This article documents four mandibular reconstructions performed using free fibula flaps. CT scan DICOM (Digital Imaging and COmmunication in Medicine) files were obtained in order to print stereolithographic models of the mandible, and in one case cutting guides for fibular osteotomies. One case study details the treatment a cancer recurrence on a right emimandibulectomy. Because of a lack of access to previous CT scans, the left part of the mandible was mirrored to obtain an accurate 3D model. In one case, due to the young age of the woman, a double barrel fibula flap was used. All cases resulted in satisfactory chewing function and aesthetic outcome, with no flap failures. The report concludes that Virtual Planning and Rapid Prototyping are helpful as they reduce costs and intraoperative times while simultaneously improving surgical precision.

Introduction: The purpose of the study was to retrospectively analyse the recurrence rate of odontogenic keratocysts and to identify eventual features of the lesions that may influence recurrence. Material and methods: This was a retrospective study carried out for a period of 3 years. The medical records of patients treated in our institution were analysed to identify all the cases of odontogenic keratocysts. Results: A total of 16 odontogenic keratocysts were recorded. These lesions were treated with simple enucleation with or without adjuvant Carnoy's solution. The relapse occurred in 4 patients treated with simple enucleation and in none of the patients that underwent enucleation and Carnoy's solution application. The kind of treatment appeared not to influence recurrence rate at statistical analysis. Conclusions: Odontogenic keratocyst is a lesion with a locally aggressive behavior and a high tendency to relapse. This tendency of recurrence may be greater with syndromic presentation of odontogenic keratocyst, with soft tissue involvement, and with teeth proximity to the lesion. The application of Carnoy's solution may be useful to minimize recurrence rate in those odontogenic keratocysts with an aggressive clinical behavior and secondly may be used for all the other lesions treated with simple enucleation that experienced relapse.

The present meta-analysis aimed to describe the methods to determine the working length in primary teeth pulpectomy, also evaluating and comparing their reliability. A systematic review was performed following the PRISMA Statement. The electronic search was conducted on PubMed, Scopus, Cochrane Library and Web of Science. After the screening protocol, a number of 14 studies were included in the qualitative analysis, while seven were included in the quantitative one. The mean working length determined by the electronic apex locator was 11.8 mm (9.0-15.55), while 12.42 mm (11.0-13.52) and 12.3 mm (9.73-15.93) were the mean working length observed with the conventional radiography and the digital radiography, respectively. No statistically significant difference was detected in quantitative analysis between the investigated methods. Introduction: The current therapeutic landscape of Alzheimer's disease (AD) is evolving rapidly. Our treatment options include new anti-amyloid-β protein disease-modifying therapies (DMTs) that decrease cognitive decline in patients with early AD (prodromal and mild AD dementia). Despite these advances, we have limited information on how neurologists would apply the results of recent DMT trials to make treatment decisions. Our goal is to identify factors associated with the use of new AD DMTs among neurologists applying concepts from behavioral economics., Methods: This non-interventional, cross-sectional, web-based study will assess 400 neurologists with expertise in AD from across Spain. Participants will start by completing demographic information, practice settings, and a behavioral battery to address their tolerance to uncertainty and risk preferences. Participants will then be presented with 10 simulated case scenarios or vignettes of common encounters in patients with early AD to evaluate treatment initiation with anti-amyloid-β DMTs (e.g., aducanumab, lecanemab, etc.). The primary outcomes will be therapeutic inertia and suboptimal decisions. Discrete choice experiments will be used to determine the weight of factors influencing treatment choices., Results: The results of this study will provide new insights into a better understanding of the most relevant factors associated with therapeutic decisions on the use of DMTs, assessing how neurologists handle uncertainty when making treatment choices, and identifying the prevalence of therapeutic inertia in the management of early AD., (© 2023. Objective: The aim of this systematic review was to describe the quality of life and oral health-related quality of life of patients affected by medication-related osteonecrosis of the jaw., Study Design: The review was conducted according to Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines. A combination of keywords and MeSH terms was used in PubMed, Scopus, Cochrane Library, Web of Science, and EBSCO up to December 13, 2020., Results: A total of 1066 results were obtained after duplicate exclusion and 11 articles were included in the final qualitative analysis. Most of the articles described the quality of life in patients with osteonecrosis of the jaw secondary to cancer treatment. The main drugs associated with the disease were bisphosphonates. Surgical treatment of the osteonecrosis improves the quality of life of these patients., Conclusions: The present systematic review suggested the negative influence of osteonecrosis of the jaw on the quality of life and oral health-related quality of life among oncologic and osteoporotic patients. The use of quality of life questionnaires in daily practice could be an important tool to better diagnose and manage patients affected by osteonecrosis of the jaws who have received or are receiving drugs associated with this complication., (Copyright © 2021 Elsevier Inc. Purpose: The aim of this meta-analysis is to assess the correlation between blood transfusions and the medical/surgical complications after head and neck reconstructive surgery., Methods: The PRISMA protocol was used and the literature search was performed on Pubmed, Scopus, Cochrane Library and Web of Science up to March 13, 2020. The risk of bias in individual studies was assessed through the Newcastle Ottawa Scale., Results: A total of 1219 records were screened after the electronic search, 22 of which were included in the qualitative analysis. Of there 22 scores, 18 articles were included in the meta-analysis. The OR for medical and surgical complications of transfused patients was 1,64 (95% CI 1.23-2.21); while, the OR for hospital readmission was 1.53 (95% CI 1.29-1.81)., Conclusions: The results of this meta-analysis suggested that blood transfusions are associated with both an increased risk of surgical and medical complications of head and neck flaps and with the hospital readmission.

