Rationale and Objectives To assess focal multiple myeloma bone lesions via dual-energy CT-based virtual noncalcium (VNCa) bone marrow imaging in relation to the overall hematological disease status and MRI findings. Materials and Methods We retrospectively evaluated 103 focal osteolytic lesions of the axial skeleton in VNCa bone marrow images of 32 patients. Region of interest-based attenuation measurements were correlated with T1w signal intensity and apparent diffusion coefficient (ADC). Results were compared between patients in active and inactive disease. Receiver operating characteristic analysis was performed to determine a cut-off value of VNCa attenuation for differentiation between the two groups. Standard of reference was the overall disease status according to International Myeloma Working Group response criteria. Results Mean attenuation difference between lesions and background bone marrow was significantly lower in inactive disease (16 HU, SD 30) compared to active disease (35 HU, SD 29). VNCa attenuation measurement allowed for differentiation between active and inactive disease with a sensitivity of 92% and a specificity of 58% at a cut-off value of -21 HU. VNCa attenuation was negatively correlated to T1w signal intensity (Spearman's ρ -0.617, p Conclusion Quantitative assessment of attenuation of focal osteolytic lesions in VNCa bone marrow images allows differentiation between overall active and inactive disease with higher attenuation signifying an increasing likelihood of active disease. This is supported by a significant positive correlation between the attenuation and the ADC, as well as a corresponding inverse correlation to T1w signal intensity.

To compare noise texture and accuracy to differentiate uric acid from non-uric acid urinary stones among four different single-source and dual-source DECT approaches in an ex vivo phantom study. Thirty-two urinary stones embedded in gelatin were mounted on a Styrofoam disk and placed into a water-filled phantom. The phantom was imaged using four different DECT approaches: (A) dual-source DECT (DS-DE); (B) 1st generation split-filter single-source DECT (SF1-TB); (C) 2nd generation split-filter single-source DECT (SF2-TB) and (D) 2nd generation split-filter single-source DECT using serial acquisitions (SF2-TS). Two different radiation doses (3 mGy and 6 mGy) were used. Noise texture was compared by assessing the average spatial frequency (fav) of the normalized noise power spectrum (nNPS). ROC curves for stone classification were computed and the accuracy for different dual-energy ratio cutoffs was derived. NNPS demonstrated comparable noise texture among A, C, and D (fav-range 0.18–0.19) but finer noise texture for B (fav = 0.27). Stone classification showed an accuracy of 96.9%, 96.9%, 93.8%, 93.8% for A, B, C, D for low-dose, respectively, and 100%, 96.9%, 96.9%, 100% for routine dose. The vendor-specified cutoff for the dual-energy ratio was optimal except for the low-dose scan in D for which the accuracy was improved from 93.8 to 100% using an optimized cutoff. Accuracy to differentiate uric acid from non-uric acid stones was high among four single-source and dual-source DECT approaches for low- and routine dose DECT scans. Noise texture differed only slightly for the first-generation split-filter approach.

To compare intravenous contrast material (CM) injection protocols for dual-energy CT pulmonary angiography (CTPA) in patients with suspected acute pulmonary embolism with regard to image quality and pulmonary perfused blood volume (PBV) values. A total of 198 studies performed with four CM injection protocols varying in CM volume and iodine delivery rates (IDR) were retrospectively included: (A) 60 ml at 5 ml/s (IDR = 1.75gI/s), (B) 50 ml at 5 ml/s (IDR = 1.75gI/s), (C) 50 ml at 4 ml/s (IDR = 1.40gI/s), (D) 40 ml at 3 ml/s (IDR = 1.05gI/s). BACKGROUND: Systemic lung cancer treatment-induced changes in bone marrow attenuation assessed via dual-energy CT-based virtual non-calcium (VNCa) imaging of the axial skeleton and their relationship to hematological laboratory have not yet been investigated. METHODS: VNCa bone marrow images of the axial skeleton derived from 93 unenhanced reduced dose dual-energy CTs of the thorax and abdomen of 31 patients were retrospectively analyzed. Each patient had received one pre-therapy baseline exam and two consecutive follow-up exams (FU1 and FU2) at a mean of 7.7 and 11.7 weeks after start of therapy. Concurrent hematologic laboratory data were available for every exam. Seven regions of interest were placed in the spine and pelvis and mean VNCa bone marrow attenuation was measured. Twenty-two Patients receiving highly myelotoxic treatment (Group A) were compared to 9 patients receiving less toxic substances (Group B). RESULTS: Median bone marrow attenuation in Group A/Group B was −31.8 HU (IQR 12.7)/40.6 HU (IQR, 12.2) at baseline, −46.5 HU (IQR, 12.5)/−43.8 HU (15.7) at FU1 and −46.9 HU (IQR, 22.0)/−38.5 HU (IQR, 18.5) at FU2. In both subgroups the reduction of the mean attenuation between baseline and FU1 was statistically significant although in Group A it was more pronounced; no significant difference was found between FU1 and FU2. The differences between Groups were not statistically significant. Leukopenia rates at FU1 and FU 2 were 50% and 54.5% in Group A and 0% and 22% in Group B. Anemia rates rose from 31.8% at baseline to 90% at FU1 and 86.4% at FU2 in Group A and fell from 77.8% at baseline to 66.7% at FU1 and further to 55.6% at FU2 in Group B. CONCLUSIONS: Both highly myelotoxic as well as—to a smaller degree—less myelotoxic systemic therapy led to a significant drop in bone marrow attenuation with no significant tendency towards subsequent elevation irrespective of the treatment’s degree of toxicity or the presence of myelosuppression and not even under hematological supportive therapy. The results suggest that in this clinical setting an increase in bone marrow attenuation should be regarded as suspicious for tumor infiltration.

Purpose: Purpose of this study was to investigate Dual-energy CT (DECT) derived virtual non-calcium (VNCa) values for absolute quantification of the bone marrow composition in the wrist. Materials and Methods: We prospectively included consecutive adult participants and examined their wrists with DECT. Ranges of VNCa and calcium values were measured in the carpal bones, radius and ulna using a semi-automatic method. Bones with bone marrow edema, assessed by two blinded radiologists, were excluded. After determining optimum parameters for the three-material decomposition, the influence of calcium values, age and sex on VNCa values was assessed using multiple linear regression. Results: 41 participants (Median age 53 years, range 20 years – 88 years, 51 % men) were enrolled and 399 bones assessed. At participant level mean VNCa values were -143 HU (SD 14 HU) using the current parameters for three-material decomposition and -104HU (SD 11 HU) with optimized parameters. There was a strong and significant influence of calcium values on VNCa values with the current parameters (p < 0.001, -0.137 HU[VNCa] / HU[Calcium]). With optimized parameters the calcium values and sex were not statistically significant predictors of VNCa values. Age was a significant, but clinically negligible, predictor (p = 0.03, -0.225 HU / year). Conclusions: After optimizing three-material decomposition parameters, calcium values, age and sex do not substantially influence virtual non-calcium values, and DECT may therefore be used for absolute quantification of the bone marrow composition - alleviating the need for reference bones or groups.

To retrospectively quantify liver iron content in haematological patients suspected of transfusional haemosiderosis using dual-energy CT (DECT) and correlate with serum ferritin levels and estimated quantity of transfused iron. One hundred forty-seven consecutive dual-source dual-energy non-contrast chest-CTs in 110 haematologic patients intended primarily for exclusion of pulmonary infection between September 2016 and June 2017 were retrospectively evaluated. Image data was post-processed with a software prototype. After material decomposition, an iron enhancement map was created and freehand ROIs were drawn including most of the partially examined liver. The virtual iron content (VIC) was calculated and expressed in milligram/millilitre. VIC was correlated with serum ferritin and estimated amount of transfused iron. Scans of patients who had not received blood products were considered controls. Forty-eight (32.7%) cases (controls) had not received any blood transfusions whereas 67.3% had received one transfusion or more. Median serum ferritin and VIC were 138.0 μg/dl (range, 6.0–2628.0 μg/dl) and 1.33 mg/ml (range, − 0.94–7.56 mg/ml) in the post-transfusional group and 27.0 μg/dl (range, 1.0–248.0 μg/dl) and 0.61 mg/ml (range, − 2.1–2.4) in the control group. Dual-energy computed tomography (CT) is an imaging technique in which the same axial slice of the patient is scanned with two different X-ray spectra in order to extract chemical information or improve diagnostic image quality. This method is useful for the detection or visualization of heavy atoms like iodine but the differentiation of soft tissue types containing only light atoms is usually not markedly improved compared with single-energy CT. The commercially available dual-energy CT scanners use different technological approaches that differ in complexity and result quality.

