2. The course of post-stroke apathy in relation to cognitive functioning: a prospective longitudinal cohort study.

01 januari 2023, Item does not contain fulltext, Apathy is common after stroke and has been associated with cognitive impairment. However, causality between post-stroke apathy and cognitive impairment remains unclear. We assessed the course of apathy in relation to changes in cognitive functioning in stroke survivors. Using the Apathy Scale (AS) and cognitive tests on memory, processing speed and executive functioning at six- and 15 months post-stroke we tested for associations between (1) AS-scores and (change in) cognitive scores; (2) apathy course (persistent/incident/resolved) and cognitive change scores. Of 117 included participants, 29% had persistent apathy, 13% apathy resolving over time and 10% apathy emerging between 6-15 months post-stroke. Higher AS-scores were cross-sectionally and longitudinally associated with lower cognitive scores. Relations between apathy and cognitive change scores were ambiguous. These inconsistent relations between apathy and changes in cognition over time suggest that post-stroke apathy does not directly impact cognitive performance. Both these sequelae of stroke require separate attention.

Contains fulltext : 282309.pdf (Publisher’s version ) (Open Access), OBJECTIVE: Cerebral venous thrombosis (CVT) caused by vaccine-induced immune thrombotic thrombocytopenia (VITT) is a rare adverse effect of adenovirus-based severe acute respiratory syndrome-coronavirus 2 (SARS-CoV-2) vaccines. In March 2021, after autoimmune pathogenesis of VITT was discovered, treatment recommendations were developed. These comprised immunomodulation, non-heparin anticoagulants, and avoidance of platelet transfusion. The aim of this study was to evaluate adherence to these recommendations and its association with mortality. METHODS: We used data from an international prospective registry of patients with CVT after the adenovirus-based SARS-CoV-2 vaccination. We analyzed possible, probable, or definite VITT-CVT cases included until January 18, 2022. Immunomodulation entailed administration of intravenous immunoglobulins and/or plasmapheresis. RESULTS: Ninety-nine patients with VITT-CVT from 71 hospitals in 17 countries were analyzed. Five of 38 (13%), 11 of 24 (46%), and 28 of 37 (76%) of the patients diagnosed in March, April, and from May onward, respectively, were treated in-line with VITT recommendations (p < 0.001). Overall, treatment according to recommendations had no statistically significant influence on mortality (14/44 [32%] vs 29/55 [52%], adjusted odds ratio [OR] = 0.43, 95% confidence interval [CI] = 0.16-1.19). However, patients who received immunomodulation had lower mortality (19/65 [29%] vs 24/34 [70%], adjusted OR = 0.19, 95% CI = 0.06-0.58). Treatment with non-heparin anticoagulants instead of heparins was not associated with lower mortality (17/51 [33%] vs 13/35 [37%], adjusted OR = 0.70, 95% CI = 0.24-2.04). Mortality was also not significantly influenced by platelet transfusion (17/27 [63%] vs 26/72 [36%], adjusted OR = 2.19, 95% CI = 0.74-6.54). CONCLUSIONS: In patients with VITT-CVT, adherence to VITT treatment recommendations improved over time. Immunomodulation seems crucial for reducing mortality of VITT-CVT. ANN NEUROL

Item does not contain fulltext, BACKGROUND: Vilobelimab, an anti-C5a monoclonal antibody, was shown to be safe in a phase 2 trial of invasively mechanically ventilated patients with COVID-19. Here, we aimed to determine whether vilobelimab in addition to standard of care improves survival outcomes in this patient population. METHODS: This randomised, double-blind, placebo-controlled, multicentre phase 3 trial was performed at 46 hospitals in the Netherlands, Germany, France, Belgium, Russia, Brazil, Peru, Mexico, and South Africa. Participants aged 18 years or older who were receiving invasive mechanical ventilation, but not more than 48 h after intubation at time of first infusion, had a PaO(2)/FiO(2) ratio of 60-200 mm Hg, and a confirmed SARS-CoV-2 infection with any variant in the past 14 days were eligible for this study. Eligible patients were randomly assigned (1:1) to receive standard of care and vilobelimab at a dose of 800 mg intravenously for a maximum of six doses (days 1, 2, 4, 8, 15, and 22) or standard of care and a matching placebo using permuted block randomisation. Treatment was not continued after hospital discharge. Participants, caregivers, and assessors were masked to group assignment. The primary outcome was defined as all-cause mortality at 28 days in the full analysis set (defined as all randomly assigned participants regardless of whether a patient started treatment, excluding patients randomly assigned in error) and measured using Kaplan-Meier analysis. Safety analyses included all patients who had received at least one infusion of either vilobelimab or placebo. This study is registered with ClinicalTrials.gov, NCT04333420. FINDINGS: From Oct 1, 2020, to Oct 4, 2021, we included 368 patients in the ITT analysis (full analysis set; 177 in the vilobelimab group and 191 in the placebo group). The phase behavior of a suspension of platelike colloids, which have a broad-size distribution and exhibit isotropic-nematic phase separation over a wide range of particle concentrations, is studied. Abstract

