Abstract

Opioid use disorder (OUD) is a substantial public health problem with an increasing prevalence worldwide. (1) From 2016 to 2021, 26 690 opioid-related deaths occurred in Canada; (1) in Ontario, [...], Background: Emergency department visits and hospital admissions for opioid toxicity are opportunities to initiate opioid agonist therapy (OAT), which reduces morbidity and mortality in patients with opioid use disorder (OUD). The study objectives were to evaluate OAT initiation rates after a hospital encounter for opioid toxicity in Ontario, Canada, and determine whether publication of a 2018 Canadian OUD management guideline was associated with increased initiation. Methods: We conducted a retrospective, population-based serial cross-sectional study of hospital encounters for opioid toxicity among patients with OUD between Jan. 1, 2013, and Mar. 31, 2020, in Ontario, Canada. The primary outcome was OAT initiation (methadone, buprenorphine-naloxone, or slow-release oral morphine) within 7 days of discharge, measured quarterly. We examined the impact of the release of the OUD management guideline on OAT initiation rates using Autoregressive Integrated Moving Average models. Results: Among 20 702 hospital visits for opioid toxicity among patients with OUD, the median age was 35 years, and 65.1% were male. Over the study period, the percentage of visits leading to OAT initiation within 7 days rose from 1.7% or less (Q1 2013) to 5.6% (Q1 2020); however, the publication of the Canadian OUD management guideline was not associated with a significant increase in these rates (0.14% slope change, 95% confidence interval -0.11% to 0.38%; p = 0.3). Interpretation: Among hospital encounters for opioid toxicity, despite rising prevalence over time, only 1 in 18 patients were dispensed OAT within a week of discharge in early 2020. These findings highlight missed opportunities to initiate therapies proven to reduce mortality in patients with OUD.

Abstract

The digenetic trematode parasite, which is a member of the genus Fasciola and has two species: Fasciola Hepatica and Fasciola Gigantica, is the source of the snail-borne trematode disease known as Fascioliasis. Fasciola gigantica is found in tropical regions of Africa and Asia, whereas Fasciola hepatica is primarily found in temperate zones. 2.4 million people are thought to be affected with fascioliasis worldwide, which has recently been recognized as a significant zoonotic parasitic illness that infects humans. Both species spread by snails belonging to the Lymnaeidae family and pose a concern to domestic ruminants and public health. Ingestion of encysted metacercariae of Fasciola species causes infection in both humans and animals. In Egypt, fascioliasis in humans and animals is endemic and causes both clinical and epidemiological health problems. Economic losses from fascioliasis include mortality, decreased carcass weight, decreased meat and milk output, decreased growth and quality of wool, liver damage, and decreased resistance, which makes animals more susceptible to subsequent consequences and other diseases. The parasite's history, taxonomic position, biology, pathophysiology, epidemiology, geographic distribution, economic losses, diagnosis, and control of infection in humans and animals are only a few of the topics covered by the authors in this work. Abstract

Fascioliasis is an important worldwide zoonotic parasitic disease. In Egypt, fascioliasis in humans and animals is endemic and causes both clinical and epidemiological health problems. The purpose of this study was to assess the influence of related risk factors in two regions of the New Valley Governorate, Egypt, as well as the prevalence of fascioliasis in humans and farm animals based on fecal examination. The fecal sedimentation technique was used to look for Fasciola eggs in the fecal samples. Between December 2022 and June 2023, 180 human stool samples and 370 animal fecal samples (300 cattle, 70 sheep) were collected. Depending on coprological examination, the prevalence of fascioliasis was 0 % (0/180) in human and 1.6% (6 out of 370) in animals, sheep showed higher infection 2.9% (2 out of 70) than cattle 1.3% (4 out of 300) with non-significant association between infection and species, age range, sex of animals, locality, and time of infection. In conclusion, even though human fascioliasis was not found in this investigation, the animal fascioliasis existence can pose a risk to public health. Thus, it's critical to implement policies that will minimize the chance that animals in the governorate of New Valley may become infected with Fasciola. [ABSTRACT FROM AUTHOR]

Abstract

Purpose: A cross-sectional study was conducted to investigate the feasibility of implementing routine collection of the Euro-Qol 5 dimensions (EQ-5D) questionnaire, to inform drug and health technology reimbursement decision making. Methods: Women with breast cancer were recruited during scheduled clinic visits to an academic cancer centre. EQ-5D-5L was self-administered using electronic tablets. Diagnostic and treatment data were abstracted from patient charts. Feasibility was assessed primarily by the proportion of patients who fully completed EQ-5D-5L and by their willingness to complete the instrument at each clinic visit. Results: 588 women were approached for study participation, 341 were enrolled. Fully completed EQ-5D-5L questionnaires were obtained in 323 participants (95% of participants, 95% CI 92–97%). Median time for EQ-5D-5L completion was 1.5 minutes (range:0.35 to 14.7). Mean age of participants was 58 years old. Most women who completed EQ-5D were White, born outside Canada and presented a high education level; one-quarter had metastatic disease. Most participants reported "No problems" in all EQ-5D-5L dimensions. Mean EQ-5D-5L index and mean EQ-5D-5L VAS values for all participants were respectively 0.83 (SD 0.13) and 75.7 (SD 17.45), with patients with metastatic disease scoring the lowest values. Seventy-eight percent of participants were willing to complete EQ-5D-5L at each clinic visit; lower Charlson comorbidity index and higher education level were predictors of willingness to continue to answer EQ-5D-5L. Conclusions: Tablet-based collection of EQ-5D-5L in the context of routine clinical practice proved to be feasible. However, many patients declined study participation or reported being in full health, raising concerns about whether this method of collecting EQ-5D adequately represents the health status of all breast cancer patients. Abstract

The CDC recommends routine voluntary HIV testing of all patients 13-64 years of age. Despite this recommendation, HIV testing rates are low even among those at identifiable risk, and many patients do not return to receive their results.To examine the costs and benefits of strategies to improve HIV testing and receipt of results.Cost-effectiveness analysis based on a Markov model. Acceptance of testing, return rates, and related costs were derived from a randomized trial of 251 patients; long-term costs and health outcomes were derived from the literature.Primary-care patients with unknown HIV status.Comparison of three intervention models for HIV counseling and testing: Model A = traditional HIV counseling and testing; Model B = nurse-initiated routine screening with traditional HIV testing and counseling; Model C = nurse-initiated routine screening with rapid HIV testing and streamlined counseling.Life-years, quality-adjusted life-years (QALYs), costs and incremental cost-effectiveness.Without consideration of the benefit from reduced HIV transmission, Model A resulted in per-patient lifetime discounted costs of $48,650 and benefits of 16.271 QALYs. Model B increased lifetime costs by $53 and benefits by 0.0013 QALYs (corresponding to 0.48 quality-adjusted life days). Model C cost $66 more than Model A with an increase of 0.0018 QALYs (0.66 quality-adjusted life days) and an incremental cost-effectiveness of $36,390/QALY. When we included the benefit from reduced HIV transmission, Model C cost $10,660/QALY relative to Model A. The cost-effectiveness of Model C was robust in sensitivity analyses.In a primary-care population, nurse-initiated routine screening with rapid HIV testing and streamlined counseling increased rates of testing and receipt of test results and was cost-effective compared with traditional HIV testing strategies.
