International audience; The female of Phlebotomus chadlii Rioux, Jumminer & Gaily, 1966 is described and illustrated for the first time from a specimen collected in El Kef region, northwest Tunisia. It was distinguished from P. ariasi by several characters of the spermathecae; 1) the enlarged portion of P. chadlii spermathecae duct appears smooth and better developed than that of P. oriasi; 2) in P. chadlii, this part comprises three quarters of the duct whereas, in P. oriasi, it covers only the half; 3 the spermathecae neck of P. chodlii is shorter than that of P. ariasi. The duct base is compatible with the large aedeagus size of P. chodlii male. Besides, the assignment of this female to the species P. chodlii is supported by: 1) the presence of moles in the same area, over the last three years; 2) the total absence in this area of P. ariasi; 3) the concomitant presence, in the same trap station, of the described female with P. chadlii moles.

Iso-enzymatic characterization of 19 Leishmania strains isolated from Tunisian dogs revealed that all correspond to Leishmania infantum MON-1. This confirms the role of dog as a reservoir of the commonest zymodeme responsible for human visceral leishmaniasis in the Mediterranean basin. However, and in spite of the high number of identifications, many zymodemes, mainly the dermotropic ones as Leishmania infantum MON-24, frequently reported in humans, had never been isolated from dogs. The study of their cultural characteristics may contribute to explain the particular iso-enzymatic profile of dogs strains., Dix-neuf souches de leishmanies, isolées chez des chiens originaires de différentes régions de la Tunisie, ont été typées par électrophorèse des iso-enzymes. Toutes se sont révélé correspondre à Leishmania infantum MON-1, confirmant la prédominance de ce zymodème chez cet animal. L’absence, aussi bien dans cette série que dans celles d’autres pays méditerranéens, de certains zymodèmes dermotropes, fréquemment rencontrés chez l’homme, pose le problème de leur réservoir. La multiplication des isolements chez les hôtes potentiels et l’étude de la cinétique en culture des différents zymodèmes pourraient apporter d’éventuelles réponses.

International audience; Toxoplasma antibodies prevalence was studied in the north of Tunisia where a mild climate prevails. Two groups of individuals were investigated: 857 living in rural area and 564 living in urb town. Sera were analysed by ELISA and indirect immunofluorescence. The overall prevalence was 58.4%. It roses from 24.5% at ten years to 52.1% at 20 years of age. A maximum level, around 70%, was reached by about 30 years. The risk of acute infection after this age seemed low as judging by the proportion of high antibodies titers observed in this group (14.2% before 30 years vs 3.7% after). A significantly higher prevalence was detected in urban residents (67% vs 52.8%). In this group, the rate of seroconversion seems the highest between ten and 20 years of age and the majority of women are infected before reaching childbearing age. In the rural area, the seropositivity is lower between ten-20 years and many women at childbearing age still susceptible to toxoplasmosis. The risk of acute infection seems higher in the youngest ones as showed by the proportion of high antibodies titers observed in the 18-30 age group (9.2%) compared to the one observed after 30 years (1.9%).; La prévalence de la toxoplasmose a été étudiée dans le nord de la Tunisie chez 857 individus d'origine rurale et 564 individus d'origine urbaine. Les anticorps anti-toxoplasmiques ont été recherchés par immunofluorescence indirecte (IFI) et par ELISA. La prévalence de la toxoplasmose dans la population générale était de 58,4 %. Elle a significativement varié avec l'âge, passant de 24,5 % à dix ans à 52, I % à 20 ans, pour atteindre un maximum autour de 70 % vers l'âge de 30 ans. A partir de cet âge, les séroconversions semblent rares à en juger par la faible proportion des sérums à titres élevés en IFI (14,2 % avant 30 ans vs 3,7 % après 30 ans). La prévalence était significativement plus élevée en milieu urbain (67 % vs 52,8 %). Dans ce milieu, c'est entre dix et vingt ans, que les séroconversions semblent les plus fréquentes, la majorité des femmes étant alors immunisées à l'âge de la procréation. En milieu rural, l'immunisation, moins intense entre dix et 20 ans, fait que le nombre de femmes réceptives à la toxoplasmose à l'âge légal du mariage (18 ans) est plus important. Ce sont donc les femmes jeunes de ce milieu qui sont les plus exposées au risque d'infection toxoplasmique comme en témoigne la proportion élevée de sérum fortement positifs dans cette population (9,2 % pour les 18-30 ans vs 1,9 % pour les 30- 48 ans).