The Machine Directive, which contains the Health and Safety Requirements to warrant safe design of industrial (and commercial) machines, is currently being evaluated for amendment. These amendments should ensure that the Machine Directive is relevant for all machines that could be introduced on the work floor in the following 30 years. In this paper, we specifically focus on what additional requirements could be included in the Machine Directive to prepare it for introduction to the work floor of machines with Machine Learning. The requirements for the Machine Directive in its current form are insufficient to warrant the safe design of machines that have become learning entities. Based on a desk study and interviews and workgroups with experts in robotics, nine potential supplementary Essential Health & Safety Requirements (in short EHSR) were defined. Although it will be a difficult task to prepare the Machine Directive for the coming 30 years, we believe that with the proposed supplementary EHSR, the 'safe design' of many future machine learning applications would be ensured.

Abstract

Purpose of review Although cities present opportunities for infectious pathogens such as HIV to spread, public health infrastructure within these cities also provides opportunities to design effective approaches to eliminate transmission of these pathogens. The HIV Transmission Elimination AMsterdam (H-TEAM) Initiative, a consortium of relevant stakeholders involved in HIV prevention and care, designed an integrated approach to curb the HIV epidemic in Amsterdam, including providing preexposure prophylaxis (PrEP), increasing awareness of acute HIV infection, offering same-day test and treat, and improving indicator disease-driven HIV testing. Recent findings In 2013, approximately 230 people in Amsterdam were newly diagnosed with HIV, largely belonging to one of two key affected populations, namely MSM and people with a migration background. Since the start of H-TEAM in 2014, a decrease in new diagnoses was observed (130 in 2017), with an increasing proportion of MSM who had been diagnosed with a recent infection. Summary The H-TEAM shows that a city-based concerted effort is feasible. However, major challenges remain, such as reducing the number of late HIV diagnoses, and identifying and providing appropriate services to a diminishing group of individuals who are likely the source of transmission.

Abstract

This correlational survey study examined the effect of gender on work-family guilt mediated by career gains and family losses. Participants included 218 mothers and 103 fathers from The Netherlands. Evidence is found for the hypothesis that women experience more work-family guilt than men do. Furthermore, this effect seems to be mediated by the perception that family suffers more from the mother’s absence than from the father’s absence. As a result of these gender differences in work-family guilt, women may take higher strain than men, which possibly leads to gendered differences in work-related decisions and to negative health outcomes in mothers.

De werkgever zorgt voor de veiligheid en de gezondheid van de werknemers inzake alle met de arbeid verbonden aspecten. Heeft u het functioneren van uw cobots en het daaraan verbonden risico voor uw medewerkers op hun werkplek volledig onder controle? Als gevolg van technologische ontwikkelingen op het gebied van leren met behulp van AI zullen organisaties op het gebied van robotisering grote stappen kunnen zetten in termen van efficiëntie. Er zullen hierdoor meer cobots op de werkvloer verschijnen. Daarbij zullen (op termijn) de primaire fysieke barrières, die mens en machine scheiden, op collaboratieve mens-robot systemen niet meer van toepassing zijn. Gezien de snelle technologische ontwikkelingen en mogelijkheden moeten bedrijven en organisaties echter blijven anticiperen en meegroeien om eventuele incidenten tussen robots en mensen voor te zijn. Het ontwerp voor de toepassing van de cobot in de gedeelde werkomgeving zal inherent veilig moeten zijn. De cobot functioneert namelijk in een zogenaamd cobot-mens–omgeving-systeem. In dit systeem werken cobot en mens als team samen in een gedeelde werkomgeving, met elk hun eigen taken en verantwoordlijkheden, die onderlinge afstemming en communicatie vergen. In een recent rapport heeft TNO samen met het ministere van Sociale Zaken en Werkgelegenheid een onderzoek gedaan naar risicobeheersfactoren voor arbeidsveiligheid in samenwerking met de Automated Guided Vehicles (AGV’s), die een basis voor de risico’s en risicobeheersmaatregelen voor veilige mens-cobot-omgeving interactie vormen. Deze kenniskaart is een praktisch hulpmiddel voor ondernemers, werkgevers en werknemers hoe een werkplek veilig en gezond kan worden ingericht bij gebruik van cobots op de werkvloer.

