RESUMO O DSAEK (Descemet stripping automated endotelial Keratoplasty) é um dos procedimentos de escolha para tratamento das doenças que acometem o endotélio corneano. Apesar do sucesso terapêutico, o procedimento pode induzir uma hipermetropia residual. Em média a refração se estabiliza em um perído que varia de 6 a 12 meses após o transplante. O objetivo desse relato é descrever a evolução dessa opção terapêutica utilizada em um paciente de 54 anos portador de Distrofia de Fuchs. O paciente foi submetido ao transplante de córnea (DSAEK) e a remoção do cristalino no mesmo tempo cirúrgico o que resultou em hipermetropia residual. Após 3 anos de acompanhamento apresentava-se com a melhor visão corrigida de 20/20 (Snellen) no olho operado com uma refração de +3,25 -1,00 (5º). Optou-se por realização de LASIK (Laser assisted in situ Keratomileusis) hipermetrópico, obtendo um resultado visual satisfatório. ABSTRACT DSAEK (Descemet stripping automated endothelial keratoplasty) is one of the options for corneal endothelium disease, which in some patients can result in a residual hyperopia after the procedure. Usually 6 to 12 months after corneal transplantation refraction is already stable. This report describes a therapeutic option used in a 54 years old patient with Fuchs' endothelial dystrophy submitted to cataract and corneal transplant that resulted in residual hyperopia, three years after the procedure the best corrected vision was 20/20 with a refraction of +3.25 -1.00 (5 º) treated with Hyperopic - LASIK (Laser-assisted In Situ Keratomileusis) with satisfactory visual result.

Purpose: To evaluate ocular straylight before and after photorefractive keratectomy (PRK) for low myopia with and without topical mitomycin (MMC) treatment. Methods: Patients who underwent PRK for low myopia were enrolled into the study. PRK without MMC was performed in 21 eyes (12 patients), whereas PRK with topical 0.02% MMC was performed in 25 eyes (14 patients). Both groups were treated using the NIDEK EC5000 excimer laser. Measurements were performed using the C-Quant straylight meter preoperatively and at two and four months postoperatively. Results: The mean patient age was 30 ± 4 years, and the mean spherical equivalent refractive error was -2.2 ± 0.75 D. The mean preoperative intraocular straylight values were 1.07 ± 0.10 in the PRK without MMC group and 1.07 ± 0.11 log(s) in the PRK with topical MMC group. At two months after surgery, there was a decrease in mean intraocular straylight values in both groups. However, a significant difference was only reached in the PRK with MMC group [0.98 ± 0.09 log(s), p=0.002] compared with preoperative values, which was likely due to a greater scatter of measurements in the PRK without MMC group [1.03 ± 0.13 log(s), p=0.082]. At four months postoperatively, ocular straylight values were not significantly different compared with those at baseline in either the PRK without MMC group [1.02 ± 0.14 log(s), p=0.26] or in the PRK with topical MMC group [1.02 ± 0.11 log(s), p=0.13]. Conclusion: PRK for low myopia decreases ocular straylight, and MMC application further reduces straylight in the early postoperative period. However, ocular straylight values do not significantly differ at four months after surgery compared with those at baseline. RESUMO Objetivo: Avaliar a dispersão de luz intraocular antes e depois da ceratectomia fotorrefrativa (PRK) para baixa miopia com e sem a aplicação tópica de mitomicina C. Métodos: Pacientes submetidos à PRK para baixa miopia foram selecionados para o estudo. PRK sem MMC foi realizado em 21 olhos (12 pacientes) e PRK com MMC tópica a 0,02% foi realizado em 25 olhos (25 pacientes). Ambos os grupos foram tratados com o excimer laser da Nidek EC5000. Avaliações foram realizadas usando o medidor de dispersão de luz C-Quant no pré-operatório e com 2 e 4 meses de pós-operatório. Resultados: A média de idade dos pacientes foi 30 ± 4 anos e a média do equivalente esférico foi -2,2 ± 0,75 D. As médias da dispersão de luz intraocular no pré-operatório foram 1,07 ± 0,10 no grupo PRK sem MMC e 1,07 ± 0,11 log(s) no grupo PRK com MMC tópica. Após 2 meses da cirurgia houve uma diminuição na média da dispersão de luz intraocular em ambos os grupos. Entretanto uma diferença estatisticamente significante, comparado com os valores pré-operatórios, foi observada apenas no grupo PRK com MMC (0,98 ± 0,09 log(s), p=0,002), provavelmente devido as medidas com maior espalhamento de luz no grupo sem MMC (1,03 ± 0,13 log(s), p=0,082). Após 4 meses de pós-operatório, os valores de dispersão de luz não apresentavam diferença estatisticamente significantes quando comparados com os valores iniciais, tanto no grupo sem MMC (1,02 ± 0,14 log(s), p=0,26) quanto no grupo com MMC tópica (1,02 ± 0,11 log(s), p=0,13). Conclusão: PRK para baixa miopia diminui a dispersão de luz ocular e a aplicação de MMC contribui para uma ainda menor dispersão de luz no período pós-operatório inicial. Entretanto, quatro meses após a cirurgia a dispersão de luz intraocular não é significantemente diferente das medidas pré-operatórias.

