Giriş ve Amaç: Sağ kolon yerleşimli kanserler genellikle gaytada gizli kanamaya bağlı zamanla oluşan anemi semptomları ile karşımıza gelir. Özellikle 50-60 yaş civarında anemi bulguları olan hastalarda sağ kolon kanseri akla getirilmelidir. Amacımız aneminin, sağ kolon yerleşimli bir tümörün daha erken evrede tanı alması açısından önemini vurgulamaktır.Gereç ve Yöntem: Araştırmamız 01.01.2012 ve 01.02.2018 tarihleri arasında S.B.Ü Gaziosmanpaşa SUAM Tıbbi Patoloji laboratuarında kolorektal karsinom tanısı almış ve Genel Cerrahi kliniğince opere edilen, patoloji raporu ve ameliyat öncesi tam kan sayımı mevcut, 18 yaş üzeri 54 hasta ile yapılmıştır. Tanı öncesindeki hemoglobin ve hematokrit değerleri ve operasyon sonrası patoloji raporları, TNM evresi, histolojik grade, invazyon derinliği, lenfovasküler invazyon, perinöral invazyon, tümör boyutu, nodal tutulum, metastaz ve hastaların demografik özellikleri hasta dosyaları üzerinden retrospektif olarak kayıt altına alınmıştır. Hemoglobin değeri erkeklerde 13 g/dL, kadınlarda 12 g/dL’nin altı anemi kabul edilmiştir.Bulgular: Hastaların %88.9’unda anemi saptanmıştır. Perinöral invazyon görülen olgularda hemoglobin düzeyleri, perinöral invazyon görülmeyen olgulardan istatistiksel olarak anlamlı düzeyde yüksek bulunmuştur. Lenf nodu metastazına göre hemoglobin değerleri arasında anlamlı bir ilişki bulunmamışken lenf nodu metastazı N2 ve N1 olan olgularda N0 olanlara göre hematokrit değerleri anlamlı şekilde yüksektir. Çalışmamızda tüm evrelerdeki sağ yerleşimli kolon kanserli hastalarda anemi sıklıkla saptanmış olup TNM evre I kanserde dahi bu oran %60’tır.Sonuç: Sağ yerleşimli kolon kanserinde anemi sık rastlanılan bir bulgudur. Anemisi olan kadın ve erkek hastalar için kolonoskopi ve üst gis endoskopisi yapılması, kanserin erken evrede saptanarak kansere bağlı mortalite ve morbiditenin azaltılabileceğini düşündürmektedir.

Amaç: Lokal ileri mide kanserinde (LİMK) tedaviye neoadjuvan kemoterapi (NAK) ile başlanması önerilir. Ancak NAK’nin faydaları gösterilmiş olsa da hangi hasta gurubunun tedaviye daha iyi yanıt verdiği tam olarak anlaşılamamıştır. Çalışmamızda hematolojik ve histopatolojik parametrelerin kemoterapi yanıtı üzerine etkisi araştırılmıştır. Yöntem ve Gereç: Bu çalışmada lokal ileri evre mide kanseri nedeni ile neoadjuvan kemoterapi görüp sonrasında ameliyat edilen 38 hasta retrospektif olarak incelendi. Ameliyat sonrası patolojik yanıt ile hastanın demografik özellikleri, tümörün endoskopik biyopsideki histopatolojik özellikleri, preoperatif hemoglobin seviyesi, nötrofil lenfosit oranı karşılaştırılıp hangi özellikteki tümörlerin daha iyi yanıt verdiği değerlendirildi. Bulgular: Ameliyat sonrası yapılan histopatolojik değerlendirmede 2 (%6) hastada patolojik tam yanıt, 9 (%24) hastada grade 1 yanıt, 13 (%34) hastada grade 2 yanıt ve 14 hastada (%36) grade 3 yanıt mevcuttu. Patolojik tam yanıt ile histopatolojik olarak perinöral invazyon olmaması istatistiksel olarak anlamlı ilişki gösterdi (p=0.023). Sonuç: Çalışmamızda NAK alan LİMK’li hastalarda perinöral invazyon varlığının kemoterapiye kötü yanıt ile ilişkisi olduğu görülmüştür. Ayrıca lenfovaskular invazyon ve düşük grade mevcudiyetinde istatistiksel olarak anlamlı olmasa da kemoterapiye kötü yanıt gözlenmiştir., Objective: In the treatment of local advanced gastric cancer (LAGC), it is recommended to start with neoadjuvant chemotherapy (NAC). Although the benefits of NAC have been shown, it is still not fully understood which patients respond better. The aim of this study was to investigate the effect of hematological and histopathological parameters on the response to chemotherapy. Material and Method: A retrospective examination was made of 38 patients who underwent surgery for LAGC after receiving NAC. Evaluations were made by comparing the demographic characteristics, histopathological characteristics in an endoscopic biopsy of the tumor, preoperative hemoglobin levels, and neutrophil-lymphocyte ratios with the postoperative pathological response to determine which tumor characteristics gave a better response. Results: In the postoperative histopathological evaluation, there was a pathological complete response to chemotherapy in two patients (6%), grade 1 in 9 patients (24%), grade 2 in 13 patients (34%), and grade 3 in 14 patients (36%). A statistically significant relationship was determined between the histopathological absence of perineural invasion and pathological complete response (p=0.023). Conclusion: A relationship between perineural invasion and poor response to chemotherapy was determined. Although not at a statistically significant level, there was also observed to be a poor response to chemotherapy in the presence of low grade and lymphovascular invasion.

