two (2) separate papers focused on the evaluation of physiopathological and clinical-endoscopic diagnostic features of caustic esophagitis in children (1st. Paper) and, on the other hand, the most relevant therapeutic considerations (2nd. Paper). We expect this guideline to become a useful tool for the physician in the difficult decision-making process when assessing patients after caustic ingestion.

Subungual osteochondroma (SO) is an infrequent and non-malignant bone tumour of the distal phalanx, especially prominent in paediatric populations. The aim of this research was to describe a case series of paediatric feet with SO which received surgical treatments. The secondary purpose was to compare these descriptive data by sex distribution. Methods: Twenty-three paediatric feet with SO confirmed by clinical or radiological features received surgical treatment. Socio-demographic (age, sex, height, weight and BMI) and clinical features (side, location, tumour or pain presence, and nail lift before surgery, as well as recurrence and adverse effects at one month after intervention) were reported. Results: Regarding clinical features before intervention, the most frequent locations of SO were the first toe (86.8%) and the right lower limb (56.5%). In addition, the presence of the tumour, pain and nail lift showed a prevalence of 91.3%, 69.5% and 47% of the study sample, respectively. Considering clinical features at one month after intervention, the most frequent adverse effect was the pain presence (69.5%). In addition, one case (4.4%) presented ulceration. Only one patient (4.4%) suffered from recurrence with a new tumour. There were not statistically significant differences by sex distribution (p >, 0.05). Conclusions: This novel study showed that surgery treatment for SO in paediatric populations presented a very low recurrence degree with minor adverse effects and without differences by sex distribution. Thus, further randomized clinical trials should be carried out in order to determine the effectiveness of this intervention in this special population.

Subungual osteochondroma (SO) is an infrequent and non-malignant bone tumour of the distal phalanx, especially prominent in paediatric populations. The aim of this research was to describe a case series of paediatric feet with SO which received surgical treatments. The secondary purpose was to compare these descriptive data by sex distribution. Methods: Twenty-three paediatric feet with SO confirmed by clinical or radiological features received surgical treatment. INTRODUCCIÓN: la torsión testicular (TT) es una patología frecuente durante la infancia que resulta de la rotación repetida del cordón espermático, conduce a un compromiso vascular de la gónada y al posterior infarto testicular. Es la primera causa de pérdida de la gónada en esta etapa de la vida. OBJETIVO: evaluar la bibliografía disponible para identificar aquellos factores relacionados con la viabilidad del testículo torsionado, revisar la incidencia, diagnóstico, tratamiento y resultado a largo plazo. MÉTODOS: revisión bibliográfica. Se realizó una búsqueda en PUBMED de estudios publicados en los últimos diez años. Los criterios de inclusión permitieron seleccionar estudios de revisión realizados en población infantil y que valorasen la edad de presentación, los síntomas y su duración, los tiempos de asistencia y los resultados finales. RESULTADOS Y DISCUSIÓN: de los 620 artículos potencialmente relevantes se seleccionaron 10 que cumplieron los criterios de inclusión. De estos, nueve fueron estudios de tipo retrospectivo y uno, prospectivo. Los diez estudios incluyeron un total de 485 casos de torsión testicular. CONCLUSIÓN: la TT no es la causa más frecuente de escroto agudo pero sí de la pérdida testicular en varones jóvenes, con una presentación bimodal. Os criterios de inclusión permitiron seleccionar estudos de revisión realizados na poboación infantil que valorasen a idade de presentación, os síntomas e a súa duración, os tempos de asistencia e os resultados finais. Is the first cause of testicular loss in young men. OBJECTIVE: evaluate the available bibliography to identify factors related to the viability of the testicle, review the incidence, diagnosis, treatment and long-term result. METHODS: bibliographic review. A PUBMED search was conducted for studies published in the last ten years. The inclusion criteria allowed us to select review studies carried out in children and assesing the age of presentation, the symptoms and their duration, the attendance times and the final results. RESULTS AND DISCUSSION: of the 620 potentially relevant articles, 10 were selected with inclusion criteria. Of these ten, nine were retrospective and one was prospective. All the studies selected included a total of 485 cases of testicular torsion. CONCLUSION: TT is not the most frequent cause of testicular loss but is the most important in young men with a bimodal presentation. Surgical exploration is the first-line treatment. Early assistance to the hospital and a quickly surgical intervention reduce the loss rate and subsequent atrophy.