Aim: To analyse lifestyle of Italian families during lockdown, evaluating its possible impact on the collaboration of children with the paediatric dentist and associated predictive factors., Materials and Methods: Only patients aged between 3 and 16 years, who had started treatment before the lockdown and had spent this periodwith both parents were included in the study. The interviews were carried out using an anonymous questionnaire that assessed school and play activities, the type of diet, the time dedicated to home oral hygiene and the activities carried out with parents. Parents' work situation during lockdown was included to correlate it with time spent at home. The Frankl Behavior Rating Scale was used to determine the degree of cooperation during dental sessions pre- and post-lockdown. Chi-square test and Fisher's Exact tests were used to identify statistical associations of improvement with each categorical variable. Student's t-test was used to highlight the differences in mean values of continuous variables between subjects considered more cooperative and unimproved subjects. Hosmer-Lemeshow test was used to assess the goodness-of-fit of the model. Assumption of linearity of independent variables and log-odds were assessed by Box-Tidwell transformation. Final selection was carried out using the Akaike criterion and all statistical analyses were carried out using the STATA statistical software package., Results: The sample consisted of 212 patients (103F; 109M). The age ranged between 3 and 16 years with an average age of 9.03 years; 82.08% (95% CI 76.24-86.99) of the subjects showed an improvement in collaboration compared to pre-lockdown. Logistic analysis reveals a statistically significant increase of the odds of improvement in patients with a lower pre-lockdown collaboration (OR: 6.05, p = 0.001), in children with a parental presence at home (jobless, OR : 30.55, p <0.001; in "smart working", OR: 23.06, p <0.001) when compared to children whose parents work away from home. From a further exploratory analysis, time dedicated to home oral hygiene was increased if associated with an increased presence of the parents at home (p = 0.015)., Conclusion: Changes in family routines and increased parental presence at home, during the COVID-19 pandemic lockdown, are associated with improved collaboration of children during dental sessions. A statistical association between the increase in time spent on oral hygiene and the increased presence of parents at home was found.