Amaç: Fibrin glue (FbGl) ve siyanoakrilatın (CyAc) maksiller sinüs mukozası üzerine histopatolojik etkilerini karşılaştırmak. Yöntemler: Çalışmaya 20 adet tavşan alındı ve her biri 10 adet olan iki gruba ayrıldı. Her iki grubun sağ maksiller sinüslerinde oluşturulan cerrahi defektleri birinci grupta (Grup 1) FbGl kullanılarak ikinci grupta (Grup 2) ise CyAc kullanılarak tedavi edildi. Her iki grubun sol maksiller sinüsleri steril salin solüsyonu ile tedavi edilerek kontrol grubu (Grup 3) olarak kullanıldı. Siyanoakrilat uygulanan ve çalışma sırasında ölen bir tavşan çalışma dışında bırakıldı. Postoperatif 21. gün bütün tavşanlar sakrifiye edildi. Her üç grubun maksiller sinus mukozasında histopatolojik olarak inflamasyon ve fibrozis yaygınlığı, yabancı cisim reaksiyonu, silia kaybı, mukoza altındaki kemikte osteogenezis artışı ve seröz gland kaybı oranları değerlendirildi. Bulgular: Her üç grup karşılaştırıldığında Grup 1(FbGl) istatistiksel olarak diğer iki gruptan daha yüksek oranda anlamlı inflamasyona (p, Objective: To compare the histopathological effects of fibrin glue (FbGl) and cyanoacrylate (CyAc) on the maxillary sinus mucosa. Methods: Twenty rabbits were divided into two groups of 10, and surgical defects were created in the maxillary sinuses. The right maxillary sinus was treated with FbGl in one group and with CyAc in the other group. As a control, the left maxillary sinuses of all rabbits were treated with sterile saline solution. One rabbit treated with CyAc died during the study and was excluded. On postoperative day 21, all animals were sacrificed. Maxillary sinus mucosa samples were studied to determine the extent of inflammation and fibrosis, foreign body reaction, cilia loss, increased osteogenesis in bony structures under the mucosa, and loss of serous glands. WOS: 000279339600006, Background and Design: Kaposi sarcomas (KS) are vascular tumors with a low malignant potential which include overlapping infectious, immunologic, and neoplastic processes. Recently, many histological subtypes have been defined. Material and Method: In the present study, 151 cutaneous classic KS lesions in 56 patients were retrospectively evaluated with regard to histological subtypes. Determination of the subtypes was based on the predominant histopathological component in the lesion. We examined changes in epidermis and dermis along with intratumoral inflammatory response characteristics in the lesions. By defining histopathological variants of the cases, differences regarding subtypes were investigated. Results: Cases that bear the ordinary characteristics of KS and those that can not be classified otherwise, comprised 82..8% of the study group. Twenty-six cases showed consistency with the subtypes outlined in the literature in terms of their histopathological properties. The most common histological subtype was the lymphangiectatic variant in 7.3% of the cases. Bullous (2.6%), lymphangioma like (2.6%), intravascular (2%), and pyogenic granuloma like (2%) variants were less common. The most uncommon histological subtype was micronodular (0.6%) type. Lymphangiectatic, bullous, intravascular, and pyogenic granuloma like variants were frequently observed in the nodular stage of KSs. Lympangioma like changes were seen to be present in the early KS lesions. Lymphangiectatic type was oftenly associated with bullous component, whereas pyogenic granuloma like type demonstrated superficial ulceration and intense inflammatory response. Lymphangioma like and intravascular types exhibited a characteristic appearance, while other variants were accompanied by components belonging to different subtypes. Conclusion: In KS, histopathological subtypes can develop as a result of different pathological processes. The next stage of the current study, which is one of the largest case series in the literature, will be investigation of the clinical and prognostic characteristics of the variants. Objective: This study aims to investigate the histopathological changes secondary to the administration of Ankaferd Blood Stopper® (ABS) into the auricular cartilage. Materials and methods: Both of the auricular cartilages of thirty New Zealand rabbits were marked with tattoo ink. A 0.2-cc ABS (study group, n: 30) and 0.2 cc physiological saline (control group, n: 30) were subcutaneously infused into the right auricle and left auricle, respectively. All layers were removed at 14 days. Results: The ABS group had significantly higher level of fibrosis, necrosis, foreign body reaction, inflammation, and cartilage degeneration, compared to the controls. Conclusion: Our study results showed that ABS administration into a closed cavity led to a significantly increased fibrosis and necrosis in the auricular cartilage.