De robotindustrie is bezig met een enorme snelheid een opmars te maken in diverse arbeidsdomeinen van zorg tot maakindustrie. In de afgelopen vijftig jaar zijn industriële machines niet alleen sneller en accurater geworden, maar zijn deze machines ook mobieler geworden. Daarnaast hebben deze machines meer bewegingsgraden gekregen, en kunnen deze machines dankzij verbeterde sensoren steeds beter met hun omgeving omgaan. Verder zien we een toenemende digitalisering (waarbinnen de ontwikkelingen parallel lopen aan die van automatisering; Eurofound, 2018), dit houdt in dat (industriële) machines steeds vaker via netwerken aan elkaar en aan het internet worden gekoppeld. Het is belangrijk om bij deze trends vooruit te blijven kijken en de machineveiligheid van morgen te definiëren, zodat een robot al in het ontwerp en de ontwikkeling ervan inherent veilig kan worden gemaakt.

Item does not contain fulltext Psychopathie geldt in de forensische psychiatrie als een belangrijk concept. In dit hoofdstuk over diagnostiek is er allereerst aandacht voor de recente discussies over de betekenis en de conceptualisering van dit begrip (zie ook hoofdstuk 1 en 5). Vervolgens gaan we in op verschillende methoden voor het diagnosticeren van psychopathie. Men kan voor de classificatie een checklist scoren op basis van een interview met betrokkene en collaterale informatie. Hiervoor worden wereldwijd de PCL-R en van de PCL-R afgeleide instrumenten gebruikt. De uitgebreide diagnostiek die de pcl-r vereist is in de praktijk echter niet altijd haalbaar, vandaar dat er de laatste decennia verscheidene zelfrapportage instrumenten ontwikkeld zijn. Zelfrapportage van psychopathische trekken staat nog relatief in de kinderschoenen, maar heeft wel een aantal voordelen ten opzichte van observatie. In paragraaf 3 gaan we in op de psychometrische eigenschappen van de diverse instrumenten. Van de PCL-R komen ook de psychometrische eigenschappen in de praktijk aan de orde, alsmede de wijze waarop in diverse contexten van dit instrument gebruik wordt gemaakt. In de conclusie formuleren we enige aanbevelingen om de diagnostiek met behulp van de PCL-R te verbeteren en voor voortgezet onderzoek, onder meer op het gebied van zelfrapportage van psychopathische trekken.

Abstract

Veel bedrijven in met name de petrochemische industrie zijn er de afgelopen jaren in geslaagd het aantal ongevallen sterk terug te dringen. Een van de middelen die is gebruikt om deze verbetering te bewerkstelligen, is de zogenaamde Kwantitatieve Risico Analyse (QRA). Een QRA is een instrument om de risico's voor een bepaalde activiteit te kwantificeren. Voor hoog-risicobedrijven (BRZO-bedrijven) is het opstellen van een QRA zelfs wettelijk verplicht maar ook in andere contexten worden QRA's gemaakt.