OBJETIVO: Comparar as técnicas de PRK e Epi-LASIK com relação à recuperação visual e sintomatologia pós-operatória. MÉTODOS: Série de casos intervencionista que incluiu 38 olhos de 19 pacientes com miopia até 5DE e astigmatismo até 1DC. Foram selecionados pacientes com erros refracionais semelhantes nos dois olhos, realizando-se, no mesmo tempo cirúrgico, PRK em um olho e Epi-LASIK no olho contralateral. Os pacientes foram acompanhados por um ano, avaliando-se a eficácia refracional e grau de desconforto pós-operatório. RESULTADOS: Durante as primeiras 12 horas, 79,9% dos pacientes (p=0,0003) referiram dor mais intensa no olho operado com a técnica Epi-LASIK. Após 24 horas, 63,2% dos pacientes (p=0,012) ainda referiam mais dor neste olho e apenas 10,5% no olho contralateral. A acuidade visual não corrigida foi melhor nos olhos do grupo PRK no primeiro dia (p=0,034). Nos demais dias não houve diferença significativa entre os grupos. Houveopacidade corneana grau 0,5 (Fantes) em três olhos do grupo PRK e em dois no grupo Epi-LASIK. CONCLUSÃO: Ambos os grupos apresentaram resultado visual refracional satisfatório, porém o grupo Epi-LASIK apresentou maior desconforto no pós-operatório imediato. OBJECTIVE: Compare PRK and Epi-LASIK techniques regarding postoperative visual recovery and symptoms. METHODS: Interventional case series study including 38 eyes of 19 patients with myopia up to 5DE and astigmatism up to 1DC. Study included patients with similar refractive errors to be submitted to PRK in one eye and Epi-LASIK in the fellow eye at the same time. Follow-up was 1 year and included refractive error analysis and postoperative discomfort. RESULTS: During the first 12 hours after surgery, 79% (P=0,0003) of patients reported more pain and discomfort in the eye submitted to Epi-LASIK. Twenty-four hours after surgery 63,2% (P=0,012) of patients still referred more pain in the eye submitted to Epi-LASIK and only 10,5% in the contralateral eye. Uncorrected visual acuity was better on the PRK group at the 1th day (p=0.034). No difference was observed at the other postoperative days after surgery. Postoperative corneal haze 0,5 (Fantes) was observed in three eyes of the PRK group and in two days of the Epi-LASIK group. CONCLUSION: Both groups presented good visual refractive results, but the Epi-LASIK group presented more discomfort immediately after surgery.

RESUMO Com o objetivo de estudar os subtipos topográficos do ceratocone, os autores estudaram através de um sistema de análise topográfica computadorizada 59 córneas de olhos não operados de 34 pacientes com ceratocone. Todos os pacientes apresentavam achados clínicos compatíveis com ceratocone em pelo menos um dos olhos ou haviam sido submetidos a ceratoplastia por ceratocone no olho contralateral. A análise topográfica corneana foi realizada em ambos os olhos em 25 pacientes e em apenas um dos olhos em nove pacientes que haviam sido submetidos à ceratoplastia por ceratocone no olho contralateral. Quanto aos subtipos topográficos, os autores encontraram padrão em 8 simétrico (2/57; 3,51%); 8 assimétrico (13/57; 22,81%); 8 distorcido (15/57;26,32%); periférico (17/57; 29,82%); central (1/57; 1,75%) e irregular (9/57; 15,79%). Em dois olhos de dois pacientes com ceratocone no olho contralateral, os padrões se assemelhavam aos observados no astigmatismo contra a regra, porém não típicos de ceratocone e por isso não incluídos na casuística. Através da comparação dos padrões topográficos de cada um dos olhos de um mesmo paciente e de diferentes pacientes em diferentes estágios evolutivos, os autores propõem uma possível rota evolutiva no desenvolvimento do ceratocone. SUMMARY To determine the topographic patterns of keratoconus, computer-assisted topographic evaluations were performed on 59 corneas of unoperated eyes of 34 patients with keratoconus. Each patient had slit-lamp findings consistent with the diagnosis of keratoconus in at least one eye or had been submitted to keratoplasty for keratoconus in the fellow eye. Twenty-five patients had topographic maps obtained for both eyes and nine patients in only one eye because of penetrating keratoplasty in the opposite eye. Patterns included symmetric bow-tie (2/57; 3,51%); asymmetric bow-tie (13/57; 22,81%); distorted bow-tie (15/57; 26,32%); peripheral (17/57; 29,82%); central (1/57; 1,75%) and irregular (9/57; 15,79%). Two eyes of two patients with keratoconus in the fellow eye had patterns similar to those observed in against rule astigmatism, but not typical of keratoconus and, therefore, were not included in the casuistic. Comparing the topographic patterns of each eye in one patient and of different patients in different stages of the disease, the authors propose a possible evolutive route for the development of keratoconus.