Benign osteoclast-like multinuclear giant cells are rarely found in tumors other than bone and soft tissue neoplasms, and they are even rarer in squamous cell carcinomas. We examined a nasopharyngeal tumor from a 52-year-old female who had undergone surgery one year earlier for hypopharyngeal squamous cell carcinoma. Histopathologically, in addition to tumor infiltration by atypical epithelial cells with squamoid differentiation, giant cells with 10-20 nuclei and a large amount of eosinophilic cytoplasm were seen infiltrating the tumor. The giant cells did not show atypia or mitosis. Immunohistochemically, the tumor cells stained for pan-keratin and epithelial membrane antigen, and the giant cells were positive for leukocyte common antigen, CD68, and Mac 387. This case was diagnosed as moderately differentiated squamous cell carcinoma with multinuclear giant cells. In this case, the giant cells infiltrating the tumor were benign and of monocytic/histiocytic origin. Studies including large case series are needed to obtain reliable information on the clinical and prognostic importance of this histological feature., Benign osteoklast benzeri multinükleer dev hücrelerin, kemik ve yumuşak doku tümörleri dışındaki tümörlere eşlik etmesi nadir bir histopatolojik bulgudur. Skuamöz hücreli karsinomlarda ise çok daha nadiren saptanmaktadır. Bir yıl önce hipofarengeal skuamöz hücreli karsinom nedeniyle opere olan 52 yaşındaki kadın hastada, nazofarinkste saptanan tümöre biyopsi yapıldı. Histopatolojik olarak skuamoid diferansiasyon gösteren atipik epitelyal hücrelerin oluşturduğu tümöral infiltrasyonun yanı sıra tümör dokusunu infiltre eden, geniş eozinofilik sitoplazmalı ve ortalama 10-20 nükleus içeren dev hücreler dikkati çekti. Dev hücrelerde atipi ve mitoz izlenmedi. İmmünhistokimyasal olarak tümör hücrelerinde pankeratin ve EMA ile dev hücrelerde ise LCA, CD68 ve Mac 387 ile reaksiyon gözlendi. Olguya, multinükleer dev hücrelerin eşlik ettiği orta derecede diferansiye skuamöz hücreli karsinom tanısı verildi. Olgumuzda tümörü infiltre eden dev hücrelerin benign karakterde ve monositik/histiyositik kökenli olduğu saptandı. Geniş olgu serilerinde yapılacak çalışmalar, bu histolojik özelliğin klinik ve prognostik önemine yönelik güvenilir bilgiler sağlayacaktır.

OBJECTIVES This study examined the frequency of chronic lymphocytic thyroiditis (CLT) in thyroid papillary carcinomas (TPC) and the relationship between the presence of tumor-infiltrating lymphocytes (TIL) and CLT. METHODS The study included 61 TPC cases. In each case, the presence of the lymphocyte infiltration in the tumor tissue and nonneoplastic parenchyma was graded semi-quantitatively using a scale of 0 to 3. The cases were classified in two groups, TPC including CLT (Group I) and TPC not including CLT (Group II). RESULTS CLT was detected in 26.3% (Group I) of the TPCs and TIL was observed in 77% of the Group I cases; no lymphoid infiltration compatible with TIL was detected in Group II. A significant difference was observed in the TIL density between Groups I and II. CONCLUSION CLT is a frequent finding in TPC, and the presence of TIL is related to CLT., AMAÇ Bu çalışmada, tiroid papiller karsinomlarında (TPK), kronik lenfositik tiroidit (KLT) görülme sıklığı araştırıldı ve tümörü infiltre eden lenfosit (TİL) varlığının KLT ile ilişkisi değerlendirildi. GEREÇ VE YÖNTEM Çalışma kapsamına 61 TPK olgusu dahil edildi. Tümör içi ve tümör çevresi parankimdeki lenfoid infiltrasyon varlığı, her biri için ayrı şekilde, semi-kantitatif olarak 0-3 arasında derecelendi. Olgular grup I (KLT içeren TPK) ve grup II (KLT içermeyen TPK) olmak üzere iki gruba ayrıldı. BULGULAR TPK'ların %26.3'ünde (grup I) KLT saptandı. Grup I'deki olguların %77'sinde TİL görülürken, grup II'deki olgularda TİL ile uyumlu olarak değerlendirilebilecek lenfoid infiltrasyon saptanmadı. Grup I ve grup II'deki olgular arasında TİL yoğunluğu açısından anlamlı farklılık belirlendi. SONUÇ Bulgularımız, TPK'da KLT'nin sık görülen bir bulgu olduğunu ve TİL varlığının KLT ile ilişkili bir parametre olduğunu göstermektedir.

Aim: The aim of this study was to assess the size and shape characteristics of prostatic adenocarcinoma cell nuclei using a computer-assisted analysis system, and to compare the results with the Gleason score. Materials and Methods: Morphometric nuclear parameters, such as roundness factor, form ellipse, area, length, and perimeter, were evaluated based on specimen slides of 130 prostatic adenocarcinoma cases (77% needle biopsies and 23% prostatectomy specimens) using a computerized image analysis system. Correlation analysis between Gleason score and morphometric results was performed. Results: The Gleason score was correlated with mean nuclear area (r = 0.516, P = 0.01), mean nuclear length (r = 0.298, P = 0.01), and mean nuclear perimeter (r = 0.303, P = 0.01) for all specimens. In the needle biopsy group the Gleason score was correlated with mean nuclear area (r = 0.522, P = 0.01), mean nuclear length (r = 0.398, P = 0.01), and mean nuclear perimeter (r = 0.432, P = 0.01), whereas in the prostatectomy group the Gleason score was correlated only with mean nuclear area (s = 0.619, P = 0.01) and mean nuclear roundness factor (s = -0.425, P = 0.05). Conclusions: Nuclear size and shape factors, especially mean nuclear area, were concordant with the Gleason score. Nuclear size and shape assessment may aid in the evaluation of the pathological status of prostatic adenocarcinoma.