Background: A putative relationship between preterm birth and developmental defects of enamel (DDE) has been described in the literature. Although systematic reviews have found preterm birth may lead to DDE, the effect size has not been quantified., Aim: The aim of this systematic review and meta-analysis was to determine the association between preterm birth and DDE., Design: An electronic search was performed in PubMed, Cochrane Library, Scopus, and Web of Science to identify relevant studies. Two independent reviewers selected the studies in a two-stage process in accordance with the PRISMA statement. The risk of bias was also analysed using the Newcastle-Ottawa Scale criteria., Results: A total of 1041 publications were considered after an electronic search, 20 of which were included in the systematic review. Of these 20 publications, 18 articles were included in a meta-analysis. The meta-analysis detected an increased risk of developing DDE in preterm children [OR: 3.27 (95% CI 2.02, 5.30; P < .001)], with a greater risk in the primary dentition. In addition to this, a subgroup analysis showed a greater risk in the development of hypoplasia in preterm children., Conclusions: The results of this meta-analysis showed a three times increased risk of developing DDE in preterm children., (© 2020 BSPD, IAPD and John Wiley & Sons A/S. Aims: Aim of the present systematic review and meta-analysis is to investigate the oral health status of children with cerebral palsy and to compare it to that of healthy children., Methods and Results: An electronic search extended to October 2019 was conducted on PubMed, Scopus, Web of Science and Cochrane Library. Two independent reviewers selected publications using a two-stage process; data were extracted according to PRISMA statement. The Newcastle Ottawa Scale was used to evaluate the risk of bias in individual studies. After screening of the 5460 studies selected 20 publications were included in the systematic review, 15 underwent quantitative analysis. In the palsy population statistical analysis showed an OR = 1.45 (95% CI: 1.05-2.00) for dental caries in the primary dentition and OR = 1.87 (95% CI: 1.07-3.24) for the simplified oral hygiene index. The OR of Angle's Class II and anterior open bite were 3.27 (95% CI: 1.22-8.81) and 14.06 (95% CI: 6.26-31.62), respectively., Conclusion: Children with cerebral palsy seem to present an increased risk of dental caries in the primary dentition, of Angle's Class II malocclusion, anterior open bite and a lower gingival status., (© 2020 Special Care Dentistry Association and Wiley Periodicals, Inc.)

Global prevalence of non-alcoholic fatty liver disease (NAFLD) has been growing in the last decades, especially in western countries, due to increased prevalence of diabetes, obesity or other components of metabolic syndrome. NAFLD recently became an important cause of hepatocellular carcinoma (HCC), even in non-cirrhotic patients. Patients with HCC-NAFLD are usually older, with more morbidities (especially cardiovascular diseases and metabolic disorders) and have advanced disease at the diagnosis due to the absence of surveillance, which is considered not cost-effective in patients without advanced fibrosis/cirrhosis, given the large prevalence of NAFLD in the general population. For these reasons, patients with HCC-NAFLD unlikely underwent curative treatments, and have been reported to have lower overall survival (OS) compared to individuals with HCC related to other aetiologies. However, this difference is not confirmed by data of patient subgroups who received curative treatment. In our review, we selected studies published over the past 8 years that analyse characteristics and outcomes of HCC-NAFLD patients who underwent surgery with the aim of identifying features that could predict outcomes and potential selection criteria. All the studies confirm that patients with HCC-NAFLD are older, with many comorbidities and that HCC occurs frequently even in non-cirrhotic livers. There is no agreement about intraoperative and perioperative complications. Regarding outcomes, all papers agree that patients with HCC in NAFLD who undergo surgery have a better OS compared to other aetiologies. Summarizing, surgery is a good curative option for patients with HCC-NAFLD, perhaps even better than transplantation in terms of OS. In this group of patients, it seems to be essential to evaluate cardio-pulmonary and general operative risk, in addition to the normal risk assessment related to liver function to avoid an underestimation, especially for patients without severe underlying fibrosis., Competing Interests: The authors report no conflicts of interest in this work., (© 2020 Campani et al.)

