1. Health facility preparedness for cholera outbreak response in four cholera-prone districts in Cameroon: a cross sectional study

Abstract

Abstract Background The risk of cholera outbreak remains high in Cameroon. This is because of the persistent cholera outbreaks in neighboring countries coupled with the poor hygiene and sanitation conditions in Cameroon. The objective of this study was to assess the readiness of health facilities to respond to cholera outbreak in four cholera-prone districts in Cameroon. Methodology A cross-sectional study was conducted targeting all health facilities in four health districts, labeled as cholera hotspots in Cameroon in August 2016. Data collection was done by interview with a questionnaire and by observation regarding the availability of resources and materials for surveillance and case management, access to water, hygiene, and sanitation. Data analysis was descriptive with STATA 11. Principal findings A total of 134 health facilities were evaluated, most of which (108/134[81%]) were urban facilities. The preparedness regarding surveillance was limited with 13 (50%) health facilities in the Far North and 22(20%) in the Littoral having cholera case definition guide. ORS for Case management was present in 8(31%) health facilities in the Far North and in 94(87%) facilities in the littoral. Less than half of the health facilities had a hand washing protocol and 7(5.1%) did not have any source of drinking water or relied on unimproved sources like lake. A total of 4(3.0%) health facilities, all in the Far North region, did not have a toilet. Conclusions The level of preparedness of health facilities in Cameroon for cholera outbreak response presents a lot of weaknesses. These are present in terms of lack of basic surveillance and case management materials and resources, low access to WaSH. If not addressed now, these facilities might not be able to play their role in case there is an outbreak and might even turn to be transmission milieus.

Abstract

INTRODUCTION: Du fait de l'accès limité au laboratoire au Cameroun, la prise en charge des cas de diarrhée dans la plus part de formations sanitaires est basée sur le diagnostic de présomption. L'objectif de notre étude était de déterminer la distribution et la sensibilité aux antibiotiques habituellement prescrits contre les bactéries pathogènes associés aux diarrhées à l'Hôpital Régional Annexe de Kousseri(HRAK) de Juillet à Octobre 2015. METHODES: il s'agissait d'une étude descriptive et transversale ciblant toute personne consentante consultant pour diarrhée à l'HRAK pendant la période d'étude. De chaque patient était collectés un échantillon de selles et les données par questionnaire anonyme administré en face à face. Chaque échantillon de selles était cultivé sur milieux spécifique aux enterobactéries et analysé suivant la méthode standard de coproculture. La sensibilité des souches isolées aux antibiotiques fréquemment prescrits, a été évaluée et les proportions des patients présentant chaque germe pathogène et de germe sensible à chaque antibiotique étaient estimées. RESULTATS: au total 45(30,0%) des 150 cas de diarrhée inclus étaient associés à une bactérie enteropathogénes dont 37(82,2%) chez les enfants de 0 à 5 ans. Escherichia coli était la bactérie la plus représentée avec 30 cas(66%) suivis des cas de Salmonella spp, 7(16%); Vibrio spp, 5(11%); Aeromonas spp, 2(4%) et Shigella spp, 1(2%). Les antibiogrammes réalisés ont montré que 17(56,7%), 14(46,7%) et 5(16,7%) E. coli étaient sensibles à la Ciprofloxacine, Ceftriaxone, au Cotrimoxazole respectivement. 4(57,14%), 2(28,57%) Salmonella spp. étaient sensibilité au Ceftriaxone et au cotrimoxazole respectivement. CONCLUSION: près du tiers des cas de diarrhées consultant à l'HRAK en saison de pluie sont associés à au moins une bactérie pathogène. La sensibilité des germes isolés aux antibiotiques couramment prescrits reste très limitée. Dans les formations sanitaires de l'Extrême Nord Cameroun ou le personnel de santé est obligé de faire des prescription d'antibiotique contre les diarrhées sur la base des diagnostic de présomption, un système de surveillance des germes associé et de la sensibilité de ceux-ci aux antibiotiques prescrits en routine devra être mis en place.

