22. Coordinated prophylactic surgical management for women with hereditary breast-ovarian cancer syndrome

Abstract

Abstract Background Women with BRCA1 or BRCA2 mutations have a substantially increased risk of breast and ovarian cancer compared with the general population. Therefore, prophylactic mastectomy (PM) and bilateral salpingo-oophorectomy (BSO) have been proposed as risk-reduction strategies for BRCA1/2 mutation carriers. We aimed to assess the feasibility of coordinated PM and BSO in hereditary breast-ovarian cancer syndrome. Methods High risk women for breast and ovarian cancer who underwent coordinated PM and BSO were included in this study. Clinical characteristics and surgical and oncologic outcomes were retrospectively reviewed. Results Twelve patients underwent coordinated PM and BSO. Ten had history of previous breast cancer. Autologous breast reconstruction was performed in ten patients. The mean age at surgery was 43 (range 34–65). Mean operating time was 9.3 hours (range 3–16) with a mean postoperative hospitalization of 5.4 days (range 4–8). Intraoperatively, there were no major surgical complications. Postoperatively, one patient developed an abdominal wound dehiscence, another reoperation for flap congestion; one had umbilical superficial epidermolysis, and one patient developed aspiration pneumonia. At a mean follow-up of 84 months, 10 of patients were cancer-free. Although no patients developed a new primary cancer, two developed a distant recurrence. Conclusion Coordinated PM and BSO is a feasible procedure with acceptable morbidity in selected high-risk patients that desire to undergo surgery at one operative setting.

23. Histologic changes of intraoral free skin flaps

Abstract

BACKGROUND: It was the purpose of this study to assess the histologic skin changes of free tissue transfers used to reconstruct intraoral defects. METHODS: Patients who were at least 12 months after intraoral free tissue reconstruction were selected for study. A 3-mm punch biopsy of the intraoral portion of the flap was performed and submitted for routine histologic analysis and periodic acid-Schiff (PAS) staining. The adjacent native oral mucosa was also biopsied and served as control. RESULTS: Eight patients (42 to 71 years old) were studied. A variety of skin flaps were employed and included free rectus abdominis (4), radial forearm (2), scapula (1), and osteocutaneous fibula flaps (1). Histologic examination demonstrated that all specimens maintained normal skin architecture and maturation. Adnexal structures were demonstrated in all but one. Acute inflammation in conjunction with diffuse parakeratosis and spongiosis were seen with 4 cases. Fungal forms consistent with Candida species were identified in the stratum comeum by PAS stain in these 4 cases. CONCLUSION: The histologic integrity of skin appears to be maintained in cutaneous free flaps placed intraorally. Previous studies have noted an inflammatory infiltrate in the epithelial tissues of intraorally placed flaps, yet have not commented on the significance of this finding even when it was noted in a chronic setting. Our study is the first to correlate these changes with the presence of fungi. The effect of the acute inflammation and associated fungal forms may suggest the need for chronic therapy and merits further study.

Abstract

The quality of life of breast cancer survivors is ameliorated by minimizing adverse effects on their physical appearance. Breast reconstruction is important for restoring the survivor's appearance. In breast reconstructive surgery, there is a need to develop technologies for quantifying surgical outcomes and understanding women's perceptions of changes in their appearance. Methods for objectively measuring breast anatomy are needed in order to help breast cancer survivors, radiation oncologists and surgeons quantify changes in appearance that occur with different breast reconstructive surgical options.In this study, we present an automated method for computing a variant of the normalized Breast Retraction Assessment, a common measure of symmetry, from routine clinical photographs taken to document breast cancer treatment procedures. The algorithms were designed using a development set of retrospectively collected clinical photographs and evaluated using a separate test set of prospectively collected clinical photographs.We have developed new algorithms for automatically localizing the umbilicus and nipples on standard anterior-posterior clinical photographs, enabling the automatic calculation of measure of breast symmetry.This study demonstrates that under standardized photographic conditions, automatic localization of fiducial points and subsequent computation of measures, such as symmetry, is feasible. The algorithms presented here for quantifying surgical outcomes in breast reconstructive surgery will provide a foundation for future work on assisting a cancer patient and her surgeons in selecting and planning reconstruction procedures that will maximize the woman's psychosocial adjustment to life as a breast cancer survivor.

47. Harvesting Technique Affects Adipose-Derived Stem Cell Yield.

Abstract

Background: The success of an autologous fat graft depends in part on its total stromal vascular fraction (SVF) and adipose-derived stem cells (ASCs). However, variations in the yields of ASCs and SVF cells as a result of different harvesting techniques and donor sites are poorly understood. Objective: To investigate the effects of adipose tissue harvesting technique and donor site on the yield of ASCs and SVF cells. Methods: Subcutaneous fat tissues from the abdomen, flank, or axilla were harvested from patients of various ages by mechanical liposuction, direct surgical excision, or Coleman's technique with or without centrifugation. Cells were isolated and then analyzed with flow cytometry to determine the yields of total SVF cells and ASCs (CD11b-, CD45-, CD34+, CD90+, D7-FIB+). Differences in ASC and total SVF yields were assessed with one-way analysis of variance. Differentiation experiments were performed to confirm the multilineage potential of cultured SVF cells. Results: Compared with Coleman's technique without centrifugation, direct excision yielded significantly more ASCs (P < .001) and total SVF cells (P = .007); liposuction yielded significantly fewer ASCs (P < .001) and total SVF cells (P < .05); and Coleman's technique with centrifugation yielded significantly more total SVF cells (P < .005), but not ASCs. The total number of SVF cells in fat harvested from the abdomen was significantly larger than the number in fat harvested from the flank or axilla (P < .05). Cultured SVF cells differentiated to adipocytes, osteocytes, and chondrocytes. Conclusions: Adipose tissue harvested from the abdomen through direct excision or Coleman's technique with centrifugation was found to yield the most SVF cells and ASCs. [ABSTRACT FROM AUTHOR]
