Abstract

Background: The impact of histologic regression on sentinel lymph node biopsy (SLNB) status and on clinical outcome is uncertain. Objective: To investigate whether and to what extent regression \75% is able to predict SLNB status and clinical outcome of patients with melanoma [1-mm thick. Methods: The study included patients with diagnoses given at 4 centers of the Italian Melanoma Intergroup. Univariable and multivariable Cox proportional hazard models stratified by center were used to analyze the effect of regression on disease-free interval and melanoma-specific survival. Results: Out of 1182 patients given primary cutaneous melanoma diagnoses during 1998-2015 with a Breslow thickness [1 mm, 954 (304 with and 650 without regression) were included in the analysis. The proportion of patients with a positive SLNB was lower in patients with regression than without (24.4% vs 31.6%, chi-squared test P = .0368). At multivariable analysis, no association was detected between regression and disease-free interval (hazard ratio 1.11, 95% confidence interval 0.85-1.46; P = .4509) or melanoma-specific survival (hazard ratio 1.05, 95% confidence interval 0.77-1.44; P = .7600). Limitation: Retrospective analysis. Conclusion: In our series, regression was not an independent prognostic factor in primary cutaneous melanoma patients with Breslow thickness [1 mm whereas it was associated with a lower incidence of SLNB positivity.

Gene expression analysis was performed on a series ( I n i = 46) of >10% regressed melanoma and a control group ( I n i = 50) of non-regressed (regression <= 10%) melanoma patients who attended the Dermatology Clinic at Turin University Hospital (Table 1). Abstract

Over the last few years functional neuroimaging studies have started unravelling unexpected functional attributes for the precuneus, a previously neglected area located in the posteromedial parietal cortex. Multiple sources of evidence from fMRI and PET investigations suggest a central role for the precuneus in a wide spectrum of highly integrated tasks, including visuo-spatial imagery, episodic memory retrieval, and selfprocessing operations, namely first-person perspective taking and an experience of agency. Furthermore, the precuneus and surrounding structures within the posteromedial parietal region are thought to represent a crucial node in the ‘default mode’ system, a network of brain structures displaying high resting metabolic rate and transient deactivations during engagement in non self-referential tasks. Therefore, it has recently been proposed that the precuneus plays a pivotal role in the neural correlates of consciousness, engaged in self-related mental representations during rest. This hypothesis is consistent with the selective hypometabolism in the posteromedial cortex reported in a wide range of pathophysiological states characterised by altered consciousness, such as sleep, vegetative state, and pharmacological anaesthesia. Functional neuroimaging studies on neuropsychiatric populations showing disruption of the ‘default mode’ activity have recently added novel insights into the understanding of the behavioural correlates of the precuneus.

Abstract

Patients affected by epilepsy show a considerably higher incidence of depression compared with the general population. Since women are twice as likely as men to suffer from depression, female gender could be considered a major risk factor for developing this condition. Converging lines of evidence suggest that sex hormones, which are known to contribute to remodelling the hippocampus, play a pivotal role in both epilepsy and depression. In women, the role of sex hormone levels may be more Important because of their physiological cyclic fluctuations. Oestrogens, more than other ovarian hormones, show an effect similar to antidepressant drugs by stimulating hippocampal synaptogenesis, thus exerting a protective role against seizures as well. This paper reviews the current knowledge on the neurobiological basis of depression in women with epilepsy. The emerging picture informs therapeutic strategies to improve the clinical management of this common comorbidity.

Abstract

The Gilles de la Tourette Syndrome (GTS) is a neurodevelopmental disorder characterised by the presence of multiple motor tics and at least one phonic tic. GTS is increasingly recognized as a relatively common neuropsychiatric disorder, usually diagnosed in early childhood. Comorbid behavioral problems occur in approximately 90% of patients, with attention deficit hyperactivity disorder (ADHD) and obsessivecompulsive disorder (OCD) being the most common ones. Depression is also common, with a lifetime risk of '0% of patients. Moreover, converging evidence shows that children with GTS may have depression ratings that are higher than those in other school age children but lower than those found in children with primary depressive disorder. Whilst the association between GTS and depressive symptoms does not appear to be mediated genetically, social and environmental factors are thought to be of etiological importance. Children with GTS have involuntary movements and noises, which might amount to a chronic stigmatizing disorder in a school setting. This in turn could isolate the individuals, make them a subject for bullying, and ultimately lead to depression. The depression in children with GTS has been shown to result in a reduced health-related quality of life, and in severe cases it may lead to hospitalization and even suicide. Further research is needed to address factors of particular relevance to the etiology of depression in children with GTS and thus improve its recognition but also treatment and outcome.

Abstract

Self injurious behavior, i.e. the deliberate, non-accidental, repetitive infliction of self harm, has been reported in a wide range of neuropsychiatric conditions. Clinical and epidemiological studies have shown that self-injurious behavior can occur in up to one third of patients with Gilles de la Tourette syndrome, a neurodevelopmental disorder, characterized by the presence of multiple motor tics and at least one vocal/phonic tic. This paper describes the case of a patient diagnosed with Gilles de la Tourette syndrome who presented with severe multiple self injurious behaviors since early childhood. On physical examination, we documented a few self-injurious behavior signs including scars in the forehead resulting from repetitive head-banging and bald patches on the scalp due to trichotillomania. Characteristically, self-injurious behaviors were distressing and resistant to treatments (both pharmacotherapy and cognitive behavioral therapy), and were associated with comorbid obsessive-compulsive disorder. This case illustrates the complex nature and difficult management of severe self-injurious behavior in the context of Gilles de la Tourette syndrome and obsessive-compulsive disorder.