The purpose of this study was to evaluate image quality of a spectral photon-counting detector (PCD) computed tomography (CT) system for evaluation of major arteries of the head and neck compared with conventional single-energy CT scans using energy-integrating detectors (EIDs).In this institutional review board-approved study, 16 asymptomatic subjects (7 men) provided informed consent and received both PCD and EID contrast-enhanced CT scans of the head and neck (mean age, 58 years; range, 46-75 years). Tube settings were (EID: 120 kVp/160 mA vs PCD: 140 kVp/108 mA) for all volunteers. Quantitative analysis included measurements of mean attenuation, image noise, and contrast-to-noise ratio (CNR). Spectral PCD data were used to reconstruct virtual monoenergetic images and iodine maps. A head phantom was used to validate iodine concentration measurements in PCD images only. Two radiologists blinded to detector type independently scored the image quality of different segments of the arteries, as well as diagnostic acceptability, image noise, and severity of artifacts of the PCD and EID images. Reproducibility was assessed with intraclass correlation coefficient. Linear mixed models that account for within-subject correlation of analyzed arterial segments were used. Linear regression and Bland-Altman analysis with 95% limits of agreement were used to calculate the accuracy of material decomposition.Photon-counting detector image quality scores were significantly higher compared with EID image quality scores with lower image noise (P0.01) and less image artifacts (P0.001). Photon-counting detector image noise was 9.1% lower than EID image noise (8.0 ± 1.3 HU vs 8.8 ± 1.5 HU, respectively, P0.001). Arterial segments showed artifacts on EID images due to beam hardening that were not present on PCD images. On PCD images of the head phantom, there was excellent correlation (R = 0.998) between actual and calculated iodine concentrations without significant bias (bias: -0.4 mg/mL [95% limits of agreements: -1.1 to 0.4 mg/mL]). Iodine maps had 20.7% higher CNR compared with nonspectral PCD (65.2 ± 9.0 vs 54.0 ± 4.5, P = 0.01), and virtual monoenergetic image at 70 keV showed similar CNR to nonspectral images (52.6 ± 4.2 vs 54.0 ± 4.5, P = 0.39).Photon-counting CT has the potential to improve the image quality of carotid and intracranial CT angiography compared with single-energy EID CT.

PURPOSE: To evaluate the accuracy and precision across phantom size of a dual-energy computed tomography (DECT) technique used to calculate relative proton stopping power (SPR) in tissue-simulating materials and a silicone implant relative to conventional single-energy CT (SECT).MATERIAL AND METHODS: A 32 cm lateral diameter (CIRS model 062M, Norfolk, Virginia) electron density phantom containing inserts which simulated the chemical composition of eight tissues in a solid-water background was scanned using SECT and DECT. A liquid water insert was included to confirm CT number accuracy. All materials were also placed in four water tanks, ranging from 15 to 45 cm in lateral width and scanned using DECT and SECT. A silicone breast implant was scanned in the same water phantoms. SPR values were calculated based on commercial software (syngo CT Dual Energy, Siemens Healthcare GmbH) and compared to reference values derived from proton beam measurements. Accuracy and precision were quantified across phantom size using percent error and standard deviation. Graphical and regression analysis were used to determine whether SECT or DECT was superior in estimating SPR across phantom size.RESULTS: Both DECT and SECT SPR data resulted in good agreement with the reference values. Percent error was ±3% for both DECT and SECT in all materials except lung and dense bone. The coefficient of variation (CV) across materials and phantom sizes was 1.12% for SECT and 0.96% for DECT. Material-specific regression and graphical analysis did not reveal size dependence for either technique but did show reduced systematic bias with DECT for dense bone and liver. Mean percent error in SPR for the implant was reduced from 11.46% for SECT to 0.49% for DECT.CONCLUSIONS: We demonstrate the superior ability of DECT to mitigate systematic bias in bones and liver and estimate SPR in a silicone breast implant.

The aim of this study was to intra-individually compare the image quality obtained by dual-source, dual-energy (DSDE) computed tomography (CT) examinations and different virtual monoenergetic reconstructions to a low single-energy (SE) scan.Third-generation DSDE-CT was performed in 49 patients with histologically proven malignant disease of the head and neck region. Weighted average images (WAIs) and virtual monoenergetic images (VMIs) for low (40 and 60 keV) and high (120 and 190 keV) energies were reconstructed. A second scan aligned to the jaw, covering the oral cavity, was performed for every patient to reduce artifacts caused by dental hardware using a SE-CT protocol with 70-kV tube voltages and matching radiation dose settings. Objective image quality was evaluated by calculating contrast-to-noise ratios. Subjective image quality was evaluated by experienced radiologists.Highest contrast-to-noise ratios for vessel and tumor attenuation were obtained in 40-keV VMI (all P0.05). Comparable objective results were found in 60-keV VMI, WAI, and the 70-kV SE examinations. Overall subjective image quality was also highest for 40-keV, but differences to 60-keV VMI, WAI, and 70-kV SE were nonsignificant (all P0.05). High kiloelectron volt VMIs reduce metal artifacts with only limited diagnostic impact because of insufficiency in case of severe dental hardware. CTDIvol did not differ significantly between both examination protocols (DSDE: 18.6 mGy; 70-kV SE: 19.4 mGy; P = 0.10).High overall image quality for tumor delineation in head and neck imaging were obtained with 40-keV VMI. However, 70-kV SE examinations are an alternative and modified projections aligned to the jaw are recommended in case of severe artifacts caused by dental hardware.

Photon-counting computed tomography (PCCT) uses a photon counting detector to count individual photons and allocate them to specific energy bins by comparing photon energy to preset thresholds. This enables simultaneous multi-energy CT with a single source and detector. Phantom studies were performed to assess the spectral performance of a research PCCT scanner by assessing the accuracy of derived images sets. Specifically, we assessed the accuracy of iodine quantification in iodine map images and of CT number accuracy in virtual monoenergetic images (VMI). Vials containing iodine with five known concentrations were scanned on the PCCT scanner after being placed in phantoms representing the attenuation of different size patients. For comparison, the same vials and phantoms were also scanned on 2nd and 3rd generation dual-source, dual-energy scanners. After material decomposition, iodine maps were generated, from which iodine concentration was measured for each vial and phantom size and compared with the known concentration. Additionally, VMIs were generated and CT number accuracy was compared to the reference standard, which was calculated based on known iodine concentration and attenuation coefficients at each keV obtained from the U.S. National Institute of Standards and Technology (NIST). Results showed accurate iodine quantification (root mean square error of 0.5 mgI/cc) and accurate CT number of VMIs (percentage error of 8.9%) using the PCCT scanner. The overall performance of the PCCT scanner, in terms of iodine quantification and VMI CT number accuracy, was comparable to that of EID-based dual-source, dual-energy scanners.

Purpose To demonstrate the feasibility of spectral imaging using photon-counting detector (PCD) x-ray computed tomography (CT) for simultaneous material decomposition of three contrast agents in vivo in a large animal model. Methods This Institutional Animal Care and Use Committee-approved study used a canine model. Bismuth subsalicylate was administered orally 24–72 h before imaging. PCD CT was performed during intravenous administration of 40–60 ml gadoterate meglumine; 3.5 min later, iopamidol 370 was injected intravenously. Renal PCD CT images were acquired every 2 s for 5–6 min to capture the wash-in and wash-out kinetics of the contrast agents. Least mean squares linear material decomposition was used to calculate the concentrations of contrast agents in the aorta, renal cortex, renal medulla and renal pelvis. Results Using reference vials with known concentrations of materials, we computed molar concentrations of the various contrast agents during each phase of CT scanning. Material concentration maps allowed simultaneous quantification of both arterial and delayed renal enhancement in a single CT acquisition. The accuracy of the material decomposition algorithm in a test phantom was −0.4 ± 2.2 mM, 0.3 ± 2.2 mM for iodine and gadolinium solutions, respectively. Peak contrast concentration of gadolinium and iodine in the aorta, renal cortex, and renal medulla were observed 16, 24, and 60 s after the start each injection, respectively. Conclusion Photon-counting spectral CT allowed simultaneous material decomposition of multiple contrast agents in vivo. Besides defining contrast agent concentrations, tissue enhancement at multiple phases was observed in a single CT acquisition, potentially obviating the need for multiphase CT scans and thus reducing radiation dose.