Objective: The study aimed to evaluate the long-term efficacy and safety of vedolizumab in a real-life cohort of patients with inflammatory bowel diseases enrolled at a tertiary referral center., Methods: Data were retrospectively collected from August 2016 to November 2018. The primary outcomes were clinical response and remission at 14, 24, and 52 weeks, and steroid-free remission rate (SFRR) at 52 weeks. Endoscopic response and remission rates at 52 weeks were the secondary outcomes., Results: Altogether 49 patients (22 with ulcerating colitis [UC] and 27 with Crohn's Disease [CD]) were enrolled. The clinical response rate gradually dropped from 85% and 50% in CD and UC, respectively, at week 14 to 59% and 25% at week 52, with significantly a higher response in CD at week 14. The endoscopic response at week 52 was 55% in CD and 25% in UC (P = 0.21). CD group had a higher SFRR than UC group (41% vs 20%) at 52 weeks, although the difference was not statistically significant. Similar clinical and endoscopic rates were observed in biologic-naive and -experienced patients. We reported no discontinuation due to adverse drug reactions, and only mild to moderate events., Conclusions: In our cohort the clinical response in the induction phase was similar to those of registered trials, despite surprising better results for CD. During the maintenance phase we observed an higher drop out than in the reported literatures. Background: Low-grade intraductal carcinomas are rare, malign tumors of salivary glands most commonly affecting parotid gland. It is a slow-growing tumor considered with a favourable prognosis after surgical excision., Methods: To define the characteristics and management of low-grade intraductal carcinoma a systematic review was performed using the electronic databases Pubmed, Cochrane and Scopus. A new case report was also described., Results: Including this case the review of literature identified only 54 cases reported thus far. Demographics, clinical presentation, diagnostic tools, treatment, follow-up and recurrence rate, histological and immunohistochemical patterns of this kind of tumor were summarized., Conclusion: Low-grade intraductal carcinoma has already been well defined but is important to focus on the fact that in few cases component of high-grade infiltrations have been reported: this may modify surgical approach because a simple tumorectomy may not be enough.

Background and Purpose: Although incidence rates vary, infectious, autoimmune, and neoplastic diseases can all cause chronic and subacute meningitis (CSM). We report a Latin-American, single center, CSM case series, analyzing the main clinical characteristics as well as ancillary diagnostic methods differentiating neoplastic from non-neoplastic etiologies., Methods: Retrospective review of CSM cases from a single center in Buenos Aires, Argentina., Results: Seventy patients with CSM diagnosis were identified, 49 with neoplastic and 21 with non-neoplastic meningitis. A history of previous cancer was significantly higher in neoplastic cases, whereas prevalence of autoimmune disease and fever was more common in non-neoplastic meningitis. C-reactive protein values were higher in non-neoplastic CSM, as was pleocytosis in cerebrospinal fluid analysis. The most frequent etiologies were breast and lung cancer for neoplastic meningitis cases; and idiopathic, tuberculous, and fungal infection for non-neoplastic cases., Conclusions: Chronic and subacute meningitis diagnosis is challenging in daily neurological practice. Purpose: The aim of this study was to investigate the risk of mandibular angle fracture associated with the presence of a mandibular third molar and its position when the mandibular fracture occurs., Methods: A systematic literary search was performed in Pubmed, Scopus, and the Cochrane Library for observational studies with at least 250 patients that included frequency of mandibular angle fracture, presence of third molar, and its position., Results: A total of seven studies were included in the review, from an initial search of 622 titles. The relative risk of mandibular angle fracture with third molar was 1.90 (95% CI = 1.47-2.46). The relative risk of mandibular angle fracture related to third molar position (according to the Pell and Gregory classification) was 1.18 (95% CI = 0.62-2.25), 1.98 (95% CI = 0.95-4.10), 2.72 (95% CI = 1.78-4.16), 1.31 (95% CI = 0.80-2.14), 2.21 (95% CI = 1.69-2.87) and 2.99 (95% CI = 2.12-4.22) for Class A, Class B, Class C, Class I, Class II, and Class III, respectively., Conclusions: Our meta-analysis reported a two-fold increased risk of mandibular angle fracture with the presence of a third molar in patients who presented with mandibular fractures. The birth of the Degree in Dentistry in Italy has been very troubled, and only in 1980 saw its effective implementation. Very "instructive" in this regard is the history on the establishment in 1924 (the period of the seizure of power by Fascism) of a "National School of Dentistry" at the University of Rome, which was withdrawn after only ten months. The biggest supporter and proponent of the School, Prof. Angelo Chiavaro, after a few years, was "punished" with the transfer from the University of Rome to that of Genoa. We present some brief notes on the biography of this courageous pioneer and the matter of which he was the protagonist.