Abstract

The Far North region has the highest rate of chronic malnutrition (35.9%) ahead of East (35.4%); North (30.6%) and Adamawa (28.7%). The study aimed to identify contextual factors of child malnutrition in line with UNICEF’s causal pattern of malnutrition (2013). This was a qualitative study. Multiple case sampling of social micro-units was used. Focus-groups; face-to-face interviews and observations were organized. Seguin method was used for data analysis (2010). Fifty people participated in the study. Mayo-danay is a landlocked area, where the lack of food processing and food conservation infrastructure promotes early and low-cost liquidation of food products. A situation that exposes families to famine and poverty a few months after the harvest. The influx of internally displaced persons (IDPs); insecurity due to terrorism has weakened cross-border trade. The household size is up to 10 members, that did not allow children to eat at their convenience, and in a diverse way. The Christian head of households did not systematically contribute to the food ration, unlike the Muslims. Fish, agricultural and livestock products are more for sale than for consumption in the household. There are frequent shortages of food supplements for the management of malnutrition in health training. The causes of malnutrition in Mayo-Danay were multifactorial. Strategies to fight against malnutrition must take into account the integrated approach to local and sustainable development.

Abstract

Background: Malnutrition is a major public health thread in Africa and Cameroon as well. A study of mothers’ feeding practices and child malnutrition can contribute to identify and optimise interventions to control malnutrition. Methods: This was an analytical case/control study with a simple random sampling.Data sources were a literature review to estimate the prevalence of referred malnutrition of children, a community detection of cases and controls. A total of 386 children were included in the study. Results: The prevalence of referred infant malnutrition was 47.89% (95% CI [45.58, 50.2]). The most affected age group was children aged between 6 to 29 months. Inappropriate feeding practices significantly increased the risk of malnutrition in children with OR = 3.44 [2.05; 5.76] (P˂0.05). After adjustment by the confounding factors we obtained an adjusted OR = 3.19 [2.08; 6.35] (P = 0.0007). The most common inappropriate feeding practices were: food taboos, non-fortified porridge, early or late introduction of supplementary foods. Conclusion: The prevalence of malnutrition remains very high in the health district of Pitoa. Poor feeding practices significantly increase the risk of malnutrition. Efficient interventions would include nutritional education and popularization of rich local foods.

Abstract

Introduction: The SMART survey (2017) in the Far North region, Cameroon presents high prevalence of malnutrition: 4.5% of global acute malnutrition (GAM) and 40.9% of chronic malnutrition (CM). Objectives: To examine the relationship between multidimensional poverty and malnutrition among children under-five in the Mayo-Danay Division. Specifically, assess the nutritional status of children, describe the living conditions of children in households and measure the association between multidimensional poverty and nutritional status of children. Methods: A cross-sectional, descriptive and analytical study was conducted on a cluster sample at two levels. Anthropometric measurements were used to assess the nutritional status of children. The non-monetary, simplified approach, based on the search for deprivation indices of the well-being of children in the household was used. Results: Out of 433 children surveyed, we observed: 4.4% GAM; 13.9% of underweight and 23.3% MC. Out of the 65,82% poor children, 13,59% had chronic malnutrition; 2.09% GAM and 15.00% underweight. Multidimensional poverty among children was associated with the CM (OR = 4, 436; (CI = 2, 767 - 7,111) and GAM (OR = 4, 194; (CI = 1, 514 - 11,416). Conclusion: The study highlights the interaction between the living conditions of children in households and the impact on their nutritional status. Multidimensional poverty is associated with malnutrition in children. We recommend to initiate a simulation and comparison study, of poverty reduction scenarios, in the study area.