RESUMO Avaliou-se a precisão e a reprodutibilidade do sistema EyeSys de videoceratografia numa superfície esférica plástica (42,50 D) e em 58 córneas de 58 pacientes divididos em quatro grupos: 30 córneas normais, 10 córneas pós-ceratotomia radial, 8 córneas pós-excimer laser e 10 córneas com ceratocone. O método de análise incluiu, para os cálculos da precisão e reprodutibilidade, todos os pontos de medida das superficies de estudo. O sistema EyeSys apresentou, na superfície esférica, uma precisão de 95% das medidas com desvio ≤ 0,25 D e níveis de reprodutibilidade maiores na região central (média de 0,0295 D) que na região periférica (média de 0,0962 D). As córneas normais, pós-ceratotomia radial e pós-excimer laser apresentaram níveis semelhantes de reprodutibilidade, nas regiões central (médias de 0,1524 D, 0,1406 D e 0,0962 D, respectivamente) e periférica (médias de 0,4484 D, 0,4673 D e 0,3138 D, respectivamente). A reprodutibilidade no ceratocone foi menor que nos demais grupos (p < 0,05), tanto na região central (média de 0,3808 D) como na periférica (média de 0,8458 D). Nos quatro grupos, a reprodutibilidade foi maior na região central da córnea (p < 0,05). O sistema EyeSys parece proporcionar informações clinicamente úteis a respeito da topografia corneana, particularmente na região central. SUMMARY Accuracy and reproducibility of the EyeSys videokeratography system was evaluated on a plastic sphere (42.50 D) and on 58 corneas of 58 patients, divided into four groups: 30 normal corneas, 10 post radial keratotomy corneas, 8 post excimer laser corneas and 10 keratoconic corneas. A method including all measurement points in the accuracy and reproducibility analysis was used. The EyeSys system showed on the plastic sphere an accuracy of 95% of the measurements within 0.25 D and higher reproducibility levels on the central (mean 0.0295 D) than on the peripheral (mean 0. 0962 D) region. The normal, post radial keratotomy and post excimer laser corneas showed similar reproducibility levels on the central (means 0.1524 D, 0.1406 D, and 0,0962 D, respectively) and peripheral (means 0.4484 D, 0.4673 D and 0.3138 D, respectively) regions. In the Keratoconus reproducibility was lower than in the other groups (p < 0.05), both on the central (mean 0.3808 D) and peripheral (mean 0.8458 D) regions. In the four groups, reproducibility was significantly higher on the central cornea (p < 0.05). The EyeSys system seems to be able to provide useful clinical information about the corneal surface, particularly over its central region.

Foram estudados retrospectivamente cem pacientes amblíopes de uma clínica particular na cidade de São Paulo, SP, Brasil, escolhidos aleatoriamente, com idades variando de 3 a 48 anos. Foram pesquisados: idade de detecção, meio de detecção, idade de início do tratamento e regularidade do retorno ao consultório. A maior parte dos casos (65%) foi detectada antes dos 7 anos de idade e dentre estes, somente 10,8% iniciaram tratamento após os 8 anos de idade. Entre aqueles que iniciaram antes dos 7 anos de idade, 44,8% retornaram irregularmente ou não retornaram. A ambliopia foi detectada predominantemente pelos pais ou pelo oftalmologista, e raramente pelo pediatra ou na escola. Os autores discutem estas observações e propõem recomendações para melhorar os resultados terapêuticos na ambliopia. This report presents retrospective data concerning 100 amblyopic patients in São Paulo, Brazil, including: the age of detection, means of detection, age in which therapy was started and follow-up of the patients. Even though most of the cases (65%) had been detected before the age of 7 years, 10.8% of them started the therapy only after 8 years of age, when the treatment of amblyopia may not be so successful. Among the patients who started their therapy before the age of 7 years, 44.8% of them returned only irregularly or didn't return at all. The amblyopia was detected, in the majority of cases, by the parents or the ophthalmologist, and rarely by the pediatrician or at school. The authors discuss such observations, and propose some ways ot improve the therapeutic results in amblyopia.