Amaç: Bu çalışmanın amacı Ankaferd “Blood Stopper”in (ABS) tükrük bezi dokusu üzerinde kanama durdurucu etki- sini incelemektir.? Gereç ve Yöntem: Çalışmada 15 erişkin Wistar Albino cin- si sıçan kullanıldı. Uygun diseksiyon işlemi sonrasında sağ submandibuler glandları tam kat kesilip 0.2 cc ABS damlatıl- dı. Sol glanda da aynı kesi işlemi sonrasında salin emdirilmiş tampon ile baskı yapıldı. İki tarafın kanama süreleri ölçüldü.? Bulgular: Kanama zamanı ABS uygulanan tarafta 21.9±8.9 saniye, salin emdirilmiş tampon ile baskı yapılan tarafta 89.8±33.9 saniye olarak bulundu. ABS uygulanan tarafta kanama zamanı anlamlı olarak kısa bulundu. Sonuç: Yeni bir hemostatik ajan olarak ABS’nin tükrük bezle- rinde kanamayı kontrol etmek için potansiyel bir yararı ola- bilir. Doku üzerinde histopatolojik etkilerini incelemek, olası toksik etkilerini belirlemek için daha çok ileriye dönük kont- rollü çalışmalara ihtiyaç vardır., Background: The aim of this study was to investigate the he- mostatic effect of Ankaferd Blood Stopper (ABS) on salivary gland tissue. Methods: Fifteen adult Wistar Albino rats were used in this study. After an appropriate dissection, the right submandibu- lar glands were incised through all layers and 0.2 cc of ABS was administered. After performing the same procedure on the left glands, compression was applied with a saline-moistened pack. The duration of bleeding on both sides was measured. Results: The duration of bleeding was 21.9±8.9 seconds in the ABS administered side, whereas the duration of bleeding was found to be 89.8±33.9 seconds in the side in which only a saline- moistened pack was applied. The duration of bleeding was sig- nificantly shorter in the ABS administered side. Conclusion: ABS as a novel hemostatic agent could have a potential benefit in controlling bleeding from salivary glands. Further prospective controlled studies are required to inves- tigate its pathological effects on the tissue and to determine possible toxic effects.

Konjenital melanositik nevüs, odontogenez sırasında ortaya çıkan ve nöral-krest’den gelişen hamartomatöz lezyonlardır. Bu çalışmada konjenital melanositik nevüsden gelişen ve gastrointestinal sistem metastazları ile bulgu veren iki malign melanoma olgusu sunulmaktadır. İlk olgu, plevral ve peritoneal efüzyon ile başvuran 71 yaşında kadın hasta olup, endoskopik incelenmede doudenumda nodüler lezyonlar saptanmış ve biyopsi yapılmıştır. Karın ağrısı yakınması ile başvuran ikinci olgu 36 yaşında erkek hasta olup, ileumda invajinasyonlara neden olan kitlesel lezyonlar nedeniyle segmenter ileal rezeksiyon uygulanmıştır. Lezyonların histopatolojik incelemesinde tüm mukoza katlarını infiltre eden diffüz neoplastik infiltrasyon görülmüştür. Belirgin atipi ve pleomorfizm içeren neoplastik hücrelerde S-100, HMB-45 ve Melan A immünreaksiyon saptanmıştır. Her iki olguya, malign melanoma tanısı verilmiştir. İlk olguda karın derisinde, ikinci olguda ise femoral bölgede konjenital melanositik nevüs saptanmış ve histopatolojik incelemelerde bu lezyonların malign melanoma yönünde transformasyon gösterdiği belirlenmiştir. Gastrointestinal sistemde malign melanoma gelişimi primer veya metastatik kökenli olabilir. Kesin tanı daima ayrıntılı klinik, histopatolojik ve immünhistokimyasal incelemeleri gerektirir., Congenital melanocytic nevi are hamartomatous lesions that develop from the neural crest and arise during odontogenesis. In this report, we present two malignant melanoma cases developed from congenital melanocytic nevi and revealed by gastrointestinal system metastases. The first case was a 71-year-old female who presented with pleural and peritoneal effusion and underwent biopsy due to detection of nodular lesions in the duodenum by endoscopic examination. The second case was a 36-year-old male patient who presented with abdominal pain in whom segmental ileal resection was performed due to mass lesions causing invaginations in the ileum. Histopathological examination of the lesions showed a diffuse neoplastic infiltration comprising the entire mucosal layers. In neoplastic cells having a marked atypia and pleomorphism, immunoreactions with S-100, HMB-45, and Melan A were detected. Both cases were diagnosed as malignant melanoma. Abdominal skin in the first case and the femoral region in the second case exhibited congenital melanocytic nevi, and those lesions were determined to show a transformation towards malignant melanoma in the histopathological studies. Malignant melanoma development in gastrointestinal system may have a primary or metastatic character. Definitive diagnosis always requires detailed clinical, histopathological and immunohistochemical analyses.