Abstract

Al decennia lang spreken robots tot de verbeelding in toekomstvisioenen in films en boeken. Karel Čapek schreef al in 1920 een toneelstuk genaamd RUR (Rossums Universele Robots). De eerste echte robot ‘Gargantuan’ werd in de periode 1935 tot 1937 geconstrueerd. Deze was volledig opgebouwd uit Meccano® 1. De huidige industriële robots lijken nog veel op de versie die in 1961 in de autoproductielijnen van General Motors zijn geïntroduceerd2. Robots zijn in de afgelopen vijftig jaar wel veel sneller en nauwkeuriger geworden, maar het blijven nu nog vaak locatie- of rail gebonden machines die automatisch een (enkele) taak uitvoeren binnen een vastgestelde gevarenzone of binnen een veiligheidskooi. In het kader op de volgende pagina worden enkele voorbeelden gegeven van de toepassing van hedendaagse industriële robots in verschillende sectoren. Deze robots staan nog ver weg van de intelligente en autonome robots zoals in sciencefiction boeken en films worden neergezet. Door deze verhalen is de beeldvorming van robots voor veel personen gericht op machines met een menselijke vorm, die zich zichzelf kunnen voortbewegen, met personen om kunnen gaan en op hun omgeving kunnen reageren. Ondanks bovenstaande beperkingen introduceren de huidige industriële robots nieuwe risico’s op de werkvloer ondanks de definiëring van veiligheidszones of kooien. Dit werd afgelopen jaar opnieuw benadrukt na een dodelijk ongeluk waarbij een werknemer door een robot werd doodgedrukt3. Aangezien industriële robots in toenemende mate in gebruik worden genomen in de land- en tuinbouw, maakindustrie en distributiemagazijnen kunnen dit soort arbeidsincidenten zich in de toekomst vaker gaan voordoen.

Abstract

For decades now, robots have been a key part of future visions in films and books. As long ago as 1920, Karel Čapek wrote a play called RUR (Rossum’s Universal Robots). The first real robot, ‘Gargantuan’, was constructed between 1935 and 1937. It was made completely out of Meccano. Today’s industrial robots strongly resemble those introduced on General Motors’ car production lines in 19612. In the past fifty years, robots have become much faster and more accurate, but in many cases they do no more than operate in one particular location or on rails, automatically carrying out perhaps only a single task within a fixed hazardous zone or inside a safety cage. The box on the next page shows several examples of the application of modern-day industrial robots in different sectors. These robots are still a far cry from the intelligent and autonomous robots described in science-fiction films and books. It is because of these depictions that many people imagine robots as being like humans - able to move independently, to interact with people, and to respond to their surroundings.

Abstract

The ever-greater integration of new technologies in work equipment is regarded by some as the fourth industrial revolution1 and referred to as ‘smart industry’2. Examples that come to mind include automation through the use of embedded software, remotely controlling heavy materials, and the connecting of work equipment to local and public networks such as the internet (for some examples, see the text box on page 6). As well as the opportunities this entails for industry, new threats are also presenting themselves. This includes employees who have to move among robots or autonomous freight vehicles, but also malicious actors who can penetrate computers and computer networks and thereby disrupt processes or bring them to a standstill. The focus in this report lies on the connection between work equipment and cyberspace, including connections to local and public networks such as the internet.

De toenemende integratie van nieuwe technologieën in arbeidsmiddelen wordt ook wel gezien als de vierde industriële revolutie1 en naar verwezen als smart industry2. Denk hierbij aan automatisering door middel van embedded software, het op afstand bedienen van zwaar materiaal en de koppeling van arbeidsmiddelen aan lokale en publieke netwerken waaronder het internet (zie kader volgende pagina voor voorbeelden). Naast de mogelijkheden die deze ontwikkelingen met zich meebrengen voor de industrie, ontstaan ook nieuwe dreigingen. Denk bijvoorbeeld aan personeel dat tussen robots of automatisch rijdende vrachtwagens moet manoeuvreren of kwaadwillenden die van afstand computers en computernetwerken binnen kunnen dringen en daardoor processen kunnen verstoren of stilleggen. De focus in dit rapport ligt op de koppeling van arbeidsmiddelen en ”cyberspace” waaronder koppelingen met lokale en publieke netwerken zoals het internet.