Purpose To assess the diagnostic performance of a third-generation dual-energy computed tomographic (CT) virtual noncalcium (VNCa) technique for detection of traumatic bone marrow edema in patients with vertebral compression fractures. Materials and Methods This prospective study was approved by the institutional review board. Informed consent was obtained from all participants. Twenty-two consecutive patients with 37 morphologic vertebral fractures were studied between October 2015 and May 2016. All patients underwent dual-energy CT (90 kV and 150 kV with a tin filter) and 3-T magnetic resonance (MR) imaging. Two independent readers visually evaluated all vertebral bodies (n = 163) for the presence of abnormal bone marrow attenuation on VNCa images by using color-coded maps and performed a quantitative analysis of CT numbers on VNCa images. MR images served as the reference standard. CT numbers were subjected to receiver operating characteristic analysis to calculate cutoff values. Results In the visual analysis, VNCa images had an overall sensitivity of 64.0%, specificity of 99.3%, accuracy of 93.9%, positive predictive value of 94.1%, and negative predictive value of 93.8%. The interobserver agreement was excellent (κ = 0.85). CT numbers obtained from VNCa images were significantly different in vertebral bodies with and without edema (P.001). Receiver operating characteristic analysis revealed an area under the curve of 0.922. A cutoff value of -47 provided sensitivity of 92.0%, specificity of 82.6%, accuracy of 84.0%, positive predictive value of 48.9%, and negative predictive value of 98.3% for the differentiation of edematous vertebral bodies. Conclusion Visual and quantitative analyses of dual-energy VNCa images showed excellent diagnostic performance for assessing traumatic bone marrow edema in vertebral compression fractures.

Background We investigated the influence of dose, spectral separation, pitch, rotation time, and reconstruction kernel on accuracy and image noise of virtual non-calcium images using a bone marrow phantom. Methods The phantom was developed at our institution and scanned using a third-generation dual-source dual-energy CT scanner at five different spectral separations by varying the tube-voltage combinations (70 kV/Sn150 kV, 80 kV/Sn150 kV, 90 kV/Sn150 kV, and 100 kV/Sn150 kV, all with 0.6-mm tin filter [Sn]; 80 kV/140 kV without tin filter) at six different doses (volume computed tomography dose index from 1 to 80 mGy). In separate experiments, rotation times, pitch, and reconstruction kernels were varied at a constant dose and tube voltage. Accuracy was determined by measuring the mean error between virtual non-calcium values in the fluid within and outside of the bone. Image noise was defined as the standard deviation of virtual non-calcium values. Results Spectral separation, dose, rotation time, or pitch did not significantly correlate (p > 0.083) with mean error. Increased spectral separation (rs-0.96, p rs-0.98, p rs 0.83, p = 0.015) and image noise (rs 1.0, p Conclusions Increased dose and increased spectral separation significantly lowered image noise in virtual non-calcium images but did not affect the accuracy. Virtual non-calcium reconstructions with similar accuracy and image noise could be achieved at a lower tube-voltage difference by increasing the dose.

Previous studies have shown that split-bolus protocols in virtual non-contrast (VNC) reconstructions of dual-energy computed tomography (DE-CT) significantly decrease radiation dose in patients with urinary stone disease. To evaluate the impact on kidney stone detection rate of stone composition, size, tube voltage, and iodine concentration for VNC reconstructions of DE-CT. In this prospective study, 16 kidney stones of different sizes (1.2–4.5 mm) and compositions (struvite, cystine, whewellite, brushite) were placed within a kidney phantom. Seventy-two scans with nine different iodine contrast agents/saline solutions with increasing attenuation (0–1400 HU) and different kilovoltage settings (70 kV/150 kV; 80 kV/150 kV; 90 kV/150 kV; 100 kV/150 kV) were performed. Two experienced radiologists independently rated the images for the presence and absence of stones. Multivariate classification tree analysis and descriptive statistics were used to evaluate the diagnostic performance. Classification tree analysis revealed a higher detection rate of renal calculi > 2 mm in size compared with that of renal calculi 2 mm, the best results were found at 70 kV/Sn 150 kV and 80 kV/Sn 150 kV in scans with contrast media attenuation of 600 HU or less, with sensitivity of 99.6% and 96.0%, respectively. Dual-energy computed tomographic angiography (DE-CTA) has been demonstrated to improve the visualization of the head and neck vessels. The aim of this study was to test the potential of split-filter single-source dual-energy CT to automatically remove bone from the final CTA data set.Dual-energy CTA was performed in 50 consecutive patients to evaluate the supra-aortic arteries, either to grade carotid artery stenosis or to rule out traumatic dissections. Dual-energy CTA was performed on a 128-slice single-source CT system equipped with a special filter array to separate the 120-kV spectrum into a high- and a low-energy spectrum for DE-based automated bone removal. Image quality of fully automated bone suppression and subsequent manual optimization was evaluated by 2 radiologists on maximum intensity projections using a 4-grade scoring system. The effect of image reconstruction with an iterative metal artifact reduction algorithm on DE postprocessing was tested using a 3-grade scoring system, and the time demand for each postprocessing step was measured.Two patients were excluded due to insufficient arterial contrast enhancement; in the remaining 48 patients, automated bone removal could be performed successfully. The addition of iterative metal artifact reduction algorithm improved image quality in 58.3% of the cases. After manual optimization, DE-CTA image quality was rated excellent in 7, good in 29, and moderate in 10 patients. Interobserver agreement was high (κ = 0.85). Stenosis grading was not influenced using DE-CTA with bone removal as compared with the original CTA. The time demand for DE image reconstruction was significantly higher than for single-energy reconstruction (42.1 vs 20.9 seconds).Our results suggest that bone removal in DE-CTA of the head and neck vessels with a single-source CT is feasible and can be performed within acceptable time and moderate user interaction.

Background and purposeTo assess the performance of dual energy unenhanced CT in the detection of acute ischemia after mechanical thrombectomy.MethodsRetrospective study, approved by the local institutional review board, including all patients that underwent intra-arterial thrombectomy in our institution over a period of 2 years. The presence of acute ischemia and hemorrhage was evaluated by three readers. Sensitivity and specificity of the non-contrast CT weighted sum image (NCCT) and the virtual non-contrast reconstructed image (VNC) were estimated and compared using generalized estimating equations to account for the non-independence of regions in each patient.Results58 patients (27 women and 31 men; mean age 70.4 years) were included in the study, yielding 580 regions of interest. Sensitivity and specificity in detecting acute ischemia were higher for all readers when using VNC, with a significant increase in sensitivity for two readers (pConclusionsDual energy unenhanced CT VNC images were superior in the identification of acute ischemia in comparison with NCCT.

OBJECTIVE: To determine the value of advanced virtual monoenergetic images (mono+) from dual-energy computed tomography (CT) of hyperattenuating and hypoattenuating liver lesions in various phantom sizes and patients in comparison with standard monoenergetic images (mono). MATERIALS AND METHODS: Anthropomorphic phantoms simulating 4 patient sizes (S, 300 × 200 mm; M, 350 × 250 mm; L, 400 × 300 mm; and XL, 600 × 450 mm) with a liver insert containing both hyperattenuating and hypoattenuating iodine-containing lesions were imaged with dose-equivalent dual-energy (100/150 Sn kilovolt [peak] [kV{p}]) and single-energy (120 kV[p]) protocols on a 192-slice dual-source CT system. In addition, 4 patients with 3 hypoattenuating and 3 hyperattenuating hepatocellular carcinoma were included and underwent dual-energy CT imaging with the same scanner at similar kV(p) settings (100/150 Sn kV[p]). Images were reconstructed with standard mono and with the mono+ algorithm at 10-kiloelectron volt (keV) intervals from 40 to 190 keV. Attenuation of the liver and lesions were measured, and contrast-to-noise ratios (CNRs) were calculated. Lesion conspicuity was rated by 2 blinded independent readers in all mono and mono+ data sets from 40 to 190 keV using a 5-point Likert scale (1, lowest conspicuity; and 5, highest conspicuity). RESULTS: Attenuation in the liver and in both hyperattenuating and hypoattenuating lesions did not differ between mono and mono+ (P = 0.41-0.49). Noise on mono+ was significantly lower than on mono for all phantom sizes (P < 0.05) and was increasing with phantom size. Hyperattenuating lesion CNR was highest for mono+ images at 40 keV in the S phantom (6.73), with significantly higher CNR for mono+ than for mono and for single energy (120 kV[p]) in all phantom sizes (all P < 0.001) except for the XL phantom. Hypoattenuating lesion CNR was highest for high-keV mono+ being significantly higher than on mono and on single-energy (120 kV[p]) images (all P < 0.001), except for the XL phantom with significantly higher CNR for mono (1.3) compared with mono+ (0.47) and 120 kV(p) (1.26). In patients, CNR curves of hyperattenuating hepatocellular carcinoma were in accordance with the phantom data, whereas hypoattenuating lesions demonstrate varying curves, some being in accordance with findings in phantoms. Interreader agreement for lesion conspicuity was very good (intraclass correlation, 0.95), with higher conspicuity scores for mono+ than for mono and single energy (120 kV[p]) at all phantom sizes (all P < 0.05) and within patients. CONCLUSION: Our ex vivo and patient data demonstrate added value for imaging of both hyperattenuating and hypoattenuating liver lesions with advanced virtual monoenergetic dual-energy CT by decreased noise, increased CNR, and higher lesion conspicuity, although with limitations in XL body sizes.