Lipoleiomyom, matür adipöz doku ve düz kas komponenti içeren, nadir görülen mezenkimal bir neoplazmdır. Tüm uterin lei omyomlar içerisinde görülme sıklığı %0,03-0,2 arasında değişmektedir. Bu çalışmada iki lipoleiomyom olgusu klinik, histopatolojik ve etiyopatogeneze yönelik bilgiler eşliğinde sunulmuş tur. İlk olgu 59 yaşında, postmenopozal kanama yakınması ile başvuran kadın hasta olup, myoma uteri klinik tanısı ile histerektomi operasyonu uygulanmıştır. İkinci olgu, postkoital kanama yakınması ile başvuran 45 yaşında kadın hastadır. Uterin serviks yerleşimli polipoid lezyon eksize edilmiştir. Her iki olguya ait operasyon materyallerinin histopatolojik incelemesinde değişen oranlarda adipöz doku ve düz kas komponenti içeren, ilk olguda intramural, ikinci olguda submukozal yerleşimli lipoleiomyom saptanmıştır. Leiomyomlarda düz kas hücreleri yanı sıra diğer heterolog elemanların görülmesi nadirdir. Adipöz dokunun %10’dan fazla olması lipoleiomyom tanısını akla getirmelidir., Lipoleiomyoma is uncommon mesenchymal neoplasm which contains mature adipose tissue and smooth muscle components. Its prevalence among all uterine leiomyomas varies between 0,03-0,2% . In the current study, we present two lipoleiomyoma cases alongside clinical and histopathological data as well as information on etiopathogenesis. The first case was a 59-year-old female patient presented with postmenopausal hemorrhage and she was subjected to hysterectomy upon clinical diagnosis of myoma uteri. Second case was a 45-year-old woman who presented with postcoital hemorrhage. Polypoid lesion which had a uterine cervix localization was excised. Histopathological examination of the operation materials belonging to these two cases, revealed intramural (first case) and submucosal (second case) lipoleiomyoma, both of which had a varying degree of adipose tissue and smooth muscle components. In leiomyoma cases, detection of other heterologous components alongside smooth muscle cells, is an uncommon event. Presence of an adipose tissue higher than 10%, should indicate lipoleiomyoma.

Congenital malformations of the lung (CML) are rare with similar embryological and clinical spectra and could result in mortality if left untreated. Bronchogenic cysts are formed during the budding of the tracheal diverticula and ventral foregut in the embryological period. In this paper we want to present a case of bronchogenic cyst with continuous intrauterine cyst aspiration follow-up. After the baby birth was operated and the postoperative period was uneventful. The pathological examination revealed a bronchogenic cyst.

Malign fibröz histiositom (MFH) agresif, yüksek gradeli bir sarkomdur. Baş ve boyun bölgesinde nadir görülür. Teşhisi immünohistokimyasal yöntemlere dayanmaktadır. 81 yaşında erkek hasta kliniğimize sol maksiller sinüste kitle ve diş ağrısı ile sevk edildi. Burun tıkanıklığı, burun kanaması veya anosmi yoktu. Hastaya orbital içeriği koruyarak sol total maksillektomi uygulandı. Ameliyat sonrası beşinci ayda hastada herhangi bir nüks ya da uzak metastaz izlenmedi. Üç boyutlu bilgisayarlı tomografi çevre dokuların tutulumunu gösterme açısından çok değerlidir. (Haseki Tıp Bülteni 2010; 48: 156-9), Malignant fibrous histiocytoma (MFH) is a high-grade and aggressive sarcoma. It is relatively rare in the head and neck region. Itsdiagnosis is based on immunohistochemical stains. An 81-year-old male patient was referred to our clinic with a mass in the left maxillary sinus and toothache. He did not have any nasal obstruction, epistaxis or anosmia. The patient received a total left maxillectomy with preservation of the orbital contents. Five months after the surgery, the patient was clinically well without any evidence of local recurrence or distant metastasis.Three-dimensional computed tomography is valuable in the detection of involvement of surrounding tissues. Gliosarkom, bifazik histolojik patern ile karakterize santral sinir sisteminin nadir tümörüdür. Amacımız, kondroblastik osteosarkomatöz farklılaşmayla birlikte olan iki gliosarkom olgusunun klinik, morfolojik ve immünhistokimyasal özelliklerini tanımlamak ve onların patogenetik mekanizmalarını tartışmaktır. Olgu 1: Anaplastik ependimom nedeniyle parietal kraniotomiye gitmiş 52 yaşında erkek hastadır. Olguya postoperatif radyoterapi ve kemoterapi verildi. İlk operasyondan sonra, dördüncü, yedinci ve onuncu aylarda rekürrensden dolayı tümör için ilave rezeksiyonlar yapıldı. Sonuncu tümör rezeksiyonundan sonra hasta exitus oldu. Postmortem biyopsinin histopatolojik incelemesinde, hem gliomatöz hem de sarkomatöz komponentler içeren bifazik morfolojik patern sergileyen neoplazi gözlendi. Olgu 2: Histolojik olarak, sarkomatöz ve glial komponent sergileyen gliosarkom tanısı alan sağ frontal lob kitlesi olan 69 yaşında erkek hastadır. İmmünhistokimyasal özellikleri genelde ilk olgununkine benzemekte, fakat p53 protein ile diffüz nükleer reaksiyon her iki komponentte saptandı. “Kondroblastik osteosarkom özellikleri ile beraber olan gliosarkom” olarak oldukça nadir histopatolojik tanılı iki olgu rapor ettik., Gliosarcoma is a rare tumor of the central nervous system characterized by a biphasic histological pattern. Our objective is to describe clinical, morphological and immunohistochemical features of two cases of gliosarcoma with chondroblastic osteosarcomatous differentiation and to discuss its pathogenetic mechanisms. Case 1: A52- year-old male patient underwent parietal craniotomy due to anaplastic ependymoma. The case had radiotherapy and chemotherapy postoperatively. After the first operation, additional resections were performed for tumor because of recurrences at the fourth, seventh and tenth months. The patient died after the last tumor resection. Histopathologic examination of the postmortem biopsy revealed neoplasm displaying a biphasic morphologic pattern including both gliomatous and sarcomatous components. Case 2: The case was a 69-year-old male patient with a right frontal lobe mass histologically diagnosed as gliosarcoma displaying sarcomatous and glial components. Immunohistochemical features were similar to those of the first case in general, but diffuse nuclear reaction with p53 protein was detected in both components. We report two cases with an extremely rare histopathologic aldiagnosis of “gliosarcoma with features of chondroblastic osteosarcoma”.