Item does not contain fulltext BACKGROUND: In adults with acute stroke, infections occur commonly and are associated with an unfavourable functional outcome. In the Preventive Antibiotics in Stroke Study (PASS) we aimed to establish whether or not preventive antimicrobial therapy with a third-generation cephalosporin, ceftriaxone, improves functional outcome in patients with acute stroke. METHODS: In this multicentre, randomised, open-label trial with masked endpoint assessment, patients with acute stroke were randomly assigned to intravenous ceftriaxone at a dose of 2 g, given every 24 h intravenously for 4 days, in addition to stroke unit care, or standard stroke unit care without preventive antimicrobial therapy; assignments were made within 24 h after symptom onset. The primary endpoint was functional outcome at 3 months, defined according to the modified Rankin Scale and analysed by intention to treat. The primary analysis was by ordinal regression of the primary outcome. Secondary outcomes included death, infection rates, antimicrobial use, and length of hospital stay. Participants and caregivers were aware of treatment allocation but assessors of outcome were masked to group assignment. This trial is registered with controlled-trials.com, number ISRCTN66140176. FINDINGS: Between July 6, 2010, and March 23, 2014, a total of 2550 patients from 30 sites in the Netherlands, including academic and non-academic medical centres, were randomly assigned to the two treatment groups: 1275 patients to ceftriaxone and 1275 patients to standard treatment (control group). 12 patients (seven in the ceftriaxone group and five in the control group) withdrew consent immediately after randomisation, leaving 2538 patients available for the intention-to-treat-analysis (1268 in the ceftriaxone group and 1270 in the control group). 2514 (99%) of 2538 patients (1257 in each group) completed 3-month follow-up. Preventive ceftriaxone did not affect the distribution of functional outcome scores on the modified Rankin Scale at 3 months (adjusted common odds ratio 0.95 [95% CI 0.82-1.09], p=0.46). Contains fulltext : 169802.pdf (publisher's version ) (Closed access), The tenascin-X (TNX) deficient type Ehlers-Danlos syndrome (EDS) is similar to the classical type of EDS. Because of the limited awareness among geneticists and the challenge of the molecular analysis of the TNXB gene, the TNX-deficient type EDS is probably to be under diagnosed. We therefore performed an observational, cross-sectional study. History and physical examination were performed. Results of serum TNX measurements were collected and mutation analysis was performed by a combination of next-generation sequencing (NGS), Sanger sequencing and multiplex ligation-dependent probe amplification (MLPA). Included were 17 patients of 11 families with autosomal recessive inheritance and childhood onset. All patients had hyperextensible skin without atrophic scarring. Hypermobility of the joints was observed in 16 of 17 patients. Deformities of the hands and feet were observed frequently. TNX serum level was tested and absent in 11 patients (seven families). Genetic testing was performed in all families+ADs- 12 different mutations were detected, most of which are suspected to lead to non-sense mRNA mediated decay. In short, patients with the TNX-deficient type EDS typically have generalized joint hypermobility, skin hyperextensibility and easy bruising. In contrast to the classical type, the inheritance pattern is autosomal recessive and atrophic scarring is absent. Molecular analysis of TNXB in a diagnostic setting is challenging.

In dit opinierende artikel wordt de waarde van de nieuwe concepten 'resilience engineering' en 'teamresilience' uiteen gezet als complementaire benaderingen van de meer traditionele methoden van veiligheidsmanagement. De auteurs betogen dat resilience of veerkracht, een onmisbare kwaliteit is voor hedendaagse bedrijven om duurzaam en veilig te blijven opereren in een con- tinu veranderende en complexer wordende wereld. Daarbij lijken teams de meest voor de hand liggende functionele eenheden te zijn die bedrijven in staat stellen om zich aan te passen aan de eisen van de voortdurend veranderende interne en externe omgeving. Veerkracht is met name van belang voor teams die opereren in een setting waar het maken van fouten ern- stige consequenties kan hebben voor mens en materieel. Dat stelt hoge eisen aan het team om blijvend adequaat te kunnen presteren. Prestaties die zowel gericht zijn op productie en rendement als op het in stand houden van bedrijfszekerheid en veiligheid van mensen en processen. Dit wordt bereikt doordat teams het vermogen hebben om hun prestaties actief aan te passen aan de gestelde eisen en aan de onverwachte situaties waarmee zij in de dagelijkse operatie kunnen worden geconfronteerd. De auteurs sluiten af met de huidige status van hun onderzoek en de vragen voor toekomstig onderzoek.
