Abstract

Normas de associação semântica, embora recentes no Brasil, são pesquisadas a mais de um século internacionalmente, constituindo-se ferramentas importantes para tarefas de avaliação da memória e linguagem. Neste estudo 108 universitários responderam a primeira palavra que lhes viesse à mente com significado relacionado a cada uma das 88 palavras-alvo apresentadas. Foi analisada força de associação entre pares e tamanho do conjunto do alvo. A maioria dos pares apresentou forte ou média associação semântica. A média de tamanho total e significativo do conjunto foi, respectivamente 37,56 e 12,40. Observaram-se correlações negativas entre força de associação e tamanho do conjunto, total e significativo. A concretude dos alvos correlacionou-se positivamente com força de correlação com nehuma das outras variáveis analisadas. Pode-se inferir que quanto maior o tamanho do conjunto do alvo, menor a força de associação entre os pares

Abstract

A lo largo de sus casi 50 años, el Instituto Nacional de la Administración Pública (INAP) tuvo un papel preponderante en el campo de la capacitación y la administración pública. Por una parte, en su carácter de órgano rector de la capacitación, es tanto efector de la capacitación como asesor de su diseño en diversas jurisdicciones de la Administración Pública Nacional (APN). Por otra parte, desde sus inicios, la producción de investigaciones orientada a fortalecer las capacidades estatales, la reflexividad sobre los procesos de administración pública y la sinergia entre las diferentes áreas del Estado, tuvo una importancia central en su devenir. En este marco, tal vez resulta menos conocido que la Biblioteca precedió al INAP. La creación del INAP en 1973 recuperó el antecedente del antiguo Instituto Superior de Administración Pública y absorbió su biblioteca especializada. Tiempo después esta biblioteca se amplió y asumió otras funciones y actividades, como la producción de documentos secundarios y el ofrecimiento de materiales pertenecientes a otras instituciones u organismos. Este hecho la convirtió en el Centro de Documentación e Información sobre Administración Pública (CEDIAP) existente hasta la actualidad. De esta manera, se pone de manifiesto la importancia crucial de la Biblioteca para el INAP, tanto por la función de proporcionar y resguardar materiales necesarios para la investigación y la capacitación, como por su relevancia simbólica: la Biblioteca fue testigo del surgimiento del INAP y acompañó su devenir institucional. Abstract

National Health Service use the Community Mental Health Service User Questionnaire (NHS-CMH) to assess care quality. However, its reliability and internal validity is uncertain. To test the NHS-CMH structure, reliability and item-level characteristics. We used data from 11,373 participants who answered the 2017 NHS-CMH survey. First, we estimated the NHS-CMH structure using Exploratory Factor Analysis (EFA) in half of the dataset. Second, we tested the best EFA-derived model with Confirmatory Factor Analysis (CFA). We tested the internal validity, construct reliability (omega – ω), explained common variance of each factor (ECV), and item thresholds. EFA suggested a 4-factor solution. The structure derived from the EFA was confirmed, demonstrating good reliability for the four correlated dimensions: "Relationship with Staff" (ω = 0.952, ECV = 40.1%), "Organizing Care" (ω = 0.855, ECV = 21.4%), "Medication and Treatments" (ω = 0.837, ECV = 13.3%), and "Support and Well-being" (ω = 0.928, ECV = 25.3%). A second-order model with a high-order domain of "Quality of Care" is also supported. The NHS-CMH can be used to reliably assess four user-informed dimensions of mental health care quality. Abstract

