Abstract Introduction Post-COVID syndrome (PCS) is characterized by a polymorphism of symptoms with hypothetical pathophysiological mechanisms. Here, we aimed to analyze the profile of inflammatory cytokines in patients with PCS and to study the relationship between this profile, the clinical symptoms as well as the endothelial function in PCS. Methods Our analytical study involved all eligible patients (n = 66) with PCS included from April 2021 to December 2021. The serum concentration of cytokines IFN-γ, IL-1α, IL-1β, IL-6, IL-8, IL-10, IL-12p70, IL-27, IP-10, MCP-1 and TNF-α was quantified by flow cytometry. Endothelial function was explored by assessing microvascular flow and reactivity using thermal probes. A comparative study was carried out according to the presence of each PCS symptom. Results The average age of our patients was 55.9 ± 16.2 years. The sex ratio was 0.69. Forty-one patients (62%) presented with a severe form of acute infection. The most frequently reported symptoms were dyspnea (67%), fatigue (50%), and memory problems (32%). Fifty-seven patients (86%) had endothelial dysfunction. The majority of patients had increased levels of IP-10 (100%), IL-8 (95%), IFN-γ (95%), MCP-1 (80%), and TNF-α (70%). The serum concentration of IL-10 was below the threshold of quantification in 89% of subjects. The severe form of acute infection was associated with elevated IL-10, MCP-1, and IL-27. Increased IL-6 and IL-27 levels were associated with fatigue while IL-8 concentrations were higher in patients who reported dyspnea. Elevation of IL-8 level was more common in patients with profound impairment of endothelial function. Conclusion Our results further support the presence of endothelial dysfunction in PCS and show an elevation of pro-inflammatory cytokines with a downmodulation of the IL-10- anti-inflammatory response. In addition, immuno-clinical phenotypes emerge, such as an inflammatory profile mediated by IL-6 and IL-27 in fatigue and IL-8 in dyspnea. The identification of immuno-clinical phenotypes would allow a better understanding of the pathophysiology of PCS symptoms.

Aim: Testing the feasibility of the determination to what extent the inability to repair DNA lesions in xeroderma pigmentosum (XP) patients, contributes to the alteration of immune responses, in the course of skin carcinogenesis. Methods: Serum samples from 11 (five XP, six non-XP) bearing skin carcinomas and from three healthy donors, were available for the quantification of IL-2, IL-4, IL-10, IFN‐γ and TNF-α cytokines concentrations. We used kits for ELISA test, by a non-competitive sandwich method. Statistical analysis of the results was performed, using non-parametric Mann-Whitney U test, with an accuracy of 5%. Results: Our results showed that the majority of XP and non-XP cancer patients showed a significant increase in the secretion of TNF-α cytokine above healthy individuals (controls). TNF-α was also found to be significantly high in the serum of XP patients above that reported for the studied non-XP cancer patients. At the same time, TNF-α was not detected in the serum of non-XP and of healthy controls. This increase in the expression level of TNF-α was statistically significant between XP and non-XP patients, and between XP patients and controls. In contrast, there were no significant differences between XP patients and healthy controls, as well as between XP and non-XP patients, for the level of serum IL-2, IL-4 and IL-10 cytokines. On the other hand, we found no detectable levels of IFN‐γ cytokine in the serum of all the studied subgroups. Conclusions: In this study, we demonstrate a general tendency to secrete inflammatory cytokines, in the cancerous groups of patients (XP and non-XP), in comparison to healthy controls, while a significantly higher propensity to develop inflammation, in XP than in non-XP cancer patients.

This study aimed to assess the kinetics of antibodies against the SARS-CoV-2, following natural infection in a cohort of employees of the Institut Pasteur de Tunis (IPT) and to assess the risk of reinfection over a 12-months follow-up period. A prospective study was conducted among an open cohort of IPT employees with confirmed SARS-CoV-2 infection that were recruited between September 2020 and March 2021. Sera samples were taken at 1, 3, 6, 9 and 12 months after confirmation of COVID-19 infection and tested for SARS-CoV-2-specific immunoglobulin G (IgG) antibodies to the spike (S-RBD) protein (IgG anti-S-RBD) and for neutralizing antibodies. Participants who had an initial decline of IgG anti-S-RBD and neutralizing antibodies followed by a subsequent rise in antibody titers as well as those who tested positive for SARS-CoV-2 by RT-PCR after at least 60 days of follow up were considered as reinfected. In total, 137 individuals were included with a mean age of 44.7 ± 12.3 years and a sex-ratio (Male/Female) of 0.33. Nearly all participants (92.7%) were symptomatic, and 2.2% required hospitalization. Among the 70 participants with three or more prospective blood samples, 32.8% were reinfected among whom 11 (47.8%) reported COVID-19 like symptoms. Up to 12 months of follow up, 100% and 42.9% of participants had detectable IgG anti-S-RBD and neutralizing antibodies, respectively. This study showed that humoral immune response following COVID-19 infection may persist up to 12 months after infection despite the potential risk for reinfection that is mainly explained by the emergence of new variants.