Thanks to the national programme of malaria eradication carried out between 1968 and 1972, there has been no active transmission of the parasitosis in Tunisia since the last indigenous case in 1979. Since 1980, with the increase in international exchanges especially with sub-Saharian countries, only imported cases of malaria have been reported in Tunisia. A retrospective and thorough survey of malaria cases diagnosed in the laboratory of parasitology of the Pasteur Institute in Tunis from 1980 to 1995 determined the epidemiological characteristics of this imported parasitosis. All in all, during the sixteen years following eradication, 245 cases were registered. The majority of cases (86.2%) was diagnosed by a systematic control of groups at risk within the national programme of malaria eradication. The remaining 13.8% cases sought medical advice when clinical symptoms appeared after their return from endemic countries. The population most affected by imported malaria were men (sex-ratio: 6.8) aged between 20 and 40 years (76% of cases); 38% were Tunisians having sojourned in an endemic country, essentially students from sub-Saharian Africa. The presumed country of contamination was African in 92.7% of the cases. Entrance into Tunisia by patients was mainly by air; 4% of the registered cases had come by land from Algeria. Sound knowledge of the epidemiological characteristics of imported malaria would make for a better follow-up of the affected population and thus reduce the probability of repeated transmission., Grâce au programme national d'éradication du paludisme entrepris entre 1968 et 1972, la Tunisie connaît depuis 1979, date du dernier cas autochtone, un arrêt de la transmission active de cette parasitose. In order to determine the social and cultural factors which delay hospitalization of children suffering from visceral leishmaniasis (VL), a study was carried out in the rural area of Kairouan (Tunisia). Qualitative techniques were used to investigate beliefs and behaviours associated with the symptoms of VL. The study has demonstrated that the folk interpretation of the disease interferes largely with the choice of the therapy. Regarding VL, the subdivision of the disease into two groups of symptoms, fever and splenomegaly, incites parents to consult doctors in the presence of fever and use traditional healers when splenomegaly appears. This is responsible for long delays before the patients can have access to official health care. This investigation clearly demonstrated that a health education programme is needed in VL endemic areas in Tunisia in order to shorten delays between the appearance of symptoms and their diagnosis. For this objective to be achieved, it would also seem necessary to implement simple serodiagnostic techniques at the periphery level., Dans le but d'appréhender les facteurs socioculturels pouvant interférer dans le délai de la prise en charge des enfants atteints de leishmaniose viscérale (LV), une enquête utilisant des techniques qualitatives de recherche a été réalisée en 1994 et 1995 dans les zones rurales de la région de Kairouan (Tunisie), où l'on compte actuellement le plus grand nombre de cas de LV.Cette étude montre que l'interprétation populaire de la maladie influence largement les choix et conduites thérapeutiques. Dans le cas de la LV, la maladie est subdivisée en deux temps, la fièvre puis la splénomégalie. Ces deux symptômes sont interprétés différemment par la population et font l'objet de recours thérapeutiques distincts. Si la fièvre conduit immédiatement chez les médecins, la splénomégalie qui se développe ensuite apparaît comme relevant de thérapies traditionnelles, ce qui écarte l'enfant du système institutionnel de santé, retardant la prise en charge en milieu pédiatrique.Ce travail illustre une collaboration qui a contribué à préciser les facteurs socioculturels en cause dans un problème de santé. Il conduit à une autre question, celle de l'éducation en vue de la prévention de cette maladie rare mais grave.

