Background Spinal anesthesia (SA) is widely administered in cesarean sections (CS), and post-dural puncture headache (PDPH) is the most unavoidable complication after the Dural or lumbar puncture (LP). Some factors are considered to be associated with the risk of recurrent PDPH. Aims To investigate the recurrent headaches after SA and their link to associated factors of age, Body Mass Index (BMI), Fast Blood Sugar (FBS), Blood Pressure (BP), time of immobilization in the supine position after SA, and a history of previous SA of PDPH of women candidates for CS, when the needle type and size were kept constant. Methods In a cross-sectional descriptive study, 80 women candidates for CS were recruited from Besat Hospital, Sanandaj, Iran. The association between age, BMI, FBS, BP, time of immobilization in the supine position after spinal anesthesia, and a history of previous SA and recurrent PDPH of women candidates for CS were investigated using statistical t-tests and chi-square test. Results The t-test and chi-square test showed no association between the quantitative and qualitative variables and PDPH after SA in women candidates for CS respectively. Conclusion The results of this study showed when the needle type and size were kept constant, the associated factors of age, BMI, FBS, BP, time of immobilization in the supine position after SA, and a history of previous SA and PDPH of women candidates for CS had no link to the recurrent headaches after SA.

Due to the critical and important structures in the thorax, transsternal surgery on thoracic vertebrae is very complex and rarely performed. We report a case of successful transsternal surgery on thoracic vertebrae. In this study, a 27-year-old man presented to the hospital with progressive lower extremity complaints. Pathologic lesions and severe destruction of the T3 vertebra were diagnosed. Considering the location of the lesion, an anterior approach was performed. The results of this operation have been very successful and have some noteworthy points. In this study, transsternal surgery is supported in patients with anterior vertebral conditions.

The use of Surgicel is one of the methods used to stop bleeding during various surgical operations, especially neurosurgery. The use of Surgicel may cause complications such as inflammation, delayed tissue repair, and necrosis. This case is a 60-year-old woman with a history of craniotomy and reaction to Surgicel who presented to the hospital with symptoms of seizures. The patient underwent craniotomy after a mass diagnosis in the parasagittal region. After reviewing this case, we concluded that the use of Surgicel can have complications for patients, Therefore, it is recommended to use Surgicel in the patient's body so that it is not treated as a mass, abscess, or hematoma in the postoperative period.

Background: Children with brain tumors experience potential neurologic complications when are treated with radiotherapy, especially if done frequently under anesthesia. Objectives: This study aimed to evaluate the neurologic complications associated with anesthesia in pediatrics treated with radiotherapy under anesthesia. Methods: This cross-sectional study consisted of 133 pediatric patients with a brain tumor who needed anesthesia for performing radiotherapy and were referred to Omid Charity Hospital and Imam Khomeini University Hospital from 2014 to 2020 by the census. Statistical values less than 0.05 were considered significant (P < 0.05). Results: The patients were in range of 1 - 8 years, and the number of 3,208 anesthesia inductions were conducted for daily radiotherapy. Major complications such as stroke, arrhythmia, tachyphylaxis, and aspiration were not observed. There was a significant relationship between the tumor side, anesthesia, and neurologic complications (P < 0.05). No significant differences were found between demographic data (age, sex, and weight) (P > 0.05). Conclusions: Anesthesia complications in this study were slight and insignificant. Some were either due to the tumor effect on other vital organs or prior exposure to radiotherapy and chemotherapy. Thus, for providing safe anesthesia, considering the tumor effect on body organs and neurologic complications caused by it can be a great help to reduce anesthesia complications in pediatrics under radiotherapy.

Backgrounds and objectives: Acute postoperative pain is still an issue in patients undergoing lumbar laminectomy surgery. We compared the effects of remifentanil and dexmedetomidine on pain intensity as well as the analgesic requirements in patients undergoing spinal surgery. Methods: This randomized clinical trial study was conducted on 60 patients in the age range of 20 to 60 years with ASA class one and two of the American Anesthesia Association under elective lumbar vertebrae laminectomy. Patients were divided into two groups: Remifentanil (group R) and Dexmedetomidine (group D). Before induction of anesthesia in R group patients, 0.01 می g / kg / min remifentanil was used. In group D, 0.3 dexmedetomidine was injected from µg / min / 50kg using a syringe pump. Postoperative pain score, nausea, and vomiting, mean arterial blood pressure, and heart rate at recovery times, 6, 12, and 24 hours after surgery were recorded. Results: The propofol infusion rate was significantly lower in the DEX group than in group R (P = 0.001). Pain scores except for 6h after surgery was significantly lower in group DEX. Purpose This study evaluates the effect of pregabalin on fasciculation and myalgia after using succinylcholine. Design This randomized double-blind prospective study was conducted among 100 patients aged 20 to 60 years old. Methods Pregabalin (300 mg) and placebo (in capsule form) were placed in similar containers. The results were analyzed by SPSS 23 software, and statistical analysis consisted of χ2 test and t test, and a P value less than .05 was considered significant. Findings The mean pain score in the group receiving pregabalin was lower than the placebo group. According to the χ2 test, there was a significant difference between the two groups in the frequency of fasciculation (P = .003). Mean fasciculation severity in the pregabalin group was lower than placebo group. According to t test, there was a significant difference in the mean fasciculation severity between the two groups (P = .002). Conclusions This study showed that 300 mg of pregabalin was effective in reducing postoperative fasciculation and myalgia in patients treated with succinylcholine.