OBJECTIVE. Dual-energy CT (DECT) enables subtraction of calcium, facilitating the visualization of bone marrow (BM) in the axial skeleton. The purpose of this study was to assess whether DECT BM images have the potential to improve the detection of multifocal and diffuse BM infiltration in multiple myeloma (MM) in comparison with regular CT with MRI as the reference standard. SUBJECTS AND METHODS. This study included 32 consecutive patients who had known MM or presented with monoclonal gammopathy of unknown significance and underwent DECT and MRI of the axial skeleton. The degrees (none, n = 14; moderate, n = 10; and high, n = 8) and patterns (diffuse, n = 10 or multifocal, n = 8) of infiltration were assessed on MR images. Attenuation in BM and CT images in known uninvolved and involved areas was measured. Cutoff values of attenuation in BM images for infiltration in lytic and nonlytic lesions were established by ROC analysis. At least 120 days later, sensitivity and specificity for reading CT images alone and when using additional BM images were evaluated. RESULTS. ROC analysis revealed larger AUC in BM images than in CT images; cutoff values for marrow invasion in BM images were 4 and -3 HU in lytic and nonlytic lesions, respectively. In the blinded reading session, BM images improved the sensitivity for the detection of diffuse infiltration from 0 to as much as 75% for cases with high-grade infiltration. In multifocal patterns, BM images did not significantly change the detection rate. CONCLUSION. BM images have the potential to improve the sensitivity for detection of diffuse BM involvement in comparison with regular CT.

One method to acquire dual-energy (DE) computed tomography (CT) data is to perform CT scans at 2 different x-ray tube voltages, typically 80 and 140 kV, either as 2 separate scans, by means of rapid kV switching, or with the use of 2 x-ray sources as in dual-source CT (DSCT) systems. In DSCT, it is possible to improve spectral separation with tin prefiltration (Sn) of the high-kV beam. Recently, x-ray tube voltages beyond the established range of 80 to 140 kV were commercially introduced, which enable additional voltage combinations for DE acquisitions, such as 80/150 Sn or 90/150 Sn kV. Here, we investigate the DE performance of several x-ray tube voltages and prefilter combinations on 2 DSCT scanners and the impact of the spectra on quantitative analysis and dose efficiency.Circular phantoms of different sizes (10-40 cm in diameter) equipped with cylindrical inserts containing water and diluted iodine contrast agent (14.5 mg/cm) were scanned using 2 different DSCT systems (SOMATOM Definition Flash and SOMATOM Force; Siemens AG, Forchheim, Germany). Five x-ray tube voltage combinations (80/140 Sn, 100/140 Sn, 80/150 Sn, 90/150 Sn, and 100/150 Sn kV) were investigated, and the results were compared with the previous standard acquisition technique (80/140 kV). As an example, 80/140 Sn kV means that 1 x-ray tube of the DSCT system was operated at 80 kV, whereas the other was operated at 140 kV with additional tin prefiltration (Sn). Dose values in terms of computed tomography dose index (CTDIvol) were kept constant between the different voltage combinations but adjusted with regard to object size according to automatic exposure control recommendations. Reconstructed images were processed using linear blending of the low- and high-kV CT images to combined images, as well as 3-material decomposition techniques to generate virtual noncontrast (VNC) images and iodine images. Contrast and pixel noise were evaluated, as well as DE ratios, which are defined as the CT value at low kV divided by the CT value at high kV.For the 10-, 20-, 30-, and 40-cm phantom, dose values in terms of CTDIvol were 1.2, 2.6, 7.3, and 21.6 mGy, respectively. In the combined images, those obtained with tin filtration showed lower noise values at similar iodine enhancement levels than did images obtained without tin filtration. The largest differences in noise were observed for the larger phantoms, in particular the 40-cm phantom. Dual-energy ratios for iodine increased with decreasing voltages of the low-kV beam and with increasing voltages of the high-kV beam, and they increased when tin prefiltration was added. In case of the 20-cm phantom, DE ratios ranged from 2.0 at 80/140 kV to 3.4 at 80/150 Sn kV. The noise level of the VNC images was strongly correlated with the inverse of the DE ratio. Irrespective of the phantom size, the lowest noise values were measured for 80/150 Sn kV.Dual-source CT systems enable DE data to be acquired using a variety of voltage combinations. Combined (or mixed) DE images provide an image impression similar to standard 120 kV images, yet the noise level depends on the DE voltage combination that is selected. Noise in decomposed VNC images is strongly influenced by the DE ratio, and it improves substantially with tin filtration of the high-voltage beam.

This study evaluates the capabilities of a whole-body photon counting CT system to differentiate between four common kidney stone materials, namely uric acid (UA), calcium oxalate monohydrate (COM), cystine (CYS),and apatite (APA) ex vivo. Two different x-ray spectra (120 kV and 140 kV) were applied and two acquisition modes were investigated; The macro-mode generates two energy threshold based image-volumes and two energy bin based image-volumes. In the chesspattern-mode, however, four energy thresholds are applied. A virtual low energy image, as well as a virtual high energy image are derived from initial threshold-based images, while considering their statistically correlated nature. The energy bin based images of the macro-mode, as well as the virtual low and high energy image of the chesspattern-mode serve as input for our dual energy evaluation. The dual energy ratio of the individually segmented kidney stones were utilized to quantify the discriminability of the different materials. The dual energy ratios of the two acquisition modes showed high correlation for both applied spectra. Wilcoxon-rank sum tests and the evaluation of the area under the receiver operating characteristics curves suggest that the UA kidney stones are best differentiable from all other materials (AUC = 1.0), followed by CYS (AUC ≈ 0.9 compared against COM and APA). COM and APA, however, are hardly distinguishable (AUC between 0.63 and 0.76). The results hold true for the measurements of both spectra and both acquisition modes.

The aim of this study was to determine whether dual-energy computed tomography (DECT) imaging is superior to conventional noncontrast computed tomography (CT) imaging for the detection of acute ischemic stroke.This was a retrospective, single-center study of 40 patients who presented to the emergency department (ED) of a major, acute care, teaching center with signs and symptoms of acute stroke. Only those patients who presented to the ED within 4 hours of symptom onset were included in this study. All 40 patients received a noncontrast DECT of the head at the time of presentation. Each patient also received standard noncontrast CT of the head 24 hours after their initial presentation to the ED. "Brain edema" images were then reconstructed using 3-material decomposition with parameters adjusted to suppress gray/white matter contrast while preserving edema and increasing its conspicuity. The initial unenhanced, mixed images, brain edema, and 24-hour follow-up true noncontrast (TNC) images were reviewed and assigned Alberta Stroke Program Early CT scores. Sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) were calculated.Of the 40 patients, 28 (70%) were diagnosed with an acute infarction. Brain edema reconstructions were better able to predict end infarction volume, with Alberta Stroke Program Early CT scores similar to the 24-hour follow-up TNC CT (7.75 vs 7.7; P0.05), whereas the mixed images routinely underestimated the extent of infarction (8.975 vs 7.7; P0.001). Initial TNC images had a sensitivity, specificity, PPV, and NPV of 80% (95% confidence interval [CI], 51.9%-95.7%), 72.7% (95% CI, 39%-94%), 80% (95% CI, 51.9%-95.7%), and 72.73% (95% CI, 51.91%-95.67%), respectively. The DECT brain edema images provided a sensitivity, specificity, PPV, and NPV of 93.33% (95% CI, 68.05%-99.83%), 100% (95% CI, 71.51%-100%), 100% (95% CI, 76.84%-100%), and 91.67% (95% CI, 61.52%-99.79%), respectively. There was very good interrater reliability across all 3 imaging techniques.Brain edema reconstructions are able to more accurately detect edema and end-infarct volume as compared with initial TNC images. This provides a better assessment of the degree and extent of infarction and may serve to better guide therapy in the future.

