Abstract

Background: The massive pulmonary neutrophil influx in respiratory distress syndrome (RDS) in preterm infants has been ascribed to the effect of leukotriene B-4 (LTB4). Objectives: To investigate whether secretory phospholipase A(2) (sPLA(2)), the rate-limiting enzyme in LTB4 production, is present in lungs of RDS infants and stimulates neutrophil migration. Methods: sPLA(2) was measured in tracheal aspirates from 15 preterm infants with RDS. The effect of aspirates on cord blood neutrophil migration was first measured, and the contribution of sPLA(2) was assessed by addition of its endogenous inhibitor Clara cell protein (CC16) or absorption of sPLA(2) from the aspirates. The role of intracellular signal transduction activation and LTB4 formation in sPLA(2)-induced neutrophil migration was determined using purified sPLA(2), several inhibitors of signal transduction, a LTB4 synthesis inhibitor and a LTB4 receptor antagonist. Results: All aspirates contained sPLA(2), which significantly stimulated neutrophil migration. Addition of CC16 or absorption of sPLA(2) abolished the stimulatory effect. All inhibitors significantly reduced sPLA(2)-induced neutrophil migration. Conclusions: sPLA(2) is present in tracheal aspirates of preterm infants with RDS. Human recombinant sPLA(2) and pancreatic type sPLA(2) stimulate in vitro cord blood neutrophil migration via activation of intracellular signal transduction pathways, LTB4 production and receptor binding. We speculate that sPLA(2) contributes to pulmonary neutrophil influx in RDS. Further studies are needed to determine the potential of sPLA(2) inhibition as a treatment for RDS. Copyright (C) 2009 S. Abstract

Årsliste 2000 Smoltkvaliteten i Otra ble evaluert våren 1999 på bakgrunn av målinger av fysiologisk og histopatologisk status samt på bakgrunn av saltvannstester. Det ble ikke påvist "skader" på smoltkvaliteten til smolt innfanget i Høyebekken og eksponert i Otra i 12 dager. Det ble påvist en liten forskjell i kvalitet mellom smolt fra Høyebekken og Straisbekken. Forskjellen var sannsynligvis ikke relatert til vannkjemi i Otra, men til forskjeller i bekkene. På tross av god smoltkvalitet ble det påvist tildels omfattende vevsforandringer samt betydelig akkumulering av Al i gjellene. Vevsforandringene som ble påvist var ikke i seg selv tilstrekkelig til å svekke saltvannstoleransen. Forandringene i gjellevev og aluminiumskonsentrasjonen påvist på og i gjeller kan skyldes responser på vinter vannkvalitet perioder når vassdraget inneholder mye aluminium. Vannkjemiske forandringer (redusert aluminiums transport) fra vinteren til våren kan ha tillatt fisken å restitueres og etablering av saltvannstoleranse. De påviste "skadene" representerer således forhistorien, hvor forhistorien antyder at vannkvaliteten ikke er optimal hele året. Smoltkvaliteten antyder at Otra etterhvert kan ha en selvreproduserende laksestamme. Bestandstettheten og alderssammensetningen av fisk i elva tyder imidlertid på at vassdraget fortsatt er "skadet" og påvirket av tidligere dårlig vannkjemi, deriblant giftutslipp i 1997. Item does not contain fulltext, The aim of the study was to evaluate the natural course of nephrocalcinosis (NC) in preterm neonates and the effect of NC on blood pressure and renal glomerular and tubular function. In a prospective observational study of 201 preterm neonates (gestational age <32 weeks) NC was present at term in 83 patients (41%), who were subsequently examined at 6, 12, and 24 months, and until August 2000 annually (with a maximum of 4 years) if NC persisted. Examination consisted of blood pressure measurement, renal ultrasonography, and glomerular and tubular function tests. The probability that NC, when present at term, would persist for 15 and 30 months was 34% [21-45, 95% confidence interval (CI)] and 15% (5-25, 95% CI) (Kaplan-Meier), respectively. Urinary tract infection did not occur more frequently in patients with NC (2.5%) than patients without NC at term (4.4%). Systolic and diastolic blood pressures above the 95th percentile were found in 39% and 48% of patients at 1 year and 30% and 34% at 2 years ( P<0.001). Mean glomerular filtration rate (GFR) (inulin clearance) at 1 and 2 years was 92 and 102 ml/min per 1.73 m(2), respectively. TP/GFR and excretion of alpha(1)-microglobulin were normal. The desmopressin test was impaired in 4 of 30 patients at 1 year and 2 of 25 at 2 years. It was concluded that while proximal tubular function is unaffected in children with neonatal NC, high blood pressure and impaired glomerular and distal tubular function might occur more frequently than in healthy children. Although no relationship can be proven between NC and hypertension or diminished renal function in this study, these results justify a large follow-up study with matched controlled study groups.