Abstract

INTRODUCTION: du fait de l'acces limite au laboratoire au Cameroun, la prise en charge des cas de diarrhee dans la plus part de formations sanitaires est basee sur le diagnostic de presomption. L'objectif de notre etude etait de determiner la distribution et la sensibilite aux antibiotiques habituellement prescrits contre les bacteries pathogenes associes aux diarrhees a l'Hopital Regional Annexe de Kousseri(HRAK) de Juillet a Octobre 2015. METHODES: il s'agissait d'une etude descriptive et transversale ciblant toute personne consentante consultant pour diarrhee a l'HRAK pendant la periode d'etude. De chaque patient etait collectes un echantillon de selles et les donnees par questionnaire anonyme administre en face a face. Chaque echantillon de selles etait cultive sur milieux specifique aux enterobacteries et analyse suivant la methode standard de coproculture. La sensibilite des souches isolees aux antibiotiques frequemment prescrits, a ete evaluee et les proportions des patients presentant chaque germe pathogene et de germe sensible a chaque antibiotique etaient estimees. RESULTATS: au total 45(30,0%) des 150 cas de diarrhee inclus etaient associes a une bacterie enteropathogenes dont 37(82,2%) chez les enfants de 0 a 5 ans. Escherichia coli etait la bacterie la plus representee avec 30 cas(66%) suivis des cas de Salmonella spp, 7(16%); Vibrio spp, 5(11%); Aeromonas spp, 2(4%) et Shigella spp, 1(2%). Les antibiogrammes realises ont montre que 17(56,7%), 14(46,7%) et 5(16,7%) E. coli etaient sensibles a la Ciprofloxacine, Ceftriaxone, au Cotrimoxazole respectivement. 4(57,14%), 2(28,57%) Salmonella spp. etaient sensibilite au Ceftriaxone et au cotrimoxazole respectivement. CONCLUSION: pres du tiers des cas de diarrhees consultant a l'HRAK en saison de pluie sont associes a au moins une bacterie pathogene. La sensibilite des germes isoles aux antibiotiques couramment prescrits reste tres limitee. Dans les formations sanitaires de l'Extreme Nord Cameroun ou le personnel de sante est oblige de faire des prescription d'antibiotique contre les diarrhees sur la base des diagnostic de presomption, un systeme de surveillance des germes associe et de la sensibilite de ceux-ci aux antibiotiques prescrits en routine devra etre mis en place.

Abstract

Introduction: In most of the health facilities in Cameroon, the management of patients with diarrhea is based on presumptive diagnosis due to limited laboratory resources. This study aimed to determine germs profile and their susceptibility to antibiotics usually prescribed against pathogenic bacteria associated with diarrheas at the Kousseri Regional Hospital Annex from July to October 2015., Methods: We conducted a descriptive and cross-sectional study of all consenting individual presenting with diarrhea to the Kousseri Regional Hospital Annex during the study period. Stool samples were collected from each patient. Patient data were collected via anonymous questionnaire with face to face interview. Each stool sample was grown on media for enterobacteria and analyzed following the standard stool culture method. The sensitivity of the isolated strains to the most frequently prescribed antibiotics was assessed and the proportions of patients with each pathogen germ and of germ sensitive to each antibiotic were estimated., Results: Out of 150 patients with diarrhea included in the study 45(30.0%) had enteropathogenic bacteria, of which 37(82.2%) in children aged 0-5 years. Escherichia coli was the the most common bacterium(30 cases, 66%) followed by Salmonella spp(7 cases, 16%), Vibrio spp,(5 cases, 11%), Aeromonas spp(2 cases, 4%), and Shigella spp(1 cases, 2%). Susceptibility tests were performed which showed that 17(56.7%), 14(46.7%) and 5(16.7%) E. coli were susceptible to ciprofloxacin, ceftriaxone, cotrimoxazole respectively; 4(57.14%), 2(28.57%) Salmonella spp. were susceptible to ceftriaxone and cotrimoxazole respectively., Conclusion: Nearly a third of the cases of diarrheas at the Kousseri Regional Hospital Annex in the season of rain were associated with at least a pathogenic bacterium. The susceptibility of isolated germs to commonly prescribed antibiotics was very limited. Health staff in the facilities in the extreme north of Cameroon prescribe antibiotic against diarrheas on the basis of presumptive diagnosis. Hence, the necessity to implement a system for the monitoring of the associated germ profile and of their susceptibility to commonly prescribed antibiotics.