Malign mikst müllerian tümör, epitelyal ve stromal elemanlara diferansiye olan müllerian mezodermden köken alan nadir bir neoplazidir. Bu olgu sunumunda uterusun malign mikst müllerian tümörünün klinik, histopatolojik ve immünhistokimyasal özellikleri ile tanısal güçlükleri tartışılmıştır. Pelvik ağrı ve postmenopozal kanama şikayeti ile başvuran 53 yaşındaki kadın olguya uygulanan bilgisayarlı tomografide, 8x5 cm boyutlarında intrauterin kitle saptanmıştır. Küretaj materyalinin “yüksek dereceli stromal sarkom” tanısı alması üzerine, hastaya total abdominal histerektomi, bilateral salpingo-ooforektomi ve omentektomi uygulanmıştır. Histopatolojik incelemede, “yüksek dereceli stromal sarkom” ile birlikte “skuamöz hücreli karsinom” varlığı izlenmiştir. Ek olarak, tümörün komşuluğunda adenomiyozis alanları gözlenmiştir. İmmünhistokimyasal olarak malign stromal hücreler vimentin, düz kas aktini ve miyoglobin ile diffüz, CD10 ve S100 protein ile fokal reaksiyon göstermiştir. Malign epitelyal hücreler ise epitelyal membran antijeni, pankeratin (AE1/AE3) ve vimentin ile diffüz, HCG ile fokal immünreaksiyon vermiştir. Olgu, serviks ve tek taraflı over invazyonu gösteren uterin malign mikst müllerian tümör (karsinosarkom) tanısı almıştır. Hastamız tanı aldıktan bir hafta sonra, böbrek yetmezliği nedeniyle eksitus olmuştur. Bu olgu, tümörün malign epitelyal bileşeninde fokal koryokarsinomatöz diferansiyasyon göstergesi sayılabilecek fokal HCG reaksiyonu izlenmesi açısından nadir bir olgudur. Adenomiyozis ile malign mikst müllerian tümör arasındaki olası ilişki değerlendirilmiştir., Malignant mixed mullerian tumor is a rare neoplasm and derives from mullerian mesoderm that differentiates in epithelial and stromal elements. In this case report, pathological and immunohistochemical profile and diagnostic difficulties in malignant mixed mullerian tumor of the uterus were discussed. The case is a 53-year- old patient who complained of pelvic pain and postmenopausal genital bleeding. Computerized tomography was revealed an intrauterine mass measuring 8x5 cm. After the evaluation of the curettage material that was diagnosed as high grade stromal sarcoma; total abdominal hysterectomy, bilateral salpingo-oophorectomy and omentectomy were performed. Histopathologically, the tumor contained squamous cell carcinoma admixed with high grade stromal sarcoma. Additionaly, areas of adenomyosis in the neighbouring of the tumor were noted. Immunohistochemically, malignant stromal cells exhibited diffuse vimentin, smooth muscle actin, myoglobin and focal CD10 and S100 protein positivity. Malignant epithelial cells revealed diffuse positive reaction for epithelial membrane antigen, pancytokeratin (AE1/AE3), vimentin and focal reaction for HCG. A diagnosis of uterin malignant mixed mullerian tumor (carcinosarcoma) with unilateral ovarian and cervix invasion was made. The patient died one week after the pathologic diagnosis because of renal failure. This case is an interesting example of the focal HCG positivity in the malignant epithelial component of the tumor that may represent focal choriocarcinomatous differentiation. Potential relationship between malignant mixed mullerian tumor and adenomyosis was evaluated.