Purpose To determine the diagnostic performance of dual-energy computed tomography (CT) for detection of bone marrow (BM) infiltration in patients with multiple myeloma by using a virtual noncalcium (VNCa) technique. Materials and Methods In this prospective study, 34 consecutive patients with multiple myeloma or monoclonal gammopathy of unknown significance sequentially underwent dual-energy CT and magnetic resonance (MR) imaging of the axial skeleton. Two independent readers visually evaluated standard CT and color-coded VNCa images for the presence of BM involvement. MR imaging served as the reference standard. Analysis on the basis of the region of interest (ROI) of VNCa CT numbers of infiltrated (n = 75) and normal (n = 170) BM was performed and CT numbers were subjected to receiver operating characteristic analysis to calculate cutoff values. Results In the visual analysis, VNCa images had an overall sensitivity of 91.3% (21 of 23), specificity of 90.9% (10 of 11), accuracy of 91.2% (31 of 34), positive predictive value of 95.5% (21 of 22), and a negative predictive value of 83.3% (10 of 12). ROI-based analysis of VNCa CT numbers showed a significant difference between infiltrated and normal BM (P.001). Receiver operating characteristic analysis revealed an area under the curve of 0.978. A cutoff of -44.9 HU provided a sensitivity of 93.3% (70 of 75), specificity of 92.4% (157 of 170), accuracy of 92.7% (227 of 245), positive predictive value of 84.3% (70 of 83), and negative predictive value of 96.9% (157 of 162) for the detection of BM infiltration. Conclusion Visual and ROI-based analyses of dual-energy VNCa images had excellent diagnostic performance for assessing BM infiltration in patients with multiple myeloma with precision comparable to that of MR imaging.

OBJECTIVES Computed tomography angiography (CTA) is a valuable tool for the assessment of carotid artery stenosis. However, blooming artifacts from calcified plaques might result in an overestimation of the stenosis grade. The aim of this study was to investigate a new dual-energy computed tomography (DECT) technique with a modified 3-material decomposition algorithm for calcium removal in extracranial carotid artery stenosis. MATERIALS AND METHODS In this retrospective, institutional review board-approved study, 30 calcified carotid plaques in 22 patients (15 men; mean age, 73 ± 10 years) with clinical suspicion of stroke were included. Dual-energy computed tomography image data were obtained using second-generation dual-source CT with tube voltages at 80 and 140Sn kVp. Conventional CTA and virtual noncalcium (VNCa) images using the modified DECT algorithm were reconstructed. By assessing spectral characteristics, the modified DECT algorithm allows for a selective removal of calcium independent of blooming. Two independent and blinded readers evaluated subjective image quality, blooming artifacts, amount of (residual) calcification, and performed stenosis measurements according to the North American Symptomatic Carotid Endarterectomy Trial (NASCET) criteria. Differences were tested using a pairwise sign test. Paired sample t tests with Bonferroni correction (P < 0.017) and Bland-Altman analyses were used to test for differences in carotid stenosis measurements between VNCa and conventional CTA using digital subtraction angiography (DSA) as the standard of reference. RESULTS Subjective image quality was similar among conventional CTA and VNCa image data sets (P = 0.82), whereas blooming artifacts were significantly reduced in VNCa images compared with conventional CTA (P < 0.001). Residual calcifications in VNCa images were absent in 11 (37%), minor in 12 (40%), medium sized in 2 (7%), and large in 5 (17%) arteries. Stenosis measurements differed significantly between VNCa (mean NASCET stenosis: 27% ± 20%) and conventional CTA images (mean NASCET stenosis: 39% ± 16%; P < 0.001) and between conventional CTA and DSA (23% ± 16%, P < 0.001). No significant differences in stenosis measurements were observed between VNCa and DSA (P = 0.189), with narrow limits of agreement (mean difference ±1.96 standard deviations: -4.7%, -35.1%, and 25.7%). CONCLUSIONS A modified 3-material decomposition DECT algorithm for calcium removal was introduced, which allows for an accurate removal of calcified carotid plaques in extracranial carotid artery disease. The algorithm might overcome the problem of overestimation of calcified stenosis due to blooming artifacts in conventional CTA.

Deep venous thrombosis (DVT) can be difficult to detect using CT due to poor and heterogeneous contrast. Dual-energy CT (DECT) allows iodine contrast optimization using noise-optimized monoenergetic extrapolations (MEIs) and iodine maps (IMs). Our aim was to assess whether MEI and IM could improve the delineation of thrombotic material within iodine-enhanced blood compared to single-energy CT (SECT). Six vessel phantoms, including human thrombus and contrast media-enhanced blood and one phantom without contrast, were placed in an attenuation phantom and scanned with DECT 100/140 kV and SECT 120 kV. IM, virtual non-contrast images (VNC), mixed images, and MEI were calculated. Attenuation of thrombi and blood were measured. Contrast and contrast-to-noise-ratios (CNRs) were calculated and compared among IM, VNC, mixed images, MEI, and SECT using paired t tests. MEI40keV and IM showed significantly higher contrast and CNR than SE120kV from high to intermediate iodine concentrations (contrast:pMEI40keV

Following the trend of low-radiation dose computed tomographic (CT) imaging, concerns regarding the detectability of low-contrast lesions have been growing. The goal of this research was to evaluate whether a new image-based algorithm (Mono+) for virtual monoenergetic imaging with a dual-energy CT scanner can improve the contrast-to-noise ratio (CNR) and conspicuity of these low-contrast objects when using iodinated contrast media.Four circular phantoms of different diameter (10-40 cm) with an iodine insert at the center were scanned at a fixed radiation dose with different single- (80, 100, 120 kV) and dual-energy protocols (80/140 kV, 80/140 Sn kV, 100/140 Sn kV) using a dual-source CT system. In addition, an anthropomorphic abdominal phantom with different low-contrast lesions was scanned with the settings previously mentioned but also at only a half and a quarter of the initial dose. Dual-energy data were processed, and virtual monoenergetic images (range, 40-190 keV) were generated. Beside the established technique, a newly developed prototype algorithm to calculate monoenergetic images (Mono+) was used. To avoid noise increase at lower calculated energies, which is a known drawback of virtual monoenergetic images at low kilo electron-volt, a regional spatial frequency-based recombination of the high signal at lower energies and the superior noise properties at medium energies is performed to optimize CNR in case of Mono+ images. The CNR and low-contrast detectability were evaluated.For all phantom sizes, the Mono+ technique provided increasing iodine CNR with decreasing kilo electron-volt, with the optimum CNR obtained at the lowest energy level of 40 keV. For all investigated phantom sizes, CNR of Mono+ images at low kilo electron-volt was superior to the CNR in single-energy images at an equivalent radiation dose and even higher than the CNR obtained with 80-kV protocols. In case of the anthropomorphic phantom, low-contrast detectability in monoenergetic images was, for all settings, similar to the circular phantoms, best for the voltage combination 80/140 Sn kV, irrespective of the dose level. For all dual-energy voltage combinations, the Mono+ algorithm led to superior results compared with single-energy imaging.With regard to optimized iodine CNR, it is more efficient to perform dual-energy scans and compute virtual monoenergetic images at 40 keV using the Mono+ technique than to perform low kilovolt scans. Given the improved CNR, the Mono+ algorithm could be very useful in improving both detection and differential diagnosis of abdominal lesions, specifically low-contrast lesions, as well as in other anatomical regions where improved iodine CNR is beneficial.

Objectives To compare dual-energy CT images (DECT) directly with gold standard histology. Methods A 85-year-old woman with chronic recurrent gouty arthritis and macroscopically visible tophi was assessed with DECT-scans of her feet. When she died 7 days later, three tophi in different regions and in different tissues of her left foot were processed for histological examination. Results Of the selected tophi, two were almost completely missed by the color-coding DECT sequences. The tophi remained in most of their volume below the detection threshold value of 150 HU (default value). Conclusions It could be demonstrated that DECT only highlights the dense tophi (corresponding of approximately 15–20 vol% urate in the tophus). Less dense tophi, despite considerable size, will be missed in the color-coded images.