Abstract

Tålegrensekart for tilførsler av sterke syrer til Norge er nå utarbeidet basert på underruter av enheten 0.5° lengde og 1.0° bredde. Totalt er tålegrensen for svovel idag overskredet i et areal på ca. 117.000 km² i hele Norge. Forsuringsskader på fisk er blitt registrert ved hjelp av systematiske intervjuundersøkelser helt fra midten av 1970-årene, og gyldigheten av metoden er bekreftet med omfattende prøvefiske. Skader på fiskebestander er fremstilt i geografiske ruteenheter basert på UTM-systemet. En skadeindeks er beregnet for hver rute. De to databasene er blitt konvertert til samme rutenett. Ialt 128.526 km² (alle fylker sør for Møre og Romsdal og Sør-Trøndelag, dvs. ca 40% av fastlands-Norges areal) er kartlagt for begge variable. Tålegrensene er overskredet i 94.000 km² av dette arealet, mens fiskeskader er registrert i ca. 86.000 km². De to datasettene er samlet inn uavhengig av hverandre. Den valgte verdi for ANC limit (20µeq/l) er anslått på grunnlag av en empirisk sammenheng mellom ANC og fiskestatus fra 1000-sjøers undersøkelsen. Ved å bruke dette utgangspunktet for tålegrenseberegninger finner en god sammenheng mellom overskrideler av tålegrenser og fiskeskader fra en uavhengig database basert på mer omfattende fiskestatusundersøkelse. Dette bekrefter at tålegrensemetoden gir et reelt bilde av forholdene, og at prognoser basert på denne metoden vil gi gode resultater for anslag av omfang av fiskeskader ved gitte fremtidige scenarier. 20. Influence of low dose allopurinol on ischaemia - Reperfusion injury during abdominal aortic surgery

Abstract

Objectives:: To ascertain whether surgery causes ischaemia-reperfusion (I-R) related injury, if this injury is augmented by preoperative shock, and reduced with low dose allopurinol. Design:: Randomised blind placebo controlled trial. Setting:: Surgical laboratory. Material and methods:: 22 pigs were randomly allocated to four groups; OP = operation/placebo, OA = operation/allopurinol, SOP = shock + operation/placebo, SOA = shock + operation/allopurinol. An aortic tube prosthesis was inserted in all. In groups SOP and SOA preoperative shock was induced by exsanguination. Allopurinol was administered in group OA on the preoperative day and peroperatively, in group SOA during shock and peroperatively. Chief outcome measures:: Perioperative blood concentrations of thiobarbituric acid reactive species (TBARS), ascorbic acid (AA), albumin, ^9^9^mTc-albumin and creatine phosphokinase (CPK) as indicators of oxidative membrane damage, antioxidant activity, microvascular permeability changes and muscular cell damage respectively. Main results:: In the OP and OA groups TBARS gradually increased, while AA, ^9^9^mTc-albumin and CPK remained unchanged and albumin decreased. No effect of allopurinol was observed in these groups. In the SOP group TBARS and AA were not significantly different from groups OP and OA. Yet, albumin, ^9^9^mTc-albumin and CPK decreased significantly more in the SOP group. Compared with the SOP group, allopurinol treatment (SOA) produced lower TBARS and higher AA levels, and reduced the effect of shock on albumin, ^9^9^mTc-albumin and CPK concentrations. Conclusion:: Aortic surgery causes no I-R related damage. Pre-operative shock produces I-R related damage, which is reduced by allopurinol.
