2. Wear Behaviour of HVOF-Sprayed Nanostructured WC-CoCr Coatings

- Abstract
HVOF-sprayed WC–10wt%Co–4wt.%Cr coatings were obtained using experimental feedstock powders (manufactured by spray-drying + sintering), containing nanometric carbide particles. Three reference coatings were also deposited using commercially-available powders containing sub-micrometric carbide particles. The coatings obtained from nanostructured powders, although affected by decarburisation phenomena, contained very fine carbide particles (~200 nm size). Those obtained from commercially-available powders simultaneously exhibited sub-micrometric (~400 nm size) and micrometric carbide particles, and were much less decarburised. Sliding wear tests performed at room temperature against sintered Al2O3 balls showed the occurrence of brittle fracture wear (detachment of near-surface material by local brittle cracking) on the nanostructured coatings, which were embrittled by decarburisation. The reference coatings, by contrast, exhibited either ductile wear behaviour (plastic deformation, pull-out of single carbide particles) or a mix of both ductile and brittle wear mechanisms. When the decarburisation of the nanostructured coatings was not too extensive, their wear loss was comparable to that of the reference ones. At 500 °C, the wear behaviour of all coatings was dominated by abrasive grooving, on account of thermal softening. The most decarburised nanostructured coatings, however, still experienced brittle cracking as well.

- Abstract
Older adults living in long-term care facilities (LTCF) are particularly vulnerable to COVID-19. We describe the clinical characteristics, the course of disease, and the care needs of 38 non-self-sufficient adults with COVID-19 in an LTCF specially set up for those who could not be discharged home or to a standard LTCF. Residents spent an average of 30.5 days in the facility (range 4-77 days). Most of them recovered and were discharged home or to their LTCF of origin. It seems feasible to set up dedicated facilities to treat and provide assistance in the activities of daily living to older adults with COVID-19.

- Abstract
Aim: To investigate the relationship between onset of delirium and time to surgery in hip fracture (HF) patients with a different degree of cognitive impairment., Methods: Retrospective analysis of a prospective database of 939 older adults, aged ≥ 75 years admitted with a fragility HF. Subjects underwent a Comprehensive Geriatric Assessment on admission, evaluating health status, prefracture functional status in basic and instrumental activities of daily living, and walking ability. According to the Short Portable Mental Status Questionnaire score, patients were stratified into three categories: cognitively healthy (0-2 errors), mildly to moderately impaired (3-7 errors) and severely impaired (8-10 errors). Time to surgery (from admission) was expressed as days. The occurrence of delirium was ascertained daily by Confusion Assessment Method., Results: Two hundred ninety-two (31.1%) patients experienced delirium during in-hospital stay. They were older, with a higher degree of comorbidity and functional impairment compared to patients without delirium. In multivariate analysis, surgical delay resulted a significant independent risk factor for delirium (HR 1.11, 95% CI 1.01-1.24), along with age, prefracture functional disability and cognitive impairment. When the analysis was performed accounting for the cognitive categories, surgical delay demonstrated to increase the risk of delirium only in the subcategory of mildly to moderately impaired patients, while no significant effect was demonstrated in patients cognitively healthy or severely impaired., Conclusions: The study supports the concept that older adults with HF should undergo surgery quickly. Patients with mild-to-moderate cognitive impairment should be primarily considered as the best target for interventions aiming to reduce time to surgery.

- Abstract
'Early psychological intervention' is defined as commencing treatment within three months of the traumatic event, with the aim to prevent or treat posttraumatic stress disorder, ongoing distress or acute stress disorder. In natural disaster situations, specific issues may limit the amount of time available for treatment and the possibility of interventions. Eye Movement Desensitisation and Reprocessing (EMDR) can be used without regard to these limits. The aim of the study is to evaluate the effects of EMDR, Recent Traumatic Episode Protocol (R-TEP) provided within three months of the traumatic event to a large sample of individuals exposed to the earthquake that hit Emilia Romagna Region (Northern Italy) in 2012. This study is based on a retrospective review of medical records collected during the activities of psychological and psychosocial unit in the immediate aftermath of earthquake. In total, 529 participants completed the Impact of Event Scale Revised (IES-R) (pre e post treatment). In order to provide a comparison similar to a waitlist-like control group, a method of cohort analysis was applied. In addition, possible time dependent effect was tested. ET (early-treated sample, participants treated within one month after the earthquake) and LT (late-treated sample, participants treated after the first month from the earthquake) reported at post-treatment an improvement to a level below the IES-R cutoff (65.8% of the ET sample and 64.02% of the LT sample). Control group analogue and time-outcome correlation suggest that positive changes in symptoms were likely due to the treatment provided and not merely to the time lapse from the traumatic event. The results of this study suggest that EMDR is a viable treatment option in response to a disaster crisis and in reducing psychological distress of acutely traumatized individuals within the context of a natural disaster.

Background: Caregiving experiences in dementia may have adverse effects on the caregiver's own physical and mental health. The Zarit Burden Interview (ZBI) is the most widely used instrument for assessing family carer burden and the purpose of the present study was to evaluate the psychometric properties of the Italian version of the ZBI (I-ZBI)., Methods: I-ZBI was administered to a non-randomized sample of 273 caregivers related to consecutively attending outpatients with a previously established primary diagnosis of dementia, according to DSM-IV criteria. Measurements used for the construct validity of the instrument were the General Health Questionnaire (GHQ-12), Hospital Anxiety and Depression Scale (HADS) and the Symptom Checklist-90-Revised (SCL-90-R). Validity and reliability were evaluated, as well as the factor structure of the instrument., Results: Most caregivers were children (65%), female (72%) and living with the patient (53%), with quite a high level of burden. The I-ZBI demonstrates good reliability with high internal consistency (α = 0.90) and split-half correlation of 0.78, and good concurrent validity with other assessment tools. A five-factor solution was obtained for the I-ZBI and the original bi-factorial structure of the ZBI was not confirmed., Conclusions: The Italian version of the Zarit Burden Interview is valid, reliable and useful for use in clinical contexts and in future studies that could lead to a better understanding of carer burden in dementia. Clarifying the factorial structure of the ZBI would help to further the understanding of the large body of research using it and would also help to advance the understanding of subjective burden.