Background Perineal groove is a rare congenital malformation that is unknown to many clinicians and is often misdiagnosed. Although it may be self-resolving during childhood, this nonepithelized mucous membrane can pose the risk of local irritation and infection, particularly urinary tract infection. Methods A retrospective study of female infants diagnosed with a perineal groove was performed, demographic characteristics and clinical features were analyzed, and a photographic review was conducted. Results Five patients with perineal groove were observed in our clinic in 2015-16. The mean age was 14 months. None had symptoms, and no treatment was required. During follow-up of 1 year, all remained asymptomatic. Conclusions Our retrospective review suggests that perineal groove may be an underdiagnosed condition. Most cases resolve spontaneously, but confusion in diagnosis may lead to misdiagnosis or misinterpretation of sexual abuse and unnecessary treatments.

In Ibero-America, there are no published reliable data on the incidence or prevalence of caustic-induced injuries, and most of the available information on clinical presentation, diagnosis, treatment, and prognosis is based on retrospective clinical series and, indeed, its clinical management is often based primarily on expert opinion. Re cently as an initiative of the Latin American Society for Pediatric Gastroenterology, Hepatology and Nutrition (LASPGHAN) and with the cooperation of the Spanish Society for Pediatric Gastroente rology, Hepatology and Nutrition (SEGHNP), we have designed a Clinical Practice Guideline that include a series of statements and recommendations aimed at optimizing patient medical care which is based on the systematic review of evidence. Two (2) successive papers focused on the evaluation of physiopathological and clinical-endoscopic diagnostic features of caustic esophagitis in children (1st. Paper) and, on the other hand, the most relevant therapeutic considerations (2nd. Paper). We expect this guideline to become a useful tool for the physician in the difficult decision-making process when assessing patients after caustic ingestion.

La información disponible sobre la presentación clínica, diagnóstico, tratamiento y pronóstico se basa en series retrospectivas de casos y, de hecho, su manejo clínico se sustenta en muchos casos fundamentalmente en la opinión de expertos. Recientemente como una iniciativa de la Sociedad Latinoamericana de Gastroenterología, Hepatología y Nutrición Pediátrica (SLAGHNP) y con la colaboración de colegas de la Sociedad Española de Gastroenterología, Hepatología y Nutrición Pediátrica (SEGHNP), hemos diseñado una Guía de Práctica Clínica (GPC) la cual incluye una serie de enunciados y recomendaciones dirigidos a optimizar la atención a los pacientes y que se basan en la revisión sistemática de la evidencia. En dos (2) manuscritos sucesivos nos hemos enfocado primero, en los aspectos fisiopatológicos y de diagnóstico clínico-endoscópico de la esofagitis cáustica en niños (1ª. Parte) y en segundo lugar, en los aspectos más relevantes del tratamiento (2ª. Parte). Esperamos esta guía se convierta en una herramienta útil para el clínico en el difícil proceso de toma de decisiones a la hora de evaluar un paciente posterior a la ingesta de una sustancia cáustica.

A34-year-old primigravida with no significant medical history underwent a routine ultrasound at 21 weeks’ gestation. Fetal sonography revealed the presence of a solid mass, 14 mm 19 mm in size, arising from the region of the glabella (Figure 1B). No other abnormalities were detected. Fetal MRI was performed at 21 weeks (Figure 2A) to clearly define the lesion and rule out calvarial defects. The patient chose to continue with the pregnancy. A male fetus was uneventfully delivered at 39 weeks. The presence of a solid friable mass located in the left internal canthus was noticed (Figure 2B). The mass was resected with no complications at 2 weeks of age (Figure 2C), and pathological study confirmed the presence of neuroglial heterotopic tissue (nasal glioma). Although benign in nature, gliomas are cosmetically unfavorable, and early surgical intervention is the treatment of choice to minimize nasal distortion.1,2 Differential diagnosis includes encephalocele, teratoma, dermoid cyst, dacryocystocele, retinoblastoma, and hemangioma.2,3 Prenatal suspected diagnosis and assessment is of paramount relevance SI