We report an uncommon clinical presentation of a placental site trophoblastic tumor. The patient presented initially with abdominal pain with, fever, bleeding and pelvic mass on ultrasonography leading to the wrong diagnosis of a pelvic abscess. Dilation and curettage were performed and pathological examination confirmed the diagnosis of placental site trophoblastic tumor. Therefore, she underwent abdominal hysterectomy. Four years after surgery, the patient is still disease free. Gestational trophoblastic diseases should be considered in every patient presenting abnormal uterine bleeding after delivery or pregnancy loss despite the associated symptoms being very unusual.

Peritonitis is a severe complication in peritoneal dialysis (PD) patients, contributing to technique failure and patient mortality. Tunisia lacks a national dialysis registry, making the incidence of peritonitis unclear. The International Society for Peritoneal Dialysis (ISPD) published guidelines from 2016 to 2022, outlining effective strategies for preventing infections in PD patients. This study was conducted between June and July 2022 to assess Tunisian nephrologists’ knowledge and practices for preventing infections. Using an online questionnaire, 44 participants (67.69% response rate) provided insights. Findings revealed an average score of 4.45 out of 7, indicating significant confusion among respondents. Many incorrectly believe that protecting the catheter under the skin until renal replacement therapy is initiated reduces infections. Questions also addressed nasal Staphylococcus aureus screening, antibiotic prophylaxis, surgical techniques, timing of dressing changes, and local antibiotic use. Responses varied, with 59.1% supporting systematic nasal screening but differing concerning eradication strategies. The study highlights discrepancies between current practices and ISPD guidelines among young Tunisian nephrologists. Recommendations include establishing a national dialysis registry to enhance surveillance and developing tailored national guidelines. A working group from the Tunisian Society of Nephrology, Dialysis, and Transplantation is already developing protocols to address identified gaps and improve PD patient outcomes.

One category of the digital signatures submitted to the NIST Post-Quantum Cryptography Standardization Process for Additional Digital Signature Schemes comprises proposals constructed leveraging the MPC-in-the-Head (MPCitH) paradigm. Typically, this framework is characterized by the computation and storage in sequence of large data structures both in signing and verification algorithms, resulting in heavy memory consumption. While some research on the efficiency of these schemes on high-performance machines has been done, studying their performance and optimization on resource-constrained ones still needs to be explored. In this work, we aim to address this gap by (1) introducing a general method to reduce the memory footprint of MPCitH schemes and analyzing its application to several MPCitH proposed schemes in the NIST Standardization Process. Additionally, (2) we conduct a detailed examination of potential memory optimizations in PERK, resulting in a streamlined version of the signing and verification algorithms with a reduced memory footprint ranging from 22 to 85 KB, down from the original 0.3 to 6 MB. Finally, (3) we introduce the first implementation of PERK tailored for Arm Cortex M4 alongside extensive experiments and comparisons against reference implementations.

Introduction: Leiomyosarcoma (LMS) of the prostate is an extremely rare and aggressive tumor that presents with nonspecific signs and symptoms. Treatment guidelines are not yet established. Case Presentation: We report two cases of LMS of the prostate. The presenting symptom was hematuria, and diagnosis was ascertained through a transurethral resection of the prostate for the 2 patients. The treatment course consisted of three courses of chemotherapy with gemcitabine and docetaxel, radical prostatectomy, and postoperative radiation therapy for the first patient and three courses of gemcitabine and radiation therapy of the prostate and the whole pelvis for the second patient. The follow-up of our 2 patients was 9 and 12 months, respectively. Recurrence occurred 10 months after treatment completion for the second case. No recurrence was noticed in the first case. Conclusion: These two cases highlight the importance of a multimodal approach to yield the best outcomes.