PURPOSE: The purposes of this study were to demonstrate the feasibility and accuracy of single-source dual-energy (DE) computed tomography (CT) with sequential data acquisition and a coregistration motion correction algorithm for urinary stone characterization and to evaluate the value of iterative reconstructions (IRs) in DE imaging. MATERIALS AND METHODS: Thirty-five urinary stones were placed in cylindrical phantoms with diameters of 30 and 40 cm. The phantoms were scanned on a 64-section CT machine with a single-source DE protocol consisting of 2 sequential acquisitions at 80 and 140 kilovolt (peak). The phantom was moved between the 80- and 140-kilovolt (peak) scans. Images were reconstructed with weighted filtered back projection (FBP) and with IR, and data were coregistered. Two independent and blinded readers assessed data sets for stone detection, overall image quality, and visibility of stones. Image noise and Hounsfield unit values of the stones were measured, and the DE index was calculated. In addition, the data sets were analyzed on color-coded images using the standard postprocessing software for differentiating uric acid- (UA) from non-UA-containing stones. RESULTS: The motion correction algorithm achieved a good coregistration of the 2 scans with different energy levels. Both readers detected all stones on all data sets with both reconstruction types. The overall image quality was rated significantly higher in IR images in the 40-cm phantom as compared with that in FBP images (P < 0.05), whereas no significant difference was found for the 30-cm phantom. Visibility of stones was rated significantly higher for both the 30- and 40-cm phantoms on IR as compared with that on FBP images, an effect that was pronounced for UA stones (P < 0.05). Noise was significantly reduced by up to 31% in the 40-cm phantom when using IR as compared with FBP (P < 0.001). The DE index was similar in the FBP and IR data sets for the 30- (P = 0.116) and 40-cm phantoms (P = 0.544), being significantly different between UA-containing stones, cystine, and struvite stones as well as stones of other compositions (P < 0.001). The postprocessing software classified all stones correctly as UA- or non-UA-containing stones on color-coded images. In the 40-cm phantom, false-positively colored voxels were found in the FBP data sets, which were not seen when using IR instead. CONCLUSIONS: Our study indicates that single-source dual-energy CT with sequential acquisitions at different energy levels and a coregistration motion correction algorithm is feasible and accurate for characterizing urinary stone composition on the basis of phantom evaluation. As compared with reconstructions with FBP, the use of IR in dual-energy CT reduces noise, improves overall image quality and visibility of stones particularly in large phantoms, and helps to avoid false classifications of urinary stones.

We evaluated a dual energy, split bolus computerized tomography protocol that provides virtual noncontrast, parenchymal and urographic phases in a single scan. We assessed the sensitivity of the virtual noncontrast phase using this protocol to detect urinary stones compared to the gold standard of the true noncontrast phase.We prospectively enrolled in the study 81 patients who underwent unenhanced single energy computerized tomography at 120 kV/200 mA as well as contrast enhanced dual energy computerized tomography on a Somatom® Definition Flash-CT (tube A 80 kV/233 mA and tube B SN 140 kV/180 mA with 1/0.8 mm slice thickness). For the split bolus protocol 400 mg/ml Iomeron® were injected at 2 time points, that is 15 ml 10 minutes before the scan and 80 ml 65 seconds before the scan. In a consensus reading 2 readers evaluated the presence and diameter of stones on the true and virtual noncontrast phases.Of the 350 stones noted on the true noncontrast phase we found 289 on the virtual noncontrast phase as well as 13 false-positive and 66 false-negative stones. Sensitivity was 98.4%, 89.8% and 82.6% per patient, segment and stone, respectively. The diameter measured on the virtual noncontrast phase corresponded to a mean ± SD 92.5% ± 31.6% of the diameter on the true noncontrast phase. The mean effective dose was 4.8 ± 1.8 and 10.5 ± 3.7 mSv for the true and virtual noncontrast phases, respectively.The proposed protocol allows for combining 3 phases in a single scan while still enabling the detection of urinary stones at high sensitivity. This technique halves the radiation dose and provides the surgeon with better anatomical information on the calyceal system. Therefore, it is a valuable diagnostic tool for kidney stone treatment planning and followup.

Inaccurate conversion of CT data to water-equivalent path length (WEPL) is one of the most important uncertainty sources in ion treatment planning. Dual energy CT (DECT) imaging might help to reduce CT number ambiguities with the additional information. In our study we scanned a series of materials (tissue substitutes, aluminum, PMMA, and other polymers) in the dual source scanner (Siemens Somatom Definition Flash). Based on the 80 kVp/140Sn kVp dual energy images, the electron densities ϱe and effective atomic numbers Zeff were calculated. We introduced a new lookup table that translates the ϱe to the WEPL. The WEPL residuals from the calibration were significantly reduced for the investigated tissue surrogates compared to the empirical Hounsfield-look-up table (single energy CT imaging) from (−1.0 ± 1.8)% to (0.1 ± 0.7)% and for non-tissue equivalent PMMA from −7.8% to −1.0%. To assess the benefit of the new DECT calibration, we conducted a treatment planning study for three different idealized cases based on tissue surrogates and PMMA. The DECT calibration yielded a significantly higher target coverage in tissue surrogates and phantom material (i.e. PMMA cylinder, mean target coverage improved from 62% to 98%). To verify the DECT calibration for real tissue, ion ranges through a frozen pig head were measured and compared to predictions calculated by the standard single energy CT calibration and the novel DECT calibration. By using this method, an improvement of ion range estimation from −2.1% water-equivalent thickness deviation (single energy CT) to 0.3% (DECT) was achieved. If one excludes raypaths located on the edge of the sample accompanied with high uncertainties, no significant difference could be observed.

To compare image quality and radiation dose of abdominal split-filter dual-energy CT (SF-DECT) combined with monoenergetic imaging to single-energy CT (SECT) with automatic tube voltage selection (ATVS). Two-hundred single-source abdominal CT scans were performed as SECT with ATVS (n = 100) and SF-DECT (n = 100). SF-DECT scans were reconstructed and subdivided into composed images (SF-CI) and monoenergetic images at 55 keV (SF-MI). Objective and subjective image quality were compared among single-energy images (SEI), SF-CI and SF-MI. CNR and FOM were separately calculated for the liver (e.g. CNRliv) and the portal vein (CNRpv). Radiation dose was compared using size-specific dose estimate (SSDE). Results of the three groups were compared using non-parametric tests. Image noise of SF-CI was 18% lower compared to SEI and 48% lower compared to SF-MI (p 0.628). Subjective sharpness was equal between single-energy and monoenergetic images and diagnostic confidence was equal between single-energy and composed images. FOMliv was highest for SF-CI. FOMpv was equal for SEI and SF-MI (p = 0.78). SSDE was significant lower for SF-DECT compared to SECT (p < 0.022). The combined use of split-filter dual-energy CT images provides comparable objective and subjective image quality at lower radiation dose compared to single-energy CT with ATVS. • Split-filter dual-energy results in 18% lower noise compared to single-energy with ATVS. • Split-filter dual-energy results in 11% lower SSDE compared to single-energy with ATVS. • Spectral shaping of split-filter dual-energy leads to an increased dose-efficiency.

Advanced virtual monochromatic reconstruction from dual-energy brain CT has not been evaluated in children. To determine the most effective advanced virtual monochromatic imaging energy level for maximizing pediatric brain parenchymal image quality in dual-energy unenhanced brain CT and to compare this technique with conventional monochromatic reconstruction and polychromatic scanning. Using both conventional (Mono) and advanced monochromatic reconstruction (Mono+) techniques, we retrospectively reconstructed 13 virtual monochromatic imaging energy levels from 40 keV to 100 keV in 5-keV increments from dual-source, dual-energy unenhanced brain CT scans obtained in 23 children. We analyzed gray and white matter noise ratios, signal-to-noise ratios and contrast-to-noise ratio, and posterior fossa artifact. We chose the optimal mono-energetic levels and compared them with conventional CT. For Mono+maximum optima were observed at 60 keV, and minimum posterior fossa artifact at 70 keV. For Mono, optima were at 65–70 keV, with minimum posterior fossa artifact at 75 keV. Mono+ was superior to Mono and to polychromatic CT for image-quality measures. Subjective analysis rated Mono+superior to other image sets. Optimal virtual monochromatic imaging using Mono+ algorithm demonstrated better image quality for gray–white matter differentiation and reduction of the artifact in the posterior fossa.