The molecular pathogenesis of pediatric astrocytomas is still poorly understood. To further understand the genetic abnormalities associated with these tumors, we performed a genome-wide analysis of DNA copy number aberrations in pediatric low-grade astrocytomas by using array-based comparative genomic hybridization. Duplication of the BRAF protooncogene was the most frequent genomic aberration, and tumors with BRAF duplication showed significantly increased mRNA levels of BRAF and a downstream target, CCND1, as compared with tumors without duplication. Furthermore, denaturing HPLC showed that activating BRAF mutations were detected in some of the tumors without BRAF duplication. Similarly, a marked proportion of low-grade astrocytomas from adult patients also had BRAF duplication. Both the stable silencing of BRAF through shRNA lentiviral transduction and pharmacological inhibition of MEK1/2, the immediate downstream phosphorylation target of BRAF, blocked the proliferation and arrested the growth of cultured tumor cells derived from low-grade gliomas. It presents itself in three dimensions: Emotional Exhaustion, Depersonalization and Sense of Personal Accomplishment. The most predisposed professions to the development of Burnout Syndrome are those whose activities encompass emotional involvement, such as professionals who work directly with other people, assisting them, or as responsible for development and well-being. A professional career that is particularly susceptible to develop Burnout Syndrome is that of the physician, consisting of a highly prevalent phenomenon amongst these professionals. The present study aimed to estimate the prevalence and identify factors associated with Burnout Syndrome in doctors at the hospitals of Francisco Beltrão - Paraná, as well as outline the profile of these professionals; evaluate the levels of Emotional Exhaustion, Depersonalization and Personal Accomplishment; identify the relationship between Burnout and sociodemographic, occupational and psychosocial characteristics, and describe the characteristics of professionals with Burnout Syndrome. This is a descriptive and quantitative research comprising 121 physicians who work in hospitals in the municipality of Francisco Beltrão - Paraná. The Maslach Burnout Inventory and a sociodemographic questionnaire were the instruments used to collect the data. The data analysis showed that the majority of the participants are male, married, with children, work on call and are satisfied with the institution where they work. Approximately half of them reported working more than 70h / week and not going on vacation in the last six months. The frequency of diagnosis of psychological disorder, consumption of anxiolytics, regular psychiatric appointments and intention to change profession varied between 20% and 25%. High levels of Emotional Exhaustion, Depersonalization and low Personal Accomplishment were found in 33.9%, 31.4% and 20.8% of respondents, respectively. The prevalence of high-level intensity in the three dimensions was 7.5%. The lowest values of Emotional Exhaustion were observed amongst the senior; those with workload between 40 and 60 hours; who do not work on call; who exercise regularly; who went on vacation in the last six months; those satisfied with the institution; with no diagnosis of psychological disorders; who don’t have regular appointments with a psychiatrist; and who do not consider changing their profession. Those who work longer hours, who attend more than 25 patients a day, who are dissatisfied with the institution and considering changing their profession showed higher Depersonalization values. Higher values of Personal Accomplishment were observed among participants with lesser weekly workload, non-outsourced workers, who do not work on call, who exercise regularly, who went on vacation less than six months ago, who are satisfied with the institution, who don’t consider changing their profession. Lastly, better indicators of the general score of Burnout Syndrome were observed in those with less workload, who exercise regularly, who went on vacation less than six months ago, who are satisfied with the institution, who have no diagnosis of psychological disorder, who have not had regular psychiatric appointments and who do not consider changing their profession. A síndrome de Burnout deriva da vivência profissional em um contexto com muitos estressores emocionais e relações sociais complexas, resultado da relação entre o profissional e o seu ambiente de trabalho. Se apresenta em três dimensões: exaustão emocional, despersonalização e realização pessoal. As profissões mais predispostas ao desenvolvimento de Síndrome de Burnout são aquelas cujas atividades têm envolvimento emocional, como profissionais que trabalham diretamente com outras pessoas, assistindo-as, ou como responsáveis pelo desenvolvimento e bem-estar. Uma carreira profissional que é suscetível a desenvolver a Síndrome de Burnout é a do médico, constituindo um fenômeno de alta prevalência entre estes profissionais. O presente estudo teve como objetivo geral estimar a prevalência e fatores associados à Síndrome de Burnout em médicos(as) dos hospitais de Francisco Beltrão – Paraná, e como objetivos específicos traçar o perfil desses profissionais da saúde; avaliar os níveis de Exaustão Emocional, Despersonalização e Realização Pessoal; identificar a relação entre Burnout e características sociodemográficas, laborais e psicossociais, e descrever as características dos profissionais com Síndrome de Burnout. Trata-se de um estudo descritivo e quantitativo envolvendo 121 médicos que trabalham nos hospitais no município de Francisco Beltrão – Paraná. Utilizou-se como instrumento para coleta dos dados o Maslach Burnout Inventory e o questionário sociodemográfico. A análise dos dados apontou que a maioria dos participantes é do sexo masculino, casados, com filhos, trabalham em regime de plantão e estão satisfeitos com a instituição em que trabalham. Aproximadamente metade deles reportaram trabalhar mais de 70h/semana e não terem tido férias nos últimos seis meses. A frequência de diagnóstico de transtorno psicológico, de consumo de ansiolíticos, atendimento com psiquiatra e intenção em mudar de profissão variou entre 20% e 25%. Níveis elevados de Exaustão Emocional, Despersonalização e baixa Realização Pessoal foram encontrados em 33,9%, 31,4% e 20,8% dos entrevistados, respectivamente. A prevalência de nível elevado nas três dimensões foi 7,5%. Os menores valores de exaustão emocional foram observados entre os mais velhos, carga horária entre 40h e 60h, que não trabalham em regime de plantão, que fazem exercício físico, que tiraram férias há menos de seis meses, satisfeitos com a instituição, sem diagnóstico de transtorno psicológico, que não consultam um psiquiatra, e que não consideram mudar de profissão. Os que trabalham em maior carga horária, que atendem mais de 25 pacientes por dia, que estão insatisfeitos com a instituição e que consideram mudar de profissão apresentaram maiores valores de despersonalização. Maiores valores de realização pessoal foram observados entre os participantes com menor carga horária semanal, os não terceirizados, que não trabalham em regime de plantão, que fazem exercício físico, que tiraram férias há menos de seis meses, estão satisfeitos com a instituição, que não consideram mudar de profissão. Por fim, melhores indicadores do escore geral de síndrome de Burnout foram observados naqueles com menor carga horária de trabalho, que fazem exercício físico, que tiraram férias há menos de seis meses, estão satisfeitos com a instituição, não têm diagnóstico de transtorno psicológico, não consultaram psiquiatras e não consideram mudar de profissão.