This study investigates the performance of cyclopentyl methyl ether (CPME) in the extraction of fenugreek seed oil, aiming to replace the conventionally used hexane. The efficiency of this alternative solvent was evaluated first through in silico methods (based on Hansen Solubility Parameters (HSPs) and Conductor-like Screening Model for Real Solvent (COSMO-RS) simulations), followed by experimental studies. Solubility computational predictions analysis revealed that CPME exhibits superior solvation power compared to hexane. Experimentally, CPME demonstrated a significantly higher oil yield (7.23%) compared to hexane (4.25%) and a better retention of beneficial unsaturated fatty acids than hexane. Additionally, the physicochemical properties of oils extracted with CPME showed enhanced oxidative stability, sterol, tocopherol, and phenolic contents, leading to superior antioxidant and antibacterial activities. Importantly, CPME’s low volatile organic compound (VOC) emissions further establish it as a more sustainable and environmentally friendly alternative to hexane, aligning with contemporary goals of reducing harmful emissions in extraction processes. Thus, this paper highlights the functional advantages of CPME, focusing on its efficiency, selectivity, and enhanced retention of bioactive compounds, positioning it as a superior extraction solvent for fenugreek seed oil compared to hexane.

Pancreatic β-cells are crucial regulators of systemic glucose homeostasis, and their dysfunction and loss are central features in type 2 diabetes. Interventions that rectify β-cell dysfunction and loss are essential to combat this deadly malady. In the current study, we sought to delineate the role of soluble epoxide hydrolase (sEH) in β-cells under diet-induced metabolic stress. The expression of sEH was upregulated in murine and macaque diabetes models and islets of diabetic human patients. We postulated that hyperglycemia-induced elevation in sEH leads to a reduction in its substrates, epoxyeicosatrienoic acids (EETs), and attenuates the function of β-cells. Genetic deficiency of sEH potentiated glucose-stimulated insulin secretion in mice, likely in a cell-autonomous manner, contributing to better systemic glucose control. Consistent with this observation, genetic and pharmacological inactivation of sEH and the treatment with EETs exhibited insulinotropic effects in isolated murine islets ex vivo. Additionally, sEH deficiency enhanced glucose sensing and metabolism with elevated ATP and cAMP concentrations. This phenotype was associated with attenuated oxidative stress and diminished β-cell death in sEH deficient islets. Moreover, pharmacological inhibition of sEH in vivo mitigated, albeit partly, high fat diet-induced β-cell loss and dedifferentiation. The current observations provide new insights into the role of sEH in β-cells and information that may be leveraged for the development of a mechanism-based intervention to rectify β-cell dysfunction and loss.

Abstract Rowshan is a distinctive architectural element and a primary component of the façades of traditional houses in western Saudi Arabia. It is window type primarily made from wood and is incorporated in contemporary architecture to express the identity of the Hejazi architectural heritage without the realization of the advantages of this rich architectural element. This study examines the Rowshan design philosophy by identifying its visual, functional, and structural determinants, and applying them to a model to reflect it in a modern context. The study adopts an analytical approach in addition to interviewing elders who lived in traditional houses. The results show that the Rowshan philosophy must apply all its structural, functional, and aesthetic characteristics like seating function, privacy, visual continuity, use of light, exploitation for shade, and the use of ornamental patterns and, openings, while adopting modularity, flexibility, and adaptability as important principles in its composition. This design philosophy can be a fundamental resource for creative designs to preserve Hejazi architectural identity. This study aims to renew interest in Rowshan elements in contemporary houses in the Mecca region and highlights the advantages of this unique architectural element by applying the Rowshan design philosophy.

Abstract Nitroglycerin (NTG) is a prodrug that has long been used in clinical practice for the treatment of angina pectoris. The biotransformation of NTG and subsequent release of nitric oxide (NO) is responsible for its vasodilatating property. Because of the remarkable ambivalence of NO in cancer disease, either protumorigenic or antitumorigenic (partly dependent on low or high concentrations), harnessing the therapeutic potential of NTG has gain interest to improve standard therapies in oncology. Cancer therapeutic resistance remains the greatest challenge to overcome in order to improve the management of cancer patients. As a NO releasing agent, NTG has been the subject of several preclinical and clinical studies used in combinatorial anticancer therapy. Here, we provide an overview of the use of NTG in cancer therapy in order to foresee new potential therapeutic avenues.

Design spaces can be read in two complementary structural and compositional dimensions: the implicit dimension, and the formal dimension. This study aimed to answer the following questions: What are the phases of the original idea transformation in the design process? Does taking advantage of the implicit dimension of others' interior spaces count as inspiration or plagiarism? Participants comprised nine postgraduate students, who were asked to copy others' project ideas by applying four design phases: idea source, test, choice, and output. The results showed that plagiarism of the concept of others' work could not occur unless the concept copies the structural, functional, and aesthetic aspects of the design space at the level of its components and the relationships between them. Otherwise, taking advantage of the implicit dimension of others' project ideas can be an inspiration. The results are expected to increase awareness among interior design instructors in higher education.