Purpose To evaluate the potential of gadolinium (Gd) as contrast material (CM) in second generation dual energy computed tomography (DECT). Material and methods In a phantom model, DECT post-processing was used to increase Gd attenuation using advanced monoenergetic extrapolation (MEI), to create virtual non-contrast images (Gd-VNC) and Gd maps and to quantify Gd content. Dilutions of Gd and iodinated CM (7–296 HU) were filled in syringes, placed in an attenuation phantom and scanned with standard DECT protocols (80 &100/Sn140 kV). MEI (40–190 keV) and VNC images as well as Gd maps were computed. The amount of Gd was quantified and the accuracy was compared to iodine images. Linear regression models were calculated to evaluate Gd attenuation of equivolume CM doses and clinical MRI doses. Results Applying monoenergetic reconstructions and using Gd as contrast agent (Gd MEI 40 keV) doubled Hounsfield-Units (HU) and 90% of the SNR (averaged: 225 HU, SNR3.1) are achievable, as compared to iodinated CM at 120 kV (averaged:110 HU, SNR3.5), at Gd doses of 1.0 mmol/kg BW. The accuracies of Gd-VNC (deviation, 6 ± 12 HU) images and Gd quantification (measurement error, 17%) were not significantly different to those of iodine enhanced images (VNC:deviation, 2 ± 11 HU; measurement error,14%). Conclusion Using monoenergetic extrapolation at 40 keV, it is possible to increase Gd-CM attenuation significantly. Thus, equivalent HU and half the SNR in comparison to a standard dose of ICM at 120 kV can be expected at a Gd-CM dose of 0.5 mmol/kg BW. Post-processing features of iodine based DECT like monoenergetic or VNC images, iodine maps or quantification of CM are feasible with the use of Gd-CM.

Rational and Objectives To investigate whether a nonlinear-blending algorithm improves tumor conspicuity and image quality in the evaluation of renal masses at dual-energy computed tomography (DECT) during nephrographic phase of enhancement. Materials and Methods The Institutional Review Board approved this retrospective study from archival material from patients consenting to the use of medical records for research purposes. A retrospective review of contrast-enhanced abdominal DECT scans in 45 patients (mean age, 59.5 years; range, 24–84 years) was performed. DECT data were reconstructed using nonlinear and linear blending. A region of interest was located within tumors and adjacent normal parenchyma; attenuation differences and contrast-to-noise ratios (CNRs) were calculated for renal masses on nonlinear- and linear-blended images. The two datasets were subjectively compared in terms of tumor detection and image quality. An exact Wilcoxon's matched pairs signed rank and marginal homogeneity tests were used to test whether differences in attenuation, CNR, and subjective assessment were greater using nonlinear blending. Results The mean difference in attenuation for renal masses and adjacent portion of renal parenchyma was 138.4 Hounsfield units ± 28.9 SD using nonlinear blending, and 121.6 HU ± 18.0 SD using linear blending ( P P P Conclusion Compared with standard linear blending, nonlinear-blending algorithm improves tumor conspicuity and image quality in renal masses at DECT evaluation during nephrographic phase of enhancement.

Objective To compare quantitative and qualitative image quality parameters in pediatric abdominopelvic dual-energy CT (DECT) using noise-optimized virtual monoenergetic image (VMI) and conventional VMI at different kiloelectron volt (keV) levels. Materials and Methods Thirty-six consecutive abdominopelvic DECT scans were retrospectively included. Noise-optimized VMI and conventional VMI were reconstructed at seven energy levels, from 40 keV to 100 keV at 10 keV intervals. The contrast-to-noise ratio (CNR) and signal-to-noise ratio (SNR) of the liver, pancreas, and aorta were objectively measured and compared. Image quality was evaluated subjectively regarding image noise, image blurring of solid organ, bowel image quality and severity of beam-hardening artifacts. Optimal monoenergetic levels in keV for both algorithms were determined based on overall image quality score. Results The maximal CNR and SNR values for all investigated organs were observed at 40 keV in noise-optimized VMI (CNR and SNR of liver, pancreas, aorta in order [CNR; 20.93, 17.34, 46.75: SNR; 37.39, 33.80, 63.21]), at 60–70 keV and at 70 keV in conventional VMI (CNR; 8.12, 5.67, 15.97: SNR; 19.57, 16.66, 26.65). In qualitative image analysis, noise-optimized VMI and conventional VMI showed the best overall image quality scores at 60 keV and at 70 keV, respectively. Noise-optimized VMI at 60 keV showed superior CNRs, SNRs, and overall image quality scores compared to conventional VMI at 70 keV (p < 0.001). Conclusion Optimal energy levels for noise-optimized VMI and conventional VMI were 60 keV and at 70 keV, respectively. Noise-optimized VMI shows superior CNRs, SNRs and subjective image quality over conventional VMI, at the optimal energy level.

Purpose We sought to evaluate the feasibility and efficiency of dual energy (DE) bone and plaque removal in head and neck CT angiography. Materials and methods 20 patients with suspected carotid stenoses received head and neck DE-CTA as part of their pre-interventional workup. Visual grading using multiplanar reformations (MPR), thick slab maximum intensity projections (MIP) and quantitative vessel analysis (QVA) of stenoses was performed prior and after DE bone removal. Results were evaluated for the detection of relevant stenoses (vessel area reduction >70%). Vessel segmentation errors were analyzed. Results Segmentation errors occurred in 19% of all vessel segments. Nevertheless, most post-bone removal artifacts could be recognized using the MPR technique for reading. Compared to MPR reading prior to bone removal, sensitivity, specificity, positive and negative predictive values after bone removal were 100%, 98%, 88% and 100% for MPR reading and 100%, 91%, 63% and 100% for exclusive MIP reading, respectively. There was a good agreement between the QVA results prior and post-DE plaque removal (r2 = 0.8858). Conclusion DE bone and plaque removal for head and neck angiography is feasible and offers a rapid and highly sensitive overview over vascular head and neck studies. Due to a slightly limited specificity of the MIP technique due to segmentation errors, possible stenoses should be verified and graded using MPR techniques.

To evaluate traumatized bone marrow with a dual-energy (DE) computed tomographic (CT) virtual noncalcium technique.In this prospective institutional review board-approved study, 21 patients with an acute knee trauma underwent DE CT and magnetic resonance (MR) imaging. A software application was used to virtually subtract calcium from the images. Presence of fractures was noted, and presence of bone bruise was rated on a four-point scale for six femoral and tibial regions by two radiologists. CT numbers were obtained in the same regions. Consensus reading of independently read MR images served as the reference standard. Image ratings and CT numbers were subjected to receiver operating characteristic curve analysis.After exclusion of 16 regions owing to artifacts, MR imaging revealed 59 bone bruises in the remaining 236 regions (19 of 114 femoral, 40 of 122 tibial). Fractures were present in eight patients. Visual rating revealed areas under the curve of 0.886 and 0.897 in the femur and 0.974 and 0.953 in the tibia for observers 1 and 2, respectively. For CT numbers, the respective areas under the curve were 0.922 and 0.974. If scores of 1 and 2 (strong or mild bone bruise) were counted as positive, sensitivities were 86.4% and 86.4% and specificities were 94.4% and 95.5% for observers 1 and 2, respectively. The kappa statistic demonstrated good to excellent agreement (femur, kappa = 0.78; tibia, kappa = 0.87).This DE CT virtual noncalcium technique can subtract calcium from cancellous bone, allowing bone marrow assessment and potentially making posttraumatic bone bruises of the knee detectable with CT.

Objectives In dual energy (DE) computed tomography (CT), spectral shaping by additional filtration of the high energy spectrum can theoretically improve dual energy contrast. The aim of this in vitro study was to examine the influence of an additional tin filter for the differentiation of human urinary calculi by dual energy CT. Materials and methods A total of 36 pure human urinary calculi (uric acid, cystine, calciumoxalate monohydrate, calciumoxalate dihydrate, carbonatapatite, brushite, average diameter 10.5 mm) were placed in a phantom and imaged with 2 dual source CT scanners. One scanner was equipped with an additional tin (Sn) filter. Different combinations of tube voltages (140/80 kV, 140/100 kV, Sn140/100 kV, Sn140/80 kV, with Sn140 referring to 140 kV with the tin filter) were applied. Tube currents were adapted to yield comparable dose indices. Low- and high energy images were reconstructed. The calculi were segmented semiautomatically in the datasets and DE ratios (attenuation@low_kV/attenuation@high_kV) and were calculated for each calculus. DE contrasts (DE-ratio_material1/DE-ratio_material2) were computed for uric acid, cystine and calcified calculi and compared between the combinations of tube voltages. Results Using exclusively DE ratios, all uric acid, cystine and calcified calculi (as a group) could be differentiated in all protocols; the calcified calculi could not be differentiated among each other in any examination protocol. The highest DE ratios and DE contrasts were measured for the Sn140/80 protocol (53%-62% higher DE contrast than in the 140/80 kV protocol without additional filtration). The DE ratios and DE contrasts of the 80/140 kV and 100/Sn140 kV protocols were comparable. Conclusion Uric acid, cystine and calcified calculi could be reliably differentiated by any of the protocols. A dose-neutral gain of DE contrast was found in the Sn-filter protocols, which might improve the differentiation of smaller calculi (Sn140/80 kV) and improve image quality and calculi differentiation in larger patients (Sn140/100 kV). However, even with the improved spectral separation of the Sn-filter protocols, the DE ratios of calcified calculi are not sufficiently distinct to allow a differentiation within this group.