Kantitatif görüntü analizi, konvansiyonel böbrek hücreli karsinomda tanısal ve prognostik amaçlarla kullanılmıştır. Bu çalışmanın amacı, konvansiyonel (berrak hücreli) böbrek hücreli karsinomda tümör boyutu, patolojik evre ve Fuhrman nükleer derece ile nükleer morfometrik ölçüm sonuçları arasında bir ilişki olup olmadığını araştırmaktır. Bilgisayar destekli görüntü analiz sistemi kullanılarak, toplam 37 konvansiyonel böbrek hücreli karsinomun hematoksilen- eozin kesitlerinde yuvarlaklık faktörü, elipslik indeksi, alan, uzunluk, genişlik ve perimetreyi içeren nükleer morfometrik parametreler değerlendirilmiştir. Morfometrik sonuçlar ile tümör boyutu, patolojik evre ve Fuhrman nükleer derece arasındaki ilişkiyi saptamak için korelasyon analizi uygulanmıştır. The aim of this study was to investigate whether there is any correlation between the results of morphometric measurements and Fuhrman nuclear grade, pathologic stage and size of conventional (clear cell) renal cell carcinoma. Morphometric nuclear parameters such as roundness factor, form ellipse, area, length, breadth and perimeter were evaluated in hematoxylin-eosin stained slides of 37 conventional renal cell carcinomas by using computer assisted image analysis system. The relationship between Fuhrman nuclear grade, pathologic stage, tumor size and morphometric results were determined by correlation analysis. Mean nuclear areas correlated with pathologic stages (r:0.413, p=0.05) and with Fuhrman nuclear grade (r:0.588, p=0.01). Mean nuclear length correlated with pathologic stage (r:0.446, p=0.01) and with Fuhrman nuclear grade (r:0.580, p=0.01). Mean nuclear breadth were correlated with pathologic stage (r:0.377, p=0.05) and with Fuhrman nuclear grade (r:0.544, p=0.01) and with tumor size (r:0.366, p=0.05). Mean nuclear perimeter correlated with pathologic stage (r:0.449, p=0.01) and Fuhrman nuclear grade (r:0.593, p=0.01). In this study, the relative concordance of morphometric results and pathologic stage and Fuhrman nuclear grade, exhibited the importance of nuclear morphometric analysis in the assessment of conventional renal cell carcinoma.

Akdeniz benekli ateşi (ABA), Rickettsia conorii'nin etken olduğu kene ile bulaşan riketsiya enfeksiyonlarından biridir. Etken insanlara genellikle enfekte kahverengi köpek keneleri (Rhipicephalus sanguineus) aracılığıyla bulaşır. Bu yazıda Bartın ilinde köpek kenesi ile temas sonrasında ABA gelişen 16 yaşında bir erkek hasta sunulmaktadır. 2007 yılı Haziran ayında hastanemize başvuran olgunun öyküsünden, beş gün önce köpeğinin kenelerini temizlediği, bundan yaklaşık 12 saat sonra bacağına bir kenenin yapıştığını fark ettiği ve keneyi bacağından cımbızla uzaklaştırdığı öğrenilmiştir. Yaklaşık üç gün sonra, olguda yüksek ateş, baş ağrısı, el ve ayak tabanları da dahil olmak üzere tüm vücutta yaygın döküntü ve bacakta kenenin ısırdığı bölgede siyah renkli bir lezyon ortaya çıkmıştır. Fizik incelemede sağ bacak ön yüzünde kenenin ısırdığı alanda saptanan ortası siyah ve çevresinde mor bir halka bulunan lezyon, ABA'ya özgül olan "tache noire" ile uyumlu bulunmuştur. Olgunun ilk başvurusunda ve başvurudan 10 gün sonra alınan serum örneklerinde çalışılan Rickettsia indirekt immünofloresan antikor (İFA) testi ilk serumda negatif, ikinci serumda ise 1/512 dilüsyonda pozitif olarak saptanmıştır. Olgu, 10 günlük doksisiklin tedavisi ile herhangi bir komplikasyon olmaksızın iyileşmiştir. Bu olgu ile, özellikle son yıllarda kene ile ilişkili enfeksiyon insidansının artış gösterdiği ülkemizde, yaz mevsiminde yüksek ateş, makülopapüler döküntü, baş ağrısı, miyalji, artralji şikayetleri ve kene ısırığı öyküsü olan, özellikle ısırık bölgesinde siyah renkli es-kar gelişen olguların ayırıcı tanısında ABA'nın düşünülmesi gerektiğinin vurgulanması amaçlanmıştır., Mediterranean spotted fever (MSF) is one of the tick-borne rickettsial infections caused by Rickettsia conorii. It is transmitted to humans by brown dog ticks (Rhipicephalus sanguineus). In this case report, a 16-years-old male patient who was diagnosed as MSF after an exposure to dog-tick in Bartin province (located at middle Black Sea region of Turkey) has been presented. His history revealed that, five days before admission to the hospital (on June, 2007) he had cleaned dog-ticks from his dog, and after 12 hours he found a stucked tick on his leg and he took it out right away with a tweezer. High fever, headache and generalized maculopapular rash including soles and palms and a black-colored lesion at the tick bite site developed three days later. In clinical examination, there was a black escar circled with a red-purple colored halo in front of the right tibia at the site of the tick bite showing high similarity to "tache noire" which was specific to MSF. Indirect immunofluorescence assay (IFA) for Rickettsia yielded negative result in the serum sample collected on admission day, however, it was found positive at 1/512 titer in the serum sample collected 10 days after admission. The patient has recovered completely without any complication after 10 days of doxycycline therapy. The aim of this presentation is to point out that MSF should be considered for the differential diagnosis of a patient with a history of tick bite, fever, maculopapular rash, headache, myalgia, arthralgia and especially with black escar during summer months in our country where the incidence of tick-borne infections has been increasing since recent years.