International audience; Natural autoantibodies (NaAb) and IgG antibodies to tetanus toxoid (TT) were analysed in the sera of 38 children with active visceral leishmaniasis (VL) previously vaccinated with TT and in 30 healthy controls matched for sex and age. Patients exhibited high levels of NaAb to a panel of self antigens (tubulin, myosin, myoglobin, actin) contrasting to a low level of IgG to TT. Analysis of the circulating B cells in 26 untreated patients showed a low percentage of CD5+ per total B cells (3-66%, mean 36.6%) compared with 14 normal controls (17.8-66.6%, mean 52.7%) (P < 0.001). Evaluation of these parameters after antimonial therapy showed a significant decrease of the level of the NaAb (P < 0.0005), and a spontaneous increase of the level of the IgG to TT without any vaccine boosting (P < 0.01). In contrast, there was a significant increase in CD5+ B cells (P < 0.0005). This result suggests that CD5+ B cells may be sequestrated in parasitized lymphoid organs and may be released after remission. These findings show that the polyclonal B cell activation that occurs during active VL involves mainly B cells bearing NaAb and are in favour of a functional dichotomy of B cells.

Resume Les taux seriques des immunoglobulines E (IgE) totales et specifiques antihydatiques (antigene utilise : liquide hydatique) sont evalues par une methode immunoenzymatique chez 38 patients Tunisiens atteints d'hydatidose (16 cas a localisation hepatique (KHF), 18 cas a localisation pulmonaire (KHF) et 4 associations), et 40 sujets temoins. Des taux eleves d'IgE totales sont observes dans 76,3 % des cas d'hydatidose et chez 17 % des sujets temoins. Les IgE totales elevees se distribuent de facon equivalente quels que soient la localisation et/ou l'etat anatomo-clinique des kystes. Par contre, les IgE specifiques antihydatiques sont observees avec une frequence significativement plus elevee dans les cas de KHF (75 %) que dans les cas de KHP (55,5 %). D'apres cette etude, cette difference semble due au fait que les KHF sont plus souvent multivesiculaires (9 sur 16) que les KHP (1 sur 18). Une correlation entre les taux eleves d'IgE seriques et la presence d'anticorps IgE antihydatique est observee. Il n'y a pas, par contre, de correlation entre les taux des IgE et ceux des anticorps hydatiques evalues par les techniques d'hemagglutination passive et l'electrosynerese.

An automated assay of anti-P1 allohemagglutinins has been carried out on 133 P2 subjects: 13 with acute hepatic distomiasis (Fascioliasis), 20 with hydatidosis and 100 healthy blood donors. Anti-P1 were detected in 100% of the distomatid sera, in 50% of the hydatid and in 29% of the healthy individuals. When compared to a reference serum test (anti-P1 standard), their concentration was found to be weak in healthy subjects, moderate in hydatidosis and exceptionally increased in acute distomiasis (up to 5-6 times the standard anti-P1 level). The 2-mercaptoethanol treatment showed that even the increased distomatid anti-P1 sera were of an IgM nature. The respective origin, synthesis and strength of these allohemagglutinins are discussed and the advantages of using distomatid IgM anti-P1 as human sera test are emphasized.

Lewis blood group specificities are detected in the liver Fluke: Fasciola hepatica. Such specificities had never been reported in nature, other than in the human body. The investigation of this phenomenon might contribute to a better understanding of the mechanisms involved in parasite-host relationship.

Four lyophilizated liver flüke antigens and one of hydatid fluid were examined for the presence of blood group like antigens P1, A, B, M and N by an hemagglutination inhibition test with hyperimmune and natural hemagglutinating sera. P1 or P1-like substance was found in the extracts of "Fasciola hepatica" and "F. gigantica" in a lower concentration than in echinoccocal fluid. This presence may explain the occurrence of anti-P1 antibodies in patients infected with liver flüke and belonging to the P2 blood group. However biochemical studies will be necessary to determine if the P1 activity is due to the blood group P1 substance of the red cell itself or due to a P1 like substance.

The present study aimed at homogenizing the use of DNA tools for Leishmania parasite characterization in two endemic countries, Algeria and Tunisia. Two genomic DNA probes, pDK10 and pDK20, previously developped in Tunisia, were here applied to a collection of 41 isolates obtained from Algerian patients having cutaneous or visceral leishmaniases. These DNA tools allowed to discriminate among and to identify causal agents of cutaneous leishmaniasis, L. infantum and L. major. Apart from the pDK20--hybridization pattern obtained usually for the species L. infantum, new hybridization patterns were identified for isolates obtained from both visceral and cutaneous leishmaniases patients. Use of DNA probes in complement to isoenzyme typing offers interesting propects for a better description of transmission cycles.