During the past years, multi-detector row CT (MDCT) has evolved into clinical practice with a rapid increase of the number of detector slices. Today's 64 slice CT systems allow whole-body examinations with sub-millimeter resolution in short scan times. As an alternative to adding even more detector slices, we describe the system concept and design of a CT scanner with two X-ray tubes and two detectors (mounted on a CT gantry with a mechanical offset of 90°) that has the potential to overcome limitations of conventional MDCT systems, such as temporal resolution for cardiac imaging. A dual source CT (DSCT) scanner provides temporal resolution equivalent to a quarter of the gantry rotation time, independent of the patient's heart rate (83 ms at 0.33 s rotation time). In addition to the benefits for cardiac scanning, it allows to go beyond conventional CT imaging by obtaining dual energy information if the two tubes are operated at different voltages. Furthermore, we discuss how both acquisition systems can be used to add the power reserve of two X-ray tubes for long scan ranges and obese patients. Finally, future advances of DSCT are highlighted.

The energy resolving capabilities of Photon Counting Detectors (PCD) in Computed Tomography (CT) facilitate energy-sensitive measurements. The provided image-information can be processed with Dual Energy and Multi Energy algorithms. A research PCD-CT firstly allows acquiring images with a close to clinical configuration of both the X-ray tube and the CT-detector. In this study, two algorithms (Material Decomposition and Virtual Non-Contrast-imaging (VNC)) are applied on a data set acquired from an anesthetized rabbit scanned using the PCD-CT system. Two contrast agents (CA) are applied: A gadolinium (Gd) based CA used to enhance contrasts for vascular imaging, and xenon (Xe) and air as a CA used to evaluate local ventilation of the animal's lung. Four different images are generated: a) A VNC image, suppressing any traces of the injected Gd imitating a native scan, b) a VNC image with a Gd-image as an overlay, where contrast enhancements in the vascular system are highlighted using colored labels, c) another VNC image with a Xe-image as an overlay, and d) a 3D rendered image of the animal's lung, filled with Xe, indicating local ventilation characteristics. All images are generated from two images based on energy bin information. It is shown that a modified version of a commercially available dual energy software framework is capable of providing images with diagnostic value obtained from the research PCD-CT system.

Purpose To develop a dual-energy contrast media-enhanced computed tomographic (CT) protocol by using time-attenuation curves from previously acquired perfusion CT data and to evaluate prospectively the relationship between iodine enhancement metrics at dual-energy CT and perfusion CT parameters in patients with hepatocellular carcinoma (HCC). Materials and Methods Institutional review board and local ethics committee approval and written informed consent were obtained. The retrospective part of this study included the development of a dual-energy CT contrast-enhanced protocol to evaluate peak arterial enhancement of HCC in the liver on the basis of time-attenuation curves from previously acquired perfusion CT data in 20 patients. The prospective part of the study consisted of an intraindividual comparison of dual-energy CT and perfusion CT data in another 20 consecutive patients with HCC. Iodine density and iodine ratio (iodine attenuation of the lesion divided by iodine attenuation in the aorta) from dual-energy CT and arterial perfusion (AP), portal venous perfusion, and total perfusion (TP) from perfusion CT were compared. Pearson R and linear correlation coefficients were calculated for AP and iodine density, AP and iodine ratio, TP and iodine density, and TP and iodine ratio. Results The dual-energy CT protocol consisted of bolus tracking in the abdominal aorta (threshold, 150 HU; scan delay, 9 seconds). The strongest intraindividual correlations in HCCs were found between iodine density and AP (r = 0.75, P = .0001). Moderate correlations were found between iodine ratio and AP (r = 0.50, P = .023) and between iodine density and TP (r = 0.56, P = .011). No further significant correlations were found. The volume CT dose index (11.4 mGy) and dose-length product (228.0 mGy · cm) of dual-energy CT was lower than those of the arterial phase of perfusion CT (36.1 mGy and 682.3 mGy · cm, respectively). Conclusion A contrast-enhanced dual-energy CT protocol developed by using time-attenuation curves from previously acquired perfusion CT data sets in patients with HCC could show good correlation between iodine density from dual-energy CT with AP from perfusion CT. The objective of our study was to show the feasibility of distinguishing between uric acid (UA) and non-UA renal stones using two consecutive spatially registered low- and high-energy scans acquired on a conventional CT system.A total of 33 patients undergoing clinically indicated dual-source dual-energy CT examinations to differentiate UA from non-UA renal stones were enrolled in this study. Immediately after patients underwent clinically indicated dual-source dual-energy CT, two consecutive scans (one at 80 kV and one at 140 kV) were obtained on a conventional CT scanner over the region limited to the stones identified on the dual-source scans. After 3D deformable registration of the 80- and 140-kV images, UA and non-UA stones were identified using commercial software. The sensitivity, specificity, and accuracy of stone classification were calculated using the dual-source results as the reference standard.A total of 469 stones were identified in the dual-source examinations (26 UA and 443 non-UA stones). The average in-plane stone diameter was 4.4 ± 2.5 (SD) mm (range, 2.0-18.9 mm). The overall sensitivity, specificity, and accuracy for identifying UA stones were 73.1%, 90.1%, and 89.1%, respectively. The sensitivity, specificity, and accuracy were 94.7%, 96.9%, and 96.8% for stones 3 mm or larger (n = 341 [19 UA and 322 non-UA]).Accurate differentiation of UA from non-UA renal stones is feasible using two consecutively acquired and spatially registered conventional CT scans.

Computed tomography (CT) is often used to assess the presence of occult fractures when plain radiographs are equivocal in the acute traumatic setting. While providing increased spatial resolution, conventional computed tomography is limited in the assessment of bone marrow edema, a finding that is readily detectable on magnetic resonance imaging (MRI).Dual-energy CT has recently been shown to demonstrate patterns of bone marrow edema similar to corresponding MRI studies. Dual-energy CT may therefore provide a convenient modality for further characterizing acute bony injury when MRI is not readily available. We report our initial experiences of 4 cases with imaging and clinical correlation.

The objective of our study was to assess the accuracy of iodine-related attenuation and iodine quantification as imaging biomarkers of iodine uptake in renal lesions on a single-phase nephrographic image with dual-energy MDCT.Fifty-nine patients (41 men, 18 women; age range, 28-84 years) with 80 renal lesions underwent contrast-enhanced dual-energy CT during the nephrographic phase of enhancement. Renal lesions were characterized as enhancing or nonenhancing on color-coded iodine overlay maps using iodine-related attenuation (in Hounsfield units) and iodine quantification (in milligrams per milliliter). For iodine-related attenuation the iodine uptake thresholds of 15 and 20 HU were tested; a threshold of 0.5 mg/mL was used for iodine quantification. The sensitivity, specificity, positive predictive value (PPV), and negative predictive value (NPV) of iodine-related attenuation and iodine quantification were calculated from chi-square tests of contingency with histopathology or imaging follow-up as the reference standard. The 95% CIs were calculated from binomial expression. Differences in sensitivity and specificity were assessed by means of McNemar analysis.A significant difference in sensitivity and specificity was found between iodine-related attenuation with the thresholds of 15 HU (sensitivity, 91.4%; specificity, 93.3%; PPV, 91.4%; NPV, 93.3%) and 20 HU (sensitivity, 77.1%; specificity, 100%; PPV, 100%; NPV, 84.9%) (p = 0.008) and between iodine quantification (sensitivity, 100%; specificity, 97.7%; PPV, 97.2%; NPV, 100%) and iodine-related attenuation with a threshold of 20 HU (p = 0.004). No significant difference in sensitivity and specificity was found between iodine quantification and iodine-related attenuation with a threshold of 15 HU.Contrast-enhanced dual-energy MDCT with iodine-related attenuation and iodine quantification allows accurate evaluation of iodine uptake in renal lesions on a single-phase nephrographic image.