Amaç: Çalışmamızda, tükürük bezinde kitle oluşturan hastalıkların tanısına ince iğne aspirasyon sitolojisinin (İİAS) katkısı, olgulara ait biyopsi tanıları ile birlikte değerlendirilerek sunulmuştur. Materyal ve Metod: 2004-2007 tarihleri arasında, Anabilim Dalı'mızda değerlendirilmiş olan 43 olguya ait tükürük bezi İİAS materyali ile bu olgulardan 19'una ait eksizyon materyali gözden geçirilmiştir. Bulgular: İİAS'lerinin 32'si (%74.4) benign, 5'i (%11.6) malignite şüphesi, 2'si (%4.7) malign ve 4'ü (%9.3) yetersiz olarak tanı almıştır. Eksizyon materyali bulunan 19 olgunun (%44) sitolojik ve histopatolojik tanıları karşılaştırıldığında; sitolojik olarak benign tanısı verilen 13 olgudan 7'si pleomorfik adenom, biri Warthin tümörü, biri malign potansiyelli miyoepitelyal neoplazi, biri mukoepidermoid karsinom, biri parotis-içi lenf nodu, ikisi ise iltihabi süreç olarak tanı almıştır. İİAS'nde malignite şüphesi tanısı alan 4 olgudan biri histopatolojik olarak malign lenfoma, diğer üçü ise skuamöz hücreli karsinom tanılarını almıştır. İİAS'nde yetersiz olarak tanı alan 2 olgunun biri histopatolojik olarak pleomorfik adenom, diğeri ise inflamatuvar süreç olarak tanı almıştır. Çalışmamızda İİAS'nin tanısal doğruluk, duyarlılık ve seçicilik oranları sırasıyla %88, %66.6 ve %100 olarak bulunmuştur. Sonuç: İİAS tükürük bezi kitlelerinde kolay uygulanabilir ve yüksek tanısal değeri olan bir yöntemdir., Aim: In this study, contribution of fine needle aspiration cytology (FNAC) in the diagnosis of salivary gland diseases forming mass was presented by correlation with biopsy diagnoses. Material and Methods: FNAC materials of salivary glands from 43 cases and excision materials from 19 cases evaluated between 2004 and 2007, in our department were reviewed. Results: FNAC diagnosis were as follows; 32 (74.4%) benign, 5 (11.6%) suspicious for malignancy, 2 (4.7%) malign and 4 (9.3%) insufficient for evaluation. When cytological and histopathologic diagnosis of 19 (44%) cases with excision material were compared, histopathologic diagnosis of 13 cases with benign cytology were as follows: 7 cases of pleomorphic adenoma, one of Warthin's tumor, one of myoepithelial neoplasm with malign potential, one of mucoepidermoid carcinoma, one of intraparotid lymph node and two of inflammatory process. One of the four cases cytologically diagnosed as suspicious for malignancy by FNAC was malign lymphoma and other three cases were diagnosedas squamous cell carcinoma. One of the two cases described as insufficient in FNAC was diagnosed as pleomorphic adenoma and the other as inflammatory process. We found that the diagnostic accuracy, sensitivity and specificity were 88%, 66.6% and 100%, respectively. Conclusion: FNAC is an easily applicable method with high diagnostic value for salivary glands masses.

Amaç: Sıçan modelinde lokal hipotermi uygulamasının iskemi-reperfüzyon hasarının farklı evrelerindeki etkileri incelendi. Çalışma planı: Grasilis kası iskemi-reperfüzyon modeli için, her birinde altı sıçan olan dört grup oluşturuldu. Femoral arterin altı saat süreyle klemplenmesiyle iskemik hasar oluşturulan kaslara sadece iskemik dönemde (İH), sadece dört saatlik reperfüzyon döneminde (RH) veya iskemi ve reperfüzyon dönemlerinde (İRH) 10 °C’de lokal hipotermi uygulandı. Methods: An isolated gracilis muscle model of ischemiareperfusion injury was used consisting of four groups, each with six rats. Ischemic injury was induced by clamping the femoral artery for six hours. Local hypothermia at 10 °C was applied during only ischemia (IH), during reperfusion (RH) for four hours, and during both ischemia and reperfusion (IRH). The control group remained untreated.
